Objectives: This study aimed to review the clinical research trends in respiratory diseases related to particulate matter (PM) to help design clinical studies on herbal medicine that protects against PM. Methods: We searched three international databases (Pubmed, EMBASE, and CENTRAL) to investigate clinical studies on respiratory diseases related to PM and to analyze their study design, respiratory-related disease, inclusion/exclusion criteria, study period, outcome measure, study results, measurement method of PM and range of PM. Results: A total of 18 studies were finally selected, including 13 observational studies and 5 randomized controlled trials. The selected studies showed an increasing trend from 1985 to 2018 and were conducted mostly in North America, followed by Europe and Asia. Subject disease and age were decided in variety by each study objective, although asthma accounted for the majority. For the outcome assessment, pulmonary function test was mostly used for lung function. Quality of life questionnaires and biomarkers in blood and sputum were also used. Conclusion: A well-designed clinical study on herbal medicine that protects against PM is needed, and this study is expected to be used as base data.
Since non-cardiac chest discomfort (NCCD) can result in substantial healthcare burden and lower quality of life, interventions such as cognitive behavioral therapy (CBT) have been investigated for the relief of NCCD. In this review, we aimed to summarize the evidence on the efficacy of the CBT for the treatment of NCCD while introducing a newly-developed computerized CBT program for NCCD. Studies applying CBT to individuals with NCCD were searched for from both English and Korean electronic databases. Among 37 studies, 11 randomized controlled trials, 4 case-control studies, 1 case series, and 2 review articles were eligible for this review. Efficacy of conventional CBT for NCCD was shown in a series of studies as most of them reported improved symptom severity of NCCD or NCCD-related anxiety. However, a substantial variability existed among these studies in participants, treatment procedures and durations. High attrition rates were also reported in these studies on conventional CBT. Computerized CBT could be an alternative to the conventional CBT as it can be standardized and more easily accessible, but it was only reported in one previous study. In addition to the literature review, we presented a newly-developed computerized CBT program for NCCD which may overcome some of the limitations of conventional CBT. A computerized CBT could be an alternative treatment of NCCD, however, need further studies on its usefulness.
Objective: This study was performed to research Korean medical treatment methods for hyperthyroidism by summarizing domestic clinical studies. Methods: The study was based on 14 papers published in Korea since 2000, on Korean medicine treatments for hyperthyroidism. Papers were searched on four domestic electronic databases. Results: 1. This paper analyzed 14 studies on the subject of clinical research on hyperthyroidism in Korea. 12 of them reported cases while two of them reported on clinical research. 2. The average age of the subjects was 39 years, 71.3% of women, 20-60 years of age accounted for 92%, and the average duration of disease was 22 months. 3. Of the 115 subjects, 102 had experienced Western medical treatment. So most of them chose Western medical treatment as the first treatment method, of which 24 experienced side effects. 4. Eumhuhwadong (陰虛火動) and Gieumyanghu (氣陰兩虛) appeared 3 times respectively. 5. The Korean herbal medicine, Ahnjeonbaekho-tang was the most frequent herbal medicine used to treat hyperthyroidism, with the next most frequent being Yanggyuksanhwa-tang. 6. The study, which was performed using herbal medicine alone, showed a marked improvement in clinical symptoms and thyroid function test, compared with administration of antithyroid drugs and herbal medicines. Conclusions: Large-scale studies and randomized controlled trials will be needed to evaluate the validity of Korean medical treatment for hyperthyroidism.
