수동 휠체어 추진 속도에 따른 상지 관절 생체역학적 영향 분석 (Upper Extremity Biomechanics of Manual Wheelchair Propulsion at Different Speeds)

  • It is known that chronic pain and injury of upper limb joint tissue in manual wheelchair users is usually caused by muscle imbalance, and the propulsion speed is reported to increase this muscle imbalance. In this study, kinematic variables, electromyography, and ultrasonographic images of the upper limb were measured and analyzed at two different propulsion speeds to provide a quantitative basis for the risk of upper extremity joint injury. Eleven patients with spinal cord injury for the experimental group (GE) and 27 healthy adults for the control group (GC) participated in this study. Joint angles and electromyography were measured while subjects performed self-selected comfortable and fast-speed wheelchair propulsion. Ultrasound images were recorded before and after each propulsion task to measure the acromiohumeral distance (AHD). The range of motion of the shoulder (14.35 deg in GE; 20.24 deg in GC) and elbow (5.25 deg in GE; 2.57 deg in GC) joints were significantly decreased (p<0.001). Muscle activation levels of the anterior deltoid, posterior deltoid, biceps brachii, and triceps brachii increased at fast propulsion. Specifically, triceps brachii showed a significant increase in muscle activation at fast propulsion. AHD decreased at fast propulsion. Moreover, the AHD of GE was already narrowed by about 60% compared to the GC from the pre-tests. Increased load on wheelchair propulsion, such as fast propulsion, is considered to cause upper limb joint impingement and soft tissue injury due to overuse of the extensor muscles in a narrow joint space. It is expected that the results of this study can be a quantitative and objective basis for training and rehabilitation for manual wheelchair users to prevent joint pain and damage.

감각통합중재에 대한 인식도 및 취업선호도에 대한 교육프로그램의 효과 및 필요성 연구 : 작업치료전공 대학생을 중심으로 (A Study on the Effects and Needs of Educational Programs on Awareness and Preference for Sensory Integration Intervention : Focusing on college students in the department of occupational therapy)

  • Purpose : This study aims to investigate occupational therapy students' perceptions of sensory integration therapy and the effect of an educational program on these perceptions and future preferences. Methods : The participants were 200 occupational therapy students in Busan and Gyeongsangnam-do. A primary survey was first conducted to shed light on students' perceptions of sensory integration therapy, followed by an education program on sensory integration provided through an online video. After this training on sensory integration therapy, an additional survey assessed the their preferred employment routes and intention to take educational courses on clinical sensory integration therapy in the future. A secondary survey was then conducted, using the same form as the primary survey, to identify changes in the students' perceptions of and preferences for sensory integration therapy. A frequency analysis using descriptive statistics was employed to identify the participants' general characteristics, employment-related preferences, and intention to take courses on sensory integration, and a paired samples t-test was used for a comparative analysis of the students' perceptions before and after the education program. Results : In terms of the students' perceptions of sensory integration therapy, the variables of efficiency, facilitation, and expertise showed statistically significant differences before and after the educational program, which resulted in a positive change in their overall perceptions of the therapy after the program. In relation to their preferred employment routes after the program, 100 students (50 %) answered the "field of adults," and 100 (50 %) students answered the "field of children." Conclusion : The findings of this study demonstrated that sensory integration education positively influences occupational therapy students' preferences for and perceptions of sensory integration therapy. Additional research is recommended to organize a more systematic education program and investigate employees in organizations related to children with disabilities.

Repair of sports bone injury based on multifunctional nanomaterial particles

  • Nanoparticles have lower size and larger specific surface area, good stability and less toxic and side effects. In recent years, with the development of nanotechnology, its application range has become wider and wider, especially in the field of biomedicine, which has received more and more attention. Bone defect repair materials with high strength, high elasticity and high tissue affinity can be prepared by nanotechnology. The purpose of this paper was to study how to analyze and study the composite materials for sports bone injury based on multifunctional nanomaterials, and described the electrospinning method. In this paper, nano-sized zirconia (ZrO2) filled micro-sized hydroxyapatite (HAP) composites were prepared according to the mechanical properties of bone substitute materials in the process of human rehabilitation. Through material tensile and compression experiments, the performance parameters of ZrO2/HAP composites with different mass fraction ratios were analyzed, the influence of filling ZrO2 particles on the mechanical properties of HAP matrix materials was clarified, and the effect of ZrO2 mass fraction on the mechanical properties of matrix materials was analyzed. From the analysis of the compressive elastic modulus, when the mass fraction of ZrO2 was 15%, the compressive elastic modulus of the material was 1222 MPa, and when 45% was 1672 MPa. From the analysis of compression ratio stiffness, when the mass fraction of ZrO2 was 15%, the compression ratio stiffness was 658.07 MPa·cm3/g, and when it was 45%, the compression ratio stiffness is 943.51MPa·cm3/g. It can be seen that by increasing the mass fraction of ZrO2, the stiffness of the composite material can be effectively increased, and the ability of the material to resist deformation would be increased. Typically, the more stressed the bone substitute material, the greater the stiffness of the compression ratio. Different mass fractions of ZrO2/HAP filling materials can be selected to meet the mechanical performance requirements of sports bone injury, and it can also provide a reference for the selection of bone substitute materials for different patients.

