Applicability of Oncoplastic Breast Conserving Surgery in Asian Breast Cancer Patients

  • Background: There are limited studies on oncoplastic breast conserving surgery in Asian women. We aimed to determine the applicability and safety of oncoplastic surgery, highlighting the specific circumstances when it will be most useful and compare our preferred technique with the worldwide practice of oncoplastic approaches. Materials and Methods: Breast cancer patients who underwent oncoplastic breast conserving surgery at a single institution from 1st May 2014-31st March 2015 were included. Data on patient demographics, tumor characteristics and the type of oncoplastic surgery performed were collected. Results: Nineteen breast cancer patients were identified. 42.1% of patients had grade I ptosis. The indications for surgery included a large tumor to breast size ratio (52.6%), multifocal/multicentric lesions (36.8%) and asymmetric breasts (10.6%), averting a mastectomy in 89.4%. Round block was the commonest technique in 63% of patients, in contrast to the inverted T pattern most frequently used in renowned institutions in the West. Mean and median tumor size and weight of specimen were 29.4/25mm (11 - 75mm) and 77g/64g (10 - 246g) respectively. Re-excision rate was 10.5%. Complete mastectomy was performed for one patient. One patient developed wound dehiscence which was treated conservatively. Cosmetic outcome was rated as excellent/good by 94.7% of patients. The patients remained clinically well after a median 16 months follow up. Conclusions: Oncoplastic breast conserving surgery is useful in a specific subgroup of Asian patients with a relatively small breast volume and minimal ptosis. Round block was the commonest technique in our series, in contrast to the worldwide utility of oncoplastic techniques. It is oncologically safe and has good cosmetic outcomes.

How Long the Effect of Social Support Would Be Continued for the Patients with Hypertension? (사회적지지 요법이 고혈압 환자 역할 행위 이행에 미친 효과의 지속에 관한 연구II)

  • The purpose of this study was to identify the effect of social support revealed in the time duration of sick role behavior compliance on the patients with hypertension using Quasiexperimental research design. Data collection was made through the interview survey technique from the hypertensive patients who received social support intervention (experimental group, n=41) and from those who were not exposed to the intervention(control group, n= 34). The subjects were registered in the cardiovascular outpatient clinic at the Chonnam National University Hospital from June 3, 1996 to November 30, 1997. $\chi^2$-test or t- test, Repeated measures ANOVA were utilized in the data analysis. The results were as follows: 1. The effect of social support intervention on sick role behavior compliance was significant in 1 month(F=69.17, p=.000), 6 months (F=11.51, p=.001), and 12 months(F=.07, p=.789) and between two groups(1 month; F=153.70, p=.000, 6 months; F=13.94, p=.000, 12 months; F=6.72, p= .011). 2. The effect of social support intervention on blood pressure was not significant through all the periods of time (F=1.21, p=.274) between the two groups(F=.12, p=.732). In conclusion, it was showed that social support had an effect on sick role behavior compliance and the effect of social support continued for twelve months(F= 10.03, p=.002) However, the score of compliance tends to decrease after 6 months of intervention. Therefore, this study indicated that social support re-intervention would be needed between six and twelve months.

  • Even though the revised operational guide for ICT education emphasizes the basic concepts of computer science, many primary and secondary schools do not deal with the changed requirements properly. Also, it is doubtful if the education considers teaching period and the levels of students or not. In this paper, we survey on the state of ICT education, the relationship between ICT education and Computer Education, and the connectivity in Computer Education between primary and secondary schools. In order to do these, we analyze the result of our questionnaire and we propose an improved method for the connectivity in Computer Education. The results are (i) primary school teachers don't consider the connectivity of Computer Education and (ii) the teachers who have higher satisfaction levels of ICT education think ICT education and Computer Education between two schools are correlated and connected. As a result, new policies to re-educate teachers are required.

