Gliomas are the most common pediatric tumors of the central nervous system. In this review, we discuss the clinical features, treatment paradigms, and evolving concepts related to two types of pediatric gliomas affecting two main locations: the optic pathway and thalamus. In particular, we discuss recently revised pathologic classification, which adopting molecular parameter. We believe that our review contribute to the readers' better understanding of pediatric glioma because pediatric glioma differs in many ways from adult glioma according to the newest advances in molecular characterization of this tumor. A better understanding of current and evolving issues in pediatric glioma is needed to ensure effective management decision.
Purpose: This study was conducted to gather descriptive data on trauma victims and to observe the general demographic characteristics and clinical profile of trauma victims who were admitted to a regional emergency medical center in the west southern Kyungsangnam-do area. Objects & Method: The study population consisted of 1,909 trauma patients who visited the emergency department of Gyeongsang National University Hospital between January 2003 and December 2004. The medical records were reviewed in a retrospective manner. Demographic data, the mechanism of injury, and clinical information were collected by three professional medical affairs recorders and an emergency physician and a Revised Trauma Score (RTS) and an Injury Severity Score (ISS) were calculated for each patient. Collected data were analyzed with SPSS software version 12.0. Results: Male patients outmembered female patients (M:F=2.54:1), and the mean age of the population was $40.5{\pm}21.4$ years. The mean RTS and ISS were $7.45{\pm}1.11$ and $8.40{\pm}7.44$, respectively. The seventies showed the highest ISS($10.94{\pm}8.66$). The most common mechanism of injury was motor-vehicle accidents (45.57%), followed by falls or slips(28.26%), and other blunt injuries(12.68%). The most frequent causes of death was cerebral herniation due to head injury(68.4%) and irreversible shock(26.3%). Conclusion: The present study clarified the demographic and clinical characteristics of trauma patients in the Kyungsangnam-do area. In the future, prospective clinical data collection is needed for a more sophisticated trauma study.
Background: The benefits of epicardial ultrasound scanning (EUS) in coronary artery bypass grafting (CABG) have not yet been established. The aim of this study was to evaluate the usefulness of EUS in CABG, including in the assessment of the quality of distal anastomoses, the identification of epicardial target vessels, and the evaluation of any graft issues other than the distal anastomoses. Methods: Fifty-three patients undergoing CABG were enrolled between March 2018 and February 2019. Intraoperative EUS was performed along with transit-time flow measurement (TTFM). Graft evaluations were performed early (shortly after surgery) and 1 year after surgery for 53 (100%) and 47 (88.7%) patients, respectively. Results: EUS was applied to assess the quality of all distal anastomoses, 32 target vessels, and 2 conduit trunks. Insufficient TTFM findings were obtained for 18 grafts. However, graft revision was performed for only 3 distal anastomoses; based on the EUS findings, the remaining 15 sites were not revised. The early and 1-year overall graft patency rates were 100% (141 anastomoses) and 96.1% (122 of 127 anastomoses), respectively. All 15 of the distal anastomoses that were not revised despite insufficient TTFM results were patent at the 1-year mark. Conclusion: The routine application of EUS in CABG could be beneficial by confirming the quality of surgery and reducing unnecessary procedures.
Objective : This study was conducted to evaluate of efficacy and safety of Vanadium Water (VW) on Cancer related Fatigue of Non-advanced Cancer Patients. Methods : Experimental Group Administration. Participants in treatment group consumed three 500 ml/bottle bottles of Jeju Island's VW, which contains 40.2 ${\mu}g/L$ of vanadium, three times a day (morning, afternoon, evening) for 4 weeks (28 days) without any additional treatments for fatigue improvement. The control group participants consumed three 500 ml/bottle bottles of ordinary water (0 ${\mu}g/L$ of vanadium) three times a day for the same period of time without any additional treatments for fatigue improvement. Results : After 4 weeks of VW consumption, total score of Revised Piper Fatigue Scale (RPFS) was reduced 30% and basal score after Fisher's Exact Test resulted statistical borderline significance (p=0.0799) and result of t-test of comparison between two groups' RPFS total score and fatigue cognitive/mood changes resulted statistical significance (p=0.0112). In detail, only 36.4% of control group (N=11) showed improvement while 77.8% of treatment group (N=9) showed fatigue improvement and thyroid hormone level changes. No other lab measures indicated any significant differences between two groups. Conclusion : Daily consumption of 1.5 L of Vanadium water for 4 weeks showed improvement of non-advanced cancer patients' fatigue.
