편측 후방 치아 잔존시 의치 설계에 따른 지대치지지 조직과 잔존 치조제의 응력 분석에 관한 연구 (STRESS ANALYSIS AT SUPPORTING TISSUE OF ABUTMENT TEETH AND RESIDUAL RIDGE ACCORDING TO DENTURE DESIGN WITH REMAINING UNILATERAL POSTERIOR TEETH)

  • This study was peformed to investigate the distribution and magnitude of stress at supporting tissue of abutment teeth and residual ridge tissue with remaining unilateral posterior teeth. Four types of removable partial dentures that included clasp retained removable partial denture, attachment retained removable partial denture, telescopic removable partial denture, and swing-lock partial denture were designed, and strain gauge was used for stress analysis. Each prosthesis was subjected to simulated vertical and oblique load. The following conclusions were drawn from this study. 1. The clasp retained removable partial denture generally distributed simulated vertical force more evenly to the supporting structure. 2. The stress at buccal side of 1st premolar was the lowest in swing-lock partial denture and that was highest in attchment retained removable partial denture. The stress at lingual side of 1st premolar was the lowest in telescopic partial denture. 3. In clasp retained removable partial denture, stress was lower at load site and ridge crest at mid-line, but it was higher at 1st premolar area on vertical load. 4. In attachment removable partial denture, stresses at buccal side of 1st premolar. lingual side of 1st premolar on vertical load, and ridge crest at midline on oblique load were higher. 5. In telescopic removable partial denture, stress at lingual side of 1st premolar was the least in all removable partial dentures, but the stress at load site was higher. 6. In swing-lock removable partial denture, stress at buccal side of 1st premolar was the lowest, and stresses at load site and distal end of residual ridge crest were higher.

한국인의 부분 치아 결손 증례와 국소의치 설계에 관한 연구 (A SURVEY OF PARTIAL EDENTULISM AND REMOVABLE PARTIAL DENTURE DESIGNS FOR PATIENTS IN KOREA)

  • There were many studies that distribute the partial edentulous states and examine the removable partial denture designs in the planning of removable partial denture treatment. This study was performed in the point of removable partial denture prescription to evaluate partial edentulism and its removable partial denture designs. The data was collected from the dental laboratory of each three dental colleges in Seoul and from two dental laboratories only for removable partial dentures as a prescription form. A total of 1411 cases with prescription form collected from dental laboratories were distributed for this study, then 788 cases were selected for this study. The case selection was done according to the contents of prescription form. The selected cases were divided into maxillary arch and mandibular and classified in terms of types of major connector and direct retainer, unbroken anterior teeth, Kennedy classification, the number of remaining teeth, and distribution of age and sex. The analyzed results were as follows : 1. The Kennedy classification I showed highest frequency both in maxilla and mandible. 2. The arch distribution of removable partial denture was 50.08% for maxilla and 49.92% for mandible. 3. The highest frequency in the distribution of direct retainer was the RPA clasp design. 4. The frequency of unbroken anterior 6 was 73.36% for maxilla and 82.30% for mandible. 5. The design of broad palatal strap and lingual bar revealed the highest prevalence in the major connector construction. 6. The mean number of remaining teeth per arch was 8.25 for maxilla and 8.37 for mandible. 7. The mean age of the patients supplied with removable partial denture was 52.25 years for men, 51.68 years for women, 52.11 years for maxilla, and 51.76 years for mandible and women showed more prevalence.

치아지지및 유리단 국소의치의 설계원칙 (THE PRINCIPLE OF THE TOOTH-BORNE AND FREE-END REMOVABLE PARTIAL DENTURE DESIGN)

  • The Applegate-Kennedy classification, the drawing of removable partial denture design using color coding, the selective tissue placement impression method in case of Class I and Class II removable partial dentures, the design of the swing lock attachment of an alternative approach to conventional removable partial denture, the design of the intracoronal or extracoronal attachment, and the removable partial denture design using a rotational path were presented. The following conclusions from the above things were presented : 1. The swing lock attachment removable partial denture can be effective to an alternative approach when the design of conventional removable partial denture is improper with markedly mobile remaining teeth or missing key abutments. 2. Intracoronal or extracoronal attachments must be selected care-fully considering the conditions of the abutment teeth and alveolar ridge whether more occlusal loads to the abutment teeth or to the alveolar ridge are distributed. 3. It must be almost prerequisite that a functional impression is taken in case of Class I and class II removable partial dentures and in case of tooth-borne removable partial dentures, a removable partial denture using rotational path is strong, hygienic, esthetic, and can be accomplished successfully in the clinical aspect when it is properly designed and fabricated through the complete understanding of an indication and a principle. 4. All necessary informations must be achieved with carefully investigated surveying procedure according to each clinical case by Applegate-Kennedy classification which can be helpful and useful in the clinical application and it is important that dentists themselves must be in the habit of drawing a reasonable partial denture design using a color coding in the paper sheet.

