Quantitative PCR for Etiologic Diagnosis of Methicillin-Resistant Staphylococcus aureus Pneumonia in Intensive Care Unit

  • Background: Ventilator-associated pneumonia (VAP) requires prompt and appropriate treatment. Since methicillin-resistant Staphylococcus aureus (MRSA) is a frequent pathogen in VAP, rapid identification of it, is pivotal. Our aim was to evaluate the utility of quantitative polymerase chain reaction (qPCR) as a useful method for etiologic diagnoses of MRSA pneumonia. Methods: We performed qPCR for mecA, S. aureus-specific femA-SA, and S. epidermidis-specific femA-SE genes from bronchoalveolar lavage or bronchial washing samples obtained from clinically-suspected VAP. Molecular identification of MRSA was based on the presence of the mecA and femA-SA gene, with the absence of the femA-SE gene. To compensate for the experimental and clinical conditions, we spiked an internal control in the course of DNA extraction. We estimated number of colony-forming units per mL (CFU/mL) of MRSA samples through a standard curve of a serially-diluted reference MRSA strain. We compared the threshold cycle (Ct) value with the microbiologic results of MRSA. Results: We obtained the mecA gene standard curve, which showed the detection limit of the mecA gene to be 100 fg, which corresponds to a copy number of 30. We chose cut-off Ct values of 27.94 (equivalent to $1{\times}10^4$ CFU/mL) and 21.78 (equivalent to $1{\times}10^5$ CFU/mL). The sensitivity and specificity of our assay were 88.9% and 88.9% respectively, when compared with quantitative cultures. Conclusion: Our results were valuable for diagnosing and identifying pathogens involved in VAP. We believe our modified qPCR is an appropriate tool for the rapid diagnosis of clinical pathogens regarding patients in the intensive care unit.

해수 및 담수사육 감성돔, Acanthopagrus schlegeli의 호흡측정실내 수용개체수에 따른 산소소비 비교 (Effect of Fish Number in Respiratory Chamber on Routine Oxygen Consumption of Black Porgy Acanthopagrus schlegeli Reared in Seawater or Freshwater)

  • 밀폐순환 유수식 산소소비 측정장치를 이용하여 해수 및 담수에서 사육한 감성돔, Acanthopagrus schlegeli의 개체수에 따른 산소소비 경향을 조사하였다. 해수 및 담수순화된 평균체중 36.4 g의 감성돔을 각각 1마리와 3마리 그룹으로 나누어 수온(15, 20, $25^{\circ}C$), 광주기(12L:12D)를 달리하여 실험하였다. 모든 실험조건에서 감성돔은 명기동안에 산소소비량이 많고, 암기동안에는 적은 뚜렷한 리듬을 보였으며, 수온상승과 비례하여 산소소비량이 증가하였다. 감성돔의 호흡수는 담수돔 보다 해수돔이 많았으며, 수온상승과 비례하여 증가하였다. 수온 15, 20, $25^{\circ}C$에서 개체수가 1마리일 때 해수돔의 산소소비량은 각각 140.0, 174.8, 282.3 mg $O^2{\cdot}kg^{-1}{\cdot}h^{-1}$, 담수돔은 177.5, 307.8, 413.1 mg $O^2{\cdot}kg^{-1}{\cdot}h^{-1}$로 해수돔에 비해 담수돔의 산소소비량이 많았다. 그러나 개체수가 3마리일 때는 해수돔 각각 200.5, 274.7, 339.1 mg $O^2{\cdot}kg^{-1}{\cdot}h^{-1}$, 담수돔 177.5, 307.8, 413.1 mg $O^2{\cdot}kg^{-1}{\cdot}h^{-1}$로 담수돔 보다 해수돔의 산소소비량이 많았다.