Objectives: The aim of this study was to develop a Phlegm Pattern Questionnaire for Functional Dyspepsia (PPQ-FD) by modifying a previously developed Phlegm Pattern Questionnaire (PPQ) and to verify its reliability and validity. Further objectives were to obtain the optimal cut-off value for the PPQ-FD for standardization and for clinical use. Methods: The PPQ-FD was developed by extracting the major symptoms of the phlegm pattern in functional dyspepsia and by using the Delphi method to administer a requested importance survey to experts. The reliability, validity, and optimal cut-off value of the PPQ-FD were analyzed by enrolling a total of 60 subjects in this study. Thirty patients were diagnosed with both functional dyspepsia and phlegm pattern and thirty patients were diagnosed with only functional dyspepsia. All participants were requested to fill out the PPQ-FD. Results: No statistically significant differences were detected in the two groups for sex distribution, age, or body mass index. Five of the survey questions negatively affected its reliability; therefore, we decided to exclude those five questions on further inspection. The Cronbach's α coefficient of the revised PPQ-FD was 0.853, and clinical validity was verified. Construct validity was analyzed by factor analysis and identified four factors. Statistically significant positive correlations were found between the revised PPQ-FD and other dyspepsia scales, such as the SQDQ, SSQD, VAS, NDI-K, and FD-QoL scales. The VAS had particularly strong positive correlations with the PPQ-FD. Conclusions: The PPQ-FD developed in this study has fundamental reliability and validity for use as a pattern-diagnosis questionnaire. The PPQ-FD can help to diagnose the phlegm pattern in patients with functional dyspepsia.

Background: Estimation of the probability of a patient having an acute pulmonary embolism (PE) for patients with a suspected PE are well established in North America and Europe. However, an assessment of the prediction rules for a PE has not been clearly defined in Korea. The aim of this study is to assess the prediction rules for patients with a suspected PE in Korea. Methods: We performed a retrospective study of 210 inpatients or patients that visited the emergency ward with a suspected PE where computed tomography pulmonary angiography was performed at a single institution between January 2005 and March 2007. Simplified Wells rules and revised Geneva rules were used to estimate the clinical probability of a PE based on information from medical records. Results: Of the 210 patients with a suspected PE, 49 (19.5%) patients had an actual diagnosis of a PE. The proportion of patients classified by Wells rules and the Geneva rules had a low probability of 1% and 21%, an intermediate probability of 62.5% and 76.2%, and a high probability of 33.8% and 2.8%, respectively. The prevalence of PE patients with a low, intermediate and high probability categorized by the Wells rules and Geneva rules was 100% and 4.5% in the low range, 18.2% and 22.5% in the intermediate range, and 19.7% and 50% in the high range, respectively. Receiver operating characteristic curve analysis showed that the revised Geneva rules had a higher accuracy than the Wells rules in terms of detecting PE. Concordance between the two prediction rules was poor ($\kappa$ coefficient=0.06). Conclusion: In the present study, the two prediction rules had a different predictive accuracy for pulmonary embolisms. Applying the revised Geneva rules to inpatients and emergency ward patients suspected of having PE may allow a more effective diagnostic process than the use of the Wells rules.
Objective : The definitions and categories of subjects related to humanities and social medicine are still controversial, and we tried to find the areas lacking compared to the standards of medical education Method : Humanities and social medicine-related subjects of the World Medical Education Standards, ASK2019, and KAS2021 were reviewed, and humanities and social medicine subjects of the College of Korean Medicine were searched. We compared subjects with the medical school learning outcomes and Korean Medicine Doctors' job competency. In order to increase validity, two authors independently classified them. In case of different opinions, we revised after sufficient discussion Results : Humanities and social medicine-related subjects in the Korean Medicine curriculum appear to be sufficient when compared with WFME and ASK2019. The humanities and social medicine-related subjects of Korean Medicine schools were not distributed evenly throughout. The areas to be supplemented in Korean Medicine education were 'a diverse understanding of life and death', 'prevention and response to patient safety incidents', 'effective communication with health-related organizations and groups', 'social responsibility and reflective practice' and 'communication between health and medical professions'. Conclusion : Humanities and social medicine-related subjects in Korean Medicine education are sufficient, but they are not evenly distributed, and the areas listed above need to be reinforced.
