자세에 따른 복식호흡 운동이 20대 성인의 자세근육 활성도 및 호흡 기능에 미치는 영향 (The effect of postural abdominal breathing exercise on posture muscle activity and respiratory function in adults in their 20s)

  • 본 연구의 목적은 4주간 20대 성인에게 앉은자세와 누운자세에 따른 복식호흡운동을 적용하였을 때 호흡기능과 배근육의 근활성도를 비교하여 어떤 자세에서의 운동이 효과적인지 알아보고자 하였다. 기립자세에서는 호흡운동 후 일초량이 가장 큰 수치를 보였고 누운자세에서는 호흡운동 전 일초량과 1초간노력성날숨량이 가장 크게 나타났다. 두자세의 복식호흡운동 전후 일초량과 날숨량 분산분석결과 운동전일초량, 운동후일초량, 운동전1초간날숨량, 운동후1초간날숨량 모두 유의한 차이를 보이며, 두 자세에서 복식호흡 전후 차이를 알 수 있었다. 기립자세, 누운자세에서 운동전에는 왼쪽 복직근, 운동후에는 오른쪽 복직근이 가장 높은 근활성도를 보였다. 두 자세 운동의 반복측정한 분산분석 결과 운동 전과 후의 양쪽 복직근과 외복사근은 근활성도 모두 차이를 보이지 않았다(p>.05). 기립자세에서 운동 전과 후에 왼쪽 외복사근이 유의한 차이를 보였고(p<0.05), 운동전과 후 FEV1/FVC와 노력성 호기량이 유의한 차이를 보였다(p<0.05). 운동 전과 후 왼쪽 외복사근이 누운자세에서 가장 높은 근활성도를 보였고 노력성 호기량은 가장 낮은 근활성도를 보였다. 운동 전과 후 호흡량 및 근활성도의 반복측정된 분산분석 결과에서 오른쪽 복직근에서 유의한 차이를 보였다(p<.05). 이를 통해 앉은 자세와 비스듬히 누운자세 두가지 자세에서 복식호흡 운동을 적용 하였을 때 호흡기능과 배근육의 근활성도 전,후를 비교하기 위해서는 두가지 자세에서의 운동 전,후 호흡과 근활성도 평가가 필요하고 평가에 따라서 어떤 자세에서의 운동이 더 효과적인지 알 수 있었다. 추후 연구에서는 좀 더 세분화되고 정확한 호흡기능과 근활성도를 알아보는 다양한 연령대의 사람들에게 장기간의 중재를 통해 파악하는 연구가 필요할 것으로 사료된다.

The Effect of Different Head Positions with Whole Body Vibration on Muscle Activation related to Postural Stability in Standing

  • Purpose: The purpose of this study was to investigate muscle activation related to postural stability depending on different head positions with whole body vibration (WBV) in standing. Methods: Eighteen healthy subjects voluntarily participated in this single-group, repeated-measures study in which the surface electromyography (EMG) data from upper trapezius, rectus abdominis, external oblique abdominis, erector spinae, gluteus maximus, rectus femoris, semitendinosus, medial gastrocnemius were collected over 3 different frequencies (0-10-20Hz) and 4 different head positions (neutral, flexion, extension, chin tuck) for each subject on WBV while standing. Results: The results of this study demonstrated that the EMG activity of all recorded muscles shows significant difference between three different frequencies and four head positions of WBV while standing (p<0.05). In the multiple comparison, significant differences could be observed for most of different frequency conditions except 0-10Hz of RA, 10-20Hz of ST. In contrast, no significant difference showed the comparison of the EMG activity depending on different head positions (p<0.05). Conclusion: These findings suggest that different head positions on WBV do not activate muscles related to postural stability. However, higher frequency on WBV is highly effective to activate whole body muscles included postural muscles regardless of different head positions.