임플란트를 이용한 국소의치 수복 (Implants in conjunction with removable partial denture)

  • The benefits of implant supported overdenture are readily apparent for the fully edentulous patients and have been well documented, however, there is deficiency of the studies regarding the combination of implants with removable partial dentures for partially edentulous patients. The purpose of this article is to review the literature concerning implants with removable partial dentures and evaluate the evidence for this clinical approach. Through many clinical case reports and studies we have searched from a broad variety of journals, we present the six considerations needed to contemplate respecting implants with removable partial denture in partially edentulous patients. First, the connection between abutment tooth and removable partial denture has to be rigid and the link between implant and removable partial denture should be hinged. Second, a mesial rest acts better in the point of force distribution for distal extension removable partial denture and splinting between implants is also a favorable choice. Third, T bar has an advantage for implants which are used as abutments in distal extension removable partial denture. Forth, as we all known functional impression is better way to reproduce movement for distal extension removable partial denture. Fifth, indirect retainer and guiding plane on the proximal surfaces of terminal abutment teeth are important in preventing denture base lifting. Sixth, implants in conjunction with removable partial denture is superior in the esthetic and phonetic as well as cost-effective point of view. We also suggest that which place we should install implants for force distribution and which diameter and length of implants should be used. in this review article, we recommend to locate the implant near of the abutment tooth for esthetics or near of first molar position for good stress distribution. The diameter and length of implant also influence to stress distribution. When we compare to conservative partial denture, patients go for removable partial denture using implants due to convenience, better support and retention according to several studies. But it is true that we need to study more on this subject and collect long term follow up cases before we discuss on it. So it is enough to bring this subject into the surface of prosthetic treatment by this article.

국부의치 설계에 관한 임상적 연구 (A CLINICAL EVALUATION ON THE DESIGN OF REMOVABLE PARTIAL DENTURE)

  • The purpose of this investigation was to evaluate the mouth preparation and design of removable partial dentures. A total of 187cases for the prefabricated partial denture frameworks in both maxillary and mandibular semi-dentulous situations (66 cases and 203 cases) was selected from this study. The evaluations of mouth preparation and design observed here involved the classification of edentulous spaces, status of abutment splinting with location, design of direct retainer and structure of maxillary major connector according to the incidence of both dental arches, ages, sexes and segment of semidentulousness. The analyzed results were as follows: 1) The order of frequency rate in removable partial denture construction was Class II (50.27%), Class I (36. 90%), Class III (10.69%), and Class IV (2.14 %). 2) The distribution on design of maxillary removable partial denture prosthesis was 33.22% and 64.11% in mandibular removable partial denture prosthesis. 3) The age distribution of removable partial denture prosthesis was prominent after40 years (41.71%). 4) The design pattern of maxillary major connectors was in order of anteroposterior bar, single palatal bar, palatal strap, U-shape connector. 5) The design pattern of direct retainer was in order of Aker's clasp, I-bar clasp, backaction clasp, cuspid universal clasp. 6) The abutment for partial denture clasp splinted between premolar and premolar and its frequency rate revealed 53.44%. 7) It seemed that the location and design of the indirect retainer showed accepatble limit.

상악 소수치 잔존 환자에서 임플란트를 이용한 가철성 부분틀니 수복 증례 (Rehabilitation of maxillary partial edentulous patients using implant assisted removable partial denture)

  • 부분 무치악 환자는 고정성 보철, 가철성 국소의치, 임플란트 지지 고정성 보철로 수복될 수 있다. 그러나 소수 잔존치아만 남은 경우, 고정성 보철로는 수복이 어려우며, 국소의치 혹은 임플란트 지지 고정성 보철을 고려하게 된다. 이 경우 임플란트 지지 고정성 보철은 고정성 수복이라는 장점이 있으나, 충분한 개수의 임플란트가 식립되어야 하며, 환자의 전신적, 국소적 여건이 합당해야 하고, 경제적 부담도 크다. 가철성 국소의치는 지대치에 의한 대칭적인 지지가 중요하며, 특히 중심선에 대해 편측으로 소수 치아만 남은 경우에는 지점선에 대해 지대치에 유해한 스트레스를 가하면서 지대치의 예후가 불량해진다. 이 경우, 전략적 위치에 임플란트를 식립함으로써 임플란트를 의치의 유지 및 지지 요소로 활용하는 가철성보철물을 설계한다면 비용 대비 기능적이며 심미적인 보철 수복이 가능해진다. 본 두 증례는 상악 Kennedy Class I 부분 무치악 환자 중 편측으로 소수 잔존치가 전치에 국한하여 존재하는 경우로, 잔존치를 지대치로 사용하면서, 전략적 위치에 임플란트를 식립함으로써 의치의 지지가 향상된 국소의치를 제작한 증례이다.