소세포 폐암에서 S-Phase Fraction의 임상적 의의 (Clinical Significance of S-Phase Fraction in Small Cell Lung Cancer)

  • 연구배경: 종양세포는 세포의 분열성장이 증가되므로, 세포분열주기 중 S-phase fraction(SPF)의 변화를 생각할 수 있다. 소세포폐암 환자에서 종양세포의 생물학적 특성인 SPF의 변화와 생존기간, 그리고 항암화학요법에 대한 반응의 정도를 비교하였다. 방법: 1990년 1월부터 1991년 12월까지 원광의대 부속병원에서 원발성 소세포폐암으로 진단받고나서, 2회 이상의 화학요법을 실시받은 후, 최소 2년 이상의 후향적 추적에 의해 사망이 확인된 42예를 대상으로 하였다. SPF 분석방법은 paraffin에 보관된 병리조직을 처리하여, 유식세포분석법에 의한 DNA histogram으로 분석 하였다. 결과: 1) 대상군의 평균 생존기간은 190(${\pm}156$)일이었고, TNM 병기, PS scale 이 진행할 수록 생존기간은 유의하게 단축되었다. 2) 대상군의 평균 SPF는 27.4(${\pm}8.5$)% 였으며, TNM 병기, PS scale의 진행에 따른 차이는 없었다. 3) 암세포의 SPF와 생존기간과의 관계는 발견되지 않았다. 4) 암세포의 SPF가 높을 수록 화학요법에 대한 반응의 정도는 양호하였다. 결론: 소세포폐암 환자에서 암세포의 SPF와 생존기간과는 관계가 없었고, 암세포의 SPF가 높을 수록, 화학요법에 대한 반응의 정도는 양호하였다. SPF는 항암화학요법 반응의 예측인자로서의 의의가 있다고 판단된다.

인체 각종 암조직에 있어서 젖산 각탄소의 산화과정 (Oxidation of Each Carbon of Lactate in Various Cancer Tissues of Human)

  • Tissue homogenates of 10 kinds of human cancer tissues were incubated in medium containing either one of $C^{14}-1,\; C^{14}-2,\;or\; C^{14}-3-lactate $ as a substrate in order to observe the oxidative pathway of lactate in cancer tissues. Lactate concentration in incubation medium was maintained at 50 mg%. At the end of incubation period, gas samples and incubation media were analyzed for total $CO_2$ production rates, radioactivities of respiratory $CO_2$, lactate uptake rates and pyruvate appearance rates. The following results were obtained. 1. Lactate uptake rates in all of cancer tissues examined were less than $2.5\;{\mu}M/hr/gm$ and much lower than those in normal tissues. 2. In the 10 kind of human cancer tissues, total $CO_2$ production rates were less than $10\;{\mu}M/hr/gm$, in all cases. These lower values impressed that oxidative metabolism in tumor tissues generally inhibited as compared with that in normal tissue. On the other hand, fractions of $CO_2$ derived from lactate to total $CO_2$ production rates were less than 15% except one case These facts showed that oxidation of lactate into $CO_2$ was greatly inhibited in tumor tissues. 3. Respiratory $CO_2$ yields from C-1 carbon of lactate in various cancer tissues were mean of 77.7% of total $CO_2$ yield from lactate and $CO_2$ yields from C-2 and C-3 carbon of lactate were mean of 9.1% and 12.6% respectively. These facts showed that carboxyl carbon of lactate oxidized more easily than ${\alpha}\;and\;{\beta}$ carbon of lactate. 4. In 10 kinds of cancer tissues, fractions of disappeared lacteate from media into $CO_2$ and pyruvate, which expressed as RLD $co_2$ and RLDpy respectively, were about 5% in except 3 cases and less than 3% except one case. These fact showed that almost of disappeared lactate from media were degraded into compounds other than $CO_2$ and pyruvate. From the above date, it was suggested that in the oxidative pathway of lactate in cancer tissues $CO_2$ was easily Produced from carboxyl carbon of lactate by oxidative decarboxylation as in the normal tissue, and further oxidation of 2 carbon unit via TCA cycle was inhibited.

