한방병원에 입원한 경추 추간판 탈출증 환자의 치료 및 경제성 평가 연구 (An Analysis of Treatment and Economic Evaluation on the Part of Cervical HIVD Inpatients at Korean Medicine Hospital)

  • Objectives Herniated Intervertebral Disc of C-spine is one of the most common diseases that causes posterior neck pain. This study was designed to analyze the general distribution and hospital cost by day and case of Korean medical treatment for Cervical Herniated Intervertebral Disc (HIVD). Methods The 132 impatients for treatment of HIVD were analyzed according to the distribution of sex, age, the duration of HIVD, the contributory factors, the Clinical grade at admission, the clinical findings at admission, the duration of hospitalization, the clinical grade at admission and the hospital cost per day and case. Results 1) The total hospital cost per case averaged 1,985,600 Won, which was consisted of room charge 584,044 Won (29.41%), performance fee 511,463 Won (25.76%), herbal medication 381,517 Won (19.21%), Korean medical physiotherapy 296,310 Won (14.92%), food expenses 199,997 Won (10.07%) in order. 2) The total hospital cost per day averaged 137,285 Won, which was consisted of room charge 39,036 Won (28.43%), performance fee 33,594 Won (24.47%), herbal medication 30,642 Won (22.32%), food expenses 12,870 Won (9.37%), and the average duration of hospitalization was 15.1 days. 3) There was statistically significant difference in the consultation fee, room charge, and herbal medication on the part of sex. 4) There was statistically significant difference not only in the performance and consultation on the part of duration of hospitalization but also the in the duration of average duration of hospitalization. 5) There was statistically significant difference in the Korean medical physiotherapy and herbal medication on the part of clinical findings. 6) There was statistically significant difference in the Korean medical physiotherapy and herbal medication on the part of duration of HIVD. 7) There was statistically significant difference in the Korean medical physiotherapy and herbal medication on the part of clinical outcome. 8) There was not only statistically significant difference in the performance, but also in the herbal medication on the part of clinical grade at admission. Conclusions This study provides plenty of information to design out the specific terms of Korean medical expenses of Cervical HIVD inpatients hospitalized at Korean medicine hospital.

  • 본 증례에서는 절식치료 중인 환자에 있어 글리세린 관장을 실시한 직후 발생한 허혈성 대장염 1례에 대해 고찰하여, 관장으로 인한 장관 내압 상승, 상온의 체온보다 낮은 관장액으로 인한 혈관 경련, 선택적 세로토닌 재흡수 억제제에 의한 출혈 경향 증가 및 글리세린 용액의 화학적 삼투작용으로 인한 점막 손상 가능성 등을 제시하였다. 관장으로 인한 이와 같은 부작용을 예방하기 위하여 임상의들의 주의가 요구된다.

The Comparative Study on the Satisfaction of the Physical Therapy facilities in the front Military hospitals and Divisions

  • Objective: This study is about the physical therapy facilities and environmental and service satisfaction of patients visiting in the front military hospitals and divisions. Design: Survey study Methods: Prior to this survey, 28 users of the front division and military hospital were surveyed in advance to verify the survey through reliability analysis. For this survey, 1,300 questionnaires were distributed to seven frontline divisions and three military hospitals, of which 578 military hospitals and 479 divisions were collected. Among them, 1,042 pieces of data were used for the analysis, excluding 15 questionnaires that exceeded the research period of the division. Analysis methods used Cronbach-α and multiple regression for mean comparison analysis, factor analysis for validation, and reliability verification to verify differences between groups. Results: As a result of the satisfaction analysis, the division's stage satisfaction was higher than that of military hospitals, and as a result of analyzing the satisfaction items by sector, accessibility, re-visit doctors, and professionalism accounted for a large portion in the field of physical therapy services. As a result of a regression analysis, the factors that affect the satisfaction of physical therapy had the greatest impact on facility and environmental satisfaction, and expertise in physical therapy services. Conclusions: In the future, it will be necessary to improve the comfort of the physical therapy room and the expertise of physical therapy in order to improve the satisfaction level of physical therapy at military medical facilities.

