Reconstruction of the Inferior Orbital Wall with Simplified Simulation Technique in Case of the Fracture Extending to the Posterior Orbital Floor

  • A 37-year-old male was assaulted and complained of severe periorbital swelling. Physical examination revealed that there were limitation of eyeball movement on upper gaze, diplopia, and hypoesthesia on the infraorbital nerve innervating region. Three-dimensional (3D) computed tomography (CT) of facial bone exhibited the fracture of orbital floor accompanying the significant amount of orbital contents' herniation extending to the far posterior part. To recover the orbital volume and restore orbital floor without threatening the optic nerve, preoperative simplified simulation was applied. The posterior margin of the fractured orbit was delineated with simulation technique using cross-linkage between the coronal and sagittal sections based on the referential axial view of the CT scans. Dissection, reduction of orbital contents, and insertion of the absorbable mesh plate molded after the prefabricated template by the simulation technique was performed. Extensive orbital floor defect was successfully reconstructed and there were no serious complications. The purpose of this report is to emphasize the necessity of preoperative simulation in case of restoring the extensive orbital floor defect.

  • Defects involving specialised areas with characteristic anatomical features, such as the nipple, upper eyelid, and lip, benefit greatly from the use of sharing procedures. The vulva, a complex 3-dimensional structure, can also be reconstructed through a sharing procedure drawing upon the contralateral vulva. In this report, we present the interesting case of a patient with chronic, massive, localised lymphedema of her left labia majora that was resected in 2011. Five years later, she presented with squamous cell carcinoma over the left vulva region, which is rarely associated with chronic lymphedema. To the best of our knowledge, our management of the radical vulvectomy defect with a labia majora sharing procedure is novel and has not been previously described. The labia major flap presented in this report is a shared flap; that is, a transposition flap based on the dorsal clitoral artery, which has consistent vascular anatomy, making this flap durable and reliable. This procedure epitomises the principle of replacing like with like, does not interfere with leg movement or patient positioning, has minimal donor site morbidity, and preserves other locoregional flap options for future reconstruction. One limitation is the need for a lax contralateral vulva. This labia majora sharing procedure is a viable option in carefully selected patients.

Reconstruction and Validation of Gridded Product of Wind/Wind-stress derived by Satellite Scatterometer Data over the World Ocean and its Impact for Air-Sea Interaction Study

  • We have persistently constructed gridded products of surface wind/wind stress over the world ocean using satellite scatterometer (ERS and Qscat). They are available for users as the Japanese Ocean Flux data sets with Use of Remote sensing Observation (J-OFURO) data together with heat flux components. Recently, a new version data of the Qscat/SeaWinds based on improved algorithm for rain flag and high wind-speed range have been delivered, and allowed us to reconstruct gridded product with higher spatial resolution. These products are validated by comparisons with in-situ measurement data by mooring buoys such as TAO/TRITON, NDBC and the Kuroshio Extension Observation (KEO) buoys, together with numerical weather prediction model products such as the NCEP-1 and 2. Results reveal that the new product has almost the same magnitude in mean difference as the previous version of Qscat product and much smaller than the NCEP-1 and 2. On the other hand, it is slightly larger root-mean-square (RMS) difference than the previous one and NCEPs for the comparison using the KEO buoy data. This may be due to the deficit of high wind speed data in the buoy measurement. The high resolution product, together with sea surface temperature (SST) one, is used to examine a new type of relationship between the lower atmosphere and upper ocean in the Kuroshio Extension region.

APPROXIMATED SEPARATION FORMULA FOR THE HELMHOLTZ EQUATION

  • The Helmholtz equation represents acoustic or electromagnetic scattering phenomena. The Method of Lines are known to have many advantages in simulation of forward and inverse scattering problems due to the usage of angle rays and Bessel functions. However, the method does not account for the jump phenomena on obstacle boundary and the approximation includes many high order Bessel functions. The high order Bessel functions have extreme blow-up or die-out features in resonance region obstacle boundary. Therefore, in particular, when we consider shape reconstruction problems, the method is suffered from severe instabilities due to the logical confliction and the severe singularities of high order Bessel functions. In this paper, two approximation formulas for the Helmholtz equation are introduced. The formulas are new and powerful. The derivation is based on Method of Lines, Huygen's principle, boundary jump relations, Addition Formula, and the orthogonality of the trigonometric functions. The formulas reduce the approximation dimension significantly so that only lower order Bessel functions are required. They overcome the severe instability near the obstacle boundary and reduce the computational time significantly. The convergence is exponential. The formulas adopt the scattering jump phenomena on the boundary, and separate the boundary information from the measured scattered fields. Thus, the sensitivities of the scattered fields caused by the boundary changes can be analyzed easily. Several numerical experiments are performed. The results show the superiority of the proposed formulas in accuracy, efficiency, and stability.