Multiple psychosocial problems and many chronic diseases of adulthood can be influenced by adolescent nutritional problems. In Korea, adolescent obesity and obesity related health risks have been increased and insufficient intakes of nutrients, such as calcium, iron and potassium, and distorted thinking about obesity are also common. However there are no comprehensive countermeasure because of the excessive burden of studies and the lack of community interest. And the nutrition guidelines that is suitable for Korean adolescent leaves something to be desired, and the pediatrician's concern is lacking yet. In the Korean dietary reference intakes 2010 that was revised according to the 2007 Korean National Growth Chart and 2007 Korean National Health and Nutrition Examination Survey, the range for adolescents is changed to the age 12-18 and dietary reference intakes of some nutrients mainly with vitamin D is changed. Recently several researches, about how school nutrition policies and media effect on eating habits and the weight concerns, and influences of public nutrition policies and fast food commercials have been actively studied to improve adolescent nutritive conditions. In this review, I summarize the dietary reference intakes for Korean adolescents that were revised in 2010, and current studies about the adolescent nutrition.

Objectives: This study aimed to compare the utility of the Psychoeducational Profile-Revised (PEP-R), Korean Wechsler Preschool and Primary Scale of Intelligence, Fourth Edition (K-WPPSI-IV), and Vineland Adaptive Behavior Scale, Second Edition (VABS-II) for evaluating developmental disabilities (DD) in preschool children. Additionally, we examined the correlations between the PEP-R, K-WPPSI-IV, and VABS-II. Methods: A total of 164 children aged 37-84 months were assessed. Children's development was evaluated using the PEP-R, K-WPPSI-IV, VABS-II, Preschool Receptive-Expressive Language Scale, and Korean Childhood Autism Rating Scale, Second Edition. Results: Of the 164 children, 103 had typical development (TD) and 61 had DD. The mean of the PEP-R Developmental Quotient (DQ), K-WPPSI-IV Full-Scale Intelligence Quotient (FSIQ), and VABS-II Adaptive Behavior Composite (ABC) scores were significantly higher in the TD group than in the DD group (p<0.001). The estimated area under the curve of the PEP-R DQ, K-WPPSI-IV FSIQ, and VABS-II ABC scores was 0.953 (95% confidence interval [CI]=0.915-0.992), 0.955 (95% CI=0.914-0.996), and 0.961 (95% CI=0.932-0.991), respectively, which did not indicate a statistically significant difference. The PEP-R DQ scores were positively correlated with the K-WPPSI-IV FSIQ (r=0.90, p<0.001) and VABS-II ABC scores (r=0.84, p<0.001). A strong correlation was observed between the K-WPPSI-IV FSIQ and VABS-II ABC scores (r=0.89, p<0.001). Conclusion: This study found that the PEP-R, K-WPPSI-IV, and VABS-II effectively distinguished DD from TD in preschool children, and no significant differences in utility were observed between them.

The taxonomy of Acanthamoeba spp., an amphizoic amoeba which causes granulomatous amoebic encephalitis and chronic amoebic keratitis, has been revised many times. The taxonomic validity of some species has yet to be assessed. In this paper, we analyzed the morphological characteristics, nuclear 18s rDNA and mitochondrial 16s rDNA sequences and the Mt DNA RFLP of the type strains of four Acanthamoeba species, which had been previously designated as A. divionensis, A. parasidionensis, A. mauritaniensis, and A. rhysodes. The four isolates revealed characteristic group II morphology. They exhibited 18S rDNA sequence differences of $0.2-1.1\%$ with each other, but more than $2\%$ difference from the other compared reference strains. Four isolates formed a different clade from that of A. castellanii Castellani and the other strains in morphological group lion the phylogenetic tree. In light of these results, A. paradivionensis, A. divionensis, and A. mauritaniensis should be regarded as synonyms for A. rhysodes.
