영상의학과 서비스 개선방안에 관한 연구 (A Study on Ways of Improving Radiology Department Services)

  • The purpose of this study is to find ways of improving radiology services provided at hospitals through examining current problems of the services and measuring patient satisfaction about them For the purpose, this researcher made a questionnaire survey of 209 patients who used the radiology departments of two university hospitals as tertiary medical institutions located in Busan. The survey was carried out from June 1 to July 30, 2008. Results of the study can be summarized as follows. The patients' satisfaction about the kindness of radiologists was generally low since it Was measured to be scored 3.0 or below in average. This indicates that they were dissatisfied with the equipment and environment. The patients' anxiety about the process of radiology was measured and scored 3.0 or below in average, indicating they generally felt anxious about the process. Factors affecting the above reliability such as their skillfulness, qualification, persuasiveness and expertise were found positively correlated with the patients' satisfaction about the radiologists. To realize Better radiological department services, in conclusion, radiologists are required to resort to conventional practices of radiology no longer, and reconsider current radiological technologies. They are also required to firmly establish their pride as a professional, and make best efforts to keep pace with this era of rapid scientific development by constantly training themselves.

의료 소비자의 의학적 지식 유무와 의료서비스만족도 관계 (The Relationship between Status of Medical Knowledge of Medical Consumers and Satisfaction of Medical Service)

  • 의료의 과학화로 인해 고가의료장비를 이용한 의료서비스 공급이 증가하고 있다. 의학 정보의 대중화로 의료소비자들의 의학적 지식수준 및 의료기관 선택능력의 향상으로 높은 수준의 의료서비스가 요구되고 있다. 이처럼 의료소비자들이 의료기관을 대하는 태도가 바뀌면서 의료기관들 역시 고객 만족도 향상을 위한 인식의 변화가 필요하다. 이에 본 연구는 의료서비스의 의식구조 변화를 지향하기 위해 PET/CT의 효용성에 대한 의학적 지식 유무에 따라 의료서비스만족도 차이를 분석하였다. 연구 결과 의료소비자 자신의 질병과 PET/CT의 효용성에 대한 의학적 지식이 있는 사람이 진료만족과 환경만족 모두 높게 나타났다. 따라서, 의료기관에서는 형식적인 의료서비스 제공이 아닌 의료소비자의 의학적 지식에 대한 지각 변화를 파악하여 의학 정보를 제공하는 실질적인 의료서비스 대응책이 필요할 것이다.

비대칭 위성 데이터 통신 시스템을 이용한 원격 진단 방사선 컨설팅 시스템 개발에 관한 연구 (A Study on the Development of Tele-Consulting System of Radiology using Asymetric Satellite Data Communication System)

  • In this paper, we present the Tele-consulting system of radiology, which uses the communication network as asymetric satellite data communication system. The asymetric satellite data communication system uses receive-only satellite links or data delivery and PSTN(Public Switched Telephone Network) modem or N-ISDN(Integrate Services Digital Network) for communication. The satellite communication linking shows the very high-speed performance than 28.8kbps modem linking. The satellite linking is 5 - 10 times aster than the modem linking. Consequently, we get the conclusion that our system is suitable or tele-radiology and telemedicine.

Utility of Noncontrast Magnetic Resonance Angiography for Aneurysm Follow-Up and Detection of Endoleaks after Endovascular Aortic Repair

  • Objective: To assess the noncontrast two-dimensional single-shot balanced turbo-field-echo magnetic resonance angiography (b-TFE MRA) features of the abdominal aortic aneurysm (AAA) status following endovascular aneurysm repair (EVAR) and evaluate to detect endoleaks (ELs). Materials and Methods: We examined four aortic stent-grafts in a phantom study to assess the degree of metallic artifacts. We enrolled 46 EVAR-treated patients with AAA and/or common iliac artery aneurysm who underwent both computed tomography angiography (CTA) and b-TFE MRA after EVAR. Vascular measurements on CTA and b-TFE MRA were compared, and signal intensity ratios (SIRs) of the aneurysmal sac were correlated with the size changes in the AAA after EVAR (AAA prognoses). Furthermore, we examined six feasible b-TFE MRA features for the assessment of ELs. Results: There were robust intermodality (r = 0.92-0.99) correlations and interobserver (intraclass correlation coefficient = 0.97-0.99) agreement. No significant differences were noted between SIRs and aneurysm prognoses. Moreover, "mottled high-intensity" and "creeping high-intensity with the low-band rim" were recognized as significant imaging findings suspicious for the presence of ELs (p < 0.001), whereas "no signal black spot" and "layered high-intensity area" were determined as significant for the absence of ELs (p < 0.03). Based on the two positive features, sensitivity, specificity, and accuracy for the detection of ELs were 77.3%, 91.7%, and 84.8%, respectively. Furthermore, the k values (0.40-0.88) displayed moderate-to-almost perfect agreement. Conclusion: Noncontrast MRA could be a promising imaging modality for ascertaining patient follow-up after EVAR.

영상치의학 분야에서의 원격판독 (Teleradiology in Oral and Maxillofacial Radiology)

  • Teleradiology is the electronic transmission of radiologic images from one location to another for the purpose of interpretation and/or consultation. Today, images can be easily transmitted to another location using the high-speed internet. In medical area, teleradiology has developed rapidly in recent 10 years due to the imbalance between the demand and availability of diagnostic services. However, teleradiology in dentistry is still inactive. I would like to overview the current teleradiology system, and discuss the necessity and the potentiality.

