Investigation of the Distance from One Tooth Center to Adjacent Tooth Center and Dental Arch Size to Develope New Removable Die System for Fabrication of Dental Prosthetic Appliance (치과보철물 제조용 모형 조립체(가철식 치형 시스템) 개발을 위한 인접 치아 중심 간격 및 악궁 크기 조사)

  • To obtain the information of dental arch size and the distance from one tooth center to adjacent tooth center of occlusal surface of each tooth which is perforated by Pindex system on working cast for removable die system, 600 dental casts in Busan were examined. The distance of center of occlusal surface of each tooth and dental arch size were digitized. The results were as follows; 1. Mean values of the distance from center of maxillary central incisor to maxillary lateral incisor(tooth number 11$\sim$12, 21$\sim$22) is 5,7 mm, 12$\sim$13(22$\sim$23) is 5.9 mm, 13$\sim$14(23$\sim$24) is 6.9 mm, 14$\sim$15(24$\sim$25) is 7.1 mm, 15$\sim$16(25$\sim$26) is 8.4 mm, 16$\sim$17(26$\sim$27) is 10.2 mm, 11$\sim$21 is 7.30 mm. Mean values of the distance from center of mandibular central incisor to mandibular lateral incisor(tooth number 31$\sim$32, 41$\sim$42) is 4.5 mm, 32$\sim$33(42$\sim$43) is 4.8 mm, 33$\sim$34(43$\sim$44) is 6.3 mm, 34$\sim$35(44$\sim$45) is 7.2 mm, 35$\sim$36(45$\sim$46) is 9.2 mm, 36$\sim$37(46$\sim$47) is 10.7 mm, 31$\sim$41 is 4.7 mm. 2. Mean values of the distance from the center of maxillary right central incisor to the center of maxillary left central incisor(11$\sim$21) is 7.3 mm, 12$\sim$22 is 18.2 mm, 13$\sim$23 is 26.9 mm, 14$\sim$24 is 37.2 mm, 15$\sim$25 is 43.2 mm, 16$\sim$26 is 48.5 mm, 17$\sim$27 is 53.5 mm. Mean values of the distance from the center of mandibular right central incisor to the center of mandibular left central incisor(31$\sim$41) is 4.7 mm, 32$\sim$42 is 13.3 mm, 33$\sim$43 is 21.7 mm, 34$\sim$44 is 31.9 mm, 35$\sim$45 is 38.2 mm, 36$\sim$46 is 44.8 mm, 37$\sim$47 is 50.7 mm. 3. Mean values of the distance from the line of between 11$\sim$21 to 12$\sim$22 is 10.9 mm, 12$\sim$22 to 13$\sim$23 is 8.7 mm, 13$\sim$23 to 14$\sim$24 is 10.3 mm, 14$\sim$24 to 15$\sim$25 is 6.0 mm, 15$\sim$25 to 16$\sim$26 is 5.3 mm, 16$\sim$26 to 17$\sim$27 is 5.0 mm. 31$\sim$41 to 32$\sim$42 is 8.6 mm, 32$\sim$42 to 33$\sim$43 is 8.4 mm, 33$\sim$43 to 34$\sim$44 is 10.2 mm, 34$\sim$44 to 35$\sim$45 is 6.3 mm, 35$\sim$45 to 36$\sim$46 is 6.6 mm, 36$\sim$46 to 37$\sim$47 is 5.9 mm. 4. We checked the bottom side of cast to verify the position of dowel pin. There is no difference upper side and lower side.