PIV 측정 및 수치해석을 이용한 구강암 수술에 따른 기도 형상 내 유동 특성 (Flow Characteristics in a Human Airway model for Oral Cancer Surgery by PIV Experiment and Numerical Simulation)

  • Oral cancer surgery typically consists of resection of lesion, neck dissection and reconstruction, and it has an impact on the position of hyoid bone. Therefore, morphological change of airway can occur since the geometric parameter of airway is correlated with the hyoid bone. Airflow is affected by geometry of the airway. In this study, flow characteristics were compared between pre- and post-surgery models by both particle image velocimetry (PIV) and numerical simulation. 3D model of upper airway was reconstructed based on CT data. Velocity is accelerated by the reduced channel area, and vortex and recirculation region are observed in pre- and post-surgery models. For the post-surgery model, high pressure distribution is developed by significantly decreased hydraulic diameter, and the longitudinal flow stream is also interrupted.

Prevention and treatment of microstomia

  • The mouth, located in the lower third of the face, is a uniquely visible structure. It serves as a vital organ both aesthetically and functionally, playing a key role in speech, expression, and fundamental oral functions. Consequently, any alterations or defects in its shape, due to various causes, can lead to aesthetic and functional deficiencies. These issues may also result in challenges with social interactions and a decrease in confidence. In cases of microstomia, various surgical approaches are proposed based on the location, extent, shape, and cause of the defect, leading to numerous case reports. Plastic surgeons are proficient in oral reconstruction; however, cases of microstomia are relatively rare, which reduces their familiarity and interest in these cases. Additionally, preferences for oral size and shape vary according to factors such as geographical region and ethnicity, further complicating the functional definition of microstomia. Therefore, both subjective patient and physician judgments play crucial roles in the diagnosis and treatment of microstomia, as these may vary depending on individual and societal aspects. This review aims to classify the various causes and definitions of microstomia, as well as its non-surgical and surgical treatment options, with the goal of the treatment of this condition.

Eruption of a venous malformation through an iliac bone harvesting site after trauma

  • Harvesting grafts from the anterior iliac bone has been associated with various complications. A 50-year-old woman presented to our department with a chief complaint of right inguinal swelling and pain. Autologous bone grafts had been harvested on two previous occasions from the right anterior iliac crest for use in the reconstruction of multiple facial fractures. Computed tomography and magnetic resonance imaging revealed a full-thickness bone defect in the right anterior iliac crest. A mass was noted in the right gluteus minimus, while a multilocular cystic mass extended from the right iliac crest defect to the right inguinal region. Both the inguinal mass and gluteal mass were removed under general anesthesia. Following histopathological analysis, the gluteal mass was diagnosed as a venous malformation (VM). Based on the patient's clinical course, iliac bone graft harvesting and trauma to the gluteal region triggered hemorrhaging from the VM. Blood components leaked out from the fragile portion of the iliac bone defect, forming a cystic lesion that developed into the inguinal mass. In this case, a coincidental VM resulted in a rare complication of iliac bone graft harvesting. These sequelae could have been avoided by planning for more appropriate ways to collect the grafts.

$^{18}F$-FDG PET 영상의 정량적 비교: PET/MR VS PET/CT (Quantitative Comparisons in $^{18}F$-FDG PET Images: PET/MR VS PET/CT)

