Toxoplasma gondii is a zoonotic parasite resulting in human infections and one of the infectious pathogens leading to uveitis and retinochoroiditis. The present study was performed to assess T. gondii infection in 20 ocular patients with chronic irregular recurrent uveitis (20 aqueous humor and 20 peripheral blood samples) using PCR. All samples were analyzed by nested PCR targeting a specific B1 gene of T. gondii. The PCR-positive rate was 25% (5/20), including 5% (1) in blood samples, 25% (5) in aqueous humor samples, and 5% (1) in both sample types. A molecular screening test for T. gondii infection in ocular patients with common clinical findings of an unclear retinal margin and an inflammatory membrane over the retina, as seen by fundus examination, may be helpful for early diagnosis and treatment.
본 연구는 외부 전원 없이 광다이오드만을 이용하여 생성한 광전 자극을 통해 신경계를 효과적으로 자극하는 방법에 대한 것이다. 광을 통한 전류원 생성 및 전달은 생체 내에 집적된 광소자를 삽입하고 외부에서 광을 통해 신호와 전력을 전달을 한다. 이 기술은 특히 '눈' 이라는 광학적인 연결통로를 이용할 수 있는 인공망막과 같은 시스템에 매우 효과적이다. 그러나 광전 소자를 내부 전원 없이 구동시키는 경우, 광전류가 생체 저항에 직접적인 영향을 받게 되므로 자극에 충분한 전류를 생성할 수 없다. 무 전원 광다이오드를 통해 생성되는 광전류를 신경 자극에 적용하기 위해서는 생체 저항의 크기에 관계없이 활동 전위 생성에 충분한 전류 공급을 할 수 있는 안정된 전류원이 필요하다. 이를 위해서 본 연구에서는 병렬 저항을 도입하였다. 병렬 저항 추가 시 생체 저항을 포함한 전체 저항 값이 낮아지므로, 광원의 세기에 따라 최대의 광전류에 근접한 값을 얻을 수 있게 된다. 그러나 병렬 저항 값의 크기를 낮출수록 자극에 쓰이지 않는 전류량이 늘어나므로, 자극 전류량의 극대 값을 찾기 위해서는 병렬 저항 값의 최적화가 필요하다. 실험을 통해 측정된 실제 자극 전류량이 최대가 되는 병렬 저항 값의 범위는 500Ω∼700Ω 이고, 이때 전류량은 580uA∼860uA 이며 전류 효율은 47.5∼59.7%이었다. 자극의 크기와 빈1도를 변화시키면서 쥐의 좌골 신경을 자극하여 눈으로 확인 가능한 떨림 현상을 확인하였으며, 다채널 기록기를 이용해 활동 전위를 측정하였다. 이를 통해, 인공 망막에서의 광 자극 가능성을 확인할 수 있었다.
The incidence of blindness resulting from diabetic retinopathy has significantly increased despite the intervention of insulin to control diabetes mellitus. Early signs are microaneurysms, exudates, intraretinal hemorrhages, cotton wool patches, microvascular abnormalities, and venous beading. Advanced stages include neovascularization, fibrous formations, preretinal and vitreous microhemorrhages, and retinal detachment. Microaneurysm count is important because it is an indicator of retinopathy progression. The purpose of this paper is to apply data mining to detect diabetic retinopathy patterns in routine fundus fluorescein angiography. Early symptoms are of principal interest and therefore the emphasis is on detecting microaneurysms rather than vessel tortuosity. The analysis does not involve image-recognition algorithms. Instead, mathematical filtering isolates microaneurysms, microhemorrhages, and exudates as objects of disconnected sets. A neural network is trained on their distribution to return fractal dimension. Hausdorff and box counting dimensions grade progression of the disease. The field is acquired on fluorescein angiography with resolution superior to color ophthalmoscopy, or on patterns produced by physical or mathematical simulations that model viscous fingering of water with additives percolated through porous media. A mathematical filter and neural network perform the screening process thereby eliminating the time consuming operation of determining fractal set dimension in every case.