Rehabilitative goals for patients undergoing lung retransplantation

  • Lung retransplantation (LRT) involves a second or subsequent lung transplant (LT) in a patient whose first transplanted graft has failed. LRT is the only treatment option for irreversible lung allograft failure caused by acute graft failure, chronic lung allograft dysfunction, or postoperative complications of bronchial anastomosis. Prehabilitation (rehabilitation before LT), while patients are on the waiting list, is recognized as an essential component of the therapeutic regimen and should be offered throughout the waiting period from the moment of listing until transplantation. LRT is particularly fraught with challenges, and prehabilitation to reduce frailty is one of the few opportunities to address modifiable risk factors (such as functional and motor impairments) in a patient population in which there is clearly room to improve outcomes. Although rehabilitative outcomes and quality of life in patients receiving or awaiting LT have gained increased interest, there is a paucity of data on rehabilitation in patients undergoing LRT. Frailty is one of the few modifiable risk factors of retransplantation that is potentially preventable. As such, it is imperative that professionals involved in the field of retransplantation conduct research specifically exploring rehabilitative techniques and outcomes of value for patients receiving LRT, because this area remains unexplored.

Syringe에 재어 놓은 약침의 시간에 따른 미생물학적 안전성 연구 (The effects of exposure at room temperature to pharmacopuncture within a syringe : an investigation of changes in microbiological safety)

  • Objectives : The main aim of this study is to evaluate changes in the microbiological safety of pharmacopuncture exposed to room temperature for an elapsed period of time. Methods : The four most frequently used pharmacopuncture products were stored in syringes at room temperature at three different hospitals for 24 hours and 48 hours respectively, and they were compared with pharmacopuncture products stored in vials through a sterility and microbial limited test. Results : Storage forms and duration of exposure to room temperature did not show significant difference in bacterial or fungal contamination, which was confirmed by the sterility and microbial limited test. Conclusions : Pharmacopuncture products stored in syringes at room temperature for 24 hours and 48 hours demonstrated their safety in terms of lack of microbiological contamination.

성인 편마비 환자의 일상생활동작 수행능력 평가 (Performance of Activities of Daily Living in Adult Hemiplegic Patients)

  • The purpose of study was that the functional gains achieved by stroke patients after rehabilitation in the acute phase have been documented. Sixty stroke inpatients who were at least 3.57 month postonset participated;32 males and 28 females;mean age, 57.4 years. By the classification of diagnosis, 20 subjects were cerebral infarction and 27 subjects were cerebral hemorrhage. Activities of daily living(ADL) performance was investigated using the Klein-Bell ADL Scale. ADL test was practiced within physical therapy room. The outcome variables were first analyzed using frequency statistics. The functional gains of the patients with sex and hemiplegic site were then using an ANOVA. Mean score of dressing, elimination, mobility, bathing/hygiene, eating, and telephone use, 26.6, 56.0, 41.4, 36.5, 82.0, and 56.7. Mean score of total, 43.0. There was no significant difference in sex and hemiplegic site.

Relationship among Stress, Anxiety-depression, Muscle Tone, and Hand Strength in Patients with Chronic Stroke: Partial Correlation

  • PURPOSE: This study was conducted to identify the relationships among stress response inventory, hospital anxiety and depression, muscle tone and stiffness, and hand strength in chronic stroke patients. METHODS: A total of 14 chronic stroke patients voluntarily agreed to this experiment and were included in this study. All measurements were performed in one day and in a room without noise. The tests conducted in this study were as follows: muscle tone and stiffness of the upper trapezius hand grip measurement. Subjects were also asked to complete surveys describing the following: stress response inventory and hospital anxiety and depression scale. RESULTS: There were significant correlations among stress response inventory and hospital anxiety and depression, stress response inventory and hand strength, and hospital anxiety and depression and hand strength (P<.05). There were high positive correlations between stress response inventory and hospital anxiety and depression (r=.979), while there were moderate negative correlations between stress response inventory and hand strength (r=-.415) and between hospital anxiety and depression and hand strength (r=-.420). CONCLUSION: The results of the present study indicate that there is a relationship among stress response inventory, hospital anxiety and depression, and hand strength in patients with chronic stroke.