들기/내리기 빈도와 회복시간 변화에 따른 몸통 근육의 피로도 분석 (Analysis of Trunk Muscle Fatigue as the Frequency of Lifting/Lowering and Recovery Time Change)

  • The purpose of this study was to investigate the relationship between the cumulative fatigue of trunk muscles and the period of recovery time during repetitive lifting and lowering task with two different frequencies(4 times/min and 6 times/ min). Eight healthy males with no prior history of LBD(low back disorders) volunteered for this study. Subjects had 2, 3, 4, and 5 minutes recovery time respectively while they were preforming the lifting and lowering task at 15% level of MVC. EMG signals from six trunk muscles were collected and the fatigue level was analyzed quantitatively. In results, the fatigue levels of LES(left elector spinae), RLD(right lattissimus dorsi), LLD(left lattissimus dorsi), RRA(right rectus abdominis) and LRA(left rectus abdominis) were recovered when 3 minutes recovery time was given at 4 times/min frequency. However, the fatgue level of RES(right elector spinae) was recovered when 4 minutes recovery was given. On the other hand, when 6 times/min frequency was used, the RLD, LLD, RRA and LRA were recovered at 5 minutes of recovery time. But for RES and LES, it took longer than 5 minutes to be recovered. This results can be applied to design the adequate length of recovery time to control the cumulative fatigue of trunk muscles in industry with repetitive lifting and lowering task.

Effects of Active Vibration Exercise on Trunk Muscle Activity, Balance, and Activities of Daily Living in Patients with Chronic Stroke

  • Purpose: This study aimed to investigate the effect of active vibration exercise on trunk muscle activity, balance, and activities of daily living in patients with chronic stroke. Methods: Twenty-four patients with chronic stroke were randomly assigned to an experimental (n=12) or a control (n=12) group. The experimental group performed an active vibration exercise workout for 20 minutes a day five times a week for 4 weeks using a flexi-bar. Trunk muscle activity was measured using surface electromyography. Balance was measured using the Berg balance scale. Activities of daily living were measured using the functional independence measure. Results: The intragroup comparison showed significant differences in the rectus abdominis, internal oblique, and external oblique muscle activity values, Berg balance scale score, and functional independence measurement values in the experimental group. The intergroup comparison showed that differences in the rectus abdominis and internal oblique external oblique muscle values, Berg balance scale score, and functional independence measurement value for the experimental group were significantly related to those in the control group. Conclusion: Based on these results, this study proved that the active vibration exercise effectively improved trunk muscle activity, balance, and activities of daily living in patients with chronic stroke.

프로와 아마추어 골퍼의 미들 아이언 스윙에 관한 근전도 분석 (The Analysis of Electromyography during Professional & Amateur Golfer's Iron Swing)

  • The purpose of this study was to describe and compare the selected electromyographical muscle activities of trunk and hip during Professional and Amateur golfer's Iron swing. Using surface electromyography, we evaluated muscle activities in 6 male professional golfers and 6 male amateur golfers during the golf iron swing. Surface electrodes were used to record the level of muscle activity in the right abdominal oblique, left abdominal oblique, right erector spinae, left erector spinae, right rectus abdominis, left rectus abdominis, right gluteus maximus, left gluteus maximus muscles during the golfer's swing. These signals were compared with IEMG(Integrated EMG) which was normalized by %RVC(Reference voluntary contraction). The golf swing was divided into three phases: take away, forward swing-acceleration, follow-through. We observed patterns of trunk muscle activity throughout three phases of the golf swing. The results can be summarized as follows: LES(Left Erector spinae) had statistically significant difference in take away and forward swing-acceleration phases. It was showed no significant difference in follow-through phase.

