골반경수술의 부인과적 응용 (Gynecologic Application of the Pelviscopic Surgery)

  • 1991년 5월 1일부터 1993년 7월 31일까지 영남대학교 의과대학 부속병원 산부인과에서 시행한 136례의 골반경수술에서 다음과 같은 결론을 얻었다. 환자의 나이 분포는 19세에서 55세였고 평균 나이는 31.2세였다. 평균 분만력은 0.96이었으며 거의 대부분의 환자는 2회 이하의 분만력을 보였다. 골반경수술의 가장 많은 적응증은 난관 임신이었고 그 다음으로는 난소낭종이었다. 시술 방법은 주로 난관절제술(58.8%)과 난소난관절제술(16.3%)이었고 입원기간은 평균 2.1일이었고 모든 경우에는 특별한 합병증이 없었다. 이러한 연구결과로 보아 골반경수술은 경제적으로나 사회적으로 또 미용학적으로도 좋은 결과를 보였으며 앞으로 다른 많은 부인과 환자에게 안전하게 적용할 수 있으리라 사료된다.

Assessment of the proximity between the mandibular third molar and inferior alveolar canal using preoperative 3D-CT to prevent inferior alveolar nerve damage

  • Background: The inferior alveolar nerve (IAN) may be injured during extraction of the mandibular third molar, causing severe postoperative complications. Many methods have been described for evaluating the relative position between the mandibular third molar and the inferior alveolar canal (IAC) on panoramic radiography and computed tomography, but conventional radiography provides limited information on the proximity of these two structures. The present study assessed the benefits of three-dimensional computed tomography (3D-CT) prior to surgical extraction of the mandibular third molar, to prevent IAN damage. Methods: This retrospective study included 4917 extractions in 3555 patients who presented for extraction of the mandibular third molars. The cases were classified into three groups, according to anatomical relationship between the mandibular third molars and the IAC on panoramic radiography and whether 3D-CT was performed. Symptoms of IAN damage were assessed using the touch-recognition test. Data were compared using the chi-square test and Fisher's exact test. Results: Among the 32 cases of IAN damage, 6 cases were included in group I (0.35 %, n = 1735 cases), 23 cases in group II (1.1 %, n = 2063 cases), and 3 cases in group III (0.27 %, n = 1119 cases). The chi-square test showed a significant difference in the incidence of IAN damage between groups I and II. No significant difference was observed between groups I and III using Fisher's exact test. In the 6 cases of IAN damage in group I, the mandibular third molar roots were located lingual relative to the IAC in 3 cases and middle relative to the IAC in 3 cases. The overlap was ${\geq}2mm$ in 3 of 6 cases and 0-2 mm in the remaining 3 cases. The mean distance between the mandibular third molar and IAC was 2.2 mm, the maximum distance 12 mm, and the minimum distance 0.5 mm. Greater than 80 % recovery was observed in 15 of 32 (46.8 %) cases of IAN damage. Conclusions: 3D-CT may be a useful tool for assessing the three-dimensional anatomical relationship and proximity between the mandibular third molar and IAC in order to prevent IAN damage during extraction of mandibular third molars.

Multiplex PCR 분석을 통한 건강한 20대 여성의 질 내에 서식하는 Lactobacillus 속 유산균의 신속한 검출 (Rapid Detection of Lactobacillus Genus Inhabiting in Vagina of 20's Healthy Women Using Multiplex PCR)

