혀의 자세가 경추의 관절가동범위에 미치는 영향 (The Effects of Tongue Positions on the Cervical Range of Motion)

  • Purpose : This study was to investigate that tongue positions have effect on the cervical range of motion (ROM). Methods : 18 subjects, 20 to 25 years of age, were participated in this study. The tongue positions were neutral position, anterior protrusion, posterior protrusion, superior protrusion, inferior protrusion, left side protrusion and right side protrusion. The neck movements were right side flexion, left side flexion, right rotation and left rotation. The cervical range of motion measured by cervical range of motion instrument (CROM, C9266-49, USA). The changes were analyzed using the paired t-test. SPSSWIN (ver. 20.0) was used for data analysis and the significance level was chosen as ${\alpha}$=0.05. Results : In the lateral flexion, the angle of left lateral flexion increased in anterior protrusion(p<0.05) and right side protrusion(p<0.05) significantly. In the rotation, the angle of right rotation increased in anterior protrusion(p<0.05), posterior protrusion (p<0.05), superior protrusion(p<0.05), inferior protrusion(p<0.05), and left side protrusion (p<0.05) significantly. Conclusions : In conclusion, we found that the tongue positions affected some cervical movements.

Comparison of Multilevel Growth Models for Respiratory Function in Patients with Tracheostomy and Stroke using Cervical Range of Motion Training

  • Objective: The purpose of this study was to investigate the effect of cervical range of motion training on the change in respiratory function growth rate at the group and individual level in stroke patients and stroke patients with tracheostomy tube. Design: A Multilevel Growth Model Methods: 8 general stroke patients and 6 stroke patients who had a tracheostomy tube inserted were subjected to cervical range of motion training 3 times a week for 4 weeks. Force vital capacity (FVC), Forced expiratory volume in the first second (FEV1), Forced expiration ratio (FEV1/FVC) and Manual assist peak cough flow (MPCF) were measured. Data were analyzed using descriptive statistics and multilevel analysis with HLM 8.0. Results: A significant difference was found in the respiratory function analysis growth rate of the entire group (p<0.05), and two groups were added to the research model. The linear growth rate of respiratory function in patients with general stroke increased with the exception of FEV1/FVC (p<0.05). Stroke patients with tracheostomy tube showed a decreasing pattern except for FVC. In particular, MPCF showed a significantly decreased result (p<0.05). Conclusions: This study found that the maintenance of improved respiratory function in stroke patients with tracheostomy tube decreased over time. However, cervical range of motion training is still a useful method for respiratory function in general stroke patients and stroke patients with tracheostomy tube.

Effect of Joint Mobilization and Kinesio Taping on Pain, Range of Motion, and Knee Function in Patients with Knee Osteoarthritis

  • Purpose: This study was conducted to investigate the effects of joint mobilization and kinesio taping on pain, range of motion and knee function in patients with knee osteoarthritis. Methods: Thirty subjects were selected and divided into three groups: group 1 was treated with joint mobilization, group 2 was treated with kinesio taping and group 3 was treated with joint mobilization and kinesio taping. Joint mobilization was performed for 20 minutes three times a week for a period of 4 weeks, after which tape was applied for the same period of time and it was not exceeded 24 hours. Pain, range of motion and knee function were then assessed to identify the effectiveness. A visual analog scale (VAS) was used for pain assessment, while active and passive range of motion (AROM, PROM) were assessed using smart phones application, and knee injury and osteoarthritis outcome score (KOOS) was used to assess knee function. Results: After intervention, the joint mobilization group and kinesio taping group showed significant improvements in VAS, AROM, PROM and KOOS (Symptom, Pain, ADL, QOL), whereas no significant difference was found in sport/recreation. The joint mobilization with kinesio taping group showed significant improvements in all items, and a significant increase was found in AROM and PROM compared to the other two groups. Conclusion: We found that joint mobilization and kinesio taping effectively improved pain, range of motion and knee function in patients with knee osteoarthritis, but that application of joint mobilization with kinesio taping was most effective.

수정된 PNF 발목 움직임 패턴이 능동적 발등 굽힘 가동범위 및 다리 근활성도에 미치는 영향 (The Effects of Modified PNF Ankle Movement Patterns on Active Dorsiflexion Range of Motion and Leg Muscle Activity)

  • Purpose: The purpose of this study was to investigate the effects of modified ankle movement patterns on participants' active dorsiflexion range of motion and leg muscle activity. Methods: This study recruited twenty-five participants, all of whom were healthy individuals with no abnormalities in the ankle or knee joints. The research methodology involved measuring the active dorsiflexion range of motion and muscle activity in each person's legs based on the presence or absence of toe extension while the subjects were in a comfortable, supine position. A statistical analysis was conducted using SPSS 25.0, and a paired samples t-test was employed. The significance level was set at 0.05. Results: When the participants demonstrated the modified ankle movement pattern with a controlled toe grip, there was an increase in their active dorsiflexion angle. However, during the proprioceptive neuromuscular facilitation technique without a controlled toe grip, a higher level of activity was observed in the leg muscles. Conclusion: The results of this study could be used as foundational data for establishing a rehabilitation exercise program designed to enhance range of motion and muscle activation in the ankle joint.

물리치료사들의 경추 가동범위에 관한 연구 (A Study for cervical range of motion in Physical Therapists)

  • The purpose of this study is to find out the influence of Physical Therapists' heavy work on their cervical spines. Subjects were 80 members of Physical Therapists, 40 males and 40 females who were between 3rd decades to 4th decades. The Cervcial Range of Motion instrument was used to measure six cervical range of motion such as flexion, extension, left and right lateral flexion, left and right rotation. The degree of rotation and both lateral flexion was appeared as decrement in 4th decades than 3rd decades(P<0.05). And the left lateral flexion in female Physical therapiest was increased than in male ones(P<0.05). By this results, it was proved that the older the PT's age, the less the cervical motion range and there were more decrement in male Physical Therapists than female Physical Therapists. And finally I think that the evaluating method used in this study will be clinically very useful and be a basic raw materials for evaluating the existence and the degree of functional disability as in the occupational hazards due to Physical Therapist's heavy work.

