급성호흡곤란증후군의 전국 실태조사 보고 (The National Survey of Acute Respiratory Distress Syndrome in Korea)

  • 연구배경 : 급성호흡곤란증후군은 국내에서도 여러 병원에서 적지 않게 경험하고 있으며 관련된 임상 및 기초연구가 지속적으로 보고되고 있으나 그 정의에 따라 발생빈도와 예후가 크게 달라질 수 있어 통일된 정의에 의한 국내 실태조사의 필요성이 점증하게 되었다. 본 학회에서는 1992년 American-European Consensus Conference에서 정한 급성호흡곤란증후군의 정의에 따라 전국적인 전향적 실태조사를 실시하였다. 본 실태조사의 목적은 첫째, 본 증후군의 발생 요인을 분석 하고 둘째, 사망률 및 사망원인을 조사하고 셋째, 예후에 관련된 인자들을 분석하여 향후 급성호흡곤란증후군의 임상 및 연구자료로 활용하고자 함이다. 방법 : 전국에 위치한 대학병원 및 400병상 이상의 종합병원 중 호흡기내과 의사가 있는 총 66개 병원을 대상으로 1995년 8월 l일 부터 1 996년 8월 31일까지 설문지 작성을 의뢰하였다. 본 실태조사에 응답한 24개의 병원의 167예를 분석하여 다음과 같은 결과를 얻었다. 통계분석은 SAS통계 프로그램을 이용하여 사망과 관련된 인자 분석에는 logistic regression법을 그 외는 $x^2$-검정 혹은 t-검정법을 시행하였으며 각 수치는 평균(${\pm}$ 표준편차) 및 위험도(95% 신뢰구간)로 표기하였다. 결과 : l. 환자들의 평균연령은 56.5세(${\pm}$ 17.2세)이었으며 남자 110명(65.9%) 여자 57명(34.1%)이었다. 2. 발생원인은 감염 (78.1%), 흡인(16.6%), 외상(11.6%), 쇽(8.5%) 등이었다. 3. 치료방법으로서 인공호흡기 치료는 95.2%(159/167예 ), 호기말 양압치료는 적용여부의 확인이 가능했던 141예 중 129예(91.5%)에서 시행 되었으며, 스테로이드를 사용한 경우는 22.8%(38/167예)였고 혈역동학적 감시장치로서 SwanGanz도자를 사용한 경우는 9예(5.4%) 이었다. 4. 사망률은 71.9%(120/167예)이었으며 발생 후 사망까지의 기간은 평균 11일(${\pm}$ 13.1일)이었다. 사망원인으로는 호흡부전이 가장 많았으며(52예, 43.7%), 패혈증(43예, 36.1%), 심부전(9예, 76%), 간부전(8예, 6.7%) 등이었다. 5. 연령이 60세 이상일 경우 사망률이 78.7%로서 60세 미만 66.7% 에 비해 사망률이 높은 경향을 보였으나(P=0.08), 성별, 유발질환이 감염성인군과 비감염성인 군들 사이, 염상 경과 중 다장기 발생이 증가된 군과 감소된 군들 사이 및 스테로이드 사용여부는 사망률의 유의한 차이가 없었다. 6. 생존군과 사망군과의 비교에서 유의한 차이를 보인 지표는 맥박 수, 혈소판 수, 알부민 치, 혈당, 24시간 소변량, 동맥혈 pH. $Pa0_2$, $PaCO_2$, $Sa0_2$, 폐포-동맥혈 산소분압 차이, 흡입가스내 산소분율, $PaO_2/FIO_2$, PEEP/$FI0_2$ 이었다. 7. 사망과 관련된 인자들로는 본 증후군 발생시 호흡부전외 다른 동반질환이 있는 경우가 없는 경우에 비하여 사망위험도가 증가하는 경향을 보였고(odd ratio 2.69; 0.88-8.22, P=0.08) 나이와 성별로 보정한 후에는 그 사망위험도가 4.30배(1.20-15.39, P<0.05)로 유의하게 증가되었다. 또한 다른 장기의 부전이 없는 경우에 비하여 동반된 타장기부전의 장기수가 l개인 경우는 사망위험도가 2.59배(1.13-5.97, P<0.05), 2개 이상인 경우는 3.89배(1.08-14.03, P<0.05)로서 타장기부전 수가 많을수록 사망위험도가 증가하였 다($x^2$=7.34, P<0.01,). 또한 발생시점의 APACHE III 점수가 높을수록 사망위험도가 증가하였는데($x^2$=9.12, P<0.01) 100점 이상일 경우 50점 미만인 경우보다 6.67배(1.39-32.08, P<0.05) 더 높았다. 8. 성별, 연령(60세 미만과 이상), 다장기부전 수 및 APACHE III 로 다변수 분석을 시행한 결과 다장기부전 수(odd ratio 1.95, 95% 신뢰구간:1.05-3.61, P=0.03) 및 APACHE III(odd ratio 1.59, 95% 신뢰구간: 1.01-2.50, P=0.04)가 독립된 위험 인자로 나타났다. 결론 : 급성호흡곤란증후군의 국내 사망률은 71.9%로 아직도 높은 치명율을 보이고 있으며, ARDS 진단 시점에서 예후에 영향을 미치는 유의한 인자는 APACHE III값, 부전 장기의 수, 동반 질환의 유무 등으로 나타났다.