  • the main purpose of this study was to inves-tigate the consumer dissatisfaction and com-plaining behavior in purchase and use by con-sumer value. The subjects participated this study were 204 female students. They were classified into 3 groups by important degree of instrumental value which is one of the 2 value categories that divided by the Rokeach. In order to assess consumer dissatishaction in purchase and use of clothing by those groups. The Clothing Purchase Dissatisfaction Inventory was used and The Consumer Complaining Behavior Inventory was adapted to asses consumer complaining behavior. The Clothing Purchase Dissatisfaction Inventory has 4 subscales(Price related factor Produt and Quality related factor Information and Service related factor Purchase decision-mak-ing related factor)and The Consumer Complaining behavior Inventory consisted in 3 subscales(Do nothing Private complaining Public complaining) Using SAS package in order to examine Clothing Purchase Dissatisfaction Inventory scores anaslysis of variance (MANOVA) was excuted And turkety test a kind of post-hoc multiple comparisons methods was adapted to compare Clothing Purchase Dissatisfaction In-ventory scores of each groups. in order to in-vestigate consumer complaining behavior by each groups and grade major pocket money a month the mean purchse price of clothing a month x2-test Frequency Percent were executed. Group made little of value tended to do nothing. 3. Grade major pocket money a monty the mean purchase price of clothing a month didn't have difference signficantly in consumer behavior. but major had difference on private complaining.

Effect of Duct Aspect Ratios on Pressure Drop in a Rotating Two-Pass Duct (덕트 종횡비가 회전덕트 내 압력강하에 미치는 영향)

  • The pressure drop characteristics in a rotating two-pass duct with rib turbulators are investigated in the present study. Three ducts of different aspect ratios (W/H=0.5, 1.0 and 2.0) are employed with a fixed hydraulic diameter ($D_h$) of 26.7 mm. $90^{\circ}$-rib turbulators with $1.5mm{\times}1.5mm$ cross-section are attached on the leading and trailing surfaces. The pitch-to-rib height ratio (p/e) is 1.0. The distance between the tip of the divider and the outer wall of the duct is 1.0 W. The thickness of divider wall is 6.0 mm o. 0.225 $D_h$. The Reynolds number (Re) based on the hydraulic diameter is kept constant at 10,000 and the .elation number (Ro) is varied from 0.0 to 0.2. As duct aspect ratio increases, high friction factor ratios show in overall regions. The reason is that the rib height-to-duct height ratio (e/H) increases, but the divider wall thickness-to-duct width ($t_d/W$) decreases. The rotation of duct produces pressure drop discrepancy between the leading and trailing surfaces. However, the pressure drop discrepancy of the high duct aspect ratio (AR=2.0) is smaller than that of the low duct aspect ratio (AR=0.5) due to the decrement of duct hight (H).

Anti-Spam for VoIP based on Turing Test (튜링 테스트 기반으로 한 VoIP 스팸방지)

  • As increasing the user of VoIP service using ITSP(Internet Telephony Service Provider), the VoIP spam becomes a big problem. The spam used in the existing public telephone is detected by using the pattern inspection of call behavior because it is difficult to filter contents for the characteristic of real-time voice communication. However there is a false-positive problem. The threat on spam remains where spam with low threshold can't be detected or users share one number. In this paper, we propose anti-spam for VoIP based on luring test. The proposed method gives a user luring test and he/she can connect to a receiver if passing turing test. A ticket is given to a user that pass luring test and it reduces overhead of luring test in re-dial. The proposed method is implemented on ASUS WL-500G wireless router and Asterisk IP-PBX. Experimental results show the effectiveness of the proposed method.

