Background: We analyzed the results of surgical reduction and fixation of ribs under thoracic epidural anesthesia and analgesia (TEA) in patients who had no more than 3 consecutive rib fractures with severe displacement to examine the clinical usefulness of this method. Materials and Methods: From May 2008 to March 2010, 35 patients underwent surgical reduction and fixation of ribs under TEA. We reviewed the indications for this technique, number of fixed ribs, combined surgical procedures for thoracic trauma, intraoperative cardiopulmonary events, postoperative complications, reestablishment of enteral nutrition, and ambulation. Results: The indications of TEA were malunion or nonunion of fractured ribs in 29 (82.9%; first operation) and incompletely ribs under previous general anesthesia in 6 (17.1%; second operation). The average number of fixed ribs per patient was 1.7 (range: 1~3). As a combined operation for thoracic trauma, 17 patients (48.6%) underwent removal of intrathoracic hematomas, and we performed repair of lung parenchyma (2), wedge resection of lung (1) for accompanying lung injury and pericardiostomy (1) for delayed hemopericardium. No patient had any intraoperative cardiopulmonary event nor did any need to switch to general anesthesia. We experienced 3 postoperative complications (8.6%): 2 extrapleural hematomas that spontaneously resolved without treatment and 1 wound infection treated with secondary closure of the wound. All patients reestablished oral feeding immediately after awakening and resumed walking ambulation the day after operation. Conclusion: Thoracic epidural anesthesia and analgesia (TEA) may positively affect cardiopulmonary function in the perioperative period. Moreover, this technique leads to an earlier return of gastrointestinal function and early ambulation without severe postoperative complications, resulting in a shortened hospital stay and lowered costs.
A 5-year-old intact male black gibbon (Hylobates concolor) was referred for evaluation of the right pelvic limb lameness following a fight against other black gibbons. Fractures of the right tibia and fibula were suspected on physical examination and palpation of the right pelvic limb, but no other injuries or abnormalities were detected. While the black gibbon was sedated, pelvic limb radiographs were taken, which revealed diaphyseal oblique fractures of the right tibia and fibula. Open reduction of the fractures was performed. The tibial fracture was repaired by use of an internal fixation technique that included a tubular dynamic compression plate and cortical screws secured along the craniomedial aspect of the tibia. There were no complications during the postoperative rehabilitation period. At 9 weeks, radiographs revealed that bridging callus was well formed over the cortices of the tibial and fibular fracture area. The cast was removed 9 weeks after surgery. The black gibbon exhibited no evidence of lameness and was released back into the group. Presently, there are no published reports of internal fracture fixation in a black gibbon where a tubular dynamic compression plate and cortical screws provided excellent stabilization of the tibia and complete fracture healing allowing normal ambulation.
Treatment of root perforation elicits special considerations due to its blood-contaminated circumstances. It is known that conventional dental restorative materials are all leaking. Calcium sulfate is the material which react with water to become chemically set. This study, therefore, was performed to develop a new compound containing calcium sulfate and to evaluate its physical and biological characteristics. Three materials were used, IRM, calcium sulfate, calcium sulfate-hydroxyapatite compound. The composition of the calcium sulfate-hydroxyapatite compound was basically 50 % of calcium sulfate and 50 % of hydroxyapatite mixed with guajacol. The materials were mixed in conventional way and underwent four physical test procedures, setting time, solubility test, compressive strength, and marginal leakage test. All materials were evaluated under the scanning electron microscope to examine the marginal sealing ability. Animal experiment was also performed to test the materials' tissue response. Twenty-four dog's premolars were tested with either furcation perforations or apical retro-fillings. From the results, we found that calcium sulfate possess the good marginal sealing ability. However, calcium sulfate creates many voids which is caused by crystal thrusting action when it reacts with water. It seemed that the voids caused disintegration of the material which eventually lead to tissue reaction. By compounding calcium sulfate and hydroxyapatite, we were able to obtain the better physical properties but it showed larger marginal gap between the material and the root surface. Within the six weeks observation period, both IRM and calcium sulfate-hydroxyapatite compound showed good tissue responses in animal experiment. It is concluded that calcium sulfate would be the material of choice in root perforation repair, but the physical property needs to be further improved.