The Association between Mortality and the Oxygen Saturation and Fraction of Inhaled Oxygen in Patients Requiring Oxygen Therapy due to COVID-19-Associated Pneumonia

  • Background: The coronavirus disease (COVID-19) can manifest in a range of symptoms, including both asymptomatic systems which appear nearly non-existent to the patient, all the way to the development of acute respiratory distress syndrome (ARDS). Specifically, COVID-19-associated pneumonia develops into ARDS due to the rapid progression of hypoxia, and although arterial blood gas analysis can assist in halting this deterioration, the current environment provided by the COVID-19 pandemic, which has led to an overall lack of medical resources or equipment, has made it difficult to administer such tests in a widespread manner. As a result, this study was conducted in order to determine whether the levels of oxygen saturation (SpO2) and the fraction of inhaled oxygen (FiO2) (SF ratio) can also serve as predictors of ARDS and the patient's risk of mortality. Methods: This was a retrospective cohort study conducted from February 2020 to Mary 2020, with the study's subjects consisting of COVID-19 pneumonia patients who had reached a state of deterioration that required the use of oxygen therapy. Of the 100 COVID-19 pneumonia cases, we compared 59 pneumonia patients who required oxygen therapy, divided into ARDS and non-ARDS pneumonia patients who required oxygen, and then investigated the different factors which affected their mortality. Results: At the time of admission, the ratios of SpO2, FiO2, and SF for the ARDS group differed significantly from those of the non-ARDS pneumonia support group who required oxygen (p<0.001). With respect to the predicting of the occurrence of ARDS, the SF ratio on admission and the SF ratio at exacerbation had an area under the curve which measured to be around 85.7% and 88.8% (p<0.001). Multivariate Cox regression analysis identified that the SF ratio at exacerbation (hazard ratio [HR], 0.916; 95% confidence interval [CI], 0.846-0.991; p=0.029) and National Early Warning Score (NEWS) (HR, 1.277; 95% CI, 1.010-1.615; p=0.041) were significant predictors of mortality. Conclusion: The SF ratio on admission and the SF ratio at exacerbation were strong predictors of the occurrence of ARDS, and the SF ratio at exacerbation and NEWS held a significant effect on mortality.

