Standardized Ultrasound Evaluation for Active Surveillance of Low-Risk Thyroid Microcarcinoma in Adults: 2024 Korean Society of Thyroid Radiology Consensus Statement

  • Active surveillance (AS) has been widely adopted as an alternative to immediate surgery owing to the indolent nature and favorable outcomes of papillary thyroid microcarcinoma (PTMC). AS is generally recommended for tumors measuring ≤1 cm without aggressive cytological subtypes, risk of gross extrathyroidal extension (ETE), lymph node metastasis (LNM), or distant metastasis. AS requires careful patient selection based on various patient and tumor characteristics, and ultrasound (US) findings. Moreover, during AS, regular US is performed to monitor any signs of tumor progression, including tumor growth, new US features of potential gross ETE, and LNM. Therefore, appropriate imaging-based assessment plays a crucial role in determining whether AS or surgery should be pursued. However, detailed recommendations concerning US evaluation are currently insufficient, necessitating the formulation of this guideline. The Korean Society of Thyroid Radiology has developed a consensus statement for low-risk PTMC, covering US assessment methods when considering AS as a management option and conducting follow-up imaging tests during AS. This guideline aims to provide optimal scientific evidence and expert opinion consensus regarding a standardized US-based assessment protocol for low-risk PTMC.

Use of Magnetic Resonance Neurography for Evaluating the Distribution and Patterns of Chronic Inflammatory Demyelinating Polyneuropathy

  • Objective: To evaluate the distribution and characteristics of peripheral nerve abnormalities in chronic inflammatory demyelinating polyneuropathy (CIDP) using magnetic resonance neurography (MRN) and to examine the diagnostic efficiency. Materials and Methods: Thirty-one CIDP patients and 21 controls underwent MR scans. Three-dimensional sampling perfections with application-optimized contrasts using different flip-angle evolutions and T1-/T2- weighted turbo spin-echo sequences were performed for neurography of the brachial and lumbosacral (LS) plexus and cauda equina, respectively. Clinical data and scores of the inflammatory Rasch-built overall disability scale (I-RODS) in CIDP were obtained. Results: The bilateral extracranial vagus (n = 11), trigeminal (n = 12), and intercostal nerves (n = 10) were hypertrophic. Plexus hypertrophies were observed in the brachial plexus of 19 patients (61.3%) and in the LS plexus of 25 patients (80.6%). Patterns of hypertrophy included uniform hypertrophy (17 [54.8%] brachial plexuses and 21 [67.7%] LS plexuses), and multifocal fusiform hypertrophy (2 [6.5%] brachial plexuses and 4 [12.9%] LS plexuses) was present. Enlarged and/or contrast-enhanced cauda equina was found in 3 (9.7%) and 13 (41.9%) patients, respectively. Diameters of the brachial and LS nerve roots were significantly larger in CIDP than in controls (p < 0.001). The largest AUC was obtained for the L5 nerve. There were no significant differences in the course duration, I-RODS score, or diameter between patients with and without hypertrophy. Conclusion: MRN is useful for the assessment of distribution and characteristics of the peripheral nerves in CIDP. Compared to other regions, LS plexus neurography is more sensitive for CIDP.

방사선학과 전공 학생들의 법의학에 대한 인식과 교육 요구도 조사 (A Survey of Radiologic Science Students' Awareness and Educational Needs of Forensic Medicine)

  • 국내 경제 및 과학의 발전으로 인하여 국민의 생활 수준은 높아졌다. 그에 따라 생명의 질 향상을 원한다. 즉, 인권을 보장하고 인간으로서 존엄과 가치를 추구한다. 그러므로 의학계는 인간의 생명을 연장하며, 건강한 삶을 유지하기 위해 도움을 준다. 인권을 수호하는 사회의학은 법의학이다. 법의학은 법의방사선학 영상으로 사망자 식별 및 원인을 분석한다. 법의방사선학은 방사선사가 의료영상을 획득하여 제공하는 것이다. 그러므로 방사선사는 법의학 관련 교과목을 이수하여 전문성을 가져야 한다. 최근 의과대학 및 간호대학에서는 의과대학 및 간호대학에서는 법의학, 법의간호학 등 다양한 교과목을 개설하여 운영 중이다. 그러나 방사선학과는 법의학 관련 교과목을 개설한 학교가 단 하나뿐이다. 향후 방사선사 및 방사선학과의 발전과 모색을 위해서는 법의학 교육을 고려하여야 한다. 이를 위해 방사선학과 학부 및 대학원에 법의학 교육의 필요성과 요구도를 조사하였다. 방사선학과 재학생들의 법의학 인식도는 2.977점으로 나타났으나, 방사선사가 법의학 교육 필요성은 3.759점으로 높게 나타났다. 또한, 재학생들의 법의학 교과목 요구도는 84.1%로 높게 나타나 대다수가 개설 운영이 필요하다고 응답하였다. 본 연구는 방사선학과 학부 및 대학원 재학생들의 법의학 관련 교과목 요구도를 알 수 있었으며, 교육의 다양성과 전문성을 모색할 필요가 있다. 법의학 및 방사선학 발전에 기초자료로 활용되길 기대한다.

병원 웹사이트 방사선 관련 픽토그램 평가 (Evaluation of Pictograms Related to Radiology on the Website of Hospitals)

  • This study reviewed whether radiology-related pictograms listed on the website of hospitals deliver clearly, accurately and quickly the necessary information. TOPSIS was carried out to select the most fitting pictograms for the department of radiology, department of radiation oncology, and department of nuclear medicine. The finally selected pictogram for the department of radiology shows an image of the chest (lungs), while that for the department of radiation oncology shows the process of treatment using equipment. Using these pictograms as standards seem to be appropriate to a certain level. However, the pictogram for department of nuclear medicine has a symbol for a nucleus and is thus inappropriate to meet the standards for a hospital pictogram. This study is meaningful in that it has established evaluation methods and procedures relaetd to radiation pictograms.