  • 본 연구는 multiplex PCR 방법을 적용하여 질 내부에 서식하는 유산균을 동시에 그리고 신속하게 검출하는 방법을 개발하는 것을 목적으로 수행되었다. 세균의 감염에 의한 질염 증상이 없는 건강한 20대 여성 166명으로부터 질 분비물을 채취하였다. 건강한 여성의 질 내부에 서식하는 것으로 확인된 Lactobacillus 속에 속하는 6종의 유산균을 종 특이 multiplex PCR primer를 제작하기 위해 선정하였다. Multiplex primer I은 L. iners, L. crispatus, L. gasseri를 선택적으로 검출하기 위해 특성화하였고 multiplex primer II는 L. acidophilus, L. jensenii, L. fermentum를 선택적으로 검출하기 위해 특성화하였다. Multiplex PCR 기술을 이용하여 분석한 결과 L. crispatus (77%)가 가장 높은 빈도로 검출되었고 L. acidophilus (57%)과 L. jensenii (57%)는 상대적으로 높은 빈도로 검출되었다. L. iners (59%)는 L. acidophilus (57%)와 L. jensenii (57%) 보다 높은 빈도로 검출되었지만 항생제 치료 과정 후 또는 세균성 질염의 경우에도 발견되는 균종으로 건강한 여성에게서 주로 서식하는 종이라고 결론 내리기는 어렵다. 결론적으로, 특정 유산균의 특이적인 primer를 이용한 multiplex PCR 기술은 질의 건강상태의 변화 또는 질의 질병으로부터 회복 과정을 예측할 수 있는 도구로서 유용할 것으로 생각된다.

2가지 내부표준물질을 이용하여 어류시료 중 Octylphenol, Nonylphenol, Bis(2-ethylhexyl)phehalate의 동시정량 (Simultaneous Determination of Octylphenol, Nonylphenol and Bis(2-ethylhexyl)phthalate in Fish Samples Using Two Internal Standards)

  • 어류근육속에 포함된 nonylphenol(NP), octylphenol(OP), bis(2-ethylhexyl)phtalate(BEHP)의 동시 정량방법을 1-phenyldecanol(PD)과 bis(2-ethylbutyl)phthalate(BEBP) 2가지 내부표준물질을 이용하여 연구하였다. 1-phenyldecanol은 nonyphenol과 octylphenol을 정량하고, bis(2-ethylbutyl)phthalate는 bis(2-ethylhexyl)phthalate를 정량하기 위해 사용하였다. NP와 OP의 경우 $0.2{\sim}20{\mu}g/g$ 농도범위에서 75% 이상의 회수율을 나타냈으며, BEHP는 $0.4{\sim}40{\mu}g/g$ 농도범위에서 102% 이상의 회수율을 보였다. 시료를 밀폐된 배양시험관에서 이염화메탄을 이용하여 추출하고 용매를 이소-헥산으로 바꾸고 aminopropyl 칼럼(2g)을 이용하여 분리한후 GC/MS-SIM 방법으로 분석하였다. 확립된 분석방법을 11점의 한국시료와 3점의 영국시료에 적용하였다. Nonylphenol은 2점의 한국어류시료에서, BEHP는 9점의 한국어류시료에서 검출되었고, 그 농도범위는 각각 $0.02{\sim}0.06{\mu}g/g$$0.18{\sim}2.03{\mu}g/g$이었다. 그러나 영국어류시료에서는 단 1점의 시료에서 BEHP가 검출되었고 그 농도는 $2.99{\mu}g/g$이었다. 이 방법에 의해서 octylphenol은 어떠한 시료에서도 검출되지 않았으며, 이와 같이 2가지 내부표준물질을 이용하는 방법은 각기 다른 특성을 갖는 분석대상물질에 각각의 특성과 유사한 내부표준물질의 사용으로 매우 적은량의 생체 시료에서 보다 정확한 정량이 가능하였다.

반회후두신경 손상을 동반하지 않은 갑상선 절제술 후 음성 변화 (Voice Changes after Thyroidectomy Without Recurrent Laryngeal Nerve Injury)

