아킬레스 건 파열의 봉합 후 발생한 심부 감염의 치료 (Treatment of Deep Infection Following Repair of Achilles Tendon Rupture)

  • Purpose: Theaim of this study was to review the results of treatment for deep infection following repair of Achilles tendon rupture using reverse sural arterialized flap and/or flexor hallucis longus transfer. Materials and Methods: Five cases of Achilles tendon infection in five patients were treated using reverse sural arterialized flap and/or flexor hallucis longus transfer at our hospital with followed up of average 23.6 months (range, 13-43 months). Three patients were male and average age at surgery was 52.0 years (range, 42-59 years). Clinical results were evaluated by the method of Percy and Conochie, and the isokinetic peak torque value was interpreted according to the guideline of Sapega. Results: The clinical result was excellent in three cases, good in one case and fair in one case. The isometric peak torque value for $30^{\circ}$ per second was normal in two cases, possibly abnormal in one case, and probably abnormal in two cases, and for $120^{\circ}$, normal in one case, probably abnormal in four cases. Five cases in five patients were satisfied with the result of treatment. Conclusion: We can expect satisfactory results of treatment for deep infection following repair of Achilles tendon rupture using reverse sural arterialized flap and/or flexor hallucis longus transfer.

The Efficacy and Safety of Ablative Fractional Resurfacing Using a 2,940-Nm Er:YAG Laser for Traumatic Scars in the Early Posttraumatic Period

  • Background : Skin injuries, such as lacerations due to trauma, are relatively common, and patients are very concerned about the resulting scars. Recently, the use of ablative and non-ablative lasers based on the fractional approach has been used to treat scars. In this study, the authors demonstrated the efficacy and safety of ablative fractional resurfacing (AFR) for traumatic scars using a 2,940-nm erbium: yttrium-aluminum-garnet (Er:YAG) laser for traumatic scars after primary repair during the early posttraumatic period. Methods : Twelve patients with fifteen scars were enrolled. All had a history of facial laceration and primary repair by suturing on the day of trauma. Laser therapy was initiated at least 4 weeks after the primary repair. Each patient was treated four times at 1-month intervals with a fractional ablative 2,940-nm Er:YAG laser using the same parameters. Posttreatment evaluations were performed 1 month after the fourth treatment session. Results : All 12 patients completed the study. After ablative fractional laser treatment, all treated portions of the scars showed improvements, as demonstrated by the Vancouver Scar Scale and the overall cosmetic scale as evaluated by 10 independent physicians, 10 independent non-physicians, and the patients themselves. Conclusions : This study shows that ablative fractional Er:YAG laser treatment of scars reduces scars fairly according to both objective results and patient satisfaction rates. The authors suggest that early scar treatment using AFR can be one adjuvant scar management method for improving the quality of life of patients with traumatic scars.

크레인 거더의 피로균열에 관한 실험적 연구 (2) -피로균열의 보수법- (Experimental Study on Fatigue Crack in Welded Crane Runway Girders (2) -Repair methods of Fatigue Crack-)

  • 크레인 주행거더에 발생된 피로균열에 대한 보수방법으로 stop-hole(5mm 또는 9mm), stop-hole에 고력볼트 체결, 용접. 첨판과 고력볼트에 의한 4가지 보수방법을 적용하였다. 이중에서 stop-hole로 보수를 한 경우와 용접보수를 한 경우에는 재균열이 발생하였다. 그러나 stop-hole에 고력볼트를 체결한 경우 및 첨판과 고력볼트로 보수한 경우에는 재균열이 발생하지 않았다. 보수전과 보수후 피로강도를 비교해보면 stop-hole 또는 stop-hole과 고력볼트로 보수를 한 경우에는 모두 보수전과 동등한 피로강도를 얻었다. 용접보수를 할 경우에는 정확한 시공이 시행되면 보수전과 동등한 피로강도를 기대할 수 있지만. 시공이 잘못되면 피로강도가 현저하게 저하될 수 있음을 알 수 있었다. 피로균열이 발생된 위치에 첨판과 소요볼트를 시공할 수 있는 조건이라면 첨판과 고력볼트로 보수하는 것이 가장 확실한 보수법임을 알 수 있었다.