  • Recently, in proportion to the remarkable development of dentistry and income increases it is growing more and more a concern about changed awareness in appearance. In this study, it had a grasp of the purpose for university hospital visiting, the motive of the commencement for orthodontic treatment and a method of the treatment as well as term. Based on these, the aim of this study is to keep more of the information between parent and doctors for mutual understanding and to grasp the characteristics for the needs of orthodontic treatment. In order to conduct researches, there has made a survey of 150 persons among orthodontic patients' parents who visit CBNU hospital, the pediatric dentistry. The study has found the results like these. 1. There was a question about the reason to visit CBNU hospital in the department of pediatric dentistry for orthodontic treatment. 52.1% of respondents, the survey found, were more likely to receive a good medical service. 25% of them were counselled from a relative or an acquaintance. 16.7% of them were recommended by another dental clinic. 2. There was a question about the expected orthodontic treatment period, when at first hospital visiting. 37.5% of the respondents answered that it was a 'more than 2 years', 12.5% of them said 'from 12 months to 18 months'. 3. There was a question about the reason to receive orthodontic treatment. 58.3% of the respondents, the survey found, answered the reason was parents' concern about the malocclusion of their children, 12.5% of them said a the orthodontic problem pointed out by entourages. 4. There was a question about the method of orthodontic treatment for patients who visit the department in pediatric dentistry. 41.7% of the respondents said that it was used as 'an intra-oral fixed appliance', 29.2% of them said 'an intra-oral removable appliance', 2.1% of them said 'an extra-oral appliance', 5. There was a question about the waiting time for treatment after a dental appointment. 60.4% of the respondents said 'from 5 minutes to 10 minutes', 4.1% of them said 'from 15 minutes to 30 minutes'. There was a question about the extent of an acceptable waiting time. It was answered to 'from 5 minutes to 10 minutes' by 60.4% of them, 2.1% of them said 'from 15 minutes to 30 minutes'.

ORTHODONTIC TRACTION OF HORIZONTALLY ERUPTED LOWER LATERAL INCISOR ON THE LINGUAL SIDE (설측으로 수평 맹출한 하악 측절치의 교정적 견인)

  • Tooth eruption is the movement of the tooth from the developing place in the alveolar bone to the functional position in the oral cavity. The permanent incisors originate from the dental lamina on the lingual side of preceding deciduous tooth and erupt to the level of the occlusion through the well developed gubernacular cord. Ectopic eruption is a developmental disturbance in the eruption pattern of the permanent dentition. Most of the ectopically erupted lower incisor has been found in lingual side. The ectopically erupted tooth could be repositioned by orthodontic force in the early mixed dentition, which could help preventing the problems of loss of space and the lingual tilting of the lower anterior teeth. An eight-year-old girl visited the department of pediatric dentistry, Yonsei Dental University Hospital, for the evaluation and the treatment of the lower right lateral incisor, which was horizontally erupted in the lingual side, parallel to the mouth floor. Her tongue was placed on the labial side of that tooth. There was no previous dental history of dental caries or trauma on the pre-occupied primary incisor. Clinical and radiographic examinations including the computed tomography(CT), showed no evidence of dilacerations on root. Therefore, we decided to start active orthodontic traction of the lower right lateral incisor. We designed the fixed type of buccal arch wire and the lip bumper with hook for the traction. Button was attached to the lingual side of the ectopically positioned tooth. Elastic was used between the appliance and the button on that tooth. After the tooth become upright over the tongue level, appliance was change to the removable type and periodic check-up with occlusal guidance was followed to monitor the position of the tooth. In this case using the fixed appliance with modified form of lip bumper and hook embedded in acrylic part instead of extraction was very efficient up-righting the ectopically erupted tooth toward the occlusal plane.

Oral Features in a Child with Noonan Syndrome : A Case Report (누난 증후군 환자의 구강 내 특징 : 증례 보고)

  • Noonan syndrome is characterized by distinctive facial features, short stature, and congenital heart disease. It is a congenital genetic disorder with a prevalence of between 1/1,000 and 1/2,500 in both genders. An 11-year-old boy with Noonan syndrome visited the hospital with an ectopically positioned tooth. A pulmonary stenosis was diagnosed and his growth and development were delayed. In many cases of this diseases there is obvious hemostasis, which he was not experiencing. His facial appearance showed characteristic features of Noonan syndrome. The patient showed a dental class II relationship, labioversion of the upper anterior teeth, and a shallow overbite. Radiographic examination revealed that the upper right canine was ectopically positioned, which led to root resorption of the upper right lateral incisor. A lateral cephalometric radiograph revealed a craniofacial pattern that was within normal limits. Surgical opening and button attachment on the impacted upper right canine were performed and traction was applied on the impacted tooth using a removable appliance. This patient was mildly affected by Noonan syndrome and showed some dental problems. However, few studies have reported the oral characteristics of Noonan syndrome despite its high incidence. Thus, this case report describes the oral features and management of Noonan syndrome.