A Needs Analysis of Educational Content for Overseas Job Applicants in the Digital Bio-health Industry

  • Background: The globalization of the healthcare industry and the increasing demand for skilled professionals in the global healthcare industry have opened up opportunities for specialized biotech healthcare professionals to seek overseas employment and career advancement. Objects: This study aimed to develop educational content essential for the overseas employment of digital bio-health professionals. Methods: A survey was conducted among 196 participants. Google Forms (Google) were utilized to create and administer the survey, employing purposive sampling, a non-probability sampling method. Data analysis was performed using IBM SPSS 25.0 (IBM Co.), including Cronbach's α and independent sample t-tests to assess significant differences. Results: About half of college students are interested in overseas employment and international careers, while the other half had not. The most common reason for wanting to work or go overseas was "foreign experience will be useful for future activities in Korea." Students who had experience taking courses from the Bio-health Convergence Open Sharing University preferred overseas programs more than those who did not have that experience. In terms of the degree of desire for overseas education courses provided by universities, contents related to human health were the highest, followed by bio-health big data. Conclusion: Many students wanted to work and go overseas if there is sufficient support from the university. The findings in this study suggest that universities are necessary to play an important role in supporting students' aspirations to work or go overseas by providing language education, education and training programs, information on overseas jobs, and mentoring programs.

Surgical Correction of Bilateral Gastrocnemius Muscle Rupture and Its Prognosis in a Korean Native Calf

  • A 3-month-old Korean native cattle (Hanwoo) calf with difficulty taking normal posture and an inability to rise was referred for a definite diagnosis and active treatment, including surgery. The calf had a history of an accident in which both hind limbs were trapped in a barn structure. After admission, a "rabbit leg" posture was observed, a typical sign of gastrocnemius muscle rupture, and both digits were knuckled downward like they were trying to grip the ground. This was considered to be a result of the superficial digital flexor not rupturing but only the gastrocnemius muscle rupturing. Physical examination revealed laceration of the metatarsus and firmness behind both stifle joints which were presumed to be the sites of gastrocnemius muscle rupture. Skeletal abnormalities, including fractures, were ruled out by radiography. Based on these findings, the patient was diagnosed with bilateral gastrocnemius muscle rupture, and surgery was performed to reconnect the head of the ruptured muscle. Because the rupture occurred perpendicular to the muscle direction, the locking loop technique, a method of suturing severed tendons, was used to reduce the tension. After surgery, the cast was used to prevent further injuries and promote voluntary rehabilitation. Follow-up was completed, with the calf showing normal posture and gait 112 days after surgery. This is the first case report in the Republic of Korea describing the successful diagnosis and treatment of bilateral gastrocnemius muscle rupture in a calf.

Effects of Y-Balance Test Difference of the Ankle Dorsi-flexion Range of Motion in K3 Soccer Players

  • 본 연구의 목적은 비 체중지지 자세 상태에서 발목관절 발등굽힘 관절가동범위에 따라 동적 균형 능력을 평가하는 Y-Balance Test와의 관계를 규명하고자 하였다. 본 연구에는 10년 이상의 축구경력을 가진 K3리그에 정기적으로 참여하는 남자축구선수 27명이 참여하였다. 발목관절의 발등굽힘과 동적균형 능력과의 관계를 검증하기 위해 The Pearson Rank Correlation Coefficient를 사용하였다. 본 연구의 결과 HADR군에서 PLRD(P<.05)와 CS(P<.01)에서 유의한 차이가 나타났으며, 발목관절 발등굽힘의 관절가동범위와 PMRD, PLRD 및 CS에서만 중증도의 유의한 상관관계가 나타났다. 따라서 발목관절 발등굽힘의 감소는 균형 능력 감소와 관련이 있으며, 축구선수의 손상을 예방하기 위해서는 정기적인 발목관절 관절가동범위의 평가뿐만 아니라 무릎과 엉덩관절의 근력 및 고유수용감각을 향상시키기 위한 트레이닝이 필요할 것으로 생각된다.

