Research Trends of Randomized Controlled Trial Studies on the Herbal Treatment of Avascular Necrosis of the Femoral Head Using China National Knowledge Infrastructure Database (대퇴골두 무혈성 괴사의 탕약 치료에 대한 무작위대조군 연구의 최신 경향 분석: 중국지망(中國知網) 검색을 중심으로)

  • Objectives This study was conducted to investigate a randomized controlled trial study on the clinical treatment of herbal medicines for avascular necrosis of the femoral head. Methods By March 2022, the China National Knowledge Infrastructure (CNKI) was reviewed about randomized controlled trial studies on the herbal treatment of avascular necrosis of the femoral head through. The publication year, evaluation criteria, treatment period, treatment method and effectiveness were analyzed for articles published from 2017 to 2022. Results 16 articles were selected and 14 kinds of herbal medicines were used. In most studies, Western medical treatments such as injection therapy, oral drug administration, traction therapy and mid-frequency electric therapy were treated alone in the control group, and a combination of Western medical treatment and herbal treatment was administered to the experimental group. Conclusions In China, there have been many studies on herbal treatment for avascular necrosis of the femoral head. In Korea, more clinical studies on the herbal treatment of avascular necrosis of the femoral head are needed. This study will be helpful for future research in Korean medicine on avascular necrosis of the femoral head.

A Study on the Significance of Sanghanron Prescription in Traffic Accident Patient (교통사고 환자에 대한 상한론 처방의 유의성 연구)

  • Objectives : The aim of this study is to make proof of availability for Sanghanron prescription. Methods : The study group comprised 80 Koreans, male:female ratio of 31:49, with mean age of 40.78 years. The SF-MPQ and PDI were checked hospitalized and discharged patients. Results : 1. The most uesd prescription is Galgeun-Tang(Gegen-tang) and the other 30 prescriptions were uesd. 2. Hospitalized patients, there was significant difference in SF-MPQ and PDI scores(P<0.05). 3. The treatment was effective to 72 patients out of 80 who had been hospitalized for less than 14 days. 4. Experimental group compared with Lee's thesis was significantly difference in 7 days of treatment, but the other compared were not significant difference. Conclusions : This data suggested that the Sanghanron prescription could be effective treatment about traffic accidental patients without surgical injury. Further study is needed about the effectiveness of Sanghanron prescription.

  • Objectives : The purpose of this study is to search for more effective methods of diagnosis and treatment of Fracture. Methods : Literature review on Fracture in view of oriental and western medicine. Conclusions : Fracture is classified by anatomical location, grade, shape of line, displacement and cause. The symptom of Fracture is pain, tenderness, deformity, attitude, abnormal mobility, crepitus, neurovascular injury. Fracture is not the same in Healing process by location. The age, endocrine system, chronic debilitating disease, stabilization is effect on healing period and process. Treatment of Fracture is classified emergency care, definite treatment and rehabilitation.

  • Objectives The objective of this study is to report clinical effect of korean traditional treatments for Operation-delayed Cauda Equina Syndrome patient. Methods The patient was treated by korean traditional treatments including acupuncture, physical treatment, herbal medication. The improvement of the clinical symptoms was observed by Modified Visual analog scale (Modified VAS), SSCES (Scoring System for Cauda Equina Syndrome), function of urination and function of defecation. Results After treatments, Modified VAS, SSCES, function of urination and function of defecation were improved in this case. Conclusions On the basis of these results, we suggest that korean traditional treatment might be an effective method to improve the clinical symptoms of Operation-delayed Cauda Equina Syndrome patient.

  • The purpose of this study is to report the effects of korean medical treatment including chuna after cervical surgery. We used acupuncture, chuna manual therapy, herbal medication on this patient and measured neck disability index (NDI) score, numerical rating scale (NRS) and cervical range of motion (ROM) to evaluate the treatment effects. Patient's NDI score and neck and upper extremity pain NRS were decreased and cervical ROM was increased.

  • The purpose of this clinical case is to evaluate the efficacy of the Korean medical treatment on a ligamentum teres tear of the hip. One patient, admitted to Daejeon Jaseng Hospital of Korean Medicine was diagnosed with a ligamentum teres tear of the hip by hip magnetic resonance imaging. We treated the patient using the Korean medical treatments, including acupunture, pharmacopuncture, herbal medication, and cupping. We measured the numerical rating scale (NRS), passive range of motion (PROM) and the changes of symptoms. After 13 days, NRS decreased from 8 to 1 respectively, PROM of hip joint has recovered, and the symptoms of the patient improved. In conclusion, this study shows that Korean medical treatment might be effective in treating the patient with a ligamentum teres tear of the hip.

