The Experimental Stydy on the Anti-Allergic Rhinitis Effects of the Sochungryong-tang (小靑龍湯이 알레르기 鼻炎에 미치는 效果에 대한 實驗的 硏究)

  • Allergic rhinitis is an inflammation of the nasal mucosa which is triggered by an allergic reaction. When exposed to certain allergens, the IgE covered mast cells degranulate releasing inflammatory mediators and cytokines which results in a local inflammatory reaction. In many recent studies, molecular biological methods have been used to investigate the role of cytokines in pathogenesis and new therapeutic targets of allergic rhinitis. This experimental study was done to research effects of Sochungryong-tang. We have studied effect of mice on OVA-induced production of IL-4, IL-5, IFN-${\gamma}$ by murine splenocytes and effect of OVA-induced total IgE and OVA-specific IgE. The results were as follows; 1. In IL-4 study, Sochungryong-tang treated group was proved significant inhibitory effect(p〈0.005). 2. In IL-5 study, Sochungryong-tang treated group was proved significant inhibitory effect(p〈0.05). 3. In JFN-${\gamma}$ study, Sochungryong-tang treated group was proved significant inhibitory effect(p〈0.000001). 4. In Total IgE, Sochungryong-tang treated group was proved significant inhibitory effect(p〈0.000001). 5. In OVA-specific IgE, Sochungryong-tang treated group didn't showed significant inhibitory effect. Depending upon above results, it is considered that Sochungryong-tang has the inhibitory effect on the allergic rhinitis of mice and suggested that it could be used in relieving patients of the symptoms which are caused by allergic rhinitis.

Open reduction of zygoma fractures with the extended transconjunctival approach and T-bar screw reduction

  • Background Zygomaticomaxillary complex (ZMC) fractures mostly occur in the form of tripod fractures. The surgical field is accessed using a combination of three classic approaches. However, the subciliary incision may have unfavorable aesthetic results. Herein, the authors report the advantages of the extended transconjunctival approach (ETA) combined with T-bar screw reduction in minimizing scarring and complications for the treatment of ZMC fractures. Methods A total of 26 patients underwent ZMC reduction through the ETA and intraoral approach. A skin incision measuring roughly 5 to 8 mm in length was placed following the lateral canthal skin crease. After releasing the inferior crus of the lateral canthal tendon for canthotomy, the medial periosteum of the lateral orbital rim was preserved for canthal reattachment. A limited subperiosteal dissection and partial relaxing incision of the orbicularis oculi were performed to expose the fracture line of the inferior orbital rim and zygomaticofrontal suture. Reduction was performed using a T-bar screw through the transconjunctival incision and an elevator through the intraoral incision. Results The aesthetic and functional results were excellent. Successful reduction was achieved and the skin incision was less than 8 mm in 20 cases (76.9%). Only six patients had an additional skin incision (less than 5 mm) to achieve reduction. No cases of ectropion, entropion, or excessive scarring were noted. Conclusions The ETA using a T-bar screw is a useful method for maximizing aesthetic results in ZMC fractures, with the advantages of minimal scarring, faster recovery, and maintenance of pretarsal fullness.

Hypoxia Induced Multidrug Resistance of Laryngeal Cancer Cells via Hypoxia-inducible Factor-1α

  • Objectives: To investigate whether hypoxia has an effect on regulation of multidrug resistance (MDR) to chemotherapeutic drugs in laryngeal carcinoma cells and explore the role of hypoxia-inducible factor-$1{\alpha}$ (HIF-$1{\alpha}$). Methods: Laryngeal cancer cells were cultured under normoxic and hypoxic conditions. The sensitivity of the cells to multiple drugs and levels of apoptosis induced by paclitaxel were determined by MTT assay and annexin-V/propidium iodide staining analysis, respectively. HIF-$1{\alpha}$ expression was blocked by RNA interference. The expression of HIF-$1{\alpha}$ gene was detected by real-time quantitative RT-PCR and Western blotting. The value of fluorescence intensity of intracellular adriamycin accumulation and retention in cells was evaluated by flow cytometry. Results: The sensitivity to multiple chemotherapy agents and induction of apoptosis by paclitaxel could be reduced by hypoxia (P<0.05). A the same time, the adriamycin releasing index of cells was increased (P<0.05). However, resistance acquisition subject to hypoxia in vitro was suppressed by down-regulating HIF-$1{\alpha}$ expression. Conclusion: HIF-$1{\alpha}$ could be considered as a key regulator for mediating hypoxia-induced MDR in laryngeal cancer cells via inhibition of drug-induced apoptosis and decrease in intracellular drug accumulation.