탄성밴드를 이용한 저항운동이 회전근개봉합술 환자의 견관절가동범위와 기능 및 통증에 미치는 효과 (Effects of Resistance Exercise using Elastic Band on Range of Motion, Function and Shoulder Pain among Patients with Rotator Cuff Repair)

  • Purpose: The purpose of this study was to examine the effect of resistive exercise using an elastic band on range of motion, function and shoulder pain. Methods: Forty two subjects who had rotator cuff surgery were assigned either to a treatment or a comparison group, twenty one each. Following a six week period after surgery those in the treatment group participated in resistant exercise using an elastic band for four weeks. The subjects in the comparison group did not participate in the exercise program. The goniometer measured range of motion, a modified tool measured function, and a self report numerical rating scale measured pain. The data were analyzed using $x^2$ test, Fisher's exact, t-test and the Mann-Whitney Test. Results: Although not statistically significant, there was a trend that more subjects in the treatment group had increased range of motion. There was a statistically significance among the treatment group in terms of increased function (p=.015). Further the treatment group reported less pain that those in the comparison group (p<.001). Conclusion: The findings support that resistance exercise is an effective strategy for patients with rotator cuff repair.

마비말장애 환자의 혀의 강도.운동범위와 말명료도에 관한 연구 (Tongue Strength, Range of Motion, and Speech Intelligibility in Dysarthric Speakers)

  • The purpose of the present study were (1) to assess differences in tongue performances and speech intelligibility between normal and dysarthric speakers; and (2) to analyze the possible interrelationships between tongue strength, range of motion, and speech intelligibility in dysarthric patients. In order to measure maximum strength of anterior and lateral tongue strength, a force transducer has been designed. And a ruler was used for guaging range of motion. 'Word intelligibility test' was administered to each group. The results were analyzed by a quantitative statistical method(t test, Pearson product- moment correlation, and one-way ANOVA). The results were as follows; (1) dysarthric speakers showed significantly poorer performance than the normal in the tongue strength, range of motion, and speech intelligibility tasks; (2) the correlation between speech intelligibility and instrumental tongue performance was high in the dysarthric group; and (3) instrumental assessment was coincide with perceptual judgement of dysarthric tongue performance. The present investigation demonstrated that tongue weakness is causally related to articulation deficits in dysarthric. The clinical use of the force transducer would help a speech pathologist to quantify the degree of tongue weakness.

Consideration from the Viewpoint of Chiropractic Correction on the Dysfunction of Temporomandibular Joint

  • This study was to investigate the needs of the functional abnormality of the Temporomandibular joint. The purpose of this study was to find out basic concept for the Chiropractic-care necessity of the neuromuscular skeletal patients with functional abnormality of the temporomandibular joint. I evaluated the change of the range of motion, neck pain, headache by post xray, orthopedic test and patient's charts. The range of motion at temporomandibular joint was improved and the necessity of chiropractic care was recognized in the neuromuscular skeletal patients with having temporomandibular joint problems.

손목관절의 기능적 운동 특성의 동작 분석 (Functional Motion Analysis of Wrist Joints)

  • Injuries of wrist in upper extremity is common onset in industrialized world. The development of joint arthroplasty and mechanical joint is area of research for biomechanical engineer and surgeon for a decade. Therefore. the knowledge of characteristic of joint motion is essential to develop the artificial wrist joint. In this study. the joint motions of wrist required for activities of daily living (ADLs). including personal hygiene and care. and general home activity were measured using flexible electrogoniometer. Total of 25 different daily activities were separated into four groups and tested on 15 subjects who did not show any abnormality of their joint functions. The maximum functional range of motion required for ADLs were obtained and standardized for analysis and comparison. Also. a least functional range of motion for ADLs were investigated. Results revealed that any significant differences were not found in least functional range of motion between left and right wrist to perform ADLs. However. a significant difference was found in different ADLs. Therefore. least range of motion obtained in this study can be used as basic data to design artificial joint and set a goal for surgeon to achieve appropriate treatment from patients.

Feasibility of a New Desktop Motion Analysis System with a Video Game Console for Assessing Various Three-Dimensional Wrist Motions

  • Background: The restriction of wrist motion results in limited hand function, and the evaluation of the range of wrist motion is related to the evaluation of wrist function. To analyze and compare the wrist motion during four selected tasks, we developed a new desktop motion analysis system using the motion controller for a home video game console. Methods: Eighteen healthy, right-handed subjects performed 15 trials of selective tasks (dart throwing, hammering, circumduction, and winding thread on a reel) with both wrists. The signals of light-emitting diode markers attached to the hand and forearm were detected by the optic receptor in the motion controller. We compared the results between both wrists and between motions with similar motion paths. Results: The parameters (range of motion, offset, coupling, and orientations of the oblique plane) for wrist motion were not significantly different between both wrists, except for radioulnar deviation for hammering and the orientation for thread winding. In each wrist, the ranges for hammering were larger than those for dart throwing. The offsets and the orientations of the oblique plane were not significantly different between circumduction and thread winding. Conclusions: The results for the parameters of dart throwing, hammering, and circumduction of our motion analysis system using the motion controller were considerably similar to those of the previous studies with three-dimensional reconstruction with computed tomography, electrogoniometer, and motion capture system. Therefore, our system may be a cost-effective and simple method for wrist motion analysis.
