신성 고혈압 백서에서 Atrial Natriuretic Peptide의 신장기능에 미치는 효과 (Effect of Atrial Natriuretic Peptide on the Renal Function in Two-Kidney One-Clip Hypertensive Rats)

  • Since the atrial receptor was suggested to be involved in the control of extracellular fluid volume, it has been shown that the granularity of atrial cardiocytes can be changed by water and salt depletion, and that an extract of atrial tissue, when injected intravenously into anesthetized rats, causes a large and rapid increase in renal excretions of sodium and water. The immunoreactive atrial natriuretic peptide (ANP) has been found in the plasma of patients suffering from various cardiovascular diseases. A high level of ANP in the plasma has been reported in essential hypertension. Several studies on the effects of ANP on renal function and arterial blood pressure have presented contradictory results showing attenuated or accentuated responses. Thus, involvement of the ANP in the development of hypertension remains unresolved. Present study was undertaken to investigate whether the ANP is involved in the development of hypertension in two-kidney one-clip Goldblatt hypertensive rats. The plasma concentration of immunoreactive ANP appeared to be significantly elevated in hypertensive rats as compared with normotensive Goldblatt operated and sham-operated rats. Plasma renin concentration was higher in hypertensive rats than in normotensive rats, as observed in earlier experiments. Intravenous infusions of ANP resulted in increases of urine flow and urinary excretions of sodium and potassium in both hypertensive and normotensive rats. The renal response to ANP was markedly accentuated in Goldblatt hypertensive rats. The plasma concentration of ANP showed a linear relationship with the arterial blood pressure. Infusions of ANP reduced blood pressure both in hypertensive and normotensive rats. These results suggest that in Goldblatt hypertensive rats an elevation of ANP level in the plasma may not be a cause, but instead a consequence of hypertension, and that the renal responsiveness to the ANP is accentuated by some unknown mechanisms.

Evaluation of Renal Function Using the Level of Neutrophil Gelatinase-Associated Lipocalin is Not Predictive of Nephrotoxicity Associated with Cisplatin-Based Chemotherapy

  • Background: For early detection of renal damage during the usage of cisplatin based chemotherapy, changes in renal function should be monitored carefully. In recent years, neutrophil gelatinase-associated lipocalin, a small polypeptide molecule, has shown promise as a marker of acute renal failure. The aim of this present study was to assess possible risk prediction of cisplatin-induced nephrotoxicity using serum NGAL. Materials and Methods: A total of 34 consecutive patients with documented serum creatinine at least 24 hours before every cycle of cisplatin-based chemotherapy were included in the study. Demographic and medical data including age, performance status, tumor characteristics and comorbid diseases were collected from medical charts. Renal function was evaluated at least 48 hours before the treatment and at the end of the treatment based on the Modification of Diet in Renal Disease (MDRD) formula. Before and after cisplatin infusion serum NGAL levels were measured for the first and 3rd cycles of chemotherapy. Results: The median age of the study population was 54 (32-70) years. Fifteen patients (41.1%) were treated on an adjuvant basis, whereas 19 patients (58.9%) were treated for metastatic disease. There was no correlation of serum NGAL levels with serum creatinine (r=0.20, p=0.26) and MDRD (r=-0.12, p=0.50) and creatinine clearance-Cockcroft-Gault (r=-0.22, p=0.22) after cisplatin infusion at the end of the 3rd cycle of chemotherapy. Conclusions: In our study, serum NGAL levels were not correlated with the cisplatin induced nephrotoxicity. Further prospective studies are needed to conclude that serum NGAL level is not a good surrogate marker to predict early cisplatin induced nephrotoxicity.

소아에서 신피막 외 침범을 동반한 신장 농양 2례 (Two Cases of Renal and Perinephric Abscesses in Children)

  • 신장 농양은 요로감염에 의한 신장 감염의 가장 심한 상태로 신주위로 파열되어 신장주위농양으로 진행할 수 있으며, 신장 흉터의 후유증을 남길 수 있다. 대개 항생제 치료가 지연되거나 적절한 항생제를 선택하지 못했을 때 발생할 수 있으며, 소아에서는 해부학적 요로계 기형을 동반하기도 한다. 저자들은 방광요관역류가 동반된 대장균에 의한 요로감염 환아에서 발열 초기에 민감한 항생제로 치료하였음에도 불구하고, 좌측 요근과 신피막 주위의 침윤을 동반한 신장 농양으로 진행하여 장기간의 항생제와 함께 경피적 흡인으로 치료된 1례와 해부학적 기형 없이 2주간 발열이 있었던 환아에서 포도알균에 의한 비장과 횡경막을 침범한 다격벽의 신장 농양이 확인되어 항생제 치료와 함께 도관삽입 후 배농하여 치료하였으나 이후 추적관찰에서 신장 흉터가 발생하였던 1례를 경험하였기에 이를 보고 하는 바이다.