Ultra Super Early Strength Cement is a material that satisfies these requirements. early hydration heat however, is significant over regular concrete, thus discretion is advised for thermal cracks in accordance with heat generation when constructing a large-scale structures. In addition, the negative point that it is difficult to achieve required strength in a short period of time following rubbing process while retaining workability, the cement is being used conditionally for engineering material and Ultra Super Early Strength Cement for maintenance material for construction doesn't exist. Magnesia Phosphate Cement, which is currently under studies in overseas uses no extra admixture and has strong points of Ultra Super Early Strength as well as favorable construction-ability and adhesive stability to the prototype concrete. These factors stem recognition that it could be used as maintenance material for construction of diverse applicability. In order to provide necessary data to increase practicality of the magnesia phosphate cement for Ultra Super Early Strength Mortar, the study carried out simulate experiment on member of framework to review field applicability.
We reviewed the records of 25 patients who were re-operated upon after primary repair of esophageal atresia with or without fistula at the Department of Pediatric Surgery, Seoul National University Children's Hospital, from January 1997 to March 2007. Types of the esophageal atresia anomalies were Gross type A in 5 patients, C in 18, and E in 2. The indications for re-operation were anastomosis stricture (n = 14), tracheo-bronchial remnant (n = 4), persistent anastomosis leakage (n = 3), recurrent tracheo-esophageal fistula (n = 2) and esophageal web (n = 2). The interval between primary and secondary surgery was from 48 days to 26 years 5 months (mean: 2 years and 4 months). Four patients required a third operation. The interval between the second and third operation was between 1 year 1 month and 3 year 10 month (mean: 2 years 5 months). Mean follow up period after last operation was 35 months (1 years-8 years 6 months). The secondary surgery was end-to-end esophageal anastomosis in 15, esophagoplasty in 5, gastric tube replacement in 5. After secondary operation, 6 patients had anastomosis stricture (4 patients were relieved of the symptoms by balloon dilatation, 2 patients underwent tertiary operation). Five patients had leakage (sealed on conservative management in all). Two patients had recurrent tracheo-esophagel fistula (1 patient received chemical cauterization and 1 patient underwent tertiary operation). Currently, only one patient has feeding problems. There were no mortalities. Secondary esophageal surgery after primary surgery for esophageal atresia was effective and safe, should be positively considered when complications do not respond to nonoperative therapy.

Purpose: Mulliken's method allows for normal nasal and lip growth, which in turn forms a natural shape of the philtrum. Therefore, we used a modified Mulliken's method to correct unilateral and bilateral cleft lip nasal deformities and followed the patients for 10 years. Methods: Ninety-one patients, who had undergone repair of unilateral and bilateral cleft lip and nasal deformity simultaneously using Mulliken's method during the time period from June 1997 to June 2009, were enrolled into this study. To follow-up of the growth of the lips and nose after the operation, the following 5 anthropometric measurements were analyzed: nasal tip protrusion, columellar length, upper lip height, cutaneous lip height, and vermilion mucosa height. Results: Using this method, we obtained a result that there was no significant difference in the development of the lip compared to the normal control group, and that the bilateral cleft lip patients' nasal projection and columellar length was shorter than that in normal persons. Both measures were statistically significant. Conclusion: Mulliken's method is a superb surgical technique, which enables the normal development of the nose and lip, which further allows for the innate philtrum appearance. The author's result does not seem to be meaningful, because the normal rate of nasal growth is slow before adolescence; however, we recommend additional follow-up and accordant treatment, if needed, once the nasal growth is complete.
The purpose of this research is to determine the deterioration characteristics of high-rise and high-density apartment complexes in 1st period new towns in order to regenerate apartments as sustainable residential environments in Korea. For this purpose, the level of deterioration was evaluated by using 'evaluating indicators of the level of deterioration of high-rise and high-density apartments'. The deterioration characteristics were determined inclusively and concretely based on evaluation results. In addition, the deterioration level according to residents' awareness of the deterioration of their apartment was evaluated. This study was conducted by methods of field data collection, staff interview, field measurement, and resident survey according to the measurement of the evaluating indicators. And the re-organized questionnaire based on the evaluating indicators was utilized for a survey of resident's awareness. The results are as follows: In the physical dimensions of apartments, the deterioration characteristics included deteriorated equipment and a decline in structural performance due to the aging of the apartment, a shortage of parking spaces, and the low flexibility of the bearing wall structure, etc. In terms of the social dimensions, the apartments showed an increase in the number of elderly households, a shortage of community facilities, and a lack of the opportunity for community participation etc. In terms of the economic dimensions, the apartments showed limitations in maintenance & repair and a lack of environmentally friendly features.