  • 감쇠 보정법과 산란 보정법은 정량적인 PET검사를 하기 위한 필수적인 방법이다. PET/CT에서는 PET에서 사용하는 소멸방사선과 CT의 X선이 같은 전리 방사선이기 때문에 측정에 의한 CT의 Hounsfield Units를 감쇠 계수로 전환해서 감쇠보정, 산란보정이 가능하다. 그러나 PET/MR에서 MR는 강한 자기장을 걸어 수소밀도와 조직의 이완률차이로 되돌아오는 변화로 신호를 획득하기 때문에 CT처럼 전환하는 것은 불가능하다. Ingenuity TF PET/MR장비는 soft tissue, lung, air로 3구역을 segment하여 MR 감쇠지도를 얻는다. 이에 신호획득원리가 완전히 상이한 PET/MR과 PET/CT에 대한 정량적 평가를 하고자 한다. Phantom study로 uniform cylinder phantom에 증류수 9293 ml와 $^{18}F$-FDG 199.8 MBq를 넣고 magnetic stirrer를 이용하여 균일하게 교반한 후 60 min부터 15분 간격으로 Ingenuity TF PET/MR, Gemini TF 64, Biograph Truepoint 40를 이용하여 각각 single-bed로 2 min씩으로 영상을 얻었다. phantom의 중심부분 10개의 slice에 대한 동일한 관심영역을 그려 SUVs를 측정하고 평균, 표준편차를 구하였다. 그리고 임상적용을 위한 평가로 $^{18}F$-FDG 섭취가 정상인 환자를 대상으로 90 sec/bed씩 Ingenuity TF PET/MR을 시행한 후 Gemini TF 64 PET/CT 검사를 실시하였다. 각각의 data에서 lung, liver, spleen, bone 위치에 동일한 관심영역을 그려 SUVs 최대값과 평균값을 측정하고, %Difference를 구하였다. 또한, PET 장비들 사이에서의 일치도를 평가하기 위해 Bland-Altman plot 분석을 하였다. Phantom study에서 3가지 장비에서 측정한 SUVs 최대값과 평균값은 Biograph Truepoint 40, Gemini TF 64, Ingenuity TF PET/MR 순으로 높은 것을 확인할 수 있었다. patients study에서는 MR과 CT로 감쇠 보정한 PET장비의 SUVs 최대값과 평균값이 서로 유의미한 차이가 없었다.(p<0.05) Lung에서 left middle lobe과 transverse bone을 제외하고는 MR로 감쇠 보정한 PET의 SUVs가 대체로 낮았다. Bland Altman Plot으로 분석한 결과 대부분의 항목에서 95% 신뢰구간의 일치한계선내에서 측정되었다. PET/CT에서는 time of flight 기능을 가진 PET이 SUVs가 낮게 측정되었다. PET/MR과 PET/CT에서 알아본 SUVs차이는 MR을 이용한 분할 감쇠 보정방법이 CT를 사용한 측정 감쇠보정방법보다 SUVs가 낮게 측정되었다. 이러한 다른 감쇠 보정법에 의한 SUVs의 차이는 임상적으로는 용인할 수준에 있었지만, 향후 PET/MR와 PET/CT의 정량적인 값을 비교 분석할 때 PET 장비들간의 특성은 고려할 필요가 있다.

복잡도 기반 적응적 샘플 오프셋 병렬화 (Complexity-based Sample Adaptive Offset Parallelism)

  • 본 논문은 High Efficiency Video Coding (HEVC)의 인-루프 필터 기술인 Sample Adaptive Offset (SAO)에 대하여 복잡도 분석기반의 병렬화 방법을 제안한다. HEVC의 SAO는 쿼드트리 기반으로 영상을 다수의 SAO영역으로 분할하고, 각 영역 단위로 에러 보정을 위한 오프셋 값을 전송함으로써 복호화된 화소의 에러를 보정한다. HEVC의 SAO는 데이터 레벨의 병렬화를 통하여 고속화할 수 있는데, SAO영역 단위의 데이터 레벨 병렬화는 영역의 크기가 일정하지 않아 멀티 코어를 사용한 병렬화시 작업량 불균형(Workload imbalance)이 발생한다. 또한, SAO는 영역 단위로 필터링 적용 여부가 결정되므로 균둥하게 SAO영역을 각 코어에 할당하더라도, 작업량 불균형이 발생할 수 있다. 본 논문에서는 SAO영역의 최소 단위인 Largest Coding Unit (LCU)를 SAO 수행의 기본단위로 하여, 각 단위에서의 SAO 파라미터 정보를 이용하여 복잡도를 미리 예측 하였다. 예측된 복잡도를 기반으로 각 코어에 균일하게 작업량이 할당될 수 있도록 영역을 코어에 적응적으로 할당하여 병렬화를 수행한 결과 순차 수행 기반 SAO에 비하여 2.38배, 영역 균등 SAO 병렬화 대비 21% 속도 향상되었다.

Ad Hoc 네트워크 기반의 가변 지오캐스팅 (Variable Geocasting based on Ad Hoc Networks)

  • 최근 산업 및 연구단체에서 이동 Ad-hoc망에 대한 관심이 높아지고 있다. 기존 연구들은 라우팅에 초점을 맞추고 있었던 것에 반해 본 논문에서는 이들 연구의 결과로 형성된 Ad-hoc망을 실제로 이용하기 위한 응용으로 가변지오캐스팅 방법을 제안한다. 지오캐스팅 프로토콜 목적은 특별한 지오캐스팅 영역에 위치한 노드들의 그룹에 패킷 데이터를 전송하는 것이다. 본 논문에서는 Ad-hoc기반의 이동 네트워크 환경에서 지오캐스트 서비스를 지원하는 기존연구에 데이터 전송경로의 비 최적화, 빈번한 지오캐스트 전송트리의 재구성으로 인한 오버헤드, 서비스단절의 문제를 해결하기 위해 이동형태기반의 가변지오캐스팅 기법을 제안한다. 가변지오캐스팅 기법은 서비스범위를 목적지 노드의 이동성에 따라 가변적으로 설정하고 자원예약기법과 다양한 실험을 통해 이동노드의 이동성에 따른 접근성 및 네트워크 오버헤드를 줄여 성능평가 하였다.