원자력 증기발생기 결함 세관 보수용 폭발 sleeving에 관한 연구 (A Study on the Explosive Sleeving of A Repair for Defective Tube/Tubeplate on the Nuclear Steam Generator)

  • Unfortunately leaks occur in heat exchangers periodically, usually at the tube to tubeplate joint. The usual method of repair is to plug off the defective area and isolate the tubes of concern from the circuit. If the leaks continua the thermal capacity of the units is progressively reduced and for this reason the alternative of using an internal bridging sleeve has been examined. This paper discusses the overall development activities that has been found necessary to bring this repair procedure to a successful conclusion for use on the nuclear steam generator. In this work we have investigated optimum explosives and explosive quality, explosive sleeving's thickness, the design of sheath stress relieving heat treatment pull-out load, hydraulic leakage, stress corrosion cracking properties. The results obtain are as follows : (1) The optimum explosives and explosive qualities are PETN and about 15~40 gr/ft of explosive sleeving in nuclear steam generator. (2) Explosive sleeving's thickness is 1.1~l.4mm, If groove of 0.35mm formed in sleeve outside existed, For the hydraulic leakage is go up, explosive sleeving of formed groove are applicate tube and turnplate. (3) If the stress relieving heat treatment are experiment in $750^\circ{C}$, $850^\circ{C}$, 15 minutes Pull-out strength of sleeving 1,500~2,300kg, hydraulic leakage is $250kg/cm^2$.

Ad Hoc 망에서 신뢰성 있는 데이터 전송을 위한 경로 재설정 기법 (A re-route method for reliable data transport in Ad Hoc Networks)

  • Ad hoc 망은 기지국이나 엑세스 포인트(AP) 같은 인프라가 없는 무선이동통신기술이다. 즉, ad hoc 망에서는 이동노드들이 라우터와 서버의 역할을 모두 한다. 기존의 경로 재 설정 방법에서는 노드의 이동으로 인하여 경로가 끊어졌을 경우 송신노드는 경로를 다시 재 설정 해야한다. 하지만 경로가 끊어진 후 다시 재 설정하는 방법은 QoS를 보장해야하는 실시간 데이터 전송에서는 적합하지 않을 수 있다. 그러므로 본 논문에서는 경로가 끊어지는 것을 미리 예방하는 경로 재 설정 기법을 제안한다. 중간노드가 이동하면서 어떤 임계영역으로 들어가는 경우 자신의 다음 노드에게 핸드오프 패킷을 발생시키고, 핸드오프 패킷을 수신한 노드는 경로가 끊어지기 전에 경로요청패킷을 중간노드의 바로 이전노드에게 브로트캐스트 함으로써 중간 노드가 경로를 벗어나더라도 지속적으로 새로운 경로를 통하여 데이터 패킷을 전송할 수 있다.

  • Supply units in each command are multi-tiered and each supply unit keeps a supply level independently, which can cause excess stock, leading false reports to the Logistics Command and increasing difficulties in managing user needs. This causes excess assets and the excess then causes deformation in demand. therefore, the supply support systems of our armed forces have become high-cost/low-efficiency and are insufficient to meet the needs of users in combat units. Civilian corporations and the US Department of Defense are downsizing the aforementioned multi-tiered supply systems thus revolutionizing efficient and effective logistics by adopting Supply Chain Management(SCM), and Prime Vendor policies. Prime Vendor policy is a logistics support method that allows users to directly request and receive supply items from suppliers, based on supply contracts between suppliers and central maintenance organizations like KDA. In other words, it is a system that allows for users to make orders to suppliers directly and suppliers to deliver goods to the users directly, cutting out the middle stage, thus allowing an efficient supply. This is a way forward in finance that cuts costs in net supplies and allows an efficient utilization of civilian assets. which is also known to fasten the speed of logistical support and stripping down the logistical structure. therefore, this report will explore Prime Vendor policies adopted in certain number of units for medical supplies that were taken in consideration of improvements in stock management in civilian organizations and the US Army, and aims to apply such policies for repair parts.

도자기 수리복원 방법의 변천과정에 관한 고찰 (A Study on the Changes Process Repair and Restoration Method of Ceramic)

  • 손상된 도자기는 고대부터 여러 가지 방법과 재료를 사용하여 수리복원 되었다. 도자기 수리복원의 역사는 전통적인 수리방법을 이용해 실생활에서 활용 가능한 재료들을 사용하던 방식에서 새로운 재료의 도입과 다양한 처리기법의 시도 및 사용된 재료의 분석에 이르는 전문적인 분야로까지 그 영역을 넓히며 발전을 거듭하여 왔다. 우리나라의 도자기 수리복원 방법의 변천과정은 출토유물과 문헌자료, 분석 등을 통해 조사한 결과, 동물이나 식물에서 추출된 접착제, 석회와 같은 복원제 등을 사용하여 수리 복원하던 천연재료 사용 시기(고대~일제 강점기 이전)와, 주사비 등 천연재료와 합성수지가 혼합되어 사용되는 시기(일제 강점기~1970년대), 새로운 처리기법과 재료가 도입되고 보존규범에 입각하여 수리복원이 이루어진 합성수지 사용 시기(80년대 이후)로 구분 할 수 있었다.

