Arthroscopic Bankart Repair with Suture Anchor (Bankart 병변의 봉합나사를 이용한 관절경적 봉합술)

  • Purpose: To evaluate the efficacy of arthroscopic Bankart repair using suture anchors for treatment of traumatic anterior instability of shoulder joint .Materials and Methods: We performed arthroscopic Bankart repair using suture anchor in 54cases and evaluated the results with the functional grading system of Rowe and Zarins after patients were followed up for more than 12 months. Results: Combined pathologies identified under arthroscopy were Hill-Sachs lesion in 28 cases, SLAP lesion in 6 cases and chondromalacia of humeral head in 6 cases. The results were excellent or good in 50 cases(92.6$\%$), and redislocation was happened only two cases. Conclusion: We conclude that arthroscopic Bankart repair with suture anchors is one of the reliableand effective method for treatment of recurrent shoulder dislocation with Bankart lesion.

The Treatment of the Acromioclavicular Dislocation using the Modified Neviaser Method (변형된 Neviaser 술식을 이용한 견봉쇄골관절 탈구의 치료)

  • Purpose: To describe a technique of the modified Neviaser method and to evaluate the clinical outcome of the technique in the treatment of the complete acromioclavicular joint dislocation, Materials and Method: We evaluated 20 patients who were treated by a modified Neviaser method from June 1996 to January 1999, They were followed up for a minimum of two years. The operative technique includes fixation of the acromioclavicular joint, repair of coracoclavicular ligament and transfer of the anterolateral band of coracoacromial ligament. Results: The 20 patients were evaluated clinically using Kang's criteria. The satisfactory results obtained in 85%. The mean coracoclavicular interval ratio was decreased from 2.22 preoperatively to 1.07 postoperatively. At the time of two year follow-up, the mean ratio was 1.20. There was no definite complication such as re-dislocation of the joint. Conclusion: In above type 3 acromioclavicular dislocation, the modified Neviaser method provided the advantage of strong and stable fixation with a low complication rate. Therefore, it is thought to be one of the useful operative technique.

  • Most pigmented villonodular synovitis (PVNS) is occurred in knee joint and finger of hand. PVNS is rarely occurred in shoulder joint. In English and French literatures, less than 30 cases were reported. We report 3 PVNS cases with rotator cuff tears, which was treated by arthroscoic extensive synovectomy, debridement and rotator cuff repair. The PVNS with rotator cuff tear in shoulder joint was rarely reported in the Korean literature.

  • Bridges are critical to the civil engineering infrastructure network as they facilitate movement of people, the transportation of goods and services. Given the aging of bridge infrastructure, federal officials mandate visual inspections biennially to identify necessary repair actions which are time, cost, and labor-intensive. Additionally, the expansion joints of bridges are rarely monitored due to cost. However, expansion joints are critical as they absorb movement from thermal effects, loadings strains, impact, abutment settlement, and vehicle motion movement. Thus, the need to monitor bridge expansion joints efficiently, at a low cost, and wirelessly is desired. This paper addresses bridge joint monitoring needs to develop a cost-effective, real-time wireless system that can be validated in a full-scale bridge structure. To this end, a wireless expansion joint monitoring was developed using commercial-off-the-shelf (COTS) sensors. An in-service bridge was selected as a testbed to validate the performance of the developed system compared with traditional displacement sensor, LVDT, temperature and humidity sensors. The short-term monitoring campaign with the wireless sensor system with the internet protocol version 6 over the time slotted channel hopping mode of IEEE 802.15.4e (6TiSCH) network showed reliable results, providing high potential of the developed system for effective joint monitoring at a low cost.