Sepsis: Early Recognition and Optimized Treatment

  • Sepsis is a life-threatening condition caused by infection and represents a substantial global health burden. Recent epidemiological studies showed that sepsis mortality rates have decreased, but that the incidence has continued to increase. Although a mortality benefit from early-goal directed therapy (EGDT) in patients with severe sepsis or septic shock was reported in 2001, three subsequent multicenter randomized studies showed no benefits of EGDT versus usual care. Nonetheless, the early administration of antibiotics and intravenous fluids is considered crucial for the treatment of sepsis. In 2016, new sepsis definitions (Sepsis-3) were issued, in which organ failure was emphasized and use of the terms "systemic inflammatory response syndrome" and "severe sepsis" was discouraged. However, early detection of sepsis with timely, appropriate interventions increases the likelihood of survival for patients with sepsis. Also, performance improvement programs have been associated with a significant increase in compliance with the sepsis bundles and a reduction in mortality. To improve sepsis management and reduce its burden, in 2017, the World Health Assembly and World Health Organization adopted a resolution that urged governments and healthcare workers to implement appropriate measures to address sepsis. Sepsis should be considered a medical emergency, and increasing the level of awareness of sepsis is essential.

한 대학병원에서 급성 폐색전증으로 진단된 환자들의 임상적 특성 및 예후 (Acute Pulmonary Embolism: Clinical Characteristics and Outcomes in a University Teaching Hospital)

  • Background: Pulmonary embolism (PE) is a common clinical problem in the West that is associated with substantial morbidity and mortality. The diagnostic modality has been changed since 2001. This study retrospectively reviewed the PE mortality with the aim of identifying the risk factors associated with mortality since the multidetector computed tomography (MDCT) was introduced. Methods: We analyzed 105 patients with acute PE proven by multidetector CT or ventilation perfusion scan. The primary outcome measure was the all-cause mortality at 3 months. The prognostic effect of the baseline factors on survival was assessed by multivariate analysis. Results: The main risk factors were prolonged immobilization, stroke, cancer and obesity. Forty nine percent of patients had 3 or more risk factors. The overall mortality at 3 months was 18.1%. Multivariate analysis revealed low diastolic blood pressure and the existence of cancer to be independent factors significantly associated with mortality. Forty two PE patients were examined for the coagulation inhibitors. Four of these patients had a protein C deficiency (9.5%), and 11 had a protein S deficiency (26%). Conclusion: PE is an important clinical problem with a high mortality rate. Close monitoring may be necessary in patients with the risk factors.

Healthcare-Associated Pneumonia among Hospitalized Patients: Is It Different from Community Acquired Pneumonia?

  • Background: The increasing number of outpatients with multidrug-resistant (MDR) pathogens has led to a new category of pneumonia, termed healthcare-associated pneumonia (HCAP). We determined the differences in etiology and outcomes between patients with HCAP and those with community-acquired pneumonia (CAP) to clarify the risk factors for HCAP mortality. Methods: A retrospective study comparing patients with HCAP and CAP at Jeju National University Hospital. The primary outcome was 30-day mortality. Results: A total of 483 patients (208 patients HCAP, 275 patients with CAP) were evaluated. Patients with HCAP were older than those with CAP (median, 74 years; interquartile range [IQR], 65-81 vs. median, 69 years; IQR, 52-78; p<0.0001). Streptococcus pneumoniae was the major pathogen in both groups, and MDR pathogens were isolated more frequently from patients with HCAP than with CAP (18.8% vs. 4.9%, p<0.0001). Initial pneumonia severity was greater in patients with HCAP than with CAP. The total 30-day mortality rate was 9.9% and was higher in patients with HCAP based on univariate analysis (16.3% vs. 5.1%; odds ratio (OR), 3.64; 95% confidence interval (CI), 1.90-6.99; p<0.0001). After adjusting for age, sex, comorbidities, and initial severity, the association between HCAP and 30-day mortality became non-significant (OR, 1.98; 95% CI, 0.94-4.18; p=0.167). Conclusion: HCAP was a common cause of hospital admissions and was associated with a high mortality rate. This increased mortality was related primarily to age and initial clinical vital signs, rather than combination antibiotic therapy or type of pneumonia.