Objectives: This study was conducted to comprehend the syndrome differentiations of dysmenorrhea and find out their clinical symptoms, tongue images and pulse patterns by analyzing previous studies. Methods: The following researches were collected by searching the medical journals published from November, 2007 to October, 2017, from KISS, OASIS, CNKI. : researches on the syndrome differentiation of dysmenorrhea, researches on the criteria of diagnosis of syndrome differentiation of dysmenorrhea, randomized controlled trials (RCT) used syndrome differentiation for treating dysmenorrhea. Results: By investigating the frequency of syndrome differentiations used in RCT studies, the frequent ones were chosen. They were qi stagnation and blood stasis (氣滯血瘀), qi-blood deficiency (氣血虛弱), congealing cold with blood stasis (寒凝血瘀), liver-kidney depletion (肝腎虧損), blood stasis with dampness-heat (濕熱瘀阻). Conclusion: 4 syndrome differentiations were frequently used in RCT studies. And the frequency of clinical symptoms on each syndrome differentiations from each RCT study was analyzed and compared. Clinical symptoms chosen as chief symptoms in more than one reference, appeared in more than half of the references, most frequent tongue images and pulse conditions were organized. The most frequent clinical symptoms included the period and pattern of pain, the accompanying symptoms of whole-body and the pattern of menstrual bleeding.
Objectives: The aim of this study was to perform a meta analysis of randomized controlled trials (RCTs) that applied Opae-san to peptic ulcer. Methods: The databases NDSL, RISS, KISS, KISTI, Oasis, DBpia, Cochrane, EMBASE, Pubmed, and CNKI were searched to identify RCTs that evaluated the therapeutic response to Opae-san on peptic ulcer. The selected studies were assessed using Cochrane Group's risk of bias tool. Results: 12 RCTs were selected from a total of 312 identified. Combined therapies of Opae-san plus triple therapy were superior to only triple therapy in achieving the effective rate (risk ratio=1.26, 95% CI: 1.17 to 1.35, p<0.001, I2=0%), the helicobacter pylori eradication rate (risk ratio=1.23, 95% CI: 1.12 to 1.34, p<0.001, I2=7%) and the recurrence rate (risk ratio=0.31, 95% CI: 0.12 to 0.82, p=0.02, I2=0%). But only Opae-san was not superior in achieving the effective rate compared to anti gastric secretion drugs. Conclusions: The current evidence suggests that combined therapies of Opae-san plus triple therapy yield a higher effective rate, and helicobacter pylori eradication rate and a lower recurrence rate. However, in most of these studies, it is difficult to evaluate the bias and therefore better designed studies are needed.
Background/Aims To evaluate the efficacy and safety of Aloe vera (AV) in patients with irritable bowel syndrome (IBS). Methods We searched the MEDLINE, EMBASE, and Cochrane databases for studies dated between 1st January 1960 and 30th December 2017. Eligible randomized controlled trials (RCTs) compared AV to placebo in patients with IBS. The primary outcome was standardized mean difference of the change in severity of IBS symptoms as measured by patient-rated scales. Secondary outcomes included response rate of IBS symptoms and adverse events. Heterogeneity among studies was assessed using Cochrane's Q and $I^2$ statistics. Results Three RCTs with a total of 151 patients with IBS were included. The meta-analysis showed a significant difference for patients with AV compared to those with placebo regarding improvement in IBS symptom score (standardized mean difference, 0.41; 95% CI, 0.07-0.75; P = 0.020). Using intention-to-treat analysis, the AV patients showed significantly better response rates of IBS symptoms compared to placebo (pooled risk ratio, 1.69; 95% CI, 1.05-2.73; P = 0.030). No adverse events related with AV were found in included studies. There was no significant heterogeneity of effects across studies (P = 0.900; $I^2=0%$). Conclusion AV is effective and safe for the treatment of patients with IBS compared to placebo.