방문재활운동 프로그램이 만성질환 노인의 균형, 일상생활활동과 우울 및 삶의 질에 미치는 영향 (Effect of home-based exercise program on balance, activities of daily living, depression and quality of life in elderly with chronic diseases)

  • Background: The purpose of this study was to identify the effect of home-based exercise program in elderly with chronic diseases on balance, activities of daily living, depression and quality of life. Design: One group pretest-posttest design Methods: The subjects consisted of 21 elderly people over the age of 65 years with chronic diseases lived in seo-gu, Gwangju. Home-based exercise program was performed twice a week, for an hour, during 6 weeks under supervision of an experienced instructor. The home-based exercise program was structured to last a total of 1 hour, consisting of 10 minutes of warm-up, 30 minutes of main exercises, and 20 minutes of concluding exercises. For the warm-up, we performed range of motion, stretching, and flexibility exercises. The main exercises comprised of strength, weight-bearing and balance exercises, and endurance exercises. The concluding exercises were tailored to the individual's home environment and needs, including task-oriented daily living training such as getting up from bed, moving to the restroom, and walking indoors. The subjects were assessed before and after the exercise program with berg balance scale(BBS), Korean-version modified barthel index(K-MBI), euro quality of life questionnaire 5-dimensional classification(EQ-5D), and geriatric depression scale short form-Korea(GDSSF-K). The collected data was analyzed using SPSS Win 18.0 program. Results: The results showed statistically significant difference in all domains of the BBS, K-MBI, EQ-5D, GDSSF-K after the exercise program. Conclusion: These results suggest that Home-based exercise program has resulted in a significant improvement on ADL, depression and quality of life in elderly with chronic diseases.

초등학교 지적장애아동의 자세조절 특성 (A Study of Postural Control Characteristics in Schoolchild with Intellectual Disability)

  • 본 연구는 초등학교 지적장애아동의 자세조절 특성을 파악하기 위해 시각, 안뜰감각 그리고 몸감각을 조합하여, 실험 조건에 따라 자세유지를 위하여 어떤 감각을 활용하고 있는지, 부적절한 감각자극에 대해서 통합시스템에서 어떻게 반응하는지, 외부 동요에 대해 신경근육의 자세조절 전략 시 동원되는 근육의 수축 개시시간과 개시순서를 각각 측정하여, 지적장애 아동의 자세조절 특성을 살펴보고, 나아가 초등학교 지적장애아동과 일반아동 간에 어떠한 차이가 있는지를 비교분석하여 자세조절능력 향상 프로그램에 적용 시 방향을 제시해주는데 있다. 이를 위하여 성별과 연령을 고려하여 초등학교 지적장애아동 26명과 일반아동 26명의 아동으로부터 자세조절능력을 비교하였다. 그 결과 자세조절과 감각계와의 관계를 파악한 평형점수와 감각활용 분석, 엉덩관절 전략에 의지하는 자세조절 전략, 근수축 개시지연과 근수축 개시순서의 변화 등을 종합해 볼 때 초등학교 지적장애아동의 자세조절의 문제는 운동시스템, 감각시스템, 통합시스템의 자세조절 시스템 전반에 걸친 총체적인 문제로 나타났다. 따라서 초등학교 지적장애아동의 자세조절 문제 해결을 위해서는 운동시스템, 감각시스템, 통합시스템을 포함하는 포괄적 자세조절 훈련 프로그램의 접근이 필요하다는 것을 알 수 있다. 자세조절 프로그램을 구성할 때, 다양한 감각을 제공하여 주고, 제공된 감각을 효율적으로 활용할 수 있는 감각훈련과 함께 고위중추에서 입력된 감각들을 정상적인 자세조절 발달에 활용할 수 있도록 훈련 시켜주어야 한다. 적응 반응을 계획하고 조직화하여 의미 있는 활동을 통해 중추신경계의 처리능력을 향상시키는 치료프로그램이 제공되어야 한다. 또한 자세조절을 안정적으로 할 수 있는 안정성 한계를 넓힐 수 있는 신경발달학적 치료와 함께, 근골격계에서 빠른 근수축 반응과 효율적인 근수축 개시순서를 유발 할 수 있도록 고유수용성 운동조절 프로그램, 자세반응 촉진 프로그램과 같은 훈련이 필요하다고 할 수 있다.

