한국 침구 치료 현황 파악을 위한 설문조사 (A Survey of Actual State of Treatment with Acupuncture and Moxibustion in Korea)

  • Objectives : This survey was accomplished to find out how Korean medical doctors take acupuncture treatment in real clinics. Methods : The survey questions were developed by the consensus from the professors who major in acupuncture and moxibustion. The questionnaire was given the 2731 Korean medical doctors at the mending education site; In addition, it was given to 793 doctors working at the 105 Korean medical hospitals listed on the National Korean Medical Hospital Associations Address book by postal mail; and also it was given to the 142 public health care Korean medical doctors who attended the conference for the municipal and provincial representatives of the public health service Korean medical doctors. Total of 1277 questionnaires were retrieved out of the 3666 subjects. Results : 1. Nine hundred out of 1277 Korean medical doctors used method of differentiation syndromes(70.4%) and 1184 out of 1277 Korean medical doctors convalescence decide with subjective symptom improvement of patient(92.6%). 2. Nine hundred eight out of 1277 Korean medical doctors used both local and remote points(71.0%) and 916 out of 1277 Korean medical doctors treat with method of reinforcement-reduction(71.7%). 3. Eleven hundred fifth five out of 1277 Korean medical doctors used cupping a boil(90.3%) and 1023 out of 1277 Korean medical doctors used moxibustion(80.1%). Conclusion : When Korean medical doctors treat with acupuncture, most of them diagnose with differentiation methods and meridian theory, decide convalescence with subjective symptom improvement of patient, select out of both local and remote acupuncture points, treat with method of basic reinforcement-reduction, apply moxa and cupping a boil when they are necessary. The cupping a boil comes to be used when being muscle and joint disease. The moxa comes to be used when being chronic disease.

의료클러스터 기반의 빅 데이터 환경에 대한 IP Spoofing 공격 발생시 상호협력 보안 모델 설계 (Designing Mutual Cooperation Security Model for IP Spoofing Attacks about Medical Cluster Basis Big Data Environment)

  • 현재 우리사회는 네트워크를 통하여 실시간으로 교류되는 다양한 정보 환경에 노출되어 있다. 특히 정부의 의료정책은 대국민의료서비스 질을 향상시키기 위해 원격진료의 시행을 서두르고 있다. 이러한 원격진료의 시행은 향후 지역에 상관없이 맞춤형 환자 진료를 위한 빅 데이터 기반의 진료 정보 구축도 함께 요구하고 있다. 본 논문은 빅 데이터 기반의 권역별 의료클러스터 구축과 이에 대한 서비스 가용성을 해치는 공격이 발생할 경우 해당 공격을 탐지하고 적절한 대응이 가능한 방어 및 보안 협력모델을 제안하고 있다. 이를 위하여 동일 병원정보시스템으로 전국에 고루 분포된 지방의료원을 권역별 가상 의료클러스터 본부로 하는 네트워크 구성을 제안하였다. 아울러 의료클러스터에 발생할 수 있는 IP Spoofing 공격과 이에 따른 DDoS 공격에 실시간으로 대응 가능한 상호협력 보안 모델을 설계하여 단일 체계, 단일 보안정책이 가지는 한계성도 극복할 수 있도록 하였다.

혈관육종 환자에서 두피의 아전절제술 (Near Total Excision in Patients with Angiosarcoma on Scalp)

  • Purpose: Angiosarcoma is a rare malignant neoplasm of endothelial type cells that line vessel walls. It tends to occur in aged male and the prognosis of angiosarcoma is very poor because of frequent local recurrence and early metastasis. The treatment regimen is yet to be established from its rare occurrence but the wide excision in early stage is known to be the most effective. The authors report two cases of near totally excised angiosarcoma with more than a safety margin of 5 cm. Methods: The two subjects were aged male patients, one of the two was diagnosed with angiosarcoma from our institution confirmed by the biopsy. The other one went through the wide excision with a safety margin of 2 cm and split-thickness skin graft but local recurrence was observed. The two patients underwent near total excision with more than a safety margin of 5 cm, leaving only the periosteum. After confirming that the angiosarcoma had not infiltrated the excision margin, reconstruction with split-thickness skin graft was performed. Results: Based on 6 months and 24 months post-surgery assessment, no local recurrence or remote metastasis in the lungs, liver, bones, and lymph nodes at the neck, where remote metastasis is common, was reported by the two subjects who underwent near total excision with a safety margin of 5 cm. Conclusion: Angiosarcoma has very poor prognosis from its frequent recurrence and metastasis. To enhance the survival rate of angiosarcoma patients, early diagnosis, timely surgical treatment, and radiotherapy after surgery are critical. In addition, authors suggest that it is necessary to further study the efficacy of wide excision using a wider safety margin as much as possible, and to apply this to more cases.