목적 이 연구의 목적은 치매 환자의 수면을 주제로 고찰 된 신뢰성 있는 연구를 통해 수면에 영향을 미치는 중재를 탐색하고 그 결과를 일반화 하는데 목적이 있다. 체계적 고찰을 통해 치매 환자의 수면 중재 효과성을 실증적으로 규명하고 이에 관한 근거를 확인하기 위한 것이다. 연구방법 자료 분석을 위하여 2000년부터 2017년까지 선행 연구 되어 온 문헌 자료를 수집 분석 하였다. PsycINFO, CINAHL, MEDLINE, PubMed, Science Direct, PEDro, Cochrane libraryA를 이용하여 dementia, sleep로 검색 하였다. 검색된 결과에 따라 선정기준에 맞는 논문을 분석하였다. 연구결과 최종적으로 5개의 논문이 선정되었고, 약물 중재 제공이 3개, 통증 관리 1개, 빛 치료 프로그램 1개로 분석되었다. 이 중 치매 환자의 수면 중재 효과가 검증된 논문이 4개 효과가 없는 것으로 나타난 논문이 1개였다. 결론 연구 결과 치매환자의 수면 중재 효과에 대한 근거를 마련할 수 있었다. 치매 환자 수면 문제 해결을 통해 환자의 행동 조절 및 질환 진행을 막을 수 있도록 적극으로 중재 하는 프로그램이 필요하다.
본 연구의 목적은 중국에서 강박증 치료에 삼전요법이 얼마나 효과가 있는지 살펴보는 것이다. 삼전요법은 도교, 불교 선종은 물론 고대 한방정신요법의 특색을 두루 담고 있는 심리치료 방법이다. 최근 국내 연구 결과에 따르면 전체 인구의 1.5%가 강박증을 앓고 있어 강박증 치료에 많은 관심이 쏠리고 있다. 중국의 임상연구 결과에 따르면 삼전요법과 양약치료를 병행한 경우 강박증 치료에 유의미한 효과를 얻었다. 삼전요법 치료만으로도 양약치료보다 강박증에 유의한 치료 효과를 거두었다. 강박증 치료를 위해 삼전요법의 활성화가 필요하다.
Objectives: Traditional oriental medicine is used in treating breast cancer-related lymphedema to alleviate symptoms. Upper limb lymphedema is a symptom that is frequently observed in patients with breast cancer, and it impairs their quality of life. This systematic review aimed to summarize the current available evidence to evaluate the effect of traditional oriental medicine on upper limb lymphedema in breast cancer patients. Methods: The review evaluated randomized controlled trials (RCTs) measuring the effect of herbal medicine, acupuncture, and moxibustion on upper limb lymphedema in breast cancer patients within four electronic databases. The Cochrane risk of bias (ROB) tool was used to assess the quality of the RCTs. Results: In total, 23 RCTs met the inclusion criteria. Among them, 22 studies reported that the rate of severity of lymphedema improved after treatment in the traditional treatment group using herbal medicine, acupuncture, or moxibustion better than in the conventional medicine group. The methodological quality of the RCTs was insufficient with an unclear and high ROB. Conclusions: Traditional oriental medicine may have a potential to improve lymphedema in patients with breast cancer. To confirm the clinical recommendation, further research with a rigorous study design is required to support the effects of traditional oriental medicine.
Objective: The purpose of this study was to investigate the effect of electroacupuncture on acute pancreatitis (AP). Method: The search engines that were used included PubMed, CNKI, OASIS, NDSL, the Korean Traditional Knowledge Portal, and J-STAGE. The search period was from the creation of the search engine to May 6, 2019; there were no language limitations. Extractions and selections from the literation were made by two authors. The study included randomized controlled trials with electroacupuncture for patients with acute pancreatitis. Cochrane's risk of bias (RoB) was used as the methodological quality assessment scale. Results: Nine studies were finally selected. It was observed that using electroacupuncture with general Western medicine treatment for AP was more effective than using only general Western medicine treatment. Although most of the studies showed a high risk of bias, there were significant differences among the indicators. Conclusion: The electroacupuncture treatment with Western medicine may be more effective in reducing symptoms of AP than Western medicine alone. However, due to the limitations of the research design, it was not enough to obtain reliable information. As a result, high quality research will be additionally required.