Effects of Agastachis Herba extract on OVA-induced allergic asthma in mice (곽향(藿香)의 난알부민으로 유도된 천식 마우스에서의 천식개선 효능연구)

  • Objectives : In this study, we investigated the effects of Agastachis Herba water (AH-W) extract on compound 48/80-induced mast cell degranulation and histamine release in human mast cells and also anti-asthmatic effect of AH-W extract on ovalbumin (OVA)-induced asthma in mice. Methods : Human mast cells, HMC-1 were treated with AH-W extract in the presence or absence of compound 48/80 (C48/80). Mast cell degranulation was observed by microscope, and the histamine release was measured in culture medium by ELISA. For preparation of asthmatic in vivo model, mice were sensitized (0, 7, and 14 days) with OVA and airway challenged (21, 23, 25, 27, and 29 days). AH-W extract at doses of 100 and 300 mg/kg/body weight was orally administered during OVA challenge once per a day. The levels of immunoglobulin (Ig) E, and Th1/Th2 cytokines, IFN-$\gamma$ and IL-4 were measured in the sera of mice by ELISA. The histopathological change of lung tissues was observed by hematoxylin and eosin (H&E) and Periodic Acid Schiff (PAS) staining. Results : The treatment of AH-W extract significantly decreased the mast cell degranulation and histamine release in C48/80-stimulated HMC-1 cells. In addition, The administration of AH-W extract at does of 100 and 300 mg/kg significantly decreased the serum levels of OVA-specific IgE compared with those of OVA control group. In H&E and PAS staining, AH-W extract inhibited OVA-induced airway inflammation, and inflammatory cells infiltration, and also histopathological damages on lung tissues such as bronchiole epithelial desquamation, goblet cells hyperplasia, and mucin releasing. Conclusions : These results indicate that AH-W extract may improve asthmatic symptoms through mast cell stabilization and inhibiting the lung inflammation in bronchial asthma.

Effect of Mixture extracted from Bupleuri Radix and Physalidis Herba on the LPS-induced depression in Rats (시호(柴胡)와 산장(酸漿) 복합제가 LPS로 유도된 우울증에 미치는 효과)

  • Objectives : The effect of mixture extracted from Bupleuri Radix and Physalidis Herba(BR+RH) on the LPS-induced Depression in rats was investigated. Methods : Rats were administered intragastrically BR+PH after injectio of LPS to induce deprssion. Immobility was examined using Tail Suspension Test(TST), Forced Swimming Test(FST). The level of plasma corticosterone was measured by an Enzyme-Linked Immunosorbent Assay(ELISA) method. The expressions of c-Fos, Corticotropin Releasing Factor(CRF), NADPH-d in the Paraventricular nucleus(PVN) and TH in the Locus coeruleus(LC) were measured by immunohistochemical method. Results : In the effect of BR + PH on TST, immobility was significantly decreased comparing with the LPS group. In FST, immobility was shown decrease tendency in the BR+PH group. The expression of c-Fos in the PVN was significantly decreased at BR + PH400 group, comparing with the LPS group. The expression of CRF in PVN was shown dto have the decrease tendency in the BR+PH group, comparing with the LPS group. The expression of NADPH-d in PVN was not significantly decreased at BR+PH groups, comparing with the LPS group. The expression of TH in the LC was shown to have the decrease tendency at BR + RH groups, but not significantly, comparing with the LPS group. Conclusions : Anti-depressant effect of mixture after extracted from Bupleuri Radix and Physalidis Herba was through the anti-inflammatory effect via inhibition of HPA axis. NO and catecholamine system is not involved.