The Effects of Anti Chemotherapy Individualized Education Relationship with Knowledge and Promoting Self Care for Cancer Patients (화학요법에 관한 개별적 환자교육이 암환자의 지식과 자가간호 수행에 미치는 효과)

  • The Cancer Patients are rapidly growing and it became one of the main cause of death. Lately with the effect of Chemotherapy, drastically improved patients quality of life and also Prolonged Life of Cancer Patients. But the Chemotherapy not only Kill the Cancer cells but also harm to normal cells witch Cancer lots of side effect. This study was designed to identify the effectiveness of patient education to reduce side effect and help promote self care. The date was collected from October, 2001 to February, 2002 by questionary Survey Method. The subjects were 24 patients Who were and Anti Chemotherapy C University Hospital. Educated booklet contained what is Chemotherapy, what are the side effects, and to promote Self care, Pre test was given after 3-4 weeks re admission period. Analysis data was done by SPSS Program Paired t-test was used to differentiate Knowledge and difference of performing self care. Relationship in between Knowledge and promoting Self care was used Pearson Correlation. As the result of education, knowledge of self care and performance was drastically increased but the relationship in between knowledge of self care performed was not significant. Therefore it is proposed as follows: 1) Further research with experienced subjects to differentiate experience group and control group. 2) Education program need to be develop and standardize for anti cancer Chemotherapy patient. 3) During 6 cycle of Chemotherapy what is ideal number of in order to get the best result.

The Study of Eye Re fraction State on Old Age in Je-ju islands (제주도 지역 노인의 굴절이상에 대한 연구)

  • The purpose of this study was to provide the accurate information on improvement of visual life by examining old age's eye refraction state and investigating distribution of ametropia in old age. The test was performed the visual acuity test by object and subject methods to over 60 years of age. The results were as follows: 1. As the result to investigate the inconvenience degree of visual life, the subjects appealed 81% for very inconvenience and 14% for a little inconvenience. 5% of them didn't feel any inconvenience in visual life. 2. The eye types were 1.7% for emmetropia, 10% for myopia, 74.7% for hyperopia, and 13.6% mixed astigmatism, respectively. 3. The axis of astigmatisms were 65% positive for indirect astigmatism, 8% for direct astigmatism, and 27% for oblique astigmatism, respectively.

  • The current VaR Model based on J. P. Morgan's RiskMetrics has problem that actual loss exceeds VaR under unstable economic conditions because the current VaR Model can't re ect future economic conditions. In general, any corporation's stock price is determined by the rm's idiosyncratic factor as well as the common systematic factor that in uences all stocks in the portfolio. In this study, we propose an One-factor VaR Model for stock portfolio which is decomposed into the common systematic factor and the rm's idiosyncratic factor. We expect that the actual loss will not exceed VaR when the One-factor Model is implemented because the common systematic factor considering the future economic conditions is estimated. Also, we can allocate the stock portfolio to minimize the loss.

Cases Study of Accidents in High Risk Organizations by System Dynamics (시스템 다이내믹스 기법을 활용한 고위험 조직 사고 사례 분석)

  • The importance of the concept of safety culture has increased in the security of high-risk facility after Chernobyl accident in 1986. This paper elaborated the concept of safety culture and its main factors by Causal Loop Diagram. Due to the decline of safety culture, the occurrence of incidents and accidents require more and more corrective actions to the members of high-risk facilities and thereby increasing their workloads. Employees who must complete the task within the given time have to have time pressures and don't comply with the rules and procedures. Also, a schedule pressure is a big stress for employees, causing mistakes in precision work. In order to improve these problems, CLD of the safety culture in this paper suggests hiring more workers, re-allocation of given workloads and strengthen the learning, communication capabilities and safety leadership. In addition, the two real accident cases were analyzed to test the feasibility of the System Dynamic simulation model through the process of structuring the fault trees on the stationary black out accident in Kori unit 1 in South Korea and Kleen Energy power station explosion in US. The simulation results show that the various safety factors cause the serious accident combined with mechanical failure and safety culture will reduce the possibility of the accidents in these high-risk organizations. This simulation model can contribute to analyzing the impact of the organizational and human factors of safety culture and can provide the alternatives in high-risk facilities.