  • 연구배경: B-type natriuretic peptide(BNP)는 심인성 및 다른 쇼크 상태를 포함하는 심혈관 질환에서 사망을 예측하는 좋은 인자 중의 하나로 알려져 있다. 그러나, 급성호흡곤란증후군환자에서 이런 관계가 잘 알려져 있지 않는 바, 저자들은 BNP가 급성호흡곤란증후군 환자에서 사망을 예측할 수 있는 지를 연구하였다. 방 법: 본 연구는 전향적 관찰로 시행되었다. 급성호흡곤란증후군으로 진단된 환자들에게 심초음파 검사를 시행한 후, 좌심실 구획률이 50% 미만이거나 확장성 심부전 양상을 보인 환자들을 제외하였다. 2003년 12월부터 2006년 2월까지 총 47명의 환자가 채택되었다. 등록된 환자들은 24시간 내로 BNP를 포함한 여러 검사실 수치를 얻었으며, APACHE(Acute Physiology and Chronic Health Evaluation) II 점수를 구하였다. 결 과: BNP와 APACHE II 점수 평균값은 생존자군과 사망자군에서 유의한 차의를 보였다(BNP: $219.5{\pm}57.7pg/mL$ vs $492.3{\pm}88.8pg/mL$; p=0.013, APACHE II 점수: $17.4{\pm}1.6$ vs $23.1{\pm}1.3$, p=0.009). BNP 는 혈중 크레아티닌 수치와 양의 상관관계를 보였으나(r=0.374, p=0.01), 좌심실 구획률과는 유의한 관계가 없었다. Receiver operating characteristic 곡선상, BNP 수치를 585 pg/mL로 잡았을 때 사망을 예측하는데 있어서 94%의 특이도를 보였으며, APACHE II 점수의 경우에는 15.5를 기준으로 하였을 때 87%의 민감도를 보였다. 이 두 요소를 결합하여 '아파치II 점수+$11{\times}logBNP$' 수치를 계산하여 기준점을 46.14로 했을 경우, 사망 예측에 있어서 민감도 63%, 특이도 82%의 결과를 얻을 수 있었다. 결 론: 좌심실 기능 부전을 보이지 않는 급성호흡곤란증후군 환자에서 BNP 수치는 생존자군과 사망자군에서 유의한 차이를 보였으며 사망을 예측할 수 있었다. 향후 급성호흡곤란증후군 환자에 있어서 BNP와 관련된 연구가 더 필요하다고 생각된다.

Humidifier Disinfectant-Associated Lung Injury: Six Years after the Tragic Event

  • In 2011, a cluster of peripartum patients were admitted to the intensive care unit of a tertiary hospital in Seoul with signs and symptoms of severe respiratory distress of unknown etiology. Subsequent epidemiological and animal studies suggested that humidifier disinfectant (HD) might represent the source of this pathology. Epidemiological studies, animal studies, and dose-response analysis demonstrated a strong association between HD use and lung injuries. The diagnostic criteria for HD-associated lung injury (HDALI) was defined on the basis of the clinical, pathological, and radiological attributes of the patients. The clinical spectrum of HDALI appears to range from asymptomatic to full-blown acute respiratory failure, and some patients have required actual lung transplantation for survival. The overall mortality of the exposed population was not significant, although peripartum patients and children who were admitted to the intensive care unit did show high mortality rates. Persistent clinical findings such as diffuse ill-defined centrilobular nodules and restrictive lung dysfunction were observed in some of the survivors. The findings of this review emphasize the importance of assessment of the level of toxicity of chemical inhalants utilized in a home setting, as well as the need to identify and monitor afflicted individuals after inhalational injury.