복합위성자료(Topex/Poseidon, ERS1)를 이용한 남인도양의 해수면 변화와 와동류 연구 (Sea Level Valiability and Eddy in the South Indian Ocean by Multi-satellite Data (Topex/Poseidon, ERS1))

  • 복합위성자료 신호처리기법을 사용해서 남인도양 지역을 중심으로 한 해수면변화와 사행 (meandering)을 연구하였다. 해수면변화는 아프리카 남부(46$^{\circ}$E)에서 크호제트 대지(Crozet basin)의 35$^{\circ}$~46$^{\circ}$S대를 걸쳐서 암스테르담-케르겔른 경로(Amsterdam-Kerguelen Passage)까지 확장된다. 이때 가장 강한 해수면변화(20~30cm)가 2개의 좁은 지역에서 발생하는데, 하나는 경도 49$^{\circ}$~57$^{\circ}$E(약 700Km) 및 위도 38$^{\circ}$~42$^{\circ}$S(약 450Km)나타났고 그리고 나머지 하나는 경도 58$^{\circ}$~64$^{\circ}$E(약 450Km) 및 위도 42$^{\circ}$~44$^{\circ}$S(약 200Km)에서 각각 나타났다. 이것은 해저지형과 관련된 강한 와동류의 형성 때문이다. 와동류 발생지역은 아규라스반류(Agulhas Return Current) 및 남극순환해류(Antarctic Circumpolar Current)와 관련하여 강한 흐름이 형성되는 지역에서 발생했다. 이때 복합위성자료(Topex/Poseidon, ERS1)는 표층순환과 해수면변동 특성을 잘 나타내었다.

지상형 원격탐사기술을 이용한 소나무 재선충 피해조사 (An Investigation of Pine Wilt Damage by Using Ground Remote Sensing Technique)

  • 소나무의 AIDS라 일컫는 소나무 재선충이 1988년 부산의 금정산에서 최초로 발견되었다. 발견된 이후 2005년 12월까지 경상남도 전역 53개 시군구지역에 피해를 끼쳤다. 피해를 입은 삼림지역에 대한 최선의 대책은 초기 방재 작업 후에 훈증을 통해 처리하는 방법이다. 그러나 관찰에 의해 초기에 방재를 하는 것은 피해 삼림지역이 매우 광범위한 지역에 걸쳐있기 때문에 어렵다. 또한 한국의 지형적 특성 상 관찰자의 접근이 어렵다는 문제가 있다. 본 연구는 근적외선 CCD 카메라를 사용하여 소나무 재선충 피해지역의 초기 방재 가능성을 제시하였다.

Infrequent Hemorrhagic Complications Following Surgical Drainage of Chronic Subdural Hematomas

  • Chronic subdural hematomas mainly occur amongst elderly people and usually develop after minor head injuries. In younger patients, subdural collections may be related to hypertension, coagulopathies, vascular abnormalities, and substance abuse. Different techniques can be used for the surgical treatment of symptomatic chronic subdural hematomas : single or double burr-hole evacuation, with or without subdural drainage, twist-drill craniostomies and classical craniotomies. Failure of the brain to re-expand, pneumocephalus, incomplete evacuation, and recurrence of the fluid collection are common complications following these procedures. Acute subdural hematomas may also occur. Rarely reported hemorrhagic complications include subarachnoid, intracerebral, intraventricular, and remote cerebellar hemorrhages. The causes of such uncommon complications are difficult to explain and remain poorly understood. Overdrainage and intracranial hypotension, rapid brain decompression and shift of the intracranial contents, cerebrospinal fluid loss, vascular dysregulation and impairment of venous outflow are the main mechanisms discussed in the literature. In this article we report three cases of different post-operative intracranial bleeding and review the related literature.