ORTHODONTIC TRACTION OF TRAUMATICALLY INTRUDED TEETH : CASE REPORT (외상에 의해 함입된 치아의 교정적 견인을 통한 치험례)

  • Traumatic injury of tooth in children is commonly occurred problem. It is classified into tooth, periodontal tissue, supporting bone, soft tissue injury by it's area and extent. Among the periodontal tissue injuries, traumatically intruded teeth are common in anterior maxillary area, though the occurrence rate is rather low, the pulp and supporting tissue injury is possible by vertical impact. The treatment method of traumatically intruded teeth is various. Observation on the spontaneous reeruption for 3-4 weeks is recommended if the traumatized teeth are deciduous teeth or slightly intruded immature permanent anterior teeth. If this did not occur because the extent of intrusion is severe or the traumatized teeth are mature permanent anterior teeth, orthodontic traction is applied by fixed/removable appliances. At this time, light and continuous force is applied for the extrusive movement of the intruded teeth. When above procedures are impossible, surgical repositioning and fixation is recommended. In these cases, we performed conventional endodontic therapy for pulp necrosis and orthodontic traction with fixed appliance. We obtained satisfactory results and will report that.

  • Ectopic eruption means the eruption of the tooth in an abnormal position due to multiple factors, which found most frequently in maxillary fist permanent molars, mandibular lateral incisors and maxillary permanent canines. Ectopic eruption of the maxillary first permanent molar occurs when the molar erupts with a more mesial angulation than normal, and locks itself in an atypical resorption on the distobuccal root of the second primary molar. The maxillary first permanent molar plays important roles for mastication and occlusion, so ectopically erupted maxillary first permanent molars should be relocated into proper position. Treatment options are separation by insertion of the brass wire or elastic rings, preparation of distal aspect of the maxillary second primary molar, using fixed or removable appliance with finger spring, and placement of space maintainer or space regainer after extraction of the maxillary second primary molar. We report three cases treated of ectopically erupted maxillary first permanent molar by re-setting of stainless steel crowns, placement of brass wire and using active plate. We could find out distal movement of maxillary first permanent molars into proper position and normal occlusion.

Self-inflicted Tongue Ulceration in a Patient with Tourette Syndrome: A Case Report (뚜렛 증후군 환자에서 자해로 인한 혀 손상 : 증례 보고)

  • Tourette's syndrome is a chronic neuropsychiatric disorder characterized by the presence of vocal and multiple motor tics. Tics are defined as brief, intermittent, repetitive, unpredictable, purposeless, and stereotyped movements or sounds. Some patients experience physical pain from intense and complex tics. In addition, motor tics can result in self-injury which is a common feature of Tourette's syndrome. A 9-year-old boy was referred by the department of neuropsychiatry because of a severe tongue laceration. His parents reported that he had been biting his tongue irregularly for 2 months before referral and suffered from an intense burning sensation. The repeated biting resulted in ulcers on the tongue, which quickly worsened and led to progressive difficulty chewing and swallowing food. We offered to give him a two-piece removable appliance to limit tongue biting; it was made of soft silicone and fitted to both the maxillary and mandibular arches. As we emphasized that the device could help alleviate his pain, he agreed to accept it and adapted well. Just 3 weeks later, his tongue lesions had healed significantly.