Chlortetracycline(CTC)의 사료 첨가가 돼지의 생산성과 호흡기 병변에 미치는 영향 (Effect of dietary chlortetracycline on productivity and respiratory disease in pigs)

  • The effects on CTC (T1) and CTC, Sulfathiazole and Penicillin Combination (T2) medication in feed through one life cycle of pigs, namely, weaning, mating, farrowing, lactation, growing, finishing and slaughter, were tested under local condition. In sow phase, productivity and the number of microflora in urine before and after medication of CTC were studied and average daily gain and feed conversion rate were checked during growing and finishing period. All pigs reached at 155 days old were slaughtered for pathological examination. 1. Litter size, farrowing rate and survival rate at birth were improved by CTC medication from weaning to 21th day after mating and mortality of piglet at weaning, 25 days after farrowing, was reduced in the CTC medication group, but no siginificant. 2. The number of microflora in the sow urine was changed with the medication at 200ppm of CTC in feed. In particular, the number of E coli, Samonella and Staphylococci were reduced by CTC medication. 3. The average daily gain and feed conversion rate of grower and finisher pigs was improved significantly in both treated groups, most in the high level CTC (T1) medicated group and was lowest in the control group. 4. The number of infected lungs was reduced not significant by both treatments (as % pneumonic lesions Co 66.7%, T1 47.1%, T2 31.4%) and the severity of lung lesions was significantly reduced by both high level of CTC and CTC combination medication in feed. 5. Although there were no statistical differences in atrophic rhinitis based on turbinate scores among the 3 groups, the number of mild and moderate (Grade 2 and 3) infections was higher in the control group (9/36) than in the treated groups (T1 2/34 & T2 4/35).

제한병기 소세포폐암에서 흉부 방사선 치료의 도입 시기에 따른 치료 효과의 비교 분석 (The effects according to the timing of thoracic radiotherapy in limited stage small cell lung cancer)

  • Background: Combination chemotherapy is now considered to be the cornerstone of small cell lung cancer (SCLC). management but the optimal management of limited SCLC is not well defined. The role of thoracic radiotherapy (TRT) is less well established. Recent meta-analyses reports revealed that TRT combined with chemotherapy produce "good" local control and prolonged survival. But other reports that survival was not changed. The liming, dose, volume and fractionation for TRT with the combined chemotherapy of SCLC remains unsettled. In this study, we analyzed the effects according to the timing of thoracic radiotherapy in limited SCLC. Method: All fifty one patients received cytoxan, adriamycin and vincristine(CAV) alternating with etoposide and cisplatin(VPP) every 3 weeks for 6 cycles were randomized prospectively into two groups: concurrent and sequential. 27 patients received 4500cGy in 30 fractions(twice daily 150cGy fractional dose) over 3 weeks 10 the primary site concurrent with the first cycle of VPP(concurrent gorup). 24 patients received 4000 to 5000cGy over 5 or 6 weeks after completion of sixth cycles of chemotherapy(sequential group). Results: 1. Response rates and response duration : Response rates were not significantly different between two groups(p=0.13). But response duration was superior in the concurrent group(p=0.03). 2. Survival duration was nor different between two groups(p=0.33). 3. Local control rate was superior in the concurrent group(p=0.00). 4. Side effects and toxicities: Hematologic toxicities, especially leukopenia, infection and frequency of radiation esophagitis were higher in the concurrent group (p=0.00, 0.03, 0.03). Conclusion: The concurrent use of TRT with chemotherapy failed to improve the survival of limited stage SCLC patients compared with the sequential use of TRT but response duration and local control rate were superior in the concurrent group. Frequency of radiation esophagitis, life threatening hematologic toxicities and infection were more frequent in the concurrent group than sequential group. So, the selection of an optimal schedule of chemotherapy combined with TRT that would lead to a major increase in survival with minimal toxicity is remained to be validated in large scale study in the future.

  • 항이뇨호르몬분비이상증후군(이하 SIADH)은 암환자의 항암화학치료 시 드물게 관찰된다. 저자 등은 선암으로 진단받고, 좌폐하엽 절제술 시행 후 보조항암화학치료 중인 51세 여자 환자에서 발작을 동반한 SIADH 1예를 문헌고찰과 함께 보고하는 바이다.