  • Object: To evaluate the efficiencies of the arthroscopic rotator cuff surgery which is Performed without the traction system in the lateral decubitus position. Methods: Twenty-nine cases of the arthroscopic rotator cuff surgery performed without the traction system in the lateral decubitus position were studied from February, 2002 to January, 2005. We performed a repair using the arthroscopic debridement and the arthroscopic rotator cuff repair, or using the mini-open incision technique after the confirmation of rotator cuff tear, then, the arthroscopic subacromial decompression was performed after the confirmation of subacromial lesions Results: We could easily find the subscapularis tear which was often overlooked in the arthroscopic rotator cuff surgery performed with the traction surgery by the relaxation of the subscapularis, as the arm position was internally rotate about 45 to 70 degrees from abducted position. We found that the operation time was reduced 14 minutes shorter than the operation time of the controlled group which had the surgery with the traction system on the average. We also found that there were no neurovascular complications from all cases. Conclusions: The arthroscopic rotator cuff surgery without traction system in the lateral decubitus position provided the better visual field, easy manipulation of the joint and reducing operation time.

  • According to the dispute cases filed for claiming the collective defect repair fees, inter-layer concrete joints turned out to be the most frequently disputed item. For this reason, this study selects the inter-layer concrete joints to further analyze the primary causes and details of each dispute case. From the results of this study, three primary causes of the disputes are found, which are 1) the absence of standard specifications for construction quality control and management after construction; 2) the absence of established standards for repair when construction defects are found. In order to prevent construction defects in inter-layer concrete joints, this study provides three suggestions including 1) the current standard specifications for inter-layer concrete joints should be further specified by the Ministry of Land, Infrastructure and Transport; 2) a construction defect should be judged according to the compliance to the standard specifications; and 3) a clear and institutional protocol needs to be established for defect repair in cases that new public apartment houses have been judged to have defects.

A Study on the Performance Characteristics for Recycled Parts of C. V. Joint in the Vehicles (자동차 중고재생 등속조인트의 성능특성에 관한 연구)

  • There are genuine parts, non-genuine parts and recycled parts which can be replaced for the repair of damaged parts of crashed cars. It is the recent trend in korea that recycled C. V joints are more popular than the genuine parts fur repairing crashed cars due to the cost. Performance of recycled C. V. joints as replacement parts was tested and analysed in this study. To examine the durability of the recycled parts, the replaced C. V joints after repair were tested and analysed periodically. The results were showed that basic performance of the recycled parts was normal. However ball cage was more frequently damaged than genuine parts. We concluded that a standard test for recycled parts is required to get a safe and durable parts.

Seismic repair of exterior R/C beam-to-column joints using two-sided and three-sided jackets

  • The use of local two-sided and three-sided jacketing for the repair and strengthening of reinforced concrete beam-column joints damaged by severe earthquakes is investigated experimentally and analytically. Two exterior beam-column joint specimens ($O_1$ and $O_2$) were submitted to a series of cyclic lateral loads to simulate severe earthquake damage. The specimens were typical of existing older structures built in the 1960s and 1970s. The specimens were then repaired and strengthened by local two-sided or three-sided jacketing according to UNIDO Manual guidelines. The strengthened specimens ($RO_1$ and $RO_2$) were then subjected to the same displacement history as that imposed on the original specimens. The repaired and strengthened specimens exhibited significantly higher strength, stiffness and better energy dissipation capacity than the original specimens.

  • Under water wet welding is an economically alternative for the repair of offshore structures. In this paper investigations on the fatigue strength of a wet welded pipe structural member have been reported. For the connection a special sleeve patch design has been developed. The joint was fatigue tested. The evaluation of the test was carried out by means of the hot-spot approach with regard to several extrapolation rules of the hot-spot stress. Obtained results have been compared to actual classfication rules and recommendations.

  • Root tear of the posterior horn of the medial meniscus can occur from trauma or chronic degeneration, leading to meniscus extrusion, articular cartilage loss, osteophyte formation, and medial joint space narrowing. It is common on middle age with or without minor trauma. We experienced a case of medial meniscus posterior horn root tear in 13 years old boy during baseball game. We performed 1 direct suture anchor repair for medial meniscus posterior horn root tear in adolescent and report clinical result.