In this thesis, two possible schemes for power feeding are considered. The first one is the network power feeding, and the other is the power feeding from the rechargeable battery. Though each scheme has its merits and demerits, the domestic network operator has proposed the latter due to the large deviation of line resistance, high power loss, and safety. This scheme, however, has several problems. The usage is limited by the life of battery leading to the failure of minimal voice service for many hours. The space occupation of that battery is another serious problem. Moreover, the network opeator should have overhead of maintenance and repair. In order to alleviate such the difficulties, the sealing current is introduced through the subscriber loop. Flowing the current into the loop prevents from rusting. This thesis designs a power controller using PEB2025 to control various amount, duration, and period of this sealing current, to achieve the optimum current supply for sealing.
There has been a lot of debate between scholars around the folding screen painting, A Beauty under the Tree[조모입여병풍], a collection of Shosoin, Japan. Even though it is preserved in Japan, its distinguished Chinese feature made many scholars to think it might be made in China and be transmitted to Japan. However, the inspection of the material during $1973{\sim}1975$ has revealed an important fact, that is, the feather used in this screen painting belonged to Japanese bird feather. Moreover, a great amount of documents written in Tenbyo(천평승보, 8th century) period, which was thought to be used as the ground material of this painting. Therefore we can get many evidences saying that this painting was made in japan. In a basic sense, I stand on this side just because it has more evidences. However, we must be careful to make a final conclusion. Especially on the precious things like this painting screen, we must take whole considerations as possible as we can. First of all, we must consider whether Japan could have ability to make this. Second, there could be other possibility that Japanese repair the original one with their feathers or papers. In order to prove this, study on other pieces including painting screens should be done. I just suggest an objective international discussion. Lastly, I can find the common feature between the pieces of Shosoin and the ones of United Shilla, which could be influenced by same impact, the Silk road trade in a broad sense.

The author studied on the effects of x-irradiation to the development of digital malformation in gestation rats. The time-matings occured between 6 p.m. and 8 a.m. and females with copulation. plugs at 8 a.m. were isolated and properly marked for evidence of copulation. The lower abdomen of mothers were exposed to x-irradiation on the 11½th day of gestation, the critical period developing digital malformation, respectively 100, 150, 200, 250, 300 and 350 rads. At 18½th day of post-conception total 50 pregnant females were dissected and the incidence of digital malformations were obtained. Rat embryos on the 12, 13, 14, 15, 16th day of gestation irradiated by 250 rads were examined for morphogenesis of digital malformation. Digital radiating lines were examined in water and histologically by H-E stain. Supra vital stain samples by Nile-blue sulfate in 37℃ normal saline were prepared for the observation of cell necrosis regions and morphogenesis of digits. The results obtained were as follows; 1. By x-irradiation on 11th day of gestation, digital malformations of Ectrodactylia, Syndactylia Polydactylia and Hematodactylia were developed. Ectrodactylia showed the effective relationship to the amount of irradiation, however Syndactylia ans Polydactylia did not. 2. By x-irradiation, cell necrosis of digital germ was appeared markedly, but in 48 hours after irradiation was depressed to the periphery of digital germ and in 72hours after irradiation was disappeared. Digital radiating line showed marked stage of malformation in 48hours after irradiation and continued to show the same amount of physiological cell necrosis as the compared control group in 72hours. after irradiaion. But in the Syndactylia, physiological cell necrosis was not able to be recognized. 3. Ectrodactylia induced by x-irradiation was considered as the direct resoult of cell necrosis of digital origin, however, Polydactylia and Syndactylia were considered as the resoult of some effect in repair process of x-irradiation damages.
