운동중추 주변에 위치한 전이성 뇌종양의 수술 후 운동 기능에 대한 평가 (Evaluation of the Postoperative Motor Function for Metastatic Brain Tumors Around the Motor Cortex)

  • Objective : Intracranial metastasis is responsible for nearly 50% of mortalities of systemic cancers. Although its frequency is increasing and there is little doubt that improving the quality and expectancy of life is the final goal, the mode of treatment is still disputed. The authors report the postoperative motor function after surgery in patients with metastatic tumors around the motor cortex. Materials and Methods : We studied 24 patients with metastatic tumors around the central sulcus during the last 22 months. Motor function was assessed pre- and post-operatively as well as its response to corticosteroids. MRI, neuronavigation system and intraoperative ultrasonography were used for tumor localization and functional MRI and cortical stimulator were used to define the motor cortex. Results : Single metastasis was found in 13 cases(54%) and 11 cases(44%) had multiple foci. Thirteen cases were located in precentral, 7 in postcentral, and 4 in superior or middle-frontal lobe. The most common primary focus was the lung(16 cases). There was no difference in postoperative motor function improvement between the steroid responsive group and non-responsive group(92% versus 90%). Ninty-two percent of the patients showed significant improvement of motor function and lived independently but there was worsening in the upper extremity in one and in another no improvement. Whole brain radiation of 3000cGy was given in all cases and 4 patients died of recurrence in primary or intracranial focus during mean follow-up periods of 14 monthes. Conclusion : Surgery may provide substantial improvement of the motor function and quality of life of the patients with metastatic tumors around the motor cortex.

  • Type 2 diabetes mellitus relavant to insulin resistance is a chronic and hard to control. In order to develop an antidiabetic agent from natural products, anti-hyperglycemic effect of herbal formula containing Mori Follium, Euonymi Lignum Suberalatum and Ginseng Radix(MEG) was investigated in db/db mice. Treatment group was administered orally with MEG formula at a dose of 300 mg/kg for 5 weeks, and blood glucose, insulin and lipid levels were determined. MEG treatment group showed a marked decrease in fasting blood glucose level and insulin resistance index(IRI) compared to those in diabetic control. Improvement of insulin resistance(60.6%) was indicative of reducing lipid levels in plasma and triglyceride contents in muscle and adipose tissue. In addition, expressions of an insulin responsive gene, glucose transporter 4(Glut4), in muscle and adipose tissue were upregulated in MEG treatment group. Compared islet morphology between groups, MEG formula prevented the ${\beta}$-cell destruction caused by high blood glucose. Taken together, MEG formula can act as an anti-hyperglycemic agent with insulin sensitizing effect, and thus deserves a clinical trial in the future.

동결견에 대한 주사요법; 통증유발부위 주사 및 신경 차단술 (Injection Treatment for Frozen Shoulder ; Trigger Point Injection and Neruologic Blockade)

  • Frozen shoulder is known as a self-limited disease. But, its long duration and pain nature can make the patients debilitative. And most patients cannot tolerate a chronically painful extremity and are concerned about the possibility of developing permanent dysfunction. In painful phase of frozen shoulder, some aggressive mordalties as like trigger point injection or suprascapular nerve block can beneficial to: reduce discomfort and pain. In order to document clinical results, we evaluated the results of 134 frozen shoulders treated with trigger point injection and/or suprascapular nerve block at Kyungpook National University Hospital, from January 1995 to April 1997. The treatment group was divided into 3 modalities: 17 cases in trigger point injection(TPI), 39 cases in suprascapular nerve block(SSB), and 78 cases in both methods. The supportive treatment including oral medication, heat and stretching exercise was also applied. The average age at the time of diagnosis was 57 years old and average follow-up time was 18 months. The results were as follows: Average time of significant improvement in pain was 9 days. Eighty-eight percent (119 cases) was improved in pain and range of motion after injecllion treatments; 82%(14/17) with TPI, 85%(33/39) with SSB, and 92%(72/78) with both. Early improvement of paih within 1 week was 72% in the treatment-responsive group, in which TPI group has 100% response(14/14) and sse has 94% response(31/33)

TERT mRNA Expression is Up-Regulated in MCF-7 Cells and a Mouse Mammary Organ Culture (MMOC) System by Endosulfan Treatment

  • Endosulfan is one of the organochlorine pesticides, which are well-known endocrine disruptors (EDs), and it acts as an estrogen agonist. Estrogen is a group of hormones that play an important role in mammary gland function and are implicated in mammary carcinogenesis. In the present study, we studied the effects of endosulfan on nodule like alveolar lesion (NLAL) formation in mouse mammary gland development using a mouse mammary gland organ culture (MMOC) system. Although endosulfan-treated mammary glands did not form NLALs, more alveolar buds were formed in this group than in the negative control (vehicle-treated) group. In addition, telomerase reverse transcriptase (TERT) mRNA expression levels were increased in endosulfan-treated mammary glands in a dose-dependent manner. Telomerase can be activated by estrogen, therefore, we examined the effects of endosulfan on telomerase activity, and found that the telomerase activity in estrogen receptor-positive MCF-7 cells was up-regulated by endosulfan treatment. Moreover, this activation was accompanied by the up­regulation of the TERT mRNA expression. Also, transient expression assays using CAT reporter plasm ids containing various fragments of the TERT promoter showed that this imperfect palindromic estrogen-responsive element is almost certainly responsible for the transcriptional activation by endosulfan. These results may help elucidate the endocrine disrupting mechanism of endosulfan.