The Effect of Supplementary Shouting Technique on Muscle Activity to Rectus Abdominis and External Oblique During Crunch Exercise in Healthy Subjects

  • Purpose: The purpose of this study was to examine the effects of a supplementary shouting technique on abdominal muscle activation to the rectus abdominis (RA) and external oblique (EO) muscles during performance of crunch exercise in healthy subjects. Methods: Sixty-four subjects were randomly allocated to two groups, crunch exercise with shouting group (SG) and crunch exercise with non-shouting group (NSG), with 32 subjects in each group, respectively. The interventions were conducted over three trials in each group, and measurements were performed on each subject by one examiner in three trials. Muscle activation of RA and EO was evaluated using electromyography (EMG) during performance of crunch exercise with or without shouting. Results: Our results showed a significantly greater increase in the EMG patterns of the RA and EO muscles during performance of crunch exercise in the SG compared to the NSG (p<0.01). Conclusion: These findings suggest that addition of the shouting technique during performance of crunch exercise, at the same time, would suggest positive evidence for improving activation of abdominal muscles.

Chest Wall Reconstruction for Chronic Intrathoracic Wounds Using Various Flaps

  • The treatment of chronic chest wounds should be focused on eradicating the infection and obliterating the dead space thus providing improved pulmonary function. Chronic chest wounds, although the incidence has decreased over the years, is still associated with high morbidity and prolong hospitalization. In cases where the disease is advanced and conventional measures fail, aggressive approaches achieve adequate resolution or significant improvement. This paper reports four cases of chronic chest wound including bronchopleural fistula and osteomyelitis managed by debridement followed by muscle coverage using latissimus dorsi, rectus abdominis, and omental flap. The intrathoracic reconstruction entails thorough debridement of empyema cavities, bronchpleural fistulas and infection focus. The infection must be completely eradicated prior to or at the time of flap transposition. The flaps used for obliteration of dead spaces provided adequate bulk, abundant blood supply, and minimal donor morbidity. The results were satisfactory with improved respiratory function without complications.

Free Flap Transplantation in Open Tibial Fracture with Vessel Injury in the Elderly

  • Two aged patients who had open tibial fractures with arterial injury caused by high energy accidents underwent emergency arterial reconstruction using a greater saphenous vein and soft tissue repair using free flaps. In the patients, soft tissue necrosis developed and tibias were exposed at postoperative third week. Follow-up angiography through the superficial femoral artery showed occlusion of the anterior tibial artery. The anterior tibial artery was reconstructed using the contralateral greater saphenous vein graft and the latissimus dorsi myocutaneous and rectus abdominis muscle free flaps were transplanted for repair of necrotic soft tissue. The reconstructed arteries showed good perfusion to the new free flaps until union of the tibias occurred. The patients were followed-up for 21 years and 17 years postoperatively, respectively. In management of open comminuted fracture of the tibia, injury of the arterial system must be ruled out by angiography in addition to evaluation of the degree of soft tissue injury.

The Change of the Trunk Muscles Activation in the Adolescent Carrying a Backpack with Belts

  • Objective: The purpose of this study is to compare the EMG activities of the trunk muscles when carrying a backpack with belts and without belts. Background: This research aimed to explore the influence of carrying a backpack with belts on the trunk muscles activity and to suggest a desirable method of wearing backpacks. Method: Seventy five adolescents participated in this study. The EMG activities of upper trapezious, cervical erector spinae and rectus abdominis were collected. One way ANOVA was used to analyze the collected data. LSD was used for post-hoc test. Results: The EMG data from treadmill walking showed significant differences between carrying a backpack with belts and without belts for the cervical erector spinae and upper trapezious(p<.05) in all group(A Group: no belt, B Group: lumbar belt, C Group: lumbar-chest belt). In descending walking, significant difference were found for the cervical erector spinae and rectus abdominis in all group. In ascending walking, there were no significant differences in all group. LSD showed that the muscle activity of the trunk muscles decreased significantly when the lumbar belt and lumbar-chest belt were worn. Conclusion: The trunk muscles activities were decreased when the subjects were wearing a backpack with belts, comparing with wearing a backpack without belts. Application: These results can be used to provide useful information on how wearing a backpack in adolescent.
