A Study on the Recognition of Students of Oriental Medical School on Cooperative System between Oriental and Western Medicine (한양방 협진에 대한 한의대생들의 인식도)

  • The purpose of this study is to examine the recognition of the student of oriental medical school on cooperative system between Oriental and Western medicine and to provide basic information for the development of oriental medicine. In order to look at the level of recognition on cooperative system between Oriental and Western medicine, from September 1st to 15th of 2005, this study had been conducted through personal interview and questionnaires to 600 students who were attending the Department of Oriental Medicine (in both prep and regular courses) in D University, located in the City of Daegu. The data has been analyzed using statistic program, the SPSS WIN 12.0. Statistical analysis tools used for this study were frequency analysis, cross-over analysis and the t-test. The results are as follows; The students of oriental medical school had relatively high level of recognition on the basic concept, interests, necessity and potential for cooperative system between Oriental and Western medicine. However, they had negative understandings on the issue of unification of the two medical systems and it's possibility in the future. The students were optimistic about possible merits of cooperative system between Oriental and Western medicine, especially in the field of rehabilitation. On the reasons for lack of development of cooperative system between Oriental and Western medicine, the students listed prejudices existing on both sides firstly, followed by their contrasting approach on human illness, lack of legal and institutional support system, the indifferences of doctors and indifferences of oriental medical doctors. In addition, students understood that the dualism existing in our current medical system is aggravating mutual distrust between the two sides, causing the confusion of patients on the choice of medical facilities, and raising their medical bills. Therefore, in order to vitalize collaboration between Oriental and Western medicine in new health care environment, the following measures should be needed decreasing the prejudices between the two medical spheres with open mind improving educational programs in Western and Oriental medical schools; promoting joint academic research or exchange programs between the schools, and increasing government effort to minimize legal and institutional restrictions cooperative system between Oriental and Western medicine.

  • Objective : This case study examined the evaluation of occupational therapy and plan to intervention of community asperger syndrome child receiving coping model. Methods : We selected child which 7-year-old boy. Evaluation periods were 2weeks which consisted of external factors and internal factors. External factors were made up interaction of subject, environments and participation of school and community. Internal factors were made up observation and structured evaluation about development state and medical conditions. Also it included observation of appropriate mood and emotions. Results : After evaluation receiving coping model, we planed to intervention. First, subject able to use his time effectively. Second, we'll have intervention program about delayed fine motor areas. Third, we'll educate self-control skills and coping skills of subject's action which not controlled himself. Fourth, we'll find the personal and physical sources to care subject. Conclusion : Our research has planed occupational therapy intervention receiving coping model of asperger syndrome subject. Future research need to practical applications.

A Measurement System for Color Environment-based Human Body Reaction (색채 환경 기반의 인체 반응 정보 측정 시스템)

  • The result of analyzing the cognitive reaction due to the color environment has been applied to various filed especially in medical field. Moreover, the study about the identification of patient's condition and examination the brain activity by collecting the bio-signal based on the color environment is being actively conducted. Even though, there were a variety of experiments by convention the color environment using a light or LED color, it still has a problem that affects the psychological information. Therefore, our proposed system using a HMD (Head Mounting display) to provide a completed color environment condition. This system uses the BMS(Biomedical System) to collect the biometric information which responds to the specific color condition and the human body response information can be measured by the development the Memory and Attention test on Mobile phone. The collection of Biometric information includes electro cardiogram(ECG), respiration, oxygen saturation (Sp02), Bio-impedance, blood pressure will store in the database. In addition, we can verify the result of the human body reaction in the color environment by Memory and Attention application. By utilizing the reaction of the human body information that is collected thought the proposed system, we can analyze the correlation between the physiological information and the color environment. And we also expect that this system can apply to the medical diagnosis and treatment. For future work, we will expand the system for prediction and treatment of Alzheimer disease by analyzing the visualization data through the proposed system. We will also do evaluation on the effectiveness of the system for using in the rehabilitation program.

Characteristics Change of Spatial and Temporal Parameters of Gait in Spastic Hemiplegic Patients by Reciprocal Inhibition (상호억제 기법에 의한 경직성 편마비 환자 보행의 공간적, 시간적 특성 변화)

