Comparison of Pain Management between before and after the Application of Guidelines in Cancer Emergency Room (암성통증관리지침 적용 전후 긴급진료실 내원 환자의 통증관리 비교)

  • Purpose: This study was to investigate the effect of the application of cancer pain management guidelines on pain management among patients in Cancer Emergency Room. Methods: This study was a retrospective descriptive study. Before application, data were collected by analyzing the Electronic Medical Record in Cancer Emergency Room in September, 2011, and after application in February, 2012. The subjects of this study consisted of 231 patients (pre-application group 83, post-application group 148), who stayed over 24 hours and complained of pain higher than Numeric Rating Scale score 4. The post-test was conducted after educating the nurses about the application of the pain management guidelines in the Electronic Medical Record. Results: This survey showed that, as the cancer pain management guidelines were applied for cancer patients with above moderate pain, the pain intensity decreased, the number of patients reaching the treatment goal score increased. Furthermore, the estimated time to reach the treatment goal decreased significantly. Conclusion: Pain intensity of the cancer patients was decreased through regular pain assessments by nurses and the medication of analgesics according to the cancer pain management guidelines. Therefore, it is necessary to develop the pain management program and to provide the physicians and nurses with intensive education about the pain management guidelines for systematic and effective pain management.

Factors associated with unexpected revisit to an emergency medical center (예고되지 않은 응급의료센터 재방문에 영향을 미치는 요인 분석)

  • The objectives of this study were to identify factors associated with unexpected revisit to an emergency medical center (EMC) located in Seoul and to examine reasons for revisit. During March, June, September and December, 2002, a total of 168 patients had unexpected revisits to the EMC within 48 hours of a previous discharge. As a 1:1 matched control, we included 136 patients who: discharged from the EMC during the same time period: did not return to the EMC; had the same diagnosis and age(${\pm}5$) with the case. In this study, factors associated with unexpected revisits were defined as characteristics of a previous discharge, which were classified into three: sociodemographic, EMC visit-related, and discharge management factors. Reasons for revisit were categorized into disease, physician, patients, and system-related factors. Data were collected by medical chart review with assistance from clinicians of the EMC. Logistic regression results showed that patients who headed home after discharge without follow-up schedule had a 27.6 times higher risk of revisiting EMC than those who were hospitalized following EMC visit. Patients discharged on his own will had a 5.9 times higher risk of revisiting than those discharged following physician's advice. Patients requiring continual observation at the time of discharge were more likely to revisit by 8.7 times than those discharged with improved condition. About 69.13% of the revisits were due to disease-related factors, followed by 13.90% due to patient-related factors, 8.64% due to system-related factors, and 8.34% due to physician-related factors. It appears that the most significant factors influencing revisits are discharge management factors such as patient's condition at discharge, whether the discharge was accorded with physician's advice, and whether returning home without follow-up schedule. Therefore, appropriate discharge management is necessary to prevent EMC revisit.

  • As cyber threats have been increased over the Internet, the invasions of personal information are constantly occurring. A malicious user can access the Web site as a normal user using leaked personal information and does illegal activities. This paper proposes an effective method which authenticates a genuine user with real-time. The method use the user's profile which is a record of user's behavior created by Membership Analysis(MA) and Markov Chain Model(MCM). In addition to, user's profile is augmented by a Time Weight(TW) which reflects the user's tendency. This method can detect a malicious user who camouflage normal user. Even if it is a genuine user, it can be determined as an abnomal user if the user acts beyond the record profile. The result of experiment showed a high accuracy, 96%, for the correct user.