Quantification of Serum Free RNA as a Predictive Biomarker for the Response to Chemotherapy in Patients with Lung Cancer: A Pilot Study

  • Background: It is well-known that cell-free nucleic acids rise in patients with many types of malignancies. Several recent experimental studies using cancer cell lines have shown that changes in cell-free RNA are predictive of the response to chemotherapy. The objective of this study was to determine whether quantification of free RNA can be used as a biomarker for clinical responses to chemotherapy in patients with lung cancer. Methods: Thirty-two patients with lung cancer (non-small cell lung cancer, n=24; small cell lung cancer, n=8) were divided into 2 groups according to their responses to chemotherapy (response group, n=19; non-response group, n=13). Blood samples were collected before and after two cycles of chemotherapy. Real-time quantitative RT-PCR was used for transcript quantification of the glyceraldehyde-3-phosphate dehydrogenase gene. Results: The pre chemotherapy values (Response group $41.36{\pm}1.72$ vs. Non-response group $41.33{\pm}1.54$, p=0.78) and post chemotherapy values (Response group $39.92{\pm}1.81$ vs. Non-response group $40.41{\pm}1.47$, p=0.40) for cell free RNA concentrations, expressed as Ct GAPDH (threshold cycle glyceraldehyde-3-phosphate dehydrogenase gene) levels, was not different between the two groups. There was no significant relationship between changes in the cell free RNA level clinical responses after chemotherapy (p=0.43). Conclusion: We did not find a correlation between quantification of serum cell free RNA levels and clinical responses to chemotherapy in patients with lung cancer. Further investigations are needed to determine whether the cell free RNA level is a useful predictor of responses to chemotherapy in patients with lung cancer.

소백산국립공원 산림생태계의 토양미생물호흡량 평가 (Estimation of Soil Microbiological Respiration Volume in Forest Ecosystem in the Sobaeksan National Park of Korea)

  • The purpose of this study is to estimate carbon dioxide emissions from soil microbial respiration by forest type of Sobaeksan National Park. As a result of estimating the annual soil microbiological respiration volume by forest type in Sobaeksan National Park, broad-leaved forests, coniferous forest, artificial forests were similar to around 19.5 CO2-ton/ha/yr. In the case of coniferous forests in sub-alpine and grassland near Birobong Peak, 12.2 CO2-ton/ha/yr and 8.1 CO2-ton/ha/yr, respectively, were lower than general forest areas. And as a result of analyzing the changes in soil microbiological respiration rate according to forest type in Sobaeksan National Park, the soil microbiological respiration rate in coniferous forests, broad-leaved forests, artificial forests, and sub-alpine areas was the highest in the July survey in summer and the lowest in November in late autumn. The change in soil microbial respiratory volume according to the measurement time in Sobaeksan National Park was the highest between 12:00 and 16:00, when the soil temperature was generally the highest among the days. It is known that the soil temperature is relatively low and the amount of soil microbial respiration decreases during winter, and the change in respiratory volume over the measurement time during the day was the smallest in November, when the amount of soil microbial respiration was relatively smaller than the May-September survey. However, this study has limitations in revealing the causal relationship of various environmental factors that affect the soil microbial respiration. Therefore, it is suggested that long-term research and investigation of various factors affecting soil respiration are needed to understand the carbon cycle of forest ecosystems.

예측 불가능한 호흡 변화에 따른 사이버나이프 종양 추적 방사선 치료의 정확도 분석 (An accuracy analysis of Cyberknife tumor tracking radiotherapy according to unpredictable change of respiration)