Ocular toxoplasmosis is a disease caused by the infection with Toxoplasma gondii through congenital or acquired routes. Once the parasite reaches the retina, it proliferates within host cells followed by rupture of the host cells and invasion into neighboring cells to make primary lesions. Sometimes the restricted parasite by the host immunity in the first scar is activated to infect another lesion nearby the scar. Blurred vision is the main complaint of ocular toxoplasmic patients and can be diagnosed by detection of antibodies or parasite DNA. Ocular toxoplasmosis needs therapy with several combinations of drugs to eliminate the parasite and accompanying inflammation; if not treated it sometimes leads to loss of vision. We describe here clinical features and currently available chemotherapy of ocular toxoplasmosis.
An image feature extraction method for the low contrast fluoresceln angiogram in dlabetes was studied. To obtain effective image segmentation, an adaptive local difference image is generated and relaxation process are applied to this difference Image. By the use of distance transformed data with segmented image, shape and location of feature regions were obtained. It was shown that the location and shape descriptions of Impaired blood vessel networks and retinal regions are can he utilized for the diagnosis of diabetes and other disease.
SCA 1 is an autosomal dominant disorder. The phenotypic manifestations of SCA 1 are not specific, and thus, the diagnosis of SCA 1 rests on molecular genetic testing. The number of CAG repeats ranges from 6-44 in normal alleles and from 39-81 repeats in disease-causing alleles(chromosomal locus 6p22-23). The main clinical features of SCA 1 are ataxia, dysarthria, ophthalmoparesis, extrapyramidal signs without retinal degeneration. A 24-year-old woman with suspected family history presented with progressive cerebellar ataxia, dysarthria, ptosis, titubation and general weakness. Brain MRI revealed a moderate cerebellar atrophy. A genomic polymerase chain reaction(PCR) analysis showed 66 repeats at the SCA 1 locus.
Objective : The purpose of this study is to report the oriental-western cooperative treatment on opthalmic and dermatologic disease caused by adverse effect of filler injection galbella region. Method : After filler injection, a patient suffers from spontenous blineness and palsy of oculomotor and trochlear nerve on right eye and erythema on glabella and nasal region. She is admitted to kyung-hee oriental hospital for 2 weeks with oriental(herbal-medicine, acupunture and nega treatment) and western(department of opthalmology, dermatology and plastic surgery measurement) treatment. Result & Conclusion : Nerve palsy was recovered near to normal. And skin legion was proceeded to recovery period without any complication. This recovery speed with oriental-western cooperative treatment is much faster than usual treatment.
본 연구에서는 당뇨망막병증 의증 진단을 위해 영상처리 알고리즘을 이용하여 안저영상을 분석하고, 비트 플레인(Bit Plane) 기법을 활용한 당뇨망막병증 질환의 특징인 경성삼출물 및 망막 출혈 등의 특정 부위를 추출한다. 분석된 안저영상을 기반으로 당뇨망막병증의 특징을 수치화하고 자동으로 진단이 가능한 시스템을 제안한다.