  • Care of patients with sepsis has improved over the last decade. However, in the recent two years, there was no significant progress in the development of a new drug for critically ill patients. In January 2011, it was announced that the worldwide phase 3 randomized trial of a novel anti-Toll-like receptor-4 compound, eritoran tetrasodium, had failed to demonstrate an improvement in the mortality of patients with severe sepsis. In October 2011, Xigris (drotrecogin alfa, a recombinant activated protein C) was withdrawn from the market following the failure of its worldwide trial that had attempted to demonstrate improved outcome. These announcements were disappointing. The recent failure of 2 promising drugs to further reduce mortality suggests that new approaches are needed. A study was published showing that sepsis can be associated to a state of immunosuppression and loss of immune function in human. However, the timing, incidence, and nature of the immunosuppression remain poorly characterized, especially in humans. This emphasizes the need for a better understanding of sepsis as well as new therapeutic strategies. Many clinical experiences of the extracorporeal membrane oxygenator (ECMO) treatment for adult acute respiratory distress syndrome (ARDS) patients, which is caused by the H1N1 influenza A virus, were reported. The use of ECMO in severe respiratory failure, particularly in the treatment of adult ARDS, is occurring more commonly.

Respiratory Severity Score as a Predictive Factor for the Mortality of Congenital Diaphragmatic Hernia

  • Purpose: Congenital diaphragmatic hernia (CDH) is rare but potentially fatal. The overall outcome is highly variable. This study aimed to identify a simple and dynamic parameter that helps predict the mortality of CDH patients in real time, without invasive tests. Methods: We conducted a retrospective chart review of 59 CDH cases. Maternal and fetal information included the gestational age at diagnosis, site of defect, presence of liver herniation, and lung-to-head ratio (LHR) at 20 to 29 weeks of gestational age. Information regarding postnatal treatment, including the number of days until surgery, the need for inhaled nitric oxide (iNO), the need for extracorporeal membrane oxygenation (ECMO), and survival, was collected. The highest respiratory severity score (RSS) within 24 hours after birth was also calculated. Results: Statistical analysis showed that a younger gestational age at the initial diagnosis (P<0.001), a lower LHR (P=0.001), and the presence of liver herniation (P=0.003) were prenatal risk factors for CDH mortality. The RSS and use of iNO and ECMO were significant factors affecting survival. In the multivariate analysis, the only remaining significant risk factor was the highest preoperative RSS within 24 hours after birth (P=0.002). The area under the receiver operating characteristic curve was 0.9375, with a sensitivity of 91.67% and specificity of 83.87% at the RSS cut-off value of 5.2. The positive and negative predictive values were 82.14% and 92.86%, respectively. Conclusion: Using the RSS as a prognostic predictor with simple calculations will help clinicians plan CDH management.

인공호흡기연관 폐렴 (Ventilator-Associated Pneumonia)

  • Ventilator-associated pneumonia (VAP) is the most frequent nosocomial infection in the intensive care unit (ICU), with an incidence ranging from 8% to 38%. Patients who acquire VAP have higher mortality rates and longer ICU and hospital stays. Because there are other potential causes of fever, leukocytosis, and pulmonary infiltrates, clinical diagnosis of VAP is overly sensitive. The only alternative approach to the clinical diagnosis of VAP is the Clinical Pulmonary Infection Score (CPIS). Employing quantitative cultures of respiratory secretions in the diagnosis of VAP leads to less antibiotic use and probably to lower mortality. With respect to microbiologic diagnosis, however, it is not clear that the use of invasive sampling using bronchoscopy is associated with better outcomes. Delayed administration of antibiotic therapy is associated with an increased mortality, and inadequate antibiotic therapy is also associated with higher mortality. Therefore, prompt initiation of adequate antibiotic therapy is a cornerstone of the treatment of VAP. The initial antibiotic therapy should be based on the most common organisms in each hospital and the most likely pathogens for that specific patient. When final cultures and susceptibilities are available, de-escalation to less broad spectrum antibiotics should be done. Since clinical improvement usually takes 2 to 3 days, clinical responses to the initial empirical therapy should be evaluated by day 3. A short course of antibiotic therapy appears to be equivalent to a traditional course of more than 14 days, except when treating non-fermenting gram-negative organisms. If patients receive initially adequate antibiotic therapy, efforts should be made to shorten the duration of therapy to as short as 7 days, provided that the etiologic pathogen is not a non-fermenting gram-negative organism.