Association between Electronic Medical Record System Adoption and Healthcare Information Technology Infrastructure

  • Objectives: The objective of this study was to investigate the relationship between the level of Electronic Medical Record (EMR) system adoption and healthcare information technology (IT) infrastructure. Methods: Both survey and various healthcare administrative datasets in Korea were used. The survey was conducted during the period from June 13 to September 25, 2017. The chief information officers of hospitals were respondents. Among them, 257 general hospitals and 273 small hospitals were analyzed. A logistic regression analysis was conducted using the SAS program. Results: The odds of having full EMR systems in general hospitals statistically significantly increased as the number of IT department staff members increased (odds ratio [OR] = 1.058, confidence interval [CI], 1.003-1.115; p = 0.038). The odds of having full EMR systems was significantly higher for small hospitals that had an IT department than those of small hospitals with no IT department (OR = 1.325; CI, 1.150-1.525; p < 0.001). Full EMR system adoption had a positive relationship with IT infrastructure in both general hospitals and small hospitals, which was statistically significant in small hospitals. The odds of having full EMR systems for small hospitals increased as IT infrastructure increased after controlling the covariates (OR = 1.527; CI, 1.317-4.135; p = 0.004). Conclusions: This study verified that full EMR adoption was closely associated with IT infrastructure, such as organizational structure, human resources, and various IT subsystems. This finding suggests that political support related to these areas is indeed necessary for the fast dispersion of EMR systems into the healthcare industry.

  • To record and evaluate the flaws which were found during pre-service/in-service inspection performance of nuclear power plants in Korea, the center line beam method described in ASME code and 6 dB drop method stated in CSA code were used. The measured through wall dimensions and lengths by these methods were compared and analyzed in this report. With the measured data analysis, the ekact understanding and use of these methods improves the reliability of flaw sizing and assures the integrity of nuclear power plant components.

A new free-living marine nematode species of the genus Phanoderma Bastian, 1865(Enoplida: Phanodermatidae) from the East Sea, Korea

  • A new species of free-living marine nematode is described from intertidal sediments of the East Sea, Korea. Phanoderma koreense sp. nov. is characterized by the presence of well-developed pharyngeal and cephalic capsule, six inner labial sensilla present as minute papillae around with circular groove, long and slender spicules with 4-5 serrated distal end, located at the base of the precloacal supplement, a series of eight to nine stout and short setae on the ventral cloacal region and conico-cylindrical tail with two pairs of blunt setae. In this study, we provide taxonomic descriptions and illustrations of a new species by differential interference contrast microscope and a pictorial key to the valid species of Phanoderma Bastian, 1865. This is the first record of the genus Phanoderma in the East Sea, Korea.

  • Amid the booming research on new and renewable energy, the photovoltaic(PV) industry has been growing around the PV advanced countries such as Japan, Germany, Europe and USA. In recent years, China became a strong performer in the world PV market share, increasing solar cell production rapidly. Both world solar cell and module production and installation rose steadily in 2007 like recent bumper years. In 2007, the PV industry produced 4.28GW and the installations reached a record high of 2.83GW, representing growth of 60percent over the previous year.

  • The species, Discothyrea sauteri Forel, 1912 (Hymenoptera: Formicidae: Proceratiinae), was documented for the first time in Korea. In this study, the morphological characteristics, representative images of this species, and a taxonomic key of the genera of the Korean Proceratiinae are provided.

  • This study examine Effect of Change of Compressive Strength of Concrete Members with Insulating Gang form on Temperature History of Concrete Positions. Test show, insulating gang forms differences and gang forms have 10℃ on peak point temperature of surface and Center if temperature history have 24Mpa by change of compressive strength. In addition, there have 14℃(16℃) on peak point temperature of surface and Center if temperature history have 40(60)Mpa. Therefore, insulating gang forms have an effect insulating performance.

  • 최근 해양 전투체계에서는 사후분석에 대한 기능이 중요시 되고 있다. 그 이유는 사후분석을 통해 작전을 분석하고 더 나은 작전을 수행할 수 있기 때문이다. 이를 가능하게하기 위해 해양 전투체계에는 전술정보 기록 및 재생 기능이 탑재되어 있다. 전술정보 기록 및 재생은 작전상황에서 전투체계 콘솔에 전시된 전술정보들을 기록해서 사후분석 및 평가를 위해 재생하는 기능이다. 본 논문에서는 전술정보 기록 및 재생 개발에 대한 효율화 방법을 제시한다. 모델 중심의 개발방식 적용과 재생시간의 효율화를 통해 기존방식 보다 효율적인 전술정보 기록 및 재생 소프트웨어를 개발하는데 그 목적을 가진다.