  • The purpose of this study was to determined the effects of reciprocal inhibition on spatial-temporal gait parameters in spastic hemiplegic patients through GaitRite system. The subjects were consisted 45 patients who had spastic hemiplegia due to stroke. All subjects randomly assigned to 3 group : manual reciprocal inhibition program group(manual group), neuromuscular electrical stimulation group(NMES group) and control group. The manual group received voluntary isometric contraction of pre-tibia muscle. The NMES group received neuromuscular electrical stimulation on tibialis anterior. The control group was not recieved any therapeutic intervention. Before and after experiments, spatial-temporal gait parameters and functional ambulatory profile was measure in all patients. The data of 30 patients who complete experimental course were statistically analysed. The results of this study were as following : 1. The percentage of change of functional ambulatory profile were markedly increased in manual group but statistically non significant(p>.01). 2. The percentage of change of gait velocity and cadence were markedly increased in manual group but statistically non significant(p>.01). 3. Asymmetry ratio of gait elements were more improved in manual group but statistically non significant(p>.01). 4. There were no statistical difference between pre-test and post-test with functional ambulatory profile, gait velocity, cadence and asymmetry ratios in NMES group(p>.01). 5. There were no statistical difference between pre-test and post-test with unctional ambulatory profile, gait velocity, cadence and asymmetry ratios in control group(p>.01). In conclusion, the present results revealed that reciprocal inhibition which produced by voluntary isometric contraction of pre-tibia muscle can be improved spatial-temporal gait parameters including functional ambulatory profile in hemiplegic patients. Therefore, reciprocal inhibition is useful to improve functional activities in hemiplegic patient. Further study should be done to analyze the effects of intervention duration of reciprocal inhibition, appropriate muscle contraction, optimal time to apply the reciprocal inhibition in more long period.

The Effects of Motor Control and Strengthening Exercise on Secondary Shoulder Impingement Syndrome at Postmastectomy (운동조절과 근력강화 운동이 유방절제술 후 견관절의 기능부전으로 유발된 견관절 충돌 증후군에 미치는 영향)

  • This study was to identify treatment effects of the shoulder control and strengthening exercise, on the subjects with secondary shoulder impingement syndrome at postmastectomy. The subjects were patients who visited our hospital due to secondary shoulder impingement syndrome(1-2 stage) at postmastectomy and they randomly allocated to two groups: a shoulder control and strengthening group (n=10) and a conservative therapy group (n=10). Both groups received conservative therapy for 5 sessions (40 minutes per week) for 4week. The shoulder control and strengthening exercises group practiced additional motor control and strengthening exercises for 60 minutes. Values of handgrip strength, pain(visual analog scale), upper extremity circumference, Disabilities of the Arm, Shoulder and Hand questionnaire, range of motion were compared with those of the conservative therapy group. There were significant differences in the amount of change of the range of motion and Disabilities of the Arm, Shoulder and Hand scale between the two groups (p<.05), however as a measure of handgrip strength, pain(visual analog scale), upper extremity circumference did not show a significant differences. These results suggest that a motor control and strengthening exercise program is feasible, secure and suitable for secondary shoulder impingement syndrome at postmastectomy.

Clinical Study on the Survival Rate and Marginal Bone Resorption of Short Implants (짧은 임플란트의 생존율과 변연골 흡수량에 관한 임상적 연구)

  • Short implants are used in parts which have anatomical structures like maxillary sinus, inferior alveolar nerve and limited alveolar height due to severe alveolar bone resorption. In these case, there are no need of additional bone augmentation so there are advantages like reduced entire treatment time, reduced patient's discomfort and protection of important anatomical structures. The aim of this study is, in implants whose length is less than 10mm, to analyze the impact of implant length, diameter, location of implant placement, presence of bone graft, presence of prosthesis splinting on survival rates and marginal bone resorption. The samples used in this study were 227 implants, less than 10mm, placed in 137 patients in Wonkwang university dental hospital implant center. From dental charts the information about implant length, diameter, location of implant placement, presence of bone graft, presence of prosthesis splinting were obtained. Emago advanced v5.6(Oral diagnostic systems, Amsterdam, The Netherlands)program was used to measure the amount of marginal bone resorption. Out of total 227 implants, resulting in 96.5 % of survival rate. There was a tendency toward higher failure rates for the maxilla and bone graft site. No significant difference in marginal bone resorption was found associated with length of implants(p>0.05) and neither with the diameter of implants. Among the risk factors examined, more failure rates of short implants can be attributed to poor bone quality in the maxilla and presence of bone graft. At implants under 10mm, length, diameter, location of implant placement, bone graft and splinting of prosthesis didn't affect marginal bone loss.

Finite Element Analysis of Stress Distribution in using Face Mask according to Traction Point (훼이스 마스크의 견인위치에 따른 응력분포에 관한 유한요소법적 연구)

  • The objective of this study was to analyse stress distribution of maxillary complex by use of face mask. The construction of the three-dimensional FEM model was based on the computed tomography(CT) scans of 13.5 years-old male subject. The CT image were digitized and converted to the finite element model by using the mimics program, with PATRAN. An anteriorly directed force of 500g was applied at the first premolar 45 degrees downwards to the FH plane and at the first molar 20 degrees downwards to the FH plane. When 45 degrees force was applied at maxillary first premolar, there were observed expansion at molar part and constriction at premolar part. The largest displacement was 0.00011mm in the x-axis. In the y-axis, anterior displacement observed generally 0.00030mm at maximum. In the z-axis, maxillary complex was displaced 0.00036 mm forward and downward. When 20 degrees force was applied at maxilla first molar, there were observed expansion at lateral nasal wall and constriction at molar part. The largest displacement was 0.001mm in the X-axis. In the Y-axis, anterior displacement observed generally 0.004mm at maximum. In the Z-axis, ANS was displaced upward and pterygoid complex was displaced downward. The largest displacement was 0.002mm.