EFFECTS OF POLYPHOSPHATE MIXED IN ACRYLIC RESIN ON THE ATTACHMENT AND GROWTH OF ORAL BACTERIA (Acrylic Resin에 혼합된 Polyphosphate가 미생물의 부착 및 성장에 미치는 영향)

  • The purpose of this present study was to develop a new way of self-curing acrylic resin, using commercially available polyphosphate, Calgon, which is known to be antimicrobial and safe. For the study, polyphosphate(polyP) was blended with acrylic powder and devided into four groups as follows: no polyP(control), 1% polyP, 2% polyP, and 3% polyP. For the experiment, Streptococcus mutans GS5, Streptococcus sobrinus 6715, Streptococcus gordonii G9B and Challis, Porphyromonas gingivalis 2561, and Candida albicans ATCC 90027 were used. Resin specimens in each group were tested in vitro for the purpose of investigating the effect of polyP on the microbial attachment, growth and hydrophobicity of the resin surface. The results were as follows. 1. PolyP added to the acrylic resin decreased attachment of S. mutans GS5, S. sobrinus 6715, S. gordonii G9B. The greater binding inhibition was found in acrylic resin polymerized with polyP at higher concentrations. 2. The addition of polyP to acrylic resin failed to significantly affect the growth of the tested microorganisms. 3. The addition of polyP to acrylic resin seemed to reduce hydrophobicity of the acrylic resin. PolyP in acrylic resin does not seem to exert a direct antibacterial activity, but rather inhibit attachment of oral bacteria, especially mutans streptococci to saliva-coated acrylic resin. The acrylic resin reduces attachment of streptococci may be due to the decreased hydrophobicity caused by polyP added to the resin. PolyP may be included to acrylic resin to inhibit dental caries which often occurs when removable acrylic resin appliance is placed.

MAXILLARY MOLAR DISTALIZATION WITH THE BONE-SUPPORTED PENDULUM (Bone-supported pendulum을 이용한 상악대구치 원심이동)

  • To distalize the maxillary molars, the traditional techniques such as extra-oral traction, Wilson distalizing arches, removable spring appliances and Schwarz plate-type appliances have been used. But, these need considerable patient cooperation. For minimal patient compliance, many practitioners use the pendulum appliances. Several clinical studies demonstrated pendulum is effective molar distalization appliance in the growing patient(using the premolars and the palate as anchorage). But unfortunately, maxillary anterior teeth also shift mesially as the molar moves distally. As a result anchorage loss is occurred. To overcome these disadvantages, we used bone-supported pendulum, combined the conventional pendulum with Skeletal Anchorage System(SAS). The miniscrew was implanted in the anterior paramedian region of the median palatal suture, which has comparatively sufficient bone thickness and is low risk to damage on the dental follicles. We report three cases, using bone-supported pendulum for the maxillary molar distalization in children. After treatment, we find out anchorage stability, minimal unfavorable anterior tooth movement and sufficient molar distalization.

THE DEVELOPMENT OF INDWELLING WIRELESS PH TELEMETRY OF INTRAORAL ACIDITY (구강 내 산도의 생체 내 측정을 위한 wireless pH telemetry의 개발)

  • The purpose of this study was to develop the wireless pH telemetry lasting longer than 24 hours in the mouth to overcome the limits of conventional wire telemetry previously used for salivary and plaque pH measurement, and to assess its effectiveness. We developed a wireless pH telemeter which can measure and store the pH profile data during more than 24 hours. It was composed of intraoral part; pH sensor of antimony electrode, battery and microprocessor for data storage, and extraoral part; control/data receiver and data analyzing software which was newly made for this device. After inspecting wireless electrode for accurate measurement, it was attached to the removable intraoral appliance and delivered to the volunteer who was told to wear except brushing time, retrieved after 24 hours and finally the pH profile data was extracted and analyzed. When compared with conventional wire telemetry, this device showed similar results and induced less discomfort to examinees. The data showed pH changes at same time when examinees ate various scheduled foods and beverages. With this method it became possible to accurately measure pH changes within mouth for long time in accordance with individual's lifestyle, definitely reducing the discomfort inflicted to the examinees' life.