Painful Jerk Test: A Predictor of Success in Nonoperative Treatment of Posteroinferior Instability of the Shoulder (통증성 Jerk 검사: 견관절 후하방 불안정성의 보전적 치료 결과의 예측)

  • The purposes of this study were to evaluate the presence or absence of pain with the jerk test as a predictor of the success of nonoperative treatment for posteroinferior instability of the shoulder and to identify pathologic lesion responsible for the pain in the jerk test. Eighty-nine shoulders(81 patients), which had posteroinferior instability with positive posterior clunk in the jerk test, were nonoperatively treated. The patients were divided into two groups with respect to the presence of pain in the jerk test: painless jerk group(54 shoulders) and painful jerk group(35 shoulders). Response to the nonoperative treatment was evaluated after at least 6 months rehabilitation program. Patients who did not respond to the rehabilitation underwent arthroscopic examination to identify any pathologic lesion. The painful jerk group had higher failure rate with nonoperative treatment (p<0.001). In the painless jerk group, fifty shoulders (93%) responded to rehabilitation program after a mean of 4 months. Four shoulders(7%) were unresponsive to the rehabilitation. In the painful jerk group, five shoulders(16%) were successful with the rehabilitation while the other thirty shoulders(84%) failed. All 34 shoulders, which were unresponsive to the rehabilitation, had a variable degree of posteroinferior labral lesions. In conclusion, the jerk test is a hallmark for predicting the prognosis of nonoperative treatment in the posteroinferior instability. Shoulders with symptomatic posteroinferior instability and a painful jerk test have posteroinferior labral lesion.

The Clinical Study of Repeatedly Performed of Balneotherapy on Cervical Pain (반복적으로 시행한 온천요법이 경항통에 미치는 영향)

  • Objectives The purpose of this study was to investigate the clinical effects of repeatedly performed of balneotherapy on cervical pain. Methods We investigate 19 cervical pain subjects in this study. Subjects took 5 session of balneotherapy once a week during 5 weeks. Each session consisted of 15 minutes of high pressure underwater shower ($32{\sim}36^{\circ}C$) and another 15 minutes of whole body bathing ($32{\sim}40^{\circ}C$). To evaluate the efficiency of balneotherapy visual analogue scale (VAS) and pain threshold were applied before treatment and after 1st, 2nd, 3rd, 4th, 5th treatment. Then the results were analysed. Results 1) The average of VAS significantly decressed from $59.11{\pm}14.67$ to $33.95{\pm}20.988$ after 5 weeks of balneothreapy treatment (p<0.001). 2) The average of pain threshold significantly increased from $5.76{\pm}1.294$ lbf to $8.74{\pm}1.126$ lbf after 5 weeks of balneothreapy treatment (p<0.001). Conclusions Balneotherapy has clinical effscts of pain reduction on cervical pain subjects. Balneotherapy can be used in addition to the Oriental physiotherapy for high effective treatment on cervical pain. Further clinical studies are required to verify these findings.

  • Objectives: The purpose of this case is to investigate and report the effectiveness of korean Medicine treatment and Harpagophytum radix. Pharmacopuncture for the type 1 complex regional pain syndrome. Methods: One patient is admitted at Bu-Chun Jaseng Hospital of Korean Medicine, diagnosed as type 1 complex regional pain syndrome and treated with korean Medicine treatment and Harpagophytum radix. pharmacopuncture. This case is measured and assessed by Neumerical Rating Scale(NRS), Oswestry Low-back Pain Disability Index(ODI) and Manual Muscle Test(MMT). Results: After treatment, the intensity and frequency of complex regional pain is gradually decreased and the result of NRS, ODI, MMT showed improvement. Conclusions: As seen in this cases of type 1 complex regional pain syndrome, korean Medicine treatment and Harpagophytum radix. pharmacopuncture appear to be effective for the pain relief and musculoskeletal function recovery.

Electro-Acupuncture for Cervical Vertigo: A Systematic Review and Meta-analysis (경추성 현훈에 대한 전침 치료: 체계적 문헌고찰 및 메타분석)

  • Objectives The objective of this study was to appraise the effect of electro-acupuncture (EA) for the treatment of cervical vertigo. Methods We performed a literature search using nine electronic databases up to the end of November 2020. We included randomized controlled trials (RCTs) which appraised the effect of EA in the treatment of cervical vertigo. The risk of bias was evaluated from the Cochrane risk of bias tool. Results Thirteen appropriate RCTs were included and analyzed. The efficacy rate of the EA group was statistically higher compared with the control group treated with western medicine (p<0.00001). EA treatment significantly improved efficacy rate when combined with western medicine than western medicine alone (p<0.0001). Also EA used with combined therapy (traction, traditional chinese medicine, injection) significantly improved efficacy rate compared with combined therapy alone (p=0.02). Meta-analysis of thirteen studies demonstrated positive results for using EA for the treatment of cervical vertigo. Conclusions Based on results, EA treatment could be effective in cervical vertigo. However, it should be noted that the studies included in this research are regionally biased and small in number. Further well-designed research should be required.