  • 목 적 : 사이버나이프 종양 추적 시스템(Cyber-knife tumor tracking system)은 환자 외부에 부착한 LED marker에서 얻어진 실시간 호흡 주기 신호와 호흡에 따라 움직이는 종양의 위치와의 상관관계를 바탕으로 종양의 위치를 미리 예측하고 종양의 움직임을 치료기와 동기화 (Synchronize) 시켜 실시간으로 종양을 추적하며 치료하는 시스템이다. 본 연구의 목적은 사이버나이프 종양 추적 방사선 치료 중 기침이나 수면 등으로 인해 예측 불가능한 갑작스러운 호흡 형태 변화에 따른 종양 추적 방사선 치료 시스템의 정확도를 평가하고자 한다. 대상 및 방법 : 연구에 사용된 호흡 Log 파일은 본원에서 호흡 동조 방사선치료(Respiratory gating radiotherapy)나 사이버나이프 호흡 추적 방사선수술(Cyber-knife tracking radiosurgery)을 받았던 환자의 호흡 Log 파일을 바탕으로, 정현곡선 형태(Sinusoidal pattern)와 갑작스런 변화 형태(Sudden change pattern)의 Log 파일을 이용하여 측정이 가능하도록 재구성하였다. 재구성 된 호흡 Log 파일을 사이버나이프 동적 흉부 팬텀에 입력하여 호흡에 따른 움직임을 구현할 수 있도록 기존 동적 흉부 팬텀의 구동장치를 추가 제작하였고, 호흡의 형태를 팬텀에 적용 시킬 수 있는 프로그램을 개발하였다. 팬텀 내부 표적(Ball cube target)의 움직임은 호흡의 크기에 따라 상하(Superior-Inferior)방향으로 5 mm, 10 mm, 20 mm 3가지 크기의 변위로 구동하게 하였다. 팬텀 내부 표적에 EBT3 필름 2장을 교차 삽입하여 표적 움직임의 변화에 따라 사이버나이프 제조사에서 제공된 End-to-End(E2E) test를 호흡의 형태에 따라 각각 5회씩 실시하고 측정하였다. 종양 추적 시스템의 정확도는 삽입된 필름을 분석하여 표적 오차(Targeting error)로 나타내었고, 추가로 E2E test가 진행되는 동안 상관관계 오차(Correlation error)를 측정하여 분석하였다. 결 과 : 표적 오차는 정현곡선 호흡 형태일 경우 표적 움직임의 크기가 5 mm, 10 mm, 20 mm 에 따라 각각 평균 $1.14{\pm}0.13mm$, $1.05{\pm}0.20mm$, $2.37{\pm}0.17mm$이고, 갑작스런 호흡 변화 형태일 경우 각각 평균 $1.87{\pm}0.19mm$, $2.15{\pm}0.21mm$, $2.44{\pm}0.26mm$으로 분석되었다. 표적 추적에 있어 변위 벡터의 길이로 정의할 수 있는 상관관계 오차는 정현곡선 호흡 형태일 경우 표적 움직임의 크기가 5 mm, 10 mm, 20 mm 에 따라 각각 평균 $0.84{\pm}0.01mm$, $0.70{\pm}0.13mm$, $1.63{\pm}0.10mm$이고, 갑작스런 호흡 변화 형태일 경우 각각 평균 $0.97{\pm}0.06mm$, $1.44{\pm}0.11mm$, $1.98{\pm}0.10mm$으로 분석되었다. 두 호흡 형태에서 모두 상관관계 오차 값이 클수록 표적 오차 값이 크게 나타났다. 정현곡선 호흡 형태의 표적 움직임 크기가 20 mm 이상일 경우, 두 오차 값 모두 사이버나이프 제조사의 권고치인 1.5 mm 이상으로 측정되었다. 결 론 : 표적 움직임의 크기가 클수록 표적 오차 값과 상관관계 오차 값이 증가하는 경향이 있었으며, 정현곡선 호흡 형태보다 갑작스런 호흡 변화 형태에서 오차 값이 크게 나타났다. 호흡의 형태가 규칙적인 정현 곡선 형태더라도 표적의 움직임이 클수록 종양 추적 시스템의 정확도가 감소하는 것으로 판단할 수 있다. 사이버나이프 종양 추적 시스템의 알고리즘을 이용하여 치료 시행 시 환자의 기침 등으로 인하여 갑작스럽게 예측 불가능한 호흡 변화가 있는 경우 치료를 멈추고 내부 표적 확인 과정을 재실시 하여야 하며 호흡 형태를 재조정해야 할 필요가 있다. 치료 중 환자가 본인의 호흡 형태를 관찰 할 수 있는 고글 모니터 등을 착용하여 규칙적인 호흡 형태를 유도하는 것이 치료의 정확도는 향상될 수 있다고 판단된다.