  • Background and Objectives : Transient minor voice changes after thyroidectomy are not infrequent complaints even in cases without any evidence of recurrent laryngeal nerve damage. However, clinical course, diagnosis and management of such voice changes are not fully understood. This study aimed to evaluate the clinical characteristics of minor voice changes after thyroidectomy. We also tried to assess the significance and feasibility of superior laryngeal nerve monitoring and to find out the optimal evaluation tools for such voice changes after thyroidectomy. Materials and Method : Nine adult patients who received total thyroidectomy without evidence of recurrent laryngeal nerve injury were enrolled for this prospective study. Voice evaluations were performed preoperatively and 3 months postoperatively ; acoustic analyses including voice range profile, aerodynamic study, stroboscopic evaluation and subjective voice assessment with questionnaires. The external branch of superior laryngeal nerve was monitored by nerve stimulator after ligation of superior thyroidal vessels. Results: Four of nine patients complained their voice change at 3 months after the surgery. Three of them reported complete recovery of their voice at 6 months after the surgery. Acoustic analysis revealed significant decrease in their phonatory range especially with high tone loss. Questionnaires related to singing was more sensitive than previously well-known "voice handicap index". Stimulation of the superior laryngeal nerve was feasible in most of the cases (94.4%), but it failed to show any correlation with minor voice changes after thyroidectomy. Conclusion : Minor voice changes were not rare events during the first 6 month after thyroidectomy. Decrease in phonatory range with high tone loss and therefore, discomfort in singing was the most common finding. Superior laryngeal monitoring was feasible but it was not a sensitive tool for the prediction of minor voice change after thyroidectomy.

표면탄성파를 이용한 아황산 가스센서 개발에 관한 연구 (The Study on development of a SAW SO$_2$ gas sensor)

  • 아황산가스를 감지하기 위해 새로운 CdS 무기박막을 이용한 표면탄성파 센서를 구현하였다. LiTaO$_3$ 단결정 압전기판에 중심주파수 54MHz인 두 개의 SAW 소자 및 발진기를 제작하였으며 아황산가스가 흡착, 탈착할 수 있는 감지막을 지연선 상에 증착시키고 다른 변수로부터의 반응을 보상하기 위해 이중지연선 구조로 제작하였다. CdS 박막은 초음파 노즐을 이용하여 분무 열분해법을 이용하여 증착하였다. 실험결과 표면탄성파 센서는 아황산가스의 농도를 0.25 ppm 까지 검출할 수 있으며 20 ppm 이내의 안정도 및 5분 이내의 빠른 반응시간을 보였다. 또 가스감응 실험의 반복을 통해 센서의 반복성을 확인함으로써 본 연구에서 개발한 센서가 이황산가스 감지용 센서로 사용될 수 있음을 확인하였다. 향후 계획으로 CdS 박막 증착시에 적절한 원소를 첨가하여 박막의 반응성을 증가시키며 또한 표면탄성파 소자의 중심주파수를 증가시켜 센서의 가스감응성을 높이고자 하였다.

자궁내막증과 동반된 불임환자의 보조생식술을 이용한 치료 (Assisted Reproductive Technology in Infertile Patients with Endometriosis)

  • Though the endometriosis is not always related with infertility, endometriosis causes infertility in some patients. There are many treatment modalities of infertile patients who have endometriosis. In recent years, Assisted Reproductive Technology(ART) have been widely accepted as being a useful tool for the treatment of infertile endometriotic patients. The objective of this study was to evaluate the outcome of ART in infertile endometriotic patients who have been carried out IVF-ET from Jan, 1992 to Dec, 1994 and to compare the results between COH/IUI and IVF-ET in the patients with endometriosis stage I. Tubal disease only patients were grouped(308 patient, 956 cycles) as a control. Endometriosis group was subdivided into 4 groups according to American Fertility Society classification; endometriosis stage I (45 patients, 61 cycles), stage II (26 patients, 39 cycles), stage III (26 pateitns, 37 cycles), stage IV (33 patients, 50 cycles). The outcomes of IVF-ET in endometriosis patients were as follows; The oocyte recovery rates were significantly lower in stage III, IV endometriosis. In case of stage III endometriosis, the fertilization rate was significantly lower than other stages of endometriosis. Clinical pregnancy rates per cycle were not different between the tubal group(22%) and the endometriosis group(25%). According to endometriosis stage, the implantation rate and clinical pregnancy rate were significantly lower in stage IV (5.6%, 16%) compared with other stages (I; 10.0%, 26%, II;9.8%, 31%, III;12.6%, 32%). It suggests that some factor like autoantibodies may inhibit implantation of embryos in stage IV endometriosis. To evaluate the possibility that simply increasing the number of gametes at the site of fertilization might account for pregnancies attributed to IVF-ET, the authors retrospectively analyzed the outcome of couples undergoing IUI during hMG cycles and CC cycles between 1992 and 1994 in the women with endometriosis stage 1. In case of stage I endometriosis, though the COH/IUI group showed lower FSH level and lesser age profile than IVF-ET group, IUI group has resulted in lower pregnancy rates(19.2%) compared with the IVF-ET group(26.2%). In conclusion, endometriotic infertile patients can get comparable pregnancy rates with the tubal factor infertility patients during IVF-ET program. Moreover even in stage I endometriosis, IVF-ET may be an more effective treatment modality than COH/IUI.