A Systematic Review on Sensory Integration Intervention in Korea: Focusing on Ayres Sensory Integration (ASI) (국내 감각통합치료 중재에 관한 체계적 고찰 - Ayres의 감각통합중재(ASI)를 중심으로)

  • Objective : The aim of this study was to investigate how many studies for sensory integration intervention in Korea reflect the principle of Ayers' sensory integration through a systemic review. Methods : The systemic review was executes using RISS, NDSL, KISS, DBpia. The key words for search were 'sensory integration therapy', 'sensory integration intervention', 'sensory integration program', 'sensory integration training' to search the articles, published in 2000-2012. To analyze ASI fidelity of 49 studies included in the present study, the numbers of items were calculated if the study describe the ASI principle. Results : The IV was the most levels of evidence, and the subjects for the studies were mental intellectual disability (15), developmental disability (14) and autism spectrum disorders (13) etc. It was described the most that presenting sensory opportunities (43), challenging postural, ocular, oral and/or bilateral motor control (37) of ASI fidelity. The studies using more than 4 ASI principle were 26.5%(13), and 7 studies using 8 or more than 8 were studied in OT. Conclusion : In OT, 34 of 49 studies performed, and they used several principles of ASI. To have unity for sensory integration principle and to improve therapy professionalism the study using various data and systematic review study will be needed in the future.

A Study on the Human Rights Sensitivity of Employees in the Vagabond Facilities (부랑인복지시설 종사자들의 인권민감성에 관한 연구)

  • This study did by purpose to be utilized to basic data for elevation of human rights consciousness of homeless facilities employees through examining their human rights consciousness by investigating human rights sensitivity that can grasp degree of human rights consciousness of homeless facilities employees. The human rights sensitivity of 390 homeless facilities employees who are working at 38 facilities around the whole country is compared and investigated. As a result, human rights sensitivity of employees displayed very low score by average 64.55 points in total score 150 points. Human rights sensitivity academic career of employees was shown meaningful difference, and human rights consciousness of substantial administrator is appeared very low in human rights sensitivity by duties. According to the existence and nonexistence of human rights education at the homeless facilities, the employs who had got human rights education shows higher human rights sensitivity than the employs who had not go human rights education. Therefore, regular program that can enforce human rights education Collectively as a formal curriculum in homeless facilities should be established and it's proposed to develop practical and efficient human rights education manual case by case related with the violation of human rights.

The Comparison of Trunk Muscle Activities During Sling and Mat Exercise (요부 안정화 운동에 따른 몸통 근육들의 근활성도 비교)

  • The purpose of this study was to verify the most effective spinal stabilization exercises program by comparing the activities of muscles contributing to spinal stabilization during four types of exercises using a sling and a mat. Twenty healthy males were recruited and each subjects performed four types of exercises. Exercise 1 was performed in a quadruped position with the subjects lifting the left arm and the opposite leg on the mat. Exercise 2 was performed in a prone position while holding a sling with the right hand and the left knee was fully extended while lifting the left arm and right leg. Exercise 3 was performed in quadruped position while holding a sling with one the right hand and lifting the opposite arm and leg. In exercise 4, subjects were instructed to maintain a balance push-up position while holding slings with both hands in 10 cm forward reaching with extended elbows. Electromyographic(EMG) activities were recorded from the multifidus, external oblique, internal oblique, abdominal rectus, and erector spinalis muscles during the exercises. The EMG amplitude of each muscle was normalized to the amplitude in the maximal voluntary isometric contraction (MVIC) of each muscle. Repeated ANOVA and Bonferroni's tests were used to compare the differences in the muscle activity according to the types of exercise. The EMG amplitudes of all the muscles were significantly different according to the types of exercises (p<.05). The highest EMG activities of each muscle was as follow; multifidus was 73.38%MVIC in exercise 3, the erector spinalis was 40.03%MVIC in exercise 3, the external oblique was 135.88%MVIC in exercise 4, the internal oblique was 128.60%MVIC in exercise 4, and the rectus abdominalis was 95.24%MVIC in Exercise 4. The types of exercises showed a significant difference in composition rate of EMG amplitudes of each muscle (p<.05). EMG composition rate of the multifidus was high in exercise 1 and 3. However, EMG composition rates of the external oblique, internal oblique, and the rectus abdominals were high in exercise 2 and 4. These results showed differences in EMG activities of muscles contributing to trunk stabilization during different therapeutic exercises. Therefore, the type of exercise should be carefully selected to effectively strengthen a specific trunk stabilizer.
