Outcomes of an outpatient home-based prehabilitation program before pancreaticoduodenectomy: A retrospective cohort study

  • Backgrounds/Aims: Prehabilitation aims for preoperative optimisation to reduce postoperative complications. However, there is a paucity of data on its use in patients undergoing pancreaticoduodenectomy (PD). Thus, this study aims to evaluate the outcomes of a home-based outpatient prehabilitation program (PP) versus no-PP in patients undergoing PD. Methods: This retrospective cohort study compared patients who underwent PP versus no-PP before elective PD from January 2016 to December 2020. Inclusion criteria for PP were < 65 years or 65-74 years with FRAIL score < 3. No-PP included dietician, case manager and anesthesia review. PP included additional physiotherapy sessions, caregiver training and interim phone consultation. Univariate and multivariate analysis were used to evaluate length of stay (LOS), morbidity, 30-day readmission, and 90-day mortality. Results: Seventy-one patients (PP: n = 50 [70.4%]; no-PP: n = 21 [29.6%]) were included in this study. Median age was 65 years (interquartile range [IQR]: 58-72 years). Majority (n = 58 [81.7%]) of patients underwent open surgery. Ductal adenocarcinoma was the most common histology (49.3%). Patient demographics were comparable between both groups. Overall median LOS was 11.0 days (IQR: 8.0-17.0 days). Compared to no-PP, PP was not independently associated with reduced intra-abdominal collections (odds ratio [OR]: 0.43; 95% confidence interval [CI]: 0.03-6.11, p = 0.532), major morbidity (OR: 1.31; 95% CI: 0.09-19.47; p = 0.845) or 30-day readmission (OR: 3.16; 95% CI: 0.26-38.27; p = 0.365). There was one (1.4%) 30-day mortality. Conclusions: Our outpatient PP with unsupervised exercise regimes did not improve postoperative outcomes following elective PD.

A Four State Rotational Frequency Detector for Fast Frequency Acquisition

  • This paper proposes a new rotational frequency detector (RFD) for phase-locked loop (PLL) or clock and data recovery (CDR) applications for fast frequency acquisition. The proposed RFD uses the four states finite state machine (FSM) model to accelerate the frequency acquisition time. It is modeled and simulated with MATLAB Simulink. The functionalities of the proposed RFD are examined and the results are compared to those of a conventional RFD. The proposed RFD's frequency acquisition time is four times faster than that of a conventional one. The proposed RFD incorporated with a phase detector (PD) in PLL or CDR is expected to improve the frequency and phase acquisition performance later greatly.

  • Carbon nanotubes(CNT) were synthesized by arc-discharge method. To fabricate CNT sensor, CNT powder was dispersed in $\alpha$-Terpinol($C_{10}$ $H_{17}$OH) solution. The CNT films were fabricated by screen printing on the interdigitated Pt/Pd alloy electrode. The microstructure of CNT film were observed by scanning electron microscopy (SEM). In order to investigate the gas sensing characteristics of the film, the CNT film was experimented to measure NH$_3$ response and recovery time. And this sensor shows better reproductibility and faster recovery time than another CNT sensors. We suggest the possibility to utilize a CNT as new sensing materials for environmental monitoring.

Recovery of Platinum Group Metals from the Leach Solution of Spent Automotive Catalysts by Cementation (자동차(自動車) 폐촉매(廢觸媒)의 침출액(浸出液)으로부터 시멘테이션에 의한 백금족(白金族) 금속(金屬)의 회수(回收))

  • The recovery of platinum group metals (PGMs) from the leach solution of spent auto-catalyst and the wash solution of the leach residue was investigated in the laboratory scale experiments by the cementation process using metal powders as the reductant. In this study, the effect of Al, Mg and Zn powders on the cementation process was particularly examined. Aluminum powder was selected as the most suitable reductant for the cementation of PGMs. At the cementation time of 10 minute under the aluminium stoichimetric amount of 19.3 and the reaction temperature of $50{\sim}60^{\circ}C$, the recovery of platinum group metals from the leach solution of the spent auto-catalyst was found to be 99.3%, 99.4%, 90.2% for Pt, Pd and Rh, respectively. Under the same conditions with the aluminium stoichimetric amount of 45, the recovery of platinum group metals from the wash solution of the leach residue of spent catalyst was observed to be 97%, 97% and 90% for Pt, Pd and Rh, respectively. In addition, it was possible to upgrade the platinum group metals in the precipitates obtained from the cementation process by about 10% through the removal of metal impurities by the nitric acid leaching at ambient temperature.

Linkage Between Catastrophic Disaster Recovery Plan and Regional Comprehensive plan in Japan - Focus on the Great East Japan Earthquake Case - (일본의 대규모재해 부흥계획과 지역종합계획과의 연계체계 연구 - 동일본 대지진 사례를 중심으로 -)

  • The purpose of this paper is to analyze the linkage between Si-Jeong-Chon (municipalities of Japan) catastrophic disaster recovery-regeneration plan (DRRP) and regional comprehensive plan (RCP), each of which was made after the Great East Japan Earthquake, and to make implications on establishment of a disaster recovery plan of Korea. In order to explore the relationship between the two plans, the DRRPs and RCPs of 30 municipalities are collected and categorized according to their characteristics. The results show that DRRPs of the municipalities are not always in a consistent form and contents because regulations and guidelines on DRRP are not definite and specific. The relationship between DRRP and RCP in terms of its form and contents can be divided into 3 categories; Reflective Type (RT), Complementary Type (CT), and Substitutive Type (ST). The 22 RT plans as the majority of those DRRPs appear to reflect the development strategies of municipalities' RCPs. Both 3 CT plans and 5 ST plans seem to be integrated with RCPs in some way. The plan in CT of a municipality supplements its RCP by adding some new strategies to its recovery section and the plan in ST of a municipality appears to replace its RCP. Finally the influential factors which are considered to determine linkage type are identified as remaining time which is legally required to re-establish its RCP, the extent of damage, and socioeconomic condition changes.

