The Clinical Feature of Reflux Esophagitis Patients Visiting a Single Korean Medicine Hospital: A Retrospective Study

  • Objectives: This study investigated the clinical characteristics and symptoms of patients with reflux esophagitis (RE) visiting a Korean medicine hospital. Methods: We retrospectively reviewed the medical records of patients with an RE diagnosis who visited a Korean medicine hospital from June 15, 2020, to April 15, 2021. We analyzed the clinical characteristics and the results of a symptoms questionnaire. Results: Our sample included 1,151 patients (711 females). The median age was 58 years; the most common age bracket was 60-69 years. A total of 837 patients (72.7%) presented with symptoms lasting more than one year, and 1,035 (89.9%) had been prescribed medications for more than three months before visiting a Korean medicine hospital. Belching (77.6%) and acid regurgitation (70.9%) were the most frequent symptoms, followed by epigastric pain (61.6%) and throat globus (58.5%). Throat globus was the most bothersome symptom, and 72.4% of the patients experienced the symptom on more than five days in the preceding two weeks. Patients with a longer symptom duration were older (p<0.01) and more likely to be overweight (p=0.03). Patients experiencing symptoms for more than five years were more likely to report throat globus (p=0.02), hoarseness (p<0.01), and coughing (p<0.01). Conclusions: Most RE patients visiting a Korean medicine hospital were females in their sixties with chronic and refractory RE. Atypical symptoms (belching, epigastric pain, and throat globus) were common. Throat globus was highly prevalent in severe cases, suggesting that it warrants attention when treating RE patients.

  • The differential white blood cell count plays an important role in the diagnosis of different diseases. It is a tedious task to count these classes of cell manually. An automatic counter using computer vision helps to perform this medical test rapidly and accurately. Most commercial-available automatic white blood cell analysis composed mainly 3 steps including segmentation, feature extraction and classification. In this paper we concentrate on the first step in automatic white-blood-cell analysis by proposing a segmentation scheme that utilizes a benefit of active contour. Specifically, the binary image is obtained by thresolding of the input blood smear image. The initial shape of active is then placed roughly inside the white blood cell and allowed to grow to fit the shape of individual white blood cell. The white blood cell is then separated using the extracted contour. The force that drives the active contour is the combination of gradient vector flow force and balloon force. Our purposed technique can handle very promising to separate the remaining red blood cells.

  • The make a decision for building remodeling, the economical value of the remodeled building should be analyzed and existing situation of the building system should be checked. For the energy issues related to the building remodeling process, careful diagnosis and re-designs of existing system should be proposed. In this research, the luminous environment for Seoul K Co. headquaters and Seoul S hotel were measured and then the re-design processes of building illumination systems were researched. With measured illuminance quantities of there cases, computer simulations for the existing situation and re-design were conducted.

Two Cases of Re-emerging Indigenous Malaria in Korean Children (자매에서 발생한 한국 토착형 소아 말라리아 2례)

  • Malaria is a febrile disease caused by protozoan parasites, genus Plasmodium. In Korea., indigenous malaria has been believed to be eradicated by 1984, and, thereafter, all of the reported cases were imported malaria. But since the first case report of re-emerging indigenous malaria in 1993, increasing number of cases were reported reaching more than 350 cases in 1996. However, indigenous malaria in children has not been reported yet. We experienced two cases of indigenous malaria in sisters who were 7 and 5 years old, respectively. Elder sister was presented with periodic fever, splenomegaly and mild headache. She had been to Guam before 4 months of the onset of symptoms. Younger sister was suffered from fever and splenomegaly and has not been abroad. They were diagnosed by examination of peripheral blood smear to be infected with Plasmodium vivax and were treated with hydroxychloroquine and primaquine successfully. These cases are believed to be first re-emerging cases of indigenous malaria in children, and malaria should be included in the differential diagnosis of unexplained febrile children.

  • The prevalence of chronic diseases including osteoporosis and sarcopenia increases as the population ages. Osteoporosis and sarcopenia are commonly associated with genetics, mechanical factors, and hormonal factors and primarily associated with aging. Many older populations, particularly those with frailty, are likely to have concurrent osteoporosis and sarcopenia, further increasing their risk of disease-related complications. Because bones and muscles are closely interconnected by anatomy, metabolic profile, and chemical components, a diagnosis should be considered for both sarcopenia and osteoporosis, which may be treated with optimal therapeutic interventions eliciting pleiotropic effects on both bones and muscles. Exercise training has been recommended as a promising therapeutic strategy to encounter the loss of bone and muscle mass due to osteosarcopenia. To stimulate the osteogenic effects for bone mass accretion, bone tissues must be exposed to mechanical load exceeding those experienced during daily living activities. Of the several exercise training programs, resistance exercise (RE) is known to be highly beneficial for the preservation of bone and muscle mass. This review summarizes the mechanisms of RE for the preservation of bone and muscle mass and supports the clinical evidences for the use of RE as a therapeutic option in osteosarcopenia.

