  • Rhabdomyosarcoma (RMS) is an aggressive and fast-growing malignant tumor. RMS predominantly arises in the head and neck of infancy and children. Metastasis is usually via the blood vessel. We report a case of a recurred RMS of the tongue base with the metastasis to multiple lymph nodes in a 37-year-old female. On the follow-up examination using advanced imaging modalities after surgical treatment of RMS, the lymph nodes should be carefully evaluated like in other malignancies, such as a carcinoma, showing frequent lymph node metastasis. • Psoriasis is a common genetically determined, chronic, inflammatory skin disease characterized by rounded crythematous, dry, scaling patches. It is caused to blood heat(血熱), shortage of blood(血燥) and blood stasis(血瘀) in oriental medicine. We treated a patient who suffered from chroinc and recurred psoriasis, and who was differenciated with endogenous wind due to shortage blood(血燥生風) with Danguiumjagagam(當歸飮子加減). After 10 days of treatment with Danguiumjagagam(當歸飮子加減) and acupunture, we observed an improvement. Based on this experience, it is considered that oriental medicine can be applied to the treatment of psoriasis.

  • A gingival mass was detected from a 1-year-old female Great Dane dog. After surgical removal, the lesions recurred in 2 weeks and died of septicemia. Characteristic histologic features were large numbers of multinucleated giant cells which were connected with capillary vessels. Neovascularization was prominent with mononuclear and polynuclear cell infiltration. Overall features of these lesions except for giant cell infiltration were similar to granuloma. From these results, a gingival mass excised from a dog was diagnosed to be a peripheral giant cell granuloma (PGCG). This is the first report of canine subcutaneous PGCG in Korea.

  • Bell's palsy is not a serious disease and recovery is spontaneous in more half of the patients, but this recovery is long delayed in many instances and the cosmetic effect of an altered facial appearance is psychologically distressing. The etiology is unknown, but Kettle's ischemic hypothesis has been widely accepted. The aim of treatment is to reduce edema and improve circulation to the facial nerve. Stellate ganglion block (SGB) resulted in abolishing cerebral vascular spasm and in increasing cerebral blood flow. Thus, stellate ganglion block is effective in treatment of Bell's palsy. From 1978 to Oct. 1988, we have treated 222 patients (20 patients were recurred cases) with facial palsy by SGB and analysed the effect of SGB. We noticed that SGB was very effective in early repetitive treatment (90% recovery). This study is reported and the literature is reviewed.

  • The goal of surgical correction for hallux valgus is to achieve a painless, shoe-wearable, and relatively straight toe with a balanced joint motion that results in aesthetically and functionally satisfactory toe. To date, there has not been a consensus on the ultimate surgical procedure for hallux valgus correction. Unfortunately, such a consensus may be difficult since it is not uncommon to encounter complications after hallux valgus correction. Postoperative soft tissue complications include difficult wound healing, infection, hypertrophy, or pain of the scar, joint stiffness, and tendon or sensory nerve damage. Postoperative bony complications include malunion, nonunion, failure of fixation, failure of angle correction, recurred deformity, osteomyelitis, and failure of balance between the metatarsal heads. Herein, we review common complications after surgical correction of hallux valgus, such as stiff joint, bony complications, recurrence of the deformity, and hallux varus.

  • A 12-year-old female was referred from a dental clinic due to a radiolucent area under the lower right first and second premolars. The lesion was diagnosed as a simple bone cyst radiographically. However, she complained lasting pain during 10 month follow up. Therefore curettage was performed and it defined as an osteoid osteoma on the histopathologic examination. As the lesion recurred, one year later, enucleation was performed. We report this case to help the diagnosis of osteoid osteoma in mandible.

  • Condyloma acuminatum is an uncommon disease but there is an increased incidence reported in recent years in prepubertal children. Anal and perianal condyloma accuminatum in children may suggest sexual abuse and treatment should include the thorough social evaluation as well as medical treatment. A 25 month old girl presented with multiple sessile nodules around her anal and perianal area, Biopsy confirmed the diagnosis of condyloma accuminatum. There was no definite evidence of sexual abuse, but her father had condyloma accuminatum involving his penis. The patient's lesions were excised totally. At three week follow up there was no anal stricture but there were newly developed small lesions around perianal area. Imiquimod onit was applied for 2 weeks and the recurred lesion disappeared completely. At postoperative 5 month, the operative area was fully epithelialized without recurrence.

  • Angiokeratoma is a cutaneous vascular disorder that occurs at any sites as trunk, extremities, fingers and toes etc. Although solitary or multiple cutaneous lesions have been reported, oral lesions have been very care. A 72-year-old man who had an exophtic, essile mass with dark red, black colored which located on buccal mucosa, was treated with excisional biopsy. He had no specific systemic history except for the medication of allopurinol, for treatment of gout since 10 years. Final diagnosis was determined as angiokeratoma by evaluation of clinical and histopathological finding, and the lesion has not been recurred for two months by decrease of allopurinol. It has been emphasized that the relationship between oral mucosal disease and the complication of allopurinol. Allopurinol is widely used for gout treatment, which we will report a case on the allopurinol-associated angiokeratoma in the oral mucosa.

  • Reconstruction of the mandible after ablative oral cancer surgery requires esthetic and functional rehabilitation. Restoring facial symmetry and dentition need accurate preoperative surgical planning and meticulous surgical technique. Free fibular flap is most useful tools to reconstruct mandible because of its adequate length and height, simultaneous harvest of soft and hard tissues and placing dental implants. In this case report, recurred squamous cell carcinoma in the right mandible had been resected and free fibular flap was utilized for mandible reconstruction using 3D rapid prototype. Simulation surgery before dental implant placement has been performed for esthetic and functional prosthodontics.

  • Inverted papilloma of the nose and paranasal sinuses has been classified as a true neoplasm by most authors. Although these tumors are histologically benign, they tend to be act clinically in a malignant fashion. They are certainly characterized by the high recurrence after inadequate removal. The authors report 2 cases of the inverted papilloma which had been diagnosed as a nasal polyp for which an intranasal operation was performed. The polyp recurred and the total mass was removed at our E.N.T. department and proved to be an inverted papillcma by histologic examination.
