The Tolosa-Hunt syndrome(THS) consists of a painful ophthalmoplegia related to a granulomatous inflammatory process in the cavernous sinus, which may be documented by cerebral magnetic resonance imaging with gadolinium enhancement. Generally, it has been reported that 2 weeks steroid therapy relieves the symptoms(periorbital pain, ophthalmoplegia) of THS. In this case, there had been no improvement of symptoms by western medical treatments(steroid & analgesics). but, the symptoms were resolved clearly by acupuncture therapy during 6 weeks(5 times/week). If more clinical trials like this are proved to be effective in THS, we can expect that acupuncture will be another therapeutical modality in THS.
Gel dosimeters have unique advantages in comparison with other dosimeters. Until now, these gels have been used in different radiotherapy techniques as a reliable dosimetric tool. Because dose distribution measurement is an important factor for appropriate treatment planning in different radiotherapy techniques, in this study, we evaluated the ability of the N-isopropylacrylamide (NIPAM) polymer gel to record the dose distribution resulting from the mixed neutron-gamma field of boron neutron capture therapy (BNCT). In this regard, a head phantom containing NIPAM gel was irradiated using the Tehran Research Reactor BNCT beam line, and then by a magnetic resonance scanner. Eventually, the $R_2$ maps were obtained in different slices of the phantom by analyzing T2-weighted images. The results show that NIPAM gel has a suitable potential for recording three-dimensional dose distribution in mixed neutron-gamma field dosimetry.
A 7-year-old, intact, female Siberian husky presented with vomiting and diarrhea after a fight with a dog with which it lived. The bitch was diagnosed with pyometra and severe dehydration. The patient received IV fluid therapy for 2 day pre- and post-operatively. Four days post-surgery, the patient had a decreased level of consciousness and suffered a tonic-clonic generalized seizure. On magnetic resonance imaging and histopathological findings, a diagnosis of laminar cortical necrosis caused by fluid overload was made. This case provides important information on the potential for fluid overload in a dehydrated patient giving rise to critical condition and death.
Intracranial chloroma may occur in leukemia, although they are rare. A 23-year-old female complained diplopia. Brain magnetic resonance MR imaging showed tumors in the both cavernous sinus, both tentorial and anterior falx. Gamma-Knife radiosurgery was performed with maximal dose; 20Gy, marginal dose; 10Gy. Peripheral blood smear revealed leukemia, and bone marrow aspiration biopsy showed acute lymphocytic leukemia. Two weeks later, MR image for the stereotactic biopsy noticed markedly decreased tumor size. Biopsy result was lymphocytic leukemia. She received conventional radiation therapy, chemotherapy, and bone marrow transplantation. Brain involvement by acute lymphocytic leukemia is very rare. Even though chloroma are sensitive to radiation therapy, prognosis is poor because of the gravity of the underlying disease and association with impending blast transformation. The authors reports a intracranial chloroma by acute lymphocytic leukemia.
Craniopharyngiomas exhibiting histologic malignancy are extremely rare. Herein, we report the case of a 26-year-old male patient who underwent suprasellar mass excision via an interhemispheric transcallosal approach. Histopathological examination indicated that the craniopharyngioma was of the adamantinomatous subtype. The patient received postoperative medical treatment for endocrine dysfunction and diabetes mellitus without radiation treatment. Two years after the operation, he presented with progressive visual disturbance and altered mentality. Magnetic resonance imaging revealed a huge mass in the suprasellar cistern and third ventricle. He underwent a second operation via the same approach. The histopathological examination showed an adamantinomatous craniopharyngioma with sheets of solid proliferation in a spindled pattern, indicating malignant transformation. Malignant transformation of craniopharyngioma in the absence of radiation therapy has been reported in only five cases, including this one. We present a case of malignant transformation of craniopharyngioma with a brief review of relevant literature.
Recent advances in the molecular and genetic characterization of central nervous system (CNS) tumors have ushered in a new era of tumor classification, diagnosis, and prognostic assessment. In this emerging and rapidly evolving molecular genetic era, imaging plays a critical role in the preoperative diagnosis and surgical planning, molecular marker prediction, targeted treatment planning, and post-therapy assessment of CNS tumors. This review provides an overview of the current imaging methods relevant to the molecular genetic classification of CNS tumors. Specifically, we focused on 1) the correlates between imaging features and specific molecular genetic markers and 2) the post-therapy imaging used for therapeutic assessment.
Brucellosis, a zoonosis with worldwide distribution, is a systemic infection caused by facultative intracellular bacteria of the genus Brucella, which can involve multiple organs and tissues. We report an uncommon case of spondylodiscitis with epidural abscess due to Brucella in a male stockbreeder. Diagnosis was based on clinical history, and supported by Brucella serology and magnetic resonance imaging. Clinical and radiological improvement were observed with a combined antimicrobial therapy of doxycycline, rifampicin, and gentamycin.
An 8-month-old 3.15 kg female Cocker-spaniel with history of ataxia referred to the Veterianary Medical Teaching Hospital, Chungnam National University. There were no abnormalities in CBC and serum chemical analysis. Agenesis of dens was found on dorsoventral view in cervical radiography. Compressed cervical spinal cord and enlarged cerebral ventricle were observed in magnetic resonance imaging. It was diagnosed as atlantoaxial instability with hydrocephalus. For conservative therapy, neck brace was applied and diuretics and prednisolone were administered. The dog's ataxia became better gradually.

Objectives : This study was performed to compare the effects of Shinbaro pharmacopuncture therapy and bee venom pharmacopuncture therapy in lumbar disc herniation Methods : We compared treatment efficacy of 12 patients with Shinbaro pharmacopuncture therapy and 10 patients with bee venom pharmacopuncture therapy, all of whom were diagnosed with lumbar disc herniation using computed tomography(CT) or magnetic resonance imaging(MRI). We performed a retrospective comparison and analysis during the course of 21 days since admission. To evaluate the treatment efficacy, we recorded the temperatures of the opposite back, hip, and legs with visual analog scale(VAS, back and legs), pain rating scale(PRS), Oswestry disability index(ODI) and digital infrared thermal imaging(DITI) based on patients' medical records. Results : Both treatments were effective in reducing pain and improving functions for lumbar disc herniation patients in terms of VAS, PRS, ODI, DITI. Shinbaro pharmacopuncture group showed slightly better results than the bee venom pharmacopuncture group, but the difference was not significant. Conclusions : For the treatment of lumbar disc herniation, it is suggested that Shinbaro pharmacopuncture therapy is thought to be as effective as bee venom pharmacopuncture therapy without side effects, although further study would be necessary.