배기밸브 보수 용접부의 부식 특성에 관한 전기화학적 연구 (An Electrochemical Study on Corrosion Property of Repair Welding Part for Exhaust Valve)

  • The diesel engine of the merchant ship has been aperated in severe environments more and more, because the temperature of the exhaust gas of a combustion chamber is getting higher and higher with increasing use of heavy oil of law quality, due to the significant increase in the price of oil in recent some years. As a result, the degree of wear and corrosion between exhaust valve and seat ring is more serious compared to other engine parts. Thus the repair welding of exhaust valve and seat ring is a unique method to prolong the life of the exhaust valve, from an economical point of view. In this study, the corrosion property of both weld metal and base metal was investigated using electrochemical methods such as measurement of corrosion potential, cathodic and anodic polarization curves, cyclic voltammogram, and polarization resistance in 5% H2SO4 solution. The test specimen was a part of an exhaust valve stem being welded as the base metal, using various welding methods. In all cases, the corrosion resistance as well as hardness of the weld metal zone was superior to the base metal. In particular, plasma welding showed relatively good properties for both corrosion resistance and hardness, compared to other welding methods. In the case of DC SMAW (Shielded metal arc welding), corrosion resistance of the weld metal zone was better than that of the base metal, although its hardness was almost same as the base metal.

교량의 생애주기비용 최적설계 (Optimal Life Cycle Cost Design of a Bridge)

  • 최근 들어 교량과 같은 구조물에 대한 생애주기비용(Life Cycle Cost, LCC) 분석의 중요성이 점차 커지고 있다. 그러나 교량의 공용수명 동안 발생할 수 있는 생애주기비용은 설계 및 시공조건 그리고 사용환경에 따라 많은 불확실성을 내포하고 있기 때문에 정확히 예측하기 힘들다. 본 논문에서는 교량의 생애주기비용 최적설계를 위한 설계방법을 제시하였다. 교량의 총생애주기비용은 초기비용, 손상비용 유지관리비용, 보수/보강비용, 사용자비용, 해제/폐기비용의 합으로 산정하였다. 생애주기비용을 목적함수로 하고 도로교설계기준을 제약조건으로 최적화를 수행하였다. 초기비용은 종합불가정보 및 참고자료를 근거로 산출하였으며, 하중 및 부재에 대한 불확실성을 고려하기 위해 손상확률의 개념을 도입하여 손상비용을 산출하였다. 교량의 내하율곡선을 이용하여 교량의 보수/보강 비용을 추정하였으며, 차량운행비용과 시간지연비용으로부터 사용자비용을 산정하였다. 이로부터 교량에 대한 생애주기비용 최적설계를 수행하고 주요인자들에 대한 영향을 살펴보았다.

보수용접봉의 종류와 용접후 열처리가 용접금속부의 내식성에 미치는 영향에 관한 전기화학적 평가 (An Electrochemical Evaluation on the Corrosion Resistance of Welding Zone due to Kinds of Repair Welding Filler Metals and Post Weld Heat Treatment)

  • Recently a fuel oil of the diesel engine of the marine ship is being changed with heavy oil of low quality as the oil price is higher more and more. Therefore the wear and corrosion in all parts of the engine such as cylinder liner, piston crown, spindle and seat ring of exhaust valves are predominantly increased. In particular the degree of wear and corrosion of piston crown is more seriously compared to the other parts of the engine due to operating in severe environment such as the high temperature of exhaust gas and repeating impact. Thus the repair weldment of the piston crown is a unique method to prolong the its life in a economical point of view. In this case, filler metals having a high corrosion and wear resistance such as stellite 6, Inconel 625 and Inconel 718 are mainly being used for repair welding. However it has been often happened that piston crown on the ship,s job site is being actually inevitably welded with mild filler metals. Therefore in this study, filler metals such as E4301, E4313 and E4316 were welded at SS401 steel as the base metal, and corrosion property of their weld metals in the case of post weld heat treatment or not was investigated with some electrochemical methods such as measurement of corrosion potential, cathodic and anodic polarization curves, cyclic voltammogram and polarization resistance etc. in 0.1% $H_2SO_4$ solution. Corrosion resistance of the weld metal of E4301 was better than the other weld metals in the case of no heat treatment, however, its resistance was considerably decreased with post weld heat treatment(annealing:$625^{\circ}C$, 2 hr) compared to other weld metals. The weld metals of E4313 and E4316 showed a relatively good corrosion resistance by post weld heat treatment.