Solvent Sublation of Trace Noble Metals by Formation of Metal Complexes with 2-Mercaptobenzothiazole

  • A solvent sublation has been studied for the determination of trace Au(III), Pt(IV) and Pd(II) in waste water with their complexes of 2-mercaptobenzothiazole (MBT). Experimental conditions such as the concentration of HCl, the amount of MBT as a ligand, the type and amount of surfactants, bubbling rate and time, and the type of organic solvent were optimized for the solvent sublation, i.e., 25.0 mL of 2.0 M HCl solution and 30mL of 0.4%(w/v) MBT ethanolic solution were added to a 1.0 L sample to form stable complexes. The addition of 4.0 mL of 1 ${\times}$$10^{-3}$ M CTAB (cetyltrimehtylammonium bromide) solution was needed for the effective flotation accomplished by bubbling nitrogen gas at the rate of 40.0 mL/min for 35 minutes. As a solvent, 20.0 mL of MIBK (methylisobuthylketone) was used to extract the floated complexes. The procedure was applied to three kinds of waste waters. Au(III) was determined as 0.68 ng/mL and 0.98 ng/mL respectively for final washed water of two plating industries in Banwol. Pd(II) and Pt(IV) were not detected in any of the three samples. The recovery, which was obtained with analyte-spiked samples, were 95-120%.

  • The purpose of this work is to develop the FES controller that can cope with the muscle fatigue which is one of the most important problems of current FES (Functional Electrical Stimulation). The feasibility of the proposed FES controller was evaluated by simulation. We used a fitness function to describe the effect of muscle fatigue and recovery process. The FES control system was developed based on the biological neuronal system. Specifically, we used PD (Proportional and Derivative) and GC (Gravity Compensation) control, which was described by the neuronal feedback structure. It was possible to control of multiple joints and muscles by using the phase-based PD and GC control method and the static optimization. As a result, the proposed FES control system could maintain the cycling motion in spite of the muscle fatigue. It is expected that the proposed FES controller will play an important role in the rehabilitation of SCI patient.

A Sensitive, Efficient, and Cost-Effective Method to Determine Rotigotine in Rat Plasma Using Liquid-Liquid Extraction (LLE) and LC-MRM

  • Rotigotine (RTG) is a non-ergot dopamine agonist used to manage the early stage of Parkinson's disease (PD) as transdermal patch. However, the poor medication compliance of PD patients and skin issues related with repeated applications of RTG patches lead to the search for alternative formulations and it also requires appropriate analytical methods for their in vivo evaluation. Thus, here, a sensitive, efficient, and cost-effective method to determine RTG in rat plasma using liquid-liquid extraction (LLE) and multiple reaction monitoring was developed. The use of 20 µL of rat plasma for sample treatment, 8-OH-DPAT as the internal standard, and methyl tert-butyl ether as the LLE solvent in the present method gives it advantages over previous methods for the analysis of RTG in biological samples. The good analytical performance of the developed method was confirmed in specificity, linearity (the coefficient of determination ≥0.999 within 0.1-100 ng/mL), sensitivity (the lower limit of quantitation at 0.1 ng/mL), accuracy (81.00-115.05%), precision (≤10.75%), and recovery (81.00-104.48%) by following the FDA guidelines. Finally, the applicability test of the validated method to the in vivo evaluation of a RTG formulation showed that the present method is the only method which can be accurately applied to that longer than 24 hours, critical for the development of formulations with reduced dosing frequencies. Therefore, the present method could contribute to the development of new RTG formulations helpful to people suffering from PD.

Adsorption behavior of platinum-group metals and Co-existing metal ions from simulated high-level liquid waste using HONTA and Crea impregnated adsorbent

  • The volume and toxicity of radioactive waste can be decreased by separating the components of high-level liquid waste according to their properties. An impregnated silica-based adsorbent was prepared in this study by combining N,N,N',N',N",N"-hexa-n-octylnitrilotriacetamide (HONTA) extractant, N',N'-di-n-hexyl-thiodiglycolamide (Crea) extractant, and macroporous silica polymer composite particles (SiO2-P). The performance of platinum-group metals adsorption and separation on prepared (HONTA + Crea)/SiO2-P adsorbent was then assessed together with that of co-existing metal ions by batch-adsorption and chromatographic separation studies. From the batch-adsorption experiment results, (HONTA + Crea)/SiO2-P adsorbent showed high adsorption performance of Pd(II) owing to an affinity between Pd(II) and Crea extractant based on the Hard and Soft Acids and Bases theory. Additionally, significant adsorption performance was observed toward Zr(IV) and Mo(VI). Compared with studies using the Crea extractant, the high adsorption performance of Zr(IV) and Mo(VI) is attributed to the HONTA extractant. As revealed from the chromatographic experiment results, most of Pd(II) was recovered from the feed solution using 0.2 M thiourea in 0.1 M HNO3. Additionally, the possibility of recovery of Zr(IV), Mo(VI), and Re(VII) was observed using the (HONTA + Crea)/SiO2-P adsorbent.
