The existence of oxytocin in the pituitary gland of chicken has been ascertained, but its physiological roles are still obscure. In the study the action of oxytocin on renal function of the chicken was investigated during water diuresis, utilizing clearance and the Sperber technique. The results obtained are summarized as follows: Oxytocin, like in many species of mammals, elicited a profound diuretic response in the chicken. Urine flow, excretion of electrolytes, as well as glomerular filteration rate increased, with intravenous infusion of $3{\sim}10\;m{\mu}/kg/min$. Oxtocin, infused into the renal portal circulation via hindleg vein in a dose of $3{\sim}13\;m{\mu}/kg/min$. elicited marked increase in urine flow, glomerular filteration rate and sodium excreted in the urine. The diuretic effect was more pronounced in the infused side. It is suggested that diuretic response to oxytocin in the chicken results from dual action of oxytocin: increase of GFR and inhibition of sodium reabsorption on the renal tubule. The possibility that oxytocin might act through some endogenous substances could be ruled out.
The effect of atractylis on the renal function of the dog was investigated in this study. Alcohol extract of atractylis, when given intravenously in dose 15mg/kg, elicited antidiuresis rather than diuresis. Also, free water clearance $(C_{H_2O})$ decreased in proportion. No alterations of the renal clearances of creatinine and PAH were detected in the range of doses which are effective in inhibiting diuresis. The effects were obtained with smaller doses when the atractylis was given through the intracarotid artery route rather than intravenously. When infused directly into a renal artery, atratylis exhibited identical action on both kidneys, indicating that the renotropic action is mediated by some endogenous humoral agents. It is therefore suggested that atractylis is capable of releasing ADH from the neurohypophysis.
Vasoactive intestinal peptide (VIP) found in duodenal mucosa originally has been suggested as a neurotransmitter. Its localization, however, now known, is not limited to the gastrointestinal tract, but scattered at many different kinds of tissues, smooth muscles, endocrine gland and exocrine gland as well as central and peripheral neural tissues. To investigate the effect of VIP on renal function, an experiment has been done in anesthetized male rats. The results obtained were: 1) Urinary output and creatinine clearance decreased significantly during the period of infusion of VIP, 2.0ug/rat/7minutes. 2) Urinary excretion of sodium, potassium and chloride decreased but without significance by infusion of VIP. 3) Blood pressure, systolic and diastolic, decreased by VIP administered intravenously in the period of infusion. 4) Changes of urinary output, sodium and chloride excretion was correlated with changes of creatinine clearance. The above data suggest that VIP administered intravenously can suppress the renal hemodynamics indirectly, and also decrease electrolyte excretion through its renal hemodynamic change.
This study was investigated to clarify the effect of Sin Bee Tang (神秘湯) on the renal function, arterial blood pressure and plasma cortisol. The results obtained were follows; 1. Urine volume and glomerular filtration rate were decreased significantly after Sin Bee Tang water extract, 0.1ml/kg, administration. 2. Glomerular filtration rate, renal plasma flow and urinary excretion of electrolytes were increased significantly after Sin Bee Tang water extract, 0.25ml/kg, administration. 3. Plasma cortisol concentration increased significantly after Sin Bee Tang water extract, 0.25ml/kg, administration. These results suggest that the therapeutic action of Sin Bee Tang for 'Su Chun (水喘)' has a relation with the increase of plasma cortisol and renal hemodynamic effect.
Proteinuria is an important risk factor for renal and cardiovascular disease. It is associated with a risk for glomerulonephritis, chronic kidney disease, and end-stage renal disease. Therefore, if persistent proteinuria is detected, kidney biopsy is considered to diagnose and treat the underlying disease. Recently, variants in the cubilin (CUBN) gene, which is associated with albuminuria, have been reported. This gene encodes cubilin, a membrane glycoprotein receptor expressed in the renal proximal tubules. Cubilin is a component of the megalin and cubilin-amnionless complex that mediates albumin reabsorption into the proximal tubules through endocytosis. A defect in cubilin leads to a reduction in albumin reuptake, resulting in albumin-dominant proteinuria. Although numerous controversies exist, several reports suggest that cubilin defects lead to proteinuria with a high portion of albuminuria but may not impair renal filtration function. If albuminuria due to reduced cubilin function is confirmed as a benign condition, we can consider using genetic studies to detect CUBN mutations in patients with proteinuria and they may not require any treatment or kidney biopsy. Here, we review recent papers on CUBN mutations and discuss the prognosis and management of individuals with this mutation.