Effects of Mixture of Bupleuri Radix and Lycii Radicis Cortex on the Change of HPA-Axis and Catecholamic System in the Forced Swimming Test (시호(柴胡)와 지골피(地骨皮) 합제(合劑)가 강제수영부하실험에서 HPA Axis와 카테콜라민 시스템에 미치는 영향)

  • Objectives : The effects of Extraction after Mixture of Bupleuri Radix and Lycii Radicis Cortex on the change of the HPA-Axis system and the Catecholamic system was investigated. Methods : After performing the Forced Swimming Test(FST), the expressions of corticotropin releasing factor(CRF), c-Fos in the paraventricular nucleus(PVN), and tyrosine hydroxylase(TH) in the ventral tegmental area(VTA) and locus coeruleus(LC) were measured by immunohistochemical method. Results : The duration of immobility in FST was significantly decreased in A 100(Extraction after Mixture of Bupleuri Radix and Lycii Radicis Cortex, 100mg/kg) and A 400(Extraction after Mixture of Bupleuri Radix and Lycii Radicis Cortex, 400mg/kg)(p<0.001). The expression of CRF in the PVN was shown the tendency to reduce in A100 and A400. The expression of c-Fos in the PVN was shown the tendency to reduce in A100 and A400. The expression of TH in the VTA was shown the tendency to reduce mildly in A100 and A400. The expression of TH in the LC was significantly reduced in A400(p<0.001). And the dose dependent reduction tendency was shown, respectively. Conclusions : According to the results above mentioned, the immobility, c-Fos and CRF expression was reduced at lower dose and was increased at higher dose. Therefore there is contradictory effects on the HPA Axis system in accordance with the dose. But in the effects on the catecholaminergic system, it significantly reduced the expression of TH in the LC. It was validated that the effect on the catecholaminergic system was ruled by Bupleuri Radix rather than Lycii Radicis Cortex via mainly the noradernergic system.

Cessation of gonadotropin-releasing hormone antagonist on triggering day in flexible multiple-dose protocol: A randomized controlled study

  • Objective: To investigate outcomes of stimulated IVF cycles in which GnRH antagonist was omitted on the ovulation triggering day. Methods: A total of 86 women who underwent controlled ovarian hyperstimulation with recombinant FSH and GnRH antagonist flexible multiple-dose protocols were recruited and prospectively randomized into the conventional group (group A) or cessation group (group B). The GnRH antagonist, 0.25 mg/day of cetrorelix, was started when the leading follicle reached 14 mm in diameter and was continuously administered until the hCG triggering day (group A, 43 cycles) or until the day before hCG administration (group B, 43 cycles). The maturity of oocytes, fertilization rate, embryo quality, and implantation and clinical pregnancy rates were evaluated. Results: The duration of ovarian stimulation, total dose of gonadotropins, serum estradiol levels on hCG administration day, and number of oocytes retrieved were not significantly different between the two groups. The total dose of GnRH antagonist was significantly lower in group B than group A ($2.5{\pm}0.9$ vs. $3.2{\pm}0.8$ ampoules, p<0.05). There was no premature luteinization in any of the subjects. The proportion of mature oocytes and fertilization rate were not significantly different in group B than group A (70.7% vs. 66.7%; 71.1% vs. 66.4%, respectively). There were no significant differences in the implantation or clinical pregnancy rates. Conclusion: Our prospective randomized study suggested that cessation of GnRH antagonist on the hCG administration day during a flexible multiple-dose protocol could reduce the total dose of GnRH antagonist without compromising its effects on pregnancy rates.