  • This paper presents the programmable logic controller (PLC) networking development for communication with the mobile phone. The proposed technique is suitable for remote sensing control systems, which can display the operation status and monitoring fault diagnosis of a system. The system operation is based on the use of personal computer (PC) to logically analyze data from PLC, and existing internet protocol for sending information messages to mobile phones.

  • Worldwide, stroke is the 2nd or 3rd leading cause of death and a major health problem. Recent advances in medical technology have significantly improved diagnosis and treatment strategies of ischemic stroke. The ischemic stroke subtype is an important determinant of mortality and long-term prognosis of patients. To estimate excess-risks of the ischemic stroke subtype, recently published article, Korean cohort study of stroke, was used as a source article. According to mortality analysis methodology from American academy of insurance medicine, the overall mortality ratio and excess death rate was the highest in patients with SOD, followed by those with CE. Calculated mortality ratio and excess death rate for subtype in this review are SOD, 920%/34‰; CE 267%/34‰; UI 209%/25‰; UM 190%/23‰; UN 188%/15‰; LAA 162%/15‰; LAC 117%/3‰.

Agricultural Drought Assessment and Diagnosis Based on Spatiotemporal Water Supply in Irrigated Area (필지단위 관개용수 공급에 따른 농업가뭄진단 평가)

  • Agricultural drought is a natural phenomenon that is not easy to observe and predict and is difficult to quantify. In South Korea, the amount of agricultural water used is large and the types of use are varied, so even if an agricultural drought occurs due to insufficient precipitation, the drought actually felt in the irrigated area is it can be temporally and spatially different. In order to interpret the general drought in the past, drought disasters were evaluated using single indicators such as drought damage area, precipitation shortage status, and drought index, and a comprehensive drought management system is needed through drought diagnosis survey. Therefore, we intend to conduct research on agricultural drought assessment and diagnosis using re-evaluation of agricultural facilities and irrigation water supply network due to changes in various conditions such as climate change, irrigation canal network, and evaluation of water supply capacity of agricultural facilities. In this study, agricultural drought diagnosis was conducted on two agricultural reservoirs located in Sangju, Gyeongsangbuk-do, with structural or non-structural evaluations to increase spatiotemporal water supply and efficiency in terms of water shortages. The results of the agricultural drought diagnosis evaluation can be used to identify irrigated areas and canal network vulnerable to drought and to prioritize drought response.

Left Ventricular Rupture after Mitral Valve Replacement - 3 cases report - (승모판막 치환술후 합병한 좌심실 파열의 외과적 고찰 - 3례 보고 -)

  • An unusual but often lethal complication of mitral valve replacement is rupture of the left ventricle. From March 1977 through June 1990, 424 mitral valve replacements were performed as isolated or combined procedures. Rupture of the posterior wall of the left ventricle was observed in 3 patients. Their was one type I and two type II rupture. Once the diagnosis was made, all of the patient were connected to the heart-lung machine again and total cardiopulmonary bypass is re-established. Repair was attempted in all of them from the outside of the heart. One of them was successively repaired but two were failed due to myocardial ischemia by circumflex coronary artery injury and failure of adequate closure of the ruptured site. From this results, we concluded that prevention is the best solution. But if we encountered this condition, early diagnosis and rapid treatment may improve the patient`s chances for survival.

SEREX; discovery of tumor antigens (종양 항원의 발견: SEREX)

  • The identification of tumor antigens is essential for the development of anticancer therapeutic vaccines and clinical diagnosis of cancer. SEREX (serological analysis of recombinant cDNA expression library)has been used to identify such tumor antigens by screening sera of cancer patients with cDNA ex-pression libraries. SEREX-defined antigens provide markers for the diagnosis of cancers. SEREX is also a powerful method for the development of anticancer therapeutics. The development of anticancer vaccines requires that tumor antigens can elicit antigen-specific antibodies or T lymphocytes. This re-view provides information on the application of SEREX for discovery of tumor antigens.