Optimization and characterization of biodiesel produced from vegetable oil

  • The world faces several issues of energy crisis and environmental deterioration due to over-dependence on single source of which is fossil fuel. Though, fuel is needed as ingredients for industrial development and growth of any country, however the fossil fuel which is a major source of energy for this purpose has always been terrifying thus the need for alternative and renewable energy sources. The search for alternative energy sources resulted into the acceptance of a biofuel as a reliable alternative energy source. This work presents the study of optimization of process of transesterification of vegetable oil to biodiesel using NaOH as catalyst. A $2^4$ factorial design method was employed to investigate the influence of ratio of oil to methanol, temperature, NaOH concentration, and transesterification time on the yield of biodiesel from vegetable oil. Low and high levels of the key factors considered were 4:1 and 6:1 mole ratio, 30 and $60^{\circ}C$ temperatures, 0.5 and 1.0 wt% catalyst concentration, and 30 and 60 min reaction time. Results obtained revealed that oil to methanol molar ratio of 6:1, tranesetrification temperature of $60^{\circ}C$, catalyst concentration of 1.0wt % and reaction time of 30 min are the best operating conditions for the optimum yield of biofuel from vegetable oil, with optimum yield of 95.8%. Results obtained on the characterizzation of the produced biodiesel indicate that the specific gravity, cloud point, flash point, sulphur content, viscosity, diesel index, centane number, acid value, free glycerine, total glycerine and total recovery are 0.8899, 4, 13, 0.0087%, 4.83, 25, 54.6. 0.228mgKOH/g, 0.018, 0.23% and 96% respectively. Results also indicate that the qualities of the biodiesel tested for are in conformity with the set standard. A model equation was developed based on the results obtained using a statistical tool. Analysis of variance (ANOVA) of data shows that mole ratio of ground nut oil to methanol and transesterification time have the most pronounced effect on the biodiesel yield with contributions of 55.06% and 9.22% respectively. It can be inferred from the results various conducted that vegetable oil locally produced from groundnut oil can be utilized as a feedstock for biodiesel production.

The Effects of Multi-identity on One's Psychological State and the Quality of Contribution in Virtual Communities: A Socio-Psychological Perspective