A Review on Acne Treatment in Korean Medicine by Analyzing Case Studies with Herbal Medicine Treatment (국내 한약 치험례 분석을 통한 한의학적 여드름 치료법에 대한 고찰)

  • Objectives : The purpose of this study is 1) to explore the general tendency of the herbs or herbal formula for acne treatment in clinical practice 2) and to provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism by analyzing the herbs or herbal formula using for acne treatment in clinical practice. Methods : In the domestic databases (Oriental medicine Advanced Searching Integrated System-OASIS, Korean Traditional Knowledge Portal-KTKP, National Discovery for Science Leader-NDSL, Research Information Sharing Service-RISS), we selected among the papers published using search terms related to "acne". Reports related to sasang constitutional medicine were excluded and a total of 23 papers were finally selected. Results : 29 prescriptions were retrieved from 23 papers. Chungan-tang (淸顔湯) notified formulas ranked first by Cheongsangbangpung- tang (淸上防風湯), Hwangnyeonhaedok-tang (黃連解毒湯), Dangguijakyaksan (當歸芍藥散) and Bipachungpe-eum (枇杷淸肺飮). Commonly used herbs are Forsythiae Fructus (連翹), Scutellariae Radix (黃芩), Gardeniae Fructus (梔子), Ligustici Rhizoma (川芎), and Angelicae Gigantis Radix (當歸). In most patients with inflammatory lesions or excessive flushing, exterior-releasing medicinal (解表藥) and heat-clearing medicinal (淸熱藥) were mainly prescribed for acne treatment, while in patients with acne accompanied hyperkeratinization, Excess sebum secretion, Dysmenorrhea or indigestion, qi-tonifying medicinal (補氣藥) or blood-tonifying medicinal (補血藥) were mainly used. KAGS (Korean Acne Grading System) was the most commonly used method for acne severity assessment. Conclusions : These findings suggest that the herbs or herbal formula for acne treatment in Korean Medicine is prescribed differently according to the cause and symptoms of acne in a unique way for control the whole body balance. Also, our findings could provide a basis for explaining the principles of acne treatment in Korean Medicine in order to make patients understand more easily based on pharmacological mechanism in clinical practice.

Results and Conclusions I) The conception of Soewnin Hwangdal(jaundice, 황달(黃疸)) is a syndrome of indigestion, which needs releasing excessive through the method of decending the interior yin. 2) In the course of Taeum syndrome(태음병(太陰炳)), Soeuwnin Hangdal(jaundice황달(黃疸)) locate beetwen Miman (bloating, 비만(?滿)) and Bujong(edema, 부종(浮腫)) on the point of view of Healthy Energy. 3)In ?SinchukBon? , Soewnin interior disease is divided two syndromes. The one is the syndrome that has diarrhea as major symptom, and the another is the syndrome that has bloating as major symptom 4) In ?SinchukBon? , Dong-Mu suggested that diuresis is an important treatment of the syndrome which has bloating as major symptom.

The Review of the Herbal Medicines with Phytoestrogenic Effect (식물성에스트로겐 작용이 보고된 한약재에 관한 고찰)

  • Objectives Medical plant has a big role in Herbal Medicines, and most medical plant has phytoestrogens. Therefore some estrogenic effects have been reported in a lot of Korean Medicine literature. Phytoestrogens should be used carefully in children. The objective of this study is to identify reported phytoestrogenic effects in Herbal Medicines and to evaluate the characteristics of the phytoestrogens in Herbal Medicines. Methods A literature search was done with using 8 databases with a limit of reports until 12/31/2013. The estrogenic effects were summarized by each individual Korean Medicine. The frequency of the phytoestrogens was also investigated depending on the Korean medical categorization by the treatment effect. Results and Conclusions Phytoestrogenic effects were reported in 89 Herbal Medicines. Phytoestrogens were act bidirectionally, and the effect was fairly weak compared to estrogen. Phytoestrogenic effect was variable on different cells and tissues. The most frequent phytoestrogenic effect was in tonifying and replenishing medicinal, the following orders were heat-clearing, exterior-releasing, and blood-activating and stasis-dispelling medicinal. Phytoestrogens were not reported in dampness-resolving, digestant, orifice-opening, emetic medicinal.