만성 폐질환 환자에서 재택 호흡재활치료방법 개발 연구 (Development of the Home-Based Pulmonary Rehabilitation Program for Patients with Chronic Lung Disease)

  • Background : Even though it is well known that pulmonary rehabilitation (PR) improves exercise capacity, and the quality of life, in patients with chronic lung diseases, not many patients can attend hospital based intensive PR in Korea. The purpose of this study was to develop a method for a home-based PR program, and study its effectiveness. Materials and Methods : Twenty patients with chronic lung diseases were randomly divided into two groups : a home PR group comprising of 10 male patients, with a mean age of 70 years, and a control group comprising of 10 male patients, with a mean age of 65 years. We developed exercise programs, depending on the exercise capacity of each patient, which were easy to do at home. The PR program consisted of a 12 week period of enforced aerobic (mostly walking) and muscle strengthening exercises, as prescribed by the exercise specialist, in accordance with the functional capacity of the patient. In addition to the education, nutritional and psychiatric consultation was undertaken, and respiratory muscle training arranged. Patients visited hospital every 2 weeks for evaluation and exercise prescription. Results : All patients finished the 12 week course of therapy. Following the home PR, the endurance times and work capacity of the upper and lower extremities were significantly increased in the treatment group in comparison to the controls. The six minute working (Eds note:should) 'working' read "walking"?) distance was increased from $465{\pm}60m$ to $508{\pm}37m$ and the maximal inspiratory pressure from $72.8{\pm}27.2cmH_2O$ to $91.4{\pm}30.9cmH_2O$. The quality of life, as assessed by St Georges Respiratory Questionnaire (SGRQ), was also improved following PR. Conclusion : The home PR program we developed seemed to be applicable, and effective, to most of the patients with chronic lung diseases in this study.

경도 인지손상을 가진 뇌졸중 환자의 상지 기능에 미치는 강제유도운동치료(CIMT)와 인지-지각 훈련의 병행 효과 (The Effect of Constraint-Induced Movement Therapy(CIMT) With Cognitive-Perceptual Training on Upper Extremity Function of Stroke Patients With Mild Cognitive Impairment)

  • 본 연구는 강제유도운동치료와 인지-지각 훈련을 병행하여 환측의 손 기능 변화, 일상생활 과제 수행 시 환측 상지의 사용빈도 변화, 그리고 환측 상지의 움직임의 질적 변화를 알아보고자 하였다. 연구의 대상은 Taub 등 (1999)이 제시한 조건을 만족하고 뇌졸중 발병 후 3개월이 경과하였고 NCSE 또는 MVPT 결과 경도 손상을 보이는 환자 10명을 대상으로 하였다. 중재 방법은 CIMT군은 4주 동안 낮 시간동안에는 변형된 resting arm-splint를 착용하도록 하여 건측 상지의 사용을 제한시켰고, CIMT+CPT군은 CIMT군과 같은 조건에서 인지-지각 훈련을 병행하여 실시하였다. 대조군은 변형된 resting arm-splint를 착용하지 않고 일반적인 작업치료를 받도록 하였다. 그 결과, Jebsen-Taylor Hand Function Test의 먹는 흉내 내기, 크고 가벼운 깡통 옮기기, 그리고 크고 무거운 깡통 옮기기 항목에서 CIMT+CPT군이 가장 큰 변화량을 보였으며 CIMT군도 CTL군과 비교하여 더 많은 호전을 보였다. 각 집단 간의 Motor Activity Log의 환측 상지의 사용빈도에 대한 변화량은 통계학적으로 유의한 차이를 보였다(p<.05). 집단 간의 환측 상지의 움직임의 질적 향상에 대한 변화량은 CIMT군과 CIMT+CPT군 모두 CTL군보다 통계학적으로 유의한 차이를 보였다(p<.05). 따라서 인지 지각 능력의 경도 손상을 가지고 있는 뇌졸중 환자에게 실시한 강제유도운동치료는 환측의 손 기능 향상과 일상생활 과제 수행 시 환측 상지의 사용 빈도와 움직임의 질적 향상에 효과를 보였고, 강제유도운동치료와 인지-지각 훈련을 병행하였을 때 더 큰 시너지 효과를 보였다.
