개심술시 체외순환이 신장기능에 미치는 영향 (Effect of the Extracorporeal Circulation on Renal Function in Adult Open Heart Patients)

  • Renal dysfunction is a common complication of open-heart surgery: a form of controlled hemorrhagic shock, and successful perioperative management of renal dysfunction depends on recognition of the risk factors and optimal management of factors influencing renal function, including cardiopulmonary bypass, and early detection of renal failure. Changes in renal functional parameters including Ccr, Cosm, CH2O, FENa, and RFI were observed prospectively in forty five patients operated on at Dept. of Thoracic and Cardiovascular Surgery, S.N.U.H., from April to June, 1985. They were 23 males and 22 females with 35 acquired and 10 congenital heart diseases and the mean age and body surface area of them were 38.010.3 years [22-63] and 1.5518 M2[1.151.92] respectively. Followings are the conclusion. 1. The Ccr, representative of renal function, is significantly improved from 90.231.3 ml/min/M2 preoperatively to 101.536.4 ml/min/M2 postoperative and day [P<0.05], and all patients were classified as postoperative renal functional class I of Abel, which representing adequate renal protection during our cardiopulmonary bypass. 2. The Cosm is significantly elevated at immediate postperfusion time and remained high at postoperative one day representing osmotic diuresis at that time, but CH2O shows no significant changes at immediate postperfusion period and is decreased significantly at postoperative one day, representing recovery of renal concentrating ability at that time with decreasing urine flow. 3. The absolute value and changing tendency in FENa and RFI during perioperative period shows no diagnostic reliability on these parameters, but those of CH2O appear to reveal future renal function more accurately than Ccr 4. The depth of hypothermia may be protective upon renal function against the ill effects of prolonged nonpulsatile cardiopulmonary bypass. 5. The depth of the hypothermia, pump time of more than 150 minutes, poor cardiac function, and intraoperative events such as embolism appear to be related with immediate postperfusion renal function. 6. Hemoglobinuria and hemolysis, poor preoperative renal function, history of cardiac surgery, and massive transfusion associated with bleeding appear not to be related with renal dysfunction.

급성 신부전을 보인 아세트아미노펜에 의한 급성 간질성 신염 1례 (A Case of Acetaminophen-induced Acute Interstitial Nephritis Presenting with Acute Renal Failure)

  • Acute interstital nephritis can occur by acetaminophen, but it is rarely presented as acute renal failure with azotemia. We report a case of acute interstitial nephritis induced by acetaminophen in a 14-year-old girl who developed non-oliguric acute renal failure. She has taken acetaminophen to control the persistent throat pain for the last two months. Renal biopsy revealed diffuse infiltration of mononuclear inflammatory cells admixed with eosinophils in the edematous interstitia. After the discontinuation of acetaminophen and the administration of corticosteroid, the serum creatinine level returned to normal.

  • The Laurence Moon-Bardet Biedl syndrome is characterized by obesity, mental retardation, visual impairment with retinitis pigmentosa, polydactyly, hypogonadism and renal manifestations. We experienced an 11 years old female with Laurence Moon-Baret Biedl syndrome associated chronic renal failure. She was diagnosed to have LMB syndrom according to the clinical manifestations of polydactyly on hands and feet, mental retardation, obesity, retinitis pigmentosa and chronic renal failure. She is on maintenance hemodialysis now.

  • An urinoma(uriniferous pseudocyst, pararenal pseudocyst) denotes an encapsulated collection of urine in the perirenal or paraureteral space. It was usually reported in relation to trauma and acquired obstructive uropathy but rarely reported in renal infection including renal candidiasis. The mechanism is believed due to rupture of fornix through weakened portion of suppurated kidney and pyelosinus backflow by increased intrapelvic pressure in obstructive uropathy and fungus ball obstruction. We report a case of urinoma in a premature as the first case in Korea which developed as a complication of renal candidiasis.

도축한우의 신장에 대한 병리학적 연구 (Abattoir survey of kidney lesions in Korean cattle)

  • To examine the prevalence and types of kidney lesions in Korean cattle, a survey was carried out at a abattoir in september 2000 and January 2001. The collected kidneys were examined grossly and histopathologically and investigated for the patterns of renal diseases with considering season and sex. Of 735 cattle (99 bulls and 246 cows in September and 140 bulls, 3 steers, and 247 cows in January) surveyed, 301 (41%) cattle had various renal lesions. The most common prominent finding was focal or multiple interstitial nephritis (36.2%). Other lesions included focal or multiple renal cyst (9.1%), infarction (0.1%), hemorrhage (0.3%), and renal atrophy (0.3%). Microscopically, main pathological lesions were acute or chronic interstitial nephritis with moderate infiltration of neutrophil or lymphocyte, respectively. Finally, the kidney lesions are more or less correlated with the meat quality, especially in female Korean cattle.

  • Numerous cases of enamel renal syndrome have been previously reported. Various terms, such as enamel renal syndrome, amelogenesis imperfecta and gingival fibromatosis syndrome, and enamel-renal-gingival syndrome, have been used for patients presenting with the dental phenotype characteristic of this condition, nephrocalcinosis or nephrolithiasis, and gingival findings. This report describes a case of amelogenesis imperfecta of the enamel agenesis variety with nephrolithiasis in a 21-year-old male patient who complained of small teeth. The imaging modalities employed were conventional radiography, cone-beam computed tomography, and renal sonography. Such cases are first encountered by dentists, as other organ or metabolic diseases are generally hidden. Hence, cases of amelogenesis imperfecta should be subjected to advanced diagnostic modalities, incorporating both dental and medical criteria, in order to facilitate comprehensive long-term management.