Tobacco Cessation in India: How Can Oral Health Professionals Contribute?

  • Tobacco use is described as the single most preventable cause of morbidity and mortality globally, with the World Bank predicting over 450 million tobacco-related deaths in the next fifty years. In India, the proportion of all deaths that can be attributed to tobacco use is expected to rise from 1.4% in 1990 to 13.3% in 2020 of which smoking alone will cause about 930,000 adult deaths by 2010. Many studies have shown that counseling from a health professional is an effective method of helping patients quit the tobacco habit. Tobacco cessation needs to be urgently expanded by training health professionals in providing routine clinical interventions, increasing availability and subsidies of pharmacotherapy, developing wide-reaching strategies such as quitlines, and costeffective strategies, including group interventions. The WHO Framework Convention on Tobacco Control (FCTC) emphasizes the vital contribution of participation of health professional bodies, as well as training and healthcare institutions in tobacco control efforts. Dentists can play an important role in helping patients quit using tobacco. One of the key strategies to reduce tobacco-related morbidity and mortality is to encourage the involvement of health professionals in tobacco-use prevention and cessation counselling. The dental office is an ideal setting for tobacco cessation services since preventive treatment services, oral screening, and patient education have always been a large part of the dental practice.

국제표준에 따른 10 MeV급 전자빔 조사시설의 흡수선량 품질보증에 관한 연구 (The Research Relating to QA of the Absorbed Dose in the 10 MeV E-beam Facility in Accordance with the International Standards)

  • 보건의료 분야에서 방사선은 의료기기 등의 멸균을 목적으로 빠르게 기존 방법을 대체하고 있으며 국제적으로, 정립된 엄격한 품질기준을 적용하고 있다. 방사선 멸균의 품질관리는 조사된 제품의 흡수선량이 요구 조건 및 기준에 부합하고 있음을 보증하는 것인데, $Co^{60}$ 동위원소를 이용하는 감마선 조사와는 달리 기계 전기적 방법에 따른 전자빔 조사는 더욱 많은 공정인자에 대해 기술적인 접근 방법이 필요하다. 국내에서는 2000년대 초반부터 전자빔 가속기의 보급이 시작되어 연구 및 산업분야에 이용되고 있으나 국제적 품질체계에 부합한 흡수선량의 품질에 관련된 연구는 매우 미흡한 실정이다. 서울방사선서비스는 2008년 10 MeV, 8 kW 사양의 대단위 전자빔 조사시설을 설치, 운영하기 시작하였는데, 전자빔 가속기, 제품운송장치, 안전장치, 기록관리 및 하위 구성장치가 통합시스템을 구성하여 우수제조기준에서 요구하는 공정품질 및 제품추적이 가능하도록 설계되었다. EN ISO11137로 대표되는 국제 표준의 이행을 위해서는 표준이 의도하는 바를 정확히 이해하고 장치의 설계기준부터 운영단계 별로 요구되는 품질시험을 정해진 절차 및 기준에 부합하도록 수행하여야 한다. 본 연구에 사용된 조사시설의 설계 시방을 제시하고 이를 구현하는데 필요한 핵심 장치의 설계 기준 및 특징에 대해 소개하였다. 또한 흡수선량 품질보증이라는 목적을 달성하기 위해 다양한 공정인자에 대한 품질시험결과를 제시하고 제시된 기준과 비교, 평가하였다.

우리나라 방사선사 과잉공급 해결방안 (Restructuring Enrollment Quotas of College for Radiological Technologists)

  • 본 논문에서는 보건의료인의 한 직종인 방사선사의 수요와 공급관련 문헌을 조사하여 방사선사를 양성하는 대학과 전문대학의 방사선(학)과의 입학정원 조정방안을 제시하였다. 오영호 등이 연구한 보건의료인력 중 장기 수급추계연구에서 방사선사는 2015년에 1,134명 ~ 2,052명의 공급과잉 현상이 전망되고, 2030년에는 12,632명 ~ 14,196명의 공급이 과잉될 것으로 전망되었다. 방사선사 공급과잉문제를 해결하기 위해서는 방사선사 교육기관의 정원 외 신입생을 5%이하로 모집하여 방사선사 응시자를 308명까지 감축하고, 3년제 방사선사 양성 교육기관을 4년제 교육기관으로 학제를 일원화하여 매년 방사선사 응시자 387명을 감축하고, 가칭 한국방사선사평가원을 설립하여 대학과 전문대학의 교육여건을 평가하고 최소한의 수준에 미달하는 대학의 방사선사 응시자격을 제한하여야 한다. 이를 위해서는 방사선사 관련단체뿐만 아니라 관련된 보건의료인 단체와의 협력이 필요하다.

SMC Full PACS로의 전환 및 System 구축

  • The purpose of this paper is to describe the transition of a 1,100 beds teritary hospital from 50% softcopy operation to full PACS operation. For the past 2 years, radiologists and clinicians have been using PACS to provide softcopy services to the outpatient clinics and inpatient wards of orthopedics surgery, neurosurgery and neurology as well as emergency room, surgical intensive care unit, medical intensive unit, pediatrics intensive care unit and neonatal intensive care unit. The examinations requested by these departments account for about 50% of hospital's radiological exams. In September 1996, we began the second phase of PACS implementation and installed additional workstations (102) in the remaining wards and clinics, interfaced to PACS additional imaging modalites, and increased the capacity of both the image server (256 Gbytes) and optical juke boxes (3 Tbytes). As of January 1997, we are in the final phase of moving away from conventional film system to full PACS operation.