Bilateral anterior dislocation in the hips: a case report

  • The hip is a stable ball-and-socket joint. Bilateral anterior dislocations of the native hip joints account for fewer than 1% of all dislocations. We present a unique case of a bilateral anterior dislocation in a patient who presented to our institution within 6 hours of trauma. The dislocations were promptly reduced under propofol anesthesia in the operating room. The patient did not suffer a concurrent fracture. After the procedure, we performed regular X-ray examinations for 2 years to rule out the development of avascular necrosis of the head of the femur. The course of the patient was unremarkable.

  • Physiotherapy may be defined as the use of physical techniques for the treatment of injuries and movement dysfunction. In the world of human medicine, physiotherapy has been proven as an indispensable aid in the recovery of many musculoskeletal conditions, as evidenced by the extensive physiotherapy departments within most hospitals. Nowadays, this important branch of medicine is also rapidly becoming a recognised tool in the prevention, cure, and rehabilitation of many equine, canine and feline injuries. In 1978, canine physical therapy techniques were described by Ann Downer, a physical therapist on faculty at Ohio State University. Animal physical therapy is a new and rapidly developing field of health care for animals. The benefits of physical therapy have long been recognized in humans. More recently, work in the veterinary field has shown the same benefits of physical therapy to be true for animal patients. Performing orthopaedic or neurological surgery, or fitting a human patient with a cast or splint, and then discharging the patient is an outdated approach. In such cases, physical therapy is clearly warranted. Similarly, recent research has shown that post-surgical rehabilitation and therapy after injuries significantly improves the functional outcomes for animals. The goals of physiotherapy are to relieve pain, restore range of motion/movement, improve function, prevent injuries and expand the physical potential of the patient. Once in the field, physical therapists actively continue their education to keep up to date on the latest treatments and technologies. Via continuing education courses, physiotherapists can learn how to apply their unique and specialized knowledge to other animal species.

The Clinical Results of Conservative Treatment of Frozen Shoulder Using Continuous Passive Motion

  • Background: The purpose of this study is to administer conservative treatment in 30 patients diagnosed with idiopathic frozen shoulder, following the suggested frozen shoulder rehabilitation program and to assess the clinical outcome using a prospective study. Methods: Thirty patients diagnosed with idiopathic frozen shoulder, treated with steroid hormone injection on the articular joint with an intra-articular steroid (triamcinolone 40 mg+lidocaine 4 ml) injection and started on stepwise shoulder extension exercise were chosen. The subjects were divided into two groups of 15 people each with one group undergoing rehabilitation with continuous passive motion (CPM) and the other group without it. Follow-ups were done before rehabilitation and at 4-week intervals with the 24th being the final week. At every follow-up, passive range of motion (ROM) was measured and surveys on pain and clinical score were administered. Results: In the last follow-up, both groups showed statistically significant improvements in all evaluation criteria. However, no statistical difference in all values of the ROM and Constant score evaluation criteria was observed between the groups. Only in the last follow-up, group 1 had a visual analog scale (VAS) score of $2.4{\pm}2.1$ points, which was lower, with statistical significance, than the VAS score of group 2, which was $4.4{\pm}3.1$ points (p<0.001). Conclusions: Study using CPM in treatment of frozen shoulder has been inadequate, meaning that there is still room for improvement and need for more study on setting a more specific protocol and guidelines for this procedure.