Primary Subcapsular Reflux as an Etiology of Subcapsular Renal Abscess

  • Herein, we report two rare cases of renal infection. The first case was renal subcapsular urine reflux in a 8-month-old girl with recurrent urinary tract infection and the second was subcapsular abscess in a 14-year-old girl with diabetes, who was successfully treated with percutaneous drainage. It has been suggested that renal subcapsular abscesses could be caused by the direct reflux of urine into the subcapsular space, rather than spread of infection from an existing parenchymal lesion, and that complete recovery can be achieved if percutaneous drainage is performed in a timely manner. We propose primary subcapsular reflux, in which urine directly refluxes upwards into the subcapsular space of the kidney, as one of the mechanisms for development of renal subcapsular abscesses.

Two pediatric cases with hematuria, normal renal function and positive antineutrophil cytoplasmic antibodies

  • Antineutrophil cytoplasmic antibody (ANCA)-associated glomerulonephritis requires prompt diagnosis and treatment, since renal function at the time of diagnosis is significantly associated with renal outcomes. Here, we report two pediatric patients with ANCA-positive glomerulonephritis initially presenting with hematuria, mild proteinuria, and normal renal function. The first patient with a high myeloperoxidase-ANCA titer (>134 IU/mL) was diagnosed with rapidly progressive glomerulonephritis based on renal biopsy and treated with immunosuppressive therapy after 10 months of follow-up. The second patient with a low myeloperoxidase-ANCA titer (11 IU/mL) maintained normal kidney function without medication. Two cases showed different clinical course according to ANCA titer.

Nutcracker syndrome in children: review of symptom, diagnosis, and treatment

  • Renal nutcracker syndrome (NCS) is the entrapment of the left renal vein between the abdominal aorta and superior mesenteric artery. Although uncommon in pediatric patients, early diagnosis is crucial to avoid potential severe complications, such as anemia or renal vein thrombosis. NCS presents a variety of symptoms, most commonly including "Triade's symptoms"-hematuria, proteinuria, and flank pain. Diagnosis and treatment include invasive and noninvasive management, although due to a lack of pediatric clinical studies, management is widely variable. Conservative diagnosis and treatment are recommended as a first-line option for pediatric patients; however, invasive surgical treatment may be recommended based on symptom severity. This review aims to provide a comprehensive overview of NCS in children to better understand the widely variable incidence, occurrence, and management from early on to allow for early-onset management.

신질환 환아에서 초음파 골량측정법을 이용한 골상태 평가 (Assesment of Bone Strength Using a New Quantitative Ultrasound Device in Children with Renal Diseases)

  • 목 적 : 소아 신질환 환자들의 골상태 평가 및 골다공증의 유병률을 산출하고 스테로이드 치료 기간 및 용량과의 연관성을 알아본 후 예방과 치료를 위한 진료에 지침을 마련하고자 시행하였다. 방 법 1994년 이후 아주대병원 소아과에서 신장질환을 진단 받은 환자들을 대상으로 초음파 골량측정법으로 골밀도를 측정한 후 한국 소아표준골밀도치를 참조하여 골상태를 평가하였고 환자의 임상소견 및 생화학적 지표들의 평균값간에 차이가 있는 지를 알아보았다. 결 과 : 골밀도를 측정 받은 환자는 총44명이었다. 진단시 나이는 6.7$\pm$4.2세였고, 골밀도 측정시 역연령은 9.3$\pm$4.2세, 골연령은 8.2$\pm$4.6세로 골연령이 저하되어 있었다. 질환별로는 특발성 신증후군 24명(54.5%), Henoch Shonlein 신염 7명(15.9%), IgA 신병증 5명(13.6%), 역류성 신병증 2명(4.5%), 기타 5명(11.4%)이었다. 환아 13명(29%)의 골밀도가 감소되어 있었고 이중 5명(11%)은 골다공증 범주에 속하였다 질환별로는 특발성 신증후군 1명(4%), Henoch Shonlein 신염 2명(29%), 역류성 신병증 1명(50%), 기타 1명(23%)이었다. 정상, 골감소증, 골다공증 세 군으로 나누었을 때 골밀도 측정시점까지 스테로이드 투여기간은 각각 26.5$\pm$27.3개월, 32.1$\pm$39.6개월, 8.9$\pm$12.9개월이었고, 투여한 축적 용량은 221.7 $\pm$248.6 mg/kg, 177.1 $\pm$302.9 mg/kg, 49.9$\pm$45.5 mg/kg이었으며 세 군간에 유의한 차이는 없었다. 스테로이드의 투여기간을 장기간(6개월이상)과 단기간(6개월 미만)으로 나누어 비교했을 때 골밀도 측정값 및 골관련 생화학적 수치들은 차이가 없었다.결 론 : 초음파 골량측정법이 보편화되지 않았지만 정상 참고치와 비교가 가능하였으며 측정결과 골감소증은 29%, 골다공증은 11%로 상당수에서 골상태가 비정상임을 알 수 있었다. 본 연구에서는 스테로이드 사용기간 및 축적용량이 골상내의 변화에 특별한 영향을 미치지 않는 것으로 나타났다. 동일 환자들의 골상태의 변화관찰과 신질환 관련 골감소의 요인을 밝혀내기 위한 추가적인 연구가 필요할 것으로 사료된다.