Development of the Interface Module for an Effective Application of a Digital Mockup

  • As the cumulative amount of spent fuel increases, the reliable and effective management of the spent fuel has become a world-wide mission. For this mission, KAERI is developing the Advanced Spent Fuel Conditioning Process (ACP) as a pre-disposal treatment process for spent fuel. Conventional approach to the development of the process and the remote operation technology is to fabricate the process equipment on the same scale as the real environment and demonstrate the remote handling operation using simulated fuel called a mock-up test. But this mock-up test is expensive and time consuming, since the design may need to be modified and the equipment fabricated again to account for the problems found during a testing. To deal with this problem, we developed a digital mockup for the ACP. Also, for an effective utilization of the digital mockup, we developed user interface modules such as the data acquisition and display module and the external input device interface module. The result of this implementation shows that a continuous motion of the manipulator using the external device interface can be represented easily and the information display screens responded well to the simulation situation.

A Bridge Transported Bilateral Force-Reflecting Servo-Manipulator for Maintenance of Nuclear Pyroprocessing Equipment

  • The Advanced Spent Fuel Conditioning Process (ACP), which is a pre-disposal treatment process for spent fuel is being developed at the Korea Atomic Energy Research Institute (KAERI). The ACP equipment is operated in an intense radiation field as well as in a high temperature. Thus, the equipment is designed in consideration of the remote handling and maintenance. This paper describes a Bridge Transported Bilateral Force-Reflecting Servo-Manipulator (BTSM) system, which is being developed to overcome the limitation of access that is a drawback of the mechanical Master-Slave Manipulators (MSMs), which are mounted on the ACP hot cell wall for the operation and the maintenance of the ACP equipment. The BTSM system was manufactured and temporally installed at the mockup to test its performance. The manufactured BTSM system will be installed at the ACP hot cell on June 2005 after the accomplishment of the performance test. The BTSM system consists of four components: a transporter with a telescoping tubeset, a slave manipulator, a master manipulator, and a remote control system. This system will highly increase the volume of coverage for the operation and maintenance of the ACP equipment.

Comparison of Topex/Poseidon sea surface heights and Tide Gauge sea levels in the South Indian Ocean

  • The comparison of Topex/Poseidon sea surface heights and Tide Gauge sea levels was studied in the South Indian Ocean after Topex/Poseidon mission of about 3 years (11- 121 cycles) from January 1993 through December 1995. The user's handbook (AVISO) for sea surface height data process was used in this study Topex/Poseidon sea suface heights ($\zeta$$^{T/P}$), satellite data at the point which is very closed to Tide Gauge station, were chosen in the same latitude of Tide Gauge station. These data were re-sampled by a linear interpolation with the interval of about 10 days, and were filtered by the gaussian filter with a 60 day-window. Tide Gauge sea levels ($\zeta$$^{Argos}$, $\zeta$$^{In-situ}$ and $\zeta$$^{Model}$), were also treated with the same method as satellite data. The main conclusions obtained from the root-mean-square and correlation coefficient were as follows: 1) to Produce Tide Gauge sea levels from bottom pressure, in-situ data of METEO-FRANCE showed very good values against to the model data of ECMWF and 2) to compare Topex/Poseidon sea surface heights of Tide Gauge sea levels, the results of the open sea areas were better than those of the coast and island areas.

  • 국토개발에 관한 각종정보, 도시환경 및 토지이용정보, 농수산자원 조사, 지질조사, 수종의 분류, 나무의 생육상태등의 많은 정보를 수집하고 추출하는데 원격탐사는 중요한 일익을 담당하고 있다. 본 연구는 원격탐사를 수행하는데 영상처리 자료의 위치 정확도를 향상시키기 위하여 실시하는 기하학적 보정을 하는데 있어 지도를 이용하는 방법과 GPS를 이용하는 방법을 서로 비교하여 보다 정확한 인공위성 영상의 좌표변환을 위한 것이다. 실제 지형지물에서 GCP를 획득할 때의 문제점과 어려운 점에 대하여 개선방안을 찾으려고 하였으며, 향후 전국적인 인공위성영상 접합을 위한 좌표계에 대한 고려를 고찰해 보았다.