Screening of Stat3 inhibitory effects of Korean herbal medicines in the A549 human lung cancer cell line

  • Background: The transcription factor signal transducer and activator of transcription 3 (Stat3)is constitutively activated in many human cancers. It promotes tumor cell proliferation,inhibits apoptosis, induces angiogenesis and metastasis, and suppresses antitumor hostimmune responses. Therefore, Stat3 has emerged as a promising molecular target for cancertherapies. In this study, we evaluated the Stat3-suppressive activity of 38 herbal medicinestraditionally used in Korea.Methods: Medicinal herb extracts in 70% ethanol were screened for their ability to suppressStat3 in the A549 human lung cancer cell line. A Stat3-responsive reporter assay system wasused to detect intracellular Stat3 activity in extract-treated cells, and Western blot analyseswere performed to measure the expression profiles of Stat3-regulated proteins.Results: Fifty percent of the 38 extracts possessed at least mild Stat3-suppressive activities(i.e., activity less than 75% of the vehicle control). Ethanol extracts of Bupleurum falcatumL., Taraxacum officinale Weber, Solanum nigrum L., Ulmus macrocarpa Hance, Euonymus alatusSieb., Artemisia capillaris Thunb., and Saururus chinensis (Lour.) Baill inhibited up to 75% of thevehicle control Stat3 activity level. A549 cells treated with these extracts also had reducedBcl-xL, Survivin, c-Myc, and Mcl-1 expression.Conclusion: Many medicinal herbs traditionally used in Korea contain Stat3 activity-suppressing substances. Because of the therapeutic impact of Stat3 inhibition, these resultscould be useful when developing novel cancer therapeutics from medicinal herbs.

Effects of mushroom waster medium and stalk residues on the growth performance and oxidative status in broilers

  • Objective: The study developed mushroom stalk residues as feed additives in the broiler diet for improving the growth performance and immunity of broilers as well as to increase the value of mushroom stalk residues. Methods: In total, 300 ROSS 308 broilers were randomly allocated into fifteen pens with five dietary treatments: i) control, basal diet; ii) CMWM, supplemented with 1% Cordyceps militaris waster medium (CM); iii) CMPE, supplemented with 0.5% CM+0.5% Pleurotus eryngii stalk residue (PE); iv) CMPS, supplemented with 0.5% CM+0.5% Pleurotus sajorcaju stalk residue (PS); v) CMFV, supplemented with 0.5% CM+0.5% Fammulina velutipes stalk residue (FV). Results: The chemical analysis results showed that CM extracts, PE extracts, PS extracts, and FV extracts contain functional components such as polysaccharides and phenols and have both 2, 2-diphenyl-1-picryl-hydrazyl-hydrate scavenging and Ferrous scavenging capacities. The group CMWM saw increased body weight gain and feed conversion rate and the promotion of jejunum villus growth, but there is no significant difference in the intestinal bacteria phase. Antioxidant genes in the nuclear factor (erythroid-derived 2)-like 2 (Nrf2)- antioxidant responsive element pathway among the groups are significantly higher than that of the control group, especially in group CMWM. Conclusion: The mushroom stalk residues have antioxidant functional components, can improve the intestinal health and body weight gain of chickens, and can activate the antioxidant pathway of Nrf2 to increase the heme oxygenase-1 expression. The treatment with 1% CM was the most promising as a feed additive.

신증후군 환아에서 1년내 재발과 관련된 위험 요인 (Risk Factors for the First-Year Relapse in Children with Nephrotic Syndrome)