성호르몬은 lacrimal system, corneal anatomy and disease, aqueous humor dynamics and glaucoma, crystalline lens and cataract, retinal disease를 포함한 안과영역의 다양한 부위에 영향을 끼칠 수 있다. Androgen은 전신에 걸친 sebaceous glands의 발달, 분화, 지질생성 등에 관여하므로 androgen deficiency는 meibomian gland의 기능장애와 건성안에서 증발을 더욱더 심화시킬 수 있는 것으로 알려져 있다. 반면에 estrogens은 sebaceous glands의 크기, activity, 지질생성을 감소시켜 meibomian gland에서는 androgen과 반대되는 작용을 할 가능성이 있다. 성호르몬은 또한 각막의 구조와 질병경과에도 영향을 줄 수 있는데, 각막이 월경주기 제2일째 및 배란주위기에 두꺼워지며, 폐경 여성에서 여성호르몬 치료 시 각막두께가 증가한다는 사실은 estrogen levels과 각막 두께와의 연관을 시사하는 소견이라고 하겠다. Fuch's dystrophy 역시 폐경 후 여성에서 더 흔히 관찰되어, 나이에 따른 호르몬 변화가 그 원인 중 하나로 지목되고 있으며, vernal keratoconjunctivitis 환자의 각막조직에서는 estrogen과 progesterone 수용체의 overexpresson이 발견되어 역시 이들 질환에서의 성호르몬의 역할을 시사한다고 할 수 있겠다. 성호르몬은 안압에도 영향을 미칠 수 있는데, intraocular pressure와 혈중 luteinizing hormone, follicular stimulating hormone (FSH), estrone, estradiol, progesterone, testosterone과는 연관이 없는 것으로 보고한 결과도 있으나, 혈중 progesterone의 경우 특히 임신중인 여성에서 안압과의 연관관계가 보고되었으며, 또한 폐경 여성에서 혈중 남성호르몬 농도가 높을수록 안압이 상승하는 경향이 있음이 보고되었다. 한편 'Female sex'는 모든 종류의 백내장의 위험요소인 것으로 알려져 있다. 백내장으로 진단받은 여성의 경우 호르몬 측정 시 연령등을 보정하고도 폐경기에 가까운 호르몬 수치를 보였으며, 이들 백내장 환자군내에서는 estradiol과 FSH 농도 사이에 유의한 음의 상관관계가 보고되었다. 또한 DHEAS의 경우 백내장 위험도를 감소시키는 것으로 보고되었고, 그 기전으로 전구호르몬인 DHEA의 역할을 제시되고 있는데. 즉 백내장의 주요 기전 중 하나인 oxidative stress를 감소시키는 antioxidant activity를 DHEA가 보이기 때문일 것으로 추정되고 있다. 성호르몬은 망막 내 혈관의 흐름에도 영향을 줄 수 있는 것으로 알려져 있는데, 안구 내 혈류에서 여성호르몬의 긍정적인 효과와 남성호르몬의 부정적인 효과가 보고되었고, age-related macular degeneration (AMD)의 경우, estradiol이 ER-$\beta$ 수용체를 통해 보호효과가 있음이 보고되어, ER-$\beta$ 수용체가 향후 AMD 치료에서 치료적 target이 될 수도 있을 것으로 알려지고 있다. 또한 DHEAS의 경우 AMD 위험도를 증가시킬 가능성이 있으며, 당뇨로 진단받은 환자에서는 혈중 testosterone 농도는 당뇨병성 망막병증의 진행과 연관이 있는 것으로 보고되었다. 성호르몬은 안과영역의 다양한 질환과의 연관관계는 아직 추가연구가 필요한 실정이나 다양한 성호르몬이 안질환의 발생 및 경과에 영향을 줄 가능성이 있으며, 이와 같은 연관관계가 규명된다면 추후 호르몬을 통해 질환의 경과 및 예방의 가능성이 제시될 수도 있을 것으로 사료된다. 특히 폐경이후 다양한 안질환이 증가한다는 기존의 연구결과들을 고려 시, 안과영역에 있어 폐경기 여성호르몬 치료의 긍정적인 역할에 관해서는 지속적인 관심이 요구된다고 하겠다.
Objectives : The purpose of this study was to analyze and report status of Korean Medicine Ophthalmology patients who did funduscopy examination. Through this, we hope that the development of our diagnosis and treatment. Methods : From June 1, 2010 to May 31, 2019, Based on the electronic medical records of patients who had funduscopy examination at Korean Medicine Ophthalmology, Busan University Korean Medicine Hospital, the gender, age, visiting motives and paths, diagnosis, examination number of years, other eye examinations and treatments method were summarized and analyzed. Results : 463 patients were able to check the electronic medical records. They were 283 females and 180 males. The mean age of the patients was 51.5 years and elderly patients who 50s and 60s were 49.3% of whole patients. The most common motives for Korean Medicine Ophthalmology visitation was 'combination treatment with other department in Korean Medicine Hospital'. Outer eye diseases were 283 cases, inner eye diseases were 198 cases. Dry eye syndrome, asthenopia, visual discomfort, conjunctivitis, and eye discomfort were most common in the outer eye diseases. Cataracts, Vitreous floater, Macular Degeneration, Glaucoma and Ocular Pain were most common in the inner eye disease. The most common parts of outer eye diseases were Conjunctival, lacrimal gland, paralytic, corneal, eyelid and front uveal, scleral disease and then in inner eye diseases parts, Retinal, lens, vitreous, glaucoma, optic nerve, behind uveal, choroid disease were most common. The number of funduscopy examination was ups and downs. Herbal medicine was the most common used. Conclusions : The funduscopy examination is essential for diagnosis and treatment of eye disease. We hope that the use of fundus examination and other ophthalmologic examination will be expanded soon in Korean Medicine Ophthalmology.