  • In a virtual community, one can possess multiple identities and pretend to be different by creating self-identity in contrast with his or her actual self. Does false identity undermine the qualitative growth of a virtual community by reducing members' accountability? Or does it stimulate their contributive behaviors by ensuring freedom of speech? It is imperative to understand the effects of multi-identity considering the distinct properties of a virtual community in which people easily change their identities at little or no cost. To answer these questions, we adopted the concept of self-discrepancy from the social psychology theory rooted in the concept of the self and developed a theoretical model to predict quality of contribution of the individual member in virtual communities. Based on the self-discrepancy theory, we first identified two different domains of the self: (1) an "actual self" that consists of attributes that the person believes he or she currently possesses in real life and (2) a "cyber self" that consists of attributes the person believes he or she possesses in a virtual community. Next, we derived an index for two different types of self-discrepancy by using the differences between the actual and the cyber identities: Personal Self-discrepancy and Social Self-discrepancy. Personal Self-discrepancy reflects the degree of discrepancy between actual and cyber identity regarding a person's intelligence, education, and expertise. Social Self-discrepancy reflects the degree of discrepancy between actual and cyber identity regarding a person's morality, sociability, and accordance with social norms. Finally, we linked them with sense of virtual community, perceived privacy rights, and quality of contribution to examine how having a multi-identity influences an individual's psychological state and contributive behaviors in a virtual community. The results of the analysis based on 266 respondents showed that Social Self-discrepancy negatively influenced both the Sense of Virtual Community and Perceived Privacy Rights, while Personal Self-discrepancy negatively influenced only Perceived Privacy Rights, thereby resulting in reduced quality of contribution in virtual communities. Based on the results of this analysis, we can explain the dysfunctions of multi-identity in virtual communities. First, people who pretend to be different by engaging in socially undesirable behaviors under their alternative identities are more likely to suffer lower levels of psychological wellbeing and thus experience lower levels of sense of virtual community than others. Second, people do not perceive a high level of privacy rights reflecting catharsis, recovery, or autonomy, even though they create different selves and engage in socially undesirable behaviors in a virtual community. Third, people who pretend to be different persons in terms of their intelligence, education, or expertise also indirectly debase the quality of contribution by decreasing perceived privacy rights. The results suggest that virtual community managers should pay more attention to the negative influences exercised by multi-identity on the quality of contribution, thereby controlling the need to create alternative identities in virtual communities. We hope that more research will be conducted on this underexplored area of multi-identity and that our theoretical framework will serve as a useful conceptual tool for all endeavors.

기후변화의 위기와 기독교 생태교육의 방향 모색 (The Crisis of Climate Change and the Direction of Christian Ecological Education)

  • 본 연구의 목적은 기후변화의 위기에 있어, 기독교교육이 생태학적 교육으로서 나아가야 할 방향을 모색해보는 데 있다. 기후변화가 보여주는 것은 불과 2-3도만 지구의 온도가 올라가도 생존이 불가능하다는 사실이다. 그러나 현재 우리의 모습은 삶을 변경하지 않는 한에서 기후변화의 위기를 염려하고 있다. 이를 위해 먼저 기후변화의 의미와 그 원인에 대해 살펴보았다. 인간중심주의적 세계관, 현대의 산업적, 과학기술적 성장, 그리고 소비지향적 사회구조 등을 그 원인으로 들 수 있는데, 인간중심주의적 세계관은 모든 것을 인간의 관점에서 정당화하며, 자연은 인간의 지배대상이나 하나의 도구로 간주해왔다. 또한 인간의 경제활동 규모가 커지면 커질수록 에너지 소비량은 증가하고, 이 증가에 따라 생태계 파괴의 위협도 증가한다. 개인의 소비증가와 풍요가 생태계 긴장을 가중시키는 것이다. 이러한 기후변화의 문제를 해결하기 위해서 성서 속의 창조를 통해 인간과 자연의 관계가 상생과 공존임을 규명하고, 안식의 원리를 통해 인간과 자연의 회복을 가져올 수 있으며, 성육신의 관점에서 세계를 하나님의 '몸'으로 이해하는 생태교육적 단초들을 발견하였다. 이 단초들을 토대로, 인간과 자연이 배려와 존중의 관계성을 가지는 생태적 패러다임으로의 전환, 창조영성의 회복, 청지기로서의 책임과 지구 차원의 연대를 이루어가기 위해 생태적 상상력의 함양을 제시함으로써 기독교 생태교육의 방향을 모색해보았다. 기후변화의 위기에 있어, 기독교의 생태교육적 단초들을 통한 기독교교육적 접근을 통해 지구의 회복이 이루어지고 책임적 존재로 다시금 설 수 있기를 기대해본다.