신장 CT 검사와 99mTc-DMSA Renal Scan 검사에서 측정한 신장 길이의 차이 및 상관분석 (Discrepancy and Correlation in the renal length between Kidney CT and 99mTc-DMSA Renal scan)

  • 핵의학 영상 검사 중에서 99mTc-DMSA Renal scan은 비침습적이면서도 신장의 기능과 형태를 동시에 평가할 수 있으며 좌우 양측 신장에 섭취를 분석함으로써 신기능을 정량적이면서 정확하게 분석할 수 있다. 신장의 크기 및 길이 평가도 신장 이상의 주요 평가 항목이며, 99mTc-DMSA Renal scan을 이용한 신장 길이의 측정은 호흡에 의한 흔들림이나 해상력의 한계로 인해서 CT 보다 신장의 길이가 과대 또는 과소평가 될 수 있다. 본 연구는 해부학적인 평가가 우수한 CT 검사와 기능적인 평가가 우수한 99mTc-DMSA Renal scan에서 측정한 신장 길이의 차이 및 상관관계를 분석하고, 99mTc-DMSA Renal scan에서 측정한 신장 길이의 정확성을 예측하는데 그 목적이 있었다. 신장 CT 검사와 99mTc-DMSA Renal scan을 검사한 200명의 환자 자료를 대상으로 연구 분석하였고, 통계 분석을 위해 SPSS Ver. 17.0 프로그램을 사용하였다. 호흡에 의한 영향과 해상력의 차이에 의해서 CT 검사 보다 99mTc-DMSA Renal scan에서 측정한 신장의 길이가 더 큰 것으로 분석되었다. 그러나 감마카메라와 연령에 따른 신장 길의 차이는 없었으나 신장 길이를 분석한 방사선사 별로는 차이가 있었다. 따라서 신장 길이 측정에서 방법의 표준화를 통한 정량 분석 결과의 일반화는 매우 중요할 것이며, 이는 99mTc-DMSA Renal scan에서 츠정한 신장 길이의 정확성을 향상할 수 있을 것이다. 또한 CT 영상과 99mTc-DMSA Renal scan에서 측정한 신장길이는 좌우측 신장 모두 매우 강한 양(+)의 상관관계가 있었다. 다수의 신장 질환에서 신기능 변화뿐만 아니라 길이나 크기가 변화하는 경우가 많으며 신장 길이를 정확하게 측정하는 방법과 표준화는 임상적인 진단에 매우 중요할 것으로 판단된다. 특히 소아의 경우에서는 다량의 방사선 피폭이 있는 CT 검사보다 99mTc-DMSA Renal scan을 이용하여 형태학적인 평가와 기능적인 평가를 모두 시행하는 경우 많으므로 99mTc-DMSA Renal scan에서 신장에 대한 기능적인 정보뿐만 아니라 크기나 길이, 위치 등과 같은 해부학적 정보를 정확하게 제공하는 것은 더욱 중요할 것으로 판단된다.

고령의 만성콩팥병 환자에서 발생한 초기 신경학적 악화가 동반된 후방순환뇌경색 치험 1례 (A Case Report on Posterior Circulation Cerebral Infarction with Early Neurological Deterioration in an Elderly Patient with Chronic Kidney Disease)

  • Objectives: This study investigated the effect of Korean medicine on an elderly patient with posterior circulation cerebral infarction, chronic kidney disease (CKD), and early neurological deterioration (END). Methods: The patient, who already had CKD, was treated with Korean medicine, comprising herbal medicine, acupuncture, moxa, and cupping combined with Western medicine (antiplatelet, diabetes) and physical therapy. A manual muscle test (MMT) and a modified Barthel index (MBI) were used to observe the treatment effects, and blood tests were performed to check estimated glomerular filtration rate (eGFR), creatinine and blood urea nitrogen (BUN), which represent renal function. Results: After the treatment, MMT, MBI, and renal function scores had increased. Conclusions: This study suggests that Korean medicine can effectively treat posterior circulation cerebral infarction with END in CKD, but further studies should be conducted.

  • Along with a development of medical technology, a variety of tests, such as laboratory tests, x-ray and endoscopies are being used in health screening tests. As the tests determine the quality of health screening, test items and methods should be carefully selected. This study was to get hold of the test items of major health screening programs in Korea Most of the health screening programmes focused upon detection of risk factors and diagnosis of life - style related diseases (diabetes, hypertension, cardiovascular diseases, hypercholesterolemia, overweight, drinking, smoking, cerebrovascular diseases, osteoporosis) ,cancers (stomach, cervix, lung, breast, liver, colon, prostate, ovary, pancreas, thyroid, esophagus) , infectious diseases (hepatitis, tuberculosis, sexually-transmitted diseases, parasites) , chronic obstructive respiratory diseases, chronic renal diseases (bacteriuria hematuria, proteinuria) , anemia, glaucoma, hearing loss, Alzheimer disease, stress, early psychiatric diseases. The health screening tests were basic physical examination, basic laboratory tests (CBC, urinalysis, liver function tests, lipid tests, glucose, HbAlc, uric acid, electrolytes, serological tests (HBsAg, HBs-Ab, HCV-Ab, HIV-Ab, VDRL) EKG, x-ray (chest PA, CT) , endoscopy (gastroscopy, colonoscopy) , sonography(abdomen, thyroid, pelvis, breast) ,cytology (cervix) , bone density, tumor markets (NMP22, alpha-FP, CEA, CA-19-9, CA12S, PSA) and eye tests. Advanced technologies, like CT, PET, MRI, MRT/Angio, molecular testings) were widely used in hospital health screening programmes .In summary, a variety of tests were utilized in health screening in Korea. Those tests were utilized by stages or according to sex and age in most of health screening programmes, however a few program used tests to excess disregarding health screening subjects.