  • 목 적 : 신증후군으로 진단 받은 환아에서 신장 조직 검사 없이 진단 당시 임상 소견 및 혈액과 소변 검사 소견만으로 1년내 재발을 예측할 수 있는 위험요인을 알아보고자 본 연구를 실시하였다. 방 법 : 1997년 7월에서 2002년 6월까지 만 5년간 고려대학교 병원 소아신장크리닉을 내원한 환아 중 스테로이드 치료에 반응을 보였던 신증후군 환아 78명을 대상으로 후향적 연구를 시행하였다. 환아는 두 군으로 분류하였는데 I군은 재발이 없거나 스테로이드 치료하여 반응을 보인 환아에서 첫 1년 내에 재발이 2회 이하로 있는 환아였고 II군은 스테로이드 치료하여 반응을 보인 환아에서 첫 1년 내에 재발이 3회 이상 있는 환아로 하였다. 모든 환아에 대해 각각 성별, 나이, 진단 당시 혈청 알부민, 콜레스테롤, 24시간 요단백, 크레아티닌 청소율, 혈뇨 및 고혈압의 유무, 초기 스테로이드에 반응하여 완해되는데 걸린 기간 등을 비교하였다. 결 과 : 78명의 환아 중 남아는 61명(78.2%), 여아는 17명(21.8%)였고 평균 연령은 $5.1{\pm}3.0$세였으며 47명(60.3%)은 I군에 속했고 31명(39.7%)은 II군에 속했다. 성별, 혈청 알부민, 콜레스테롤, 24시간 요단백, 크레아티닌 청소율과 혈뇨 및 고혈압 유무는 두 군에서 유의한 차이를 보이지 않았다. 두 군의 평균 발병연령은 I군에서 $5.1{\pm}3.1$세, II군에서 $4.9{\pm}2.9$세로 II군의 발병연령이 더 낮은 경향을 보였으나 통계적으로 유의한 차이는 없었고 발병 연령을 3세를 기준으로 구분하였을 때 3세 이하의 환아 중 많은 수가 II군에 속했으며 통계적으로도 유의한 차이를 보였다(P<0.05). 또한 초기 스테로이드에 반응하여 완해가 일어나는 시기를 비교했을 때 I군은 $12.9{\pm}0.5$일, II군은 $16.2{\pm}1.1$일로 II군에서 완해되는 시간이 더 오래 걸린 양상을 보였다(P<0.05). 결 론 : 신증후군으로 진단된 환아에서 초기 진단 당시 연령이 3세 이하로 어리거나 초기 스테로이드에 치료에 반응하여 완해 되는데 걸리는 기간이 길수록 나쁜 예후를 보이는 양상을 보였다. 따라서 추후 신증후군 환아 진단시 예후를 예측하는데 도움이 되리라 생각된다.

웹소켓을 이용한 모바일 앱 테스트 시스템 설계 (A Design of Mobile App Test System Using Web Socket)

  • In this paper, we propose a testing method of the quality of device application effectively reflecting the necessity of increasing mobile device application quality test, and designed and developed it as a web system. We have studied traditional software testing methods for designing and developing a system. We have developed a next-generation alternative technology that can analyze and compensate for the weaknesses of the web and weaknesses of the Android in order to realize the access system and the responsive web system. As an example applied to the system, we analyzed the requirements of the system implementation and expressed the functions of the system through the use case diagram. The test scenarios were conducted by connecting 5,000 devices to each system using the existing HTTP polling method and the proposed web socket method. After setting the comparative group, the result of the proposed web management system was observed and compared through repeated tests. The research shows that the proposed web socket system has better performance than HTTP polling. The more the number of simultaneous processing of the devices, the lower the server stability but the better the time efficiency. In conclusion, the more the number of devices to be tested is, the more the number of devices can be effectively tested using the proposed web management system.

Controlled Release Behavior of pH-Responsive Composite Hydrogel Containing Activated Carbon

  • Activated carbon (AC) is one of the most effective adsorbents for organic compounds because of their extended surface area, high adsorption capacity, microporous structure and special surface reactivity. The composites of pH-sensitive hydrogel and activated carbon were prepared in order to improve the loading capacity of drug. The pH-sensitive hydrogel matrix swelled well in the basic condition to release the drug loaded in AC. The release of drug was controlled depending on both the pH due to the ionization of the carboxylic acid group and the AC due to the surface properties.

육군자탕(六君子湯)이 위액(胃液) 분비(分泌) 및 적출(摘出) 위(胃) 운동(運動)에 미치는 영향(影響)에 관한 연구(硏究) (Effects of Yukgunja-Tang on Secretion of Gastric Juice and Movements of Isolated Stomach)

  • The extracts of Yukgunja-tang which has been used in some of gastroin testinal disorders were examined for secretion of gastric juice and movement of isolated stomach of rabbit. Experiments were performed with the dried extract of Yukgunja-tang (sample I) and the dried extract compound made of each drug of Yukgunja-tang (sample II). The secretion of gastric juice at the doses of 25.0 and 50.0mg/kg p.o. of sample I showed the decreases of 34.3 and 43.3%, respectively, as compared with control group, whereas the secretion at a dose of 76.8g/kg of sample II showed 43.1%. In gastric acid secretion, sample I showed 50.0% decrease at a dose of 50.0mg/kg and sample II showed 46.2% decrease at a dose of 76.8mg/kg. In pepsin output sample I showed 17.8 and 23.4% decreases at a dose of 25.0 and 50.0mg/kg, respectively, whereas sample II showed 22.5% decrease at a dose of 76.8mg/kg. The isolated stomach of rabbit showed dose-responsive contraction by addition of sample I, however the contraction was not observed by sample II.