  • The purpose of this study was to evaluate the stress patterns developed in abutment and residual ridge according to removable denture design in case of remaining mandibular canines. The removable denture designs in this study were as foolows : 1. Removable partial denture with non-splinted abutment 2. Removable partial denture with splinted abutment 3. Overdenture with telescopic crown 4. Overdenture with O-Ring attachment 5. Overdenture with combination bar attachment Photoelastic stress analysis was used to record the isochromatic fringe patterns and to calculate principal stress components at measuring points. The results were as follows : 1. In case of removable partial denture with non-splinted abutment, stress of root area at the loaded side was the largest. No significant differences in stress of root area were observed between loaded side and unloaded side. 2. No significant differences in stress of residual ridge at the loaded side were observed between removable partial denture with splinted abutment and removable partial denture with non-splinted abutment. 3. In case of combination bar attachment retained overdenture, stress of root area was the largest and in case of telescopic crown retained overdenture, stress of root area was the lowest. 4. In case of attachment retained overdenture, stress of residual ridge was lower but stress of root area was larger than in case of removable partial denture.

STRAIN ON THE LABIAL PLATES AROUND ABUTMENTS SUPPORTING REMOVABLE PARTIAL DENTURES WITH VARIOUS PROSTHETIC DESIGNS: AN IN VITRO STUDY

  • Statement of problem. In distal extension removable partial denture, the preservation of health of abutment teeth is very important, but abutment teeth are subjected to unfavorable stress. Purpose. The purpose of this study was to investigate the biomechanical effects of mandibular removable partial dentures with various prosthetic designs using strain gauge analysis. Material and methods. Artificial teeth of both canines were anchored bilaterally in a mandibular edentulous model made of resin. Bilateral distal extension removable partial dentures with splinted and unsplinted abutments were fabricated. Group 1 : Clasp-retained mandibular removable partial denture with unsplinted abuhnents Group 2 : Clasp-retained mandibular removable partial denture with splinted abutments by 6-unit bridge Group 3 : Bar-retained mandibular removable partial denture Strain gauges were bonded on the labial plate of the mandibular resin model, approximately 2 mm close to the abutments. Two vertical experimental loadings (100N and 200N) were applied subsequently via two miniature load cells that were placed at mandibular first molar regions. Strain measurements were performed and simultaneously monitored from a computer connected to data acquisition system. For within-group evaluations, t-test was used to compare the strain values and for between-group comparisons, a one-way analysis of variance (ANOVA) was used and Duncan test was used as post hoc comparisons. Results. Strain values increased as the applied load increased from 100N to 200N for all groups (p<.05). The strain values of group 1 and 2 were tensile under loadings. In contrast, strain values of group 3 were compressive in nature. Under 100N loading, group 1 showed higher strain values than group 3 in absolute quantity (p<.05). Under 200N loading, group 3 showed higher strain values than group 1 and 2 in absolute quantity (p<.05). Group 1 showed higher strain values than group 2 (p<.05). Conclusion. Splinting of two isolated abutments by bridge reduced the peri-abutment strain in comparison with unsplinted abutments. Strain of bar-retained removable partial denture increased much more as applied load increased, but was compressive in nature.

An implant-supported removable partial denture for a patient with post-inflammatory scar contracture caused by burn complications: a clinical report

  • The scars and contracture around the oral-facial region may cause difficulty in prosthodontic treatment to restore esthetics and function for the patients, who suffered severe burns. This article presents a technique that uses a fixed partial denture prepared with a conventional milling technique and an attachment to support anterior cantilever removable partial denture, thereby providing a more esthetically acceptable and functional result.

Double crown retained RPD의 4년 임상 관찰 (DOUBLE CROWN RETAINED REMOVABLE PARTIAL DENTURE - 4 YEARS IN RETROSPECT)

  • Statment of problem : Little in known about the importance of selection of various double crowns as retainer in determining the outcome of treatment with double crown retained removable partial denture. Purpose : To obtain information about the effects and the results of this treatment modality. Material and methods : This study describes 61 double crown retained removable partial dentures worn by 51 patients from Samsung Medical Center, Seoul a time ranging between 6 and 48 months and evaluate occlusal contacts on the denture teeth, denture movement, incidence of denture relining, denture retention, fracture of dentures and abutments, hygiene, residual ridge inflammatory changes, number of lost abutment,. interruption of denture use. Results : One tenth of all the restorations were relined. Restorations in 21 arches fractured repeatedly for various reasons . There was no apparent interrelationship between fractures and the five groups. Conclusion : Good prognoses of removable partial dentures were shown in all groups(Kennedy Classes I, II, III, combination and few remaining abutment).