The most common anatomic variant seen in donor kidneys for renal transplantation is the presence of multiple renal arteries, which can cause an increased risk of complications. Accessory renal arteries should be anastomosed to the proper source arteries to improve renal perfusion via the appropriate vascular reconstruction techniques. In microsurgery, 2 kinds of vascular augmentation methods, known as 'supercharging' and 'turbocharging,' have been introduced to ensure vascular perfusion in the transferred flap. Supercharging uses a distant source of the vessels, while turbocharging uses vascular sources within the same flap territory. These technical concepts can also be applied in renal transplantation, and in this report, we describe 2 patients who underwent procedures using supercharging and turbocharging. In one case, the ipsilateral deep inferior epigastric artery was transposed to the accessory renal artery (supercharging), and in the other case, the accessory renal artery was anastomosed to the corresponding main renal artery with a vascular graft (turbocharging). The transplanted kidneys showed good perfusion and proper function. No cases of renal failure, hypertension, rejection, or urologic complications were observed. These microsurgical techniques can be safely utilized for renal transplantation with donor kidneys that have multiple arteries with a lower complication rate and better outcome.
This study was performed in order to certify the antidiuretic action and to investigate the mechanism of antidiuretic action of debrisoquin infused into a renal artery in dog. Debrisoquin, when infused into a renal artery, exhibited the antidiuretic action accompanied the reductions of glomerular filtration rate and renal plasma flow, and the decreased amounts of sodium and potassium excreted in urine, limited only to the infused side, while control kidney function remained unchanged at all. The antidiuretic action of debrisoquin infused into a renal artery was blocked by pretreament of prazosin, $\alpha$$_1$-adrenergic blocking agent, or reserpine, catecholamine depleting agent. These results suggest that debrisoquin infused into a renal artery elicits antidiuretic action through indirect stimulation of renal sympathetic nerves.
Branchio-oto-renal syndrome (BOR) is a rare autosomal dominant disorder. The features include branchial cysts, hearing loss, ear malformation, preauricular pits, retrognathia, congenital heart disease, and renal abnormalities. However, anesthetic management of these patients has seldom been reported. We report a case in which general anesthesia was performed for dental treatment in a patient with BOR. Airway management, renal function, and hemodynamic changes can be of critical concern during anesthetic management. A 13-year-old girl diagnosed with BOR had severe right hearing loss, right external ear malformation, renal abnormalities, and postoperative patent ductus arteriosus (PDA). Dental extraction under general anesthesia was scheduled for a supernumerary tooth. The procedure was completed with sufficient urine volume, adequate airway management, and stable hemodynamics.
The actions of Atractylodes rhizoma alba on renal function of dog were investigated with water and methanol extract of it, utilizing clearance technique. Water extract elicited diuresis while methanol extract induced antidiuresis in the dog when they were given intravenously. The diuresis induced by the intravenous water extract seemed to be closely related to the decrement of reabsorption of electrolytes in renal tubules. On the contrary, the antidiuresis by intravenous methanol extract appeared to be related to the increment of renal reabsorption of sodium. Water and methanol extract, when infused into a renal artery, exhibited identical resposes to the intravenous actions confined only to the infused kidney, inferring that renal actions of water and methanol extract are not mediated by any endognous humoral extract are not mediated by any endogenous humoral agents. From these observations it is concluded that diuretic action of water extract and antidiuretic action of methanol extract were brought about by direct action in the renal tubules.
