혈관 조영 검사 및 중재적 방사선 시술시 방사선량에 대한 참고 기준치 : 대뇌 중심으로 (Reference Levels for Radiation Dose in Angiography and Interventional Radiology : In the Cerebrum)

  • 혈관 조영 검사 및 중재적 방사선 시술을 통한 환자에게 주어지는 피폭선량에 대해 대뇌를 중심으로 환자 체형을 표준화하여 검사 및 시술 차이에 따른 투시시간, 면적 선량률(kerma area product rate)을 관측하고 참고 기준치를 비교하고자 하였다. 그 결과 질병 분류에 따른 면적 선량률은 지주막하출혈(subarachnoid hemorrhage)이 가장 높았고, 뇌동맥류가 가장 낮았다. 또한 검사 및 시술에 따른 면적 선량률은 양쪽 내경동맥, 양쪽 총경동맥 및 추골동맥으로 하는 방법이 가장 높았으며, guglielmi detachable coil(GDC)이 가장 낮게 나타났다. 따라서 이를 혈관 조영 검사 및 중재적 방사선 시술시 환자 피폭 선량에 대한 참고 기준치로 이용할 수 있을 것이며 향후 환자 피폭 선량 교육과 관리에 유용할 것이라 사료된다.

실시간 비대면 수업환경을 2년간 경험한 학생들의 만족도 조사 연구: 방사선전공학생들을 대상으로 (The Study on Satisfactory Rate with Students Which Experienced Non-face-to-face Online Class Environment for Two Years: For Radiology Majoring Students)

  • This study is a questionnaire about the lesson environment that radiation major students prefer in a non-face-to-face live online lesson environment for a total of 133 students, 65 second graders and 68 third graders who are enrolled in the department of radiology at a university located in the Seoul metropolitan area. And checked the satisfactory level by grade. The questionnaire consists of three categories: 1st real-time non-face-to-face lectures, 2nd professor lectures, and 3rd corona lectures. A total of 14 questions, with multiple choice and descriptive response methods. As an evaluation method, in the case of a multiple-choice question, the average was calculated using a 5-point Likert scale. As a result of conducting the independent sample T-test of the SPSS program, the response by grade was P > 0.05, and no significant result was shown by the contents of the questionnaire survey of the second grade. As for the lecture method of the department of radiology after the end of Covid-19 virus, it is better to promote face-to-face lessons in radiation training subjects and non-face-to-face real-time education in subjects centered on radiation theory.

연구 및 진료에서의 이해상충 (Conflicts of Interest in Research and Clinical Practice)

  • 최근 의사들이나 연구자들은 진료나 연구에서 독자적으로 수행하기보다는, 회사, 연구기관, 보건의료기관, 및 학술 저널의 관련인들과 관계를 맺게 된다, 혹은 그들이 관련 회사에서의 자문 및 주주 참여 등과 같은 다중적 이해관계(multiple interests)를 가지기도 한다. 그러한 이해관계가 연구자의 의사결정에 지대한 영향을 미치는 경우 이해상충이 발생할 수 있다. 이해상충은 연구 진실성을 침해하여 피험자나 환자를 위험에 빠뜨리기도 하고 공공에 대한 위험을 초래할 수 있고 과학에 대한 대중의 인식을 악화시킬 수도 있기에 매우 중요한 문제라고 할 수 있다. 이 종설에서는 이해상충의 개념과 구체적인 사례 및 이에 대한 해법을 살펴보고자 한다.

Interactive learning in oral and maxillofacial radiology

  • Purpose: The use of electronic tools in teaching is growing rapidly in all fields, and there are many options to choose from. We present one such platform, Learning Catalytics$^{TM}$ (LC) (Pearson, New York, NY, USA), which we utilized in our oral and maxillofacial radiology course for second-year dental students. Materials and Methods: The aim of our study was to assess the correlation between students' performance on course exams and self-assessment LC quizzes. The performance of 354 predoctoral dental students from 2 consecutive classes on the course exams and LC quizzes was assessed to identify correlations using the Spearman rank correlation test. The first class was given in-class LC quizzes that were graded for accuracy. The second class was given out-of-class quizzes that were treated as online self-assessment exercises. The grading in the self-assessment exercises was for participation only and not accuracy. All quizzes were scheduled 1-2 weeks before the course examinations. Results: A positive but weak correlation was found between the overall quiz scores and exam scores when the two classes were combined (P<0.0001). A positive but weak correlation was likewise found between students' performance on exams and on in-class LC quizzes (class of 2016) (P<0.0001) as well as on exams and online LC quizzes (class of 2017) (P<0.0001). Conclusion: It is not just the introduction of technological tools that impacts learning, but also their use in enabling an interactive learning environment. The LC platform provides an excellent technological tool for enhancing learning by improving bidirectional communication in a learning environment.

한국원자력연구시설을 이용한 방사선학과 대학생 인력양성 (Cultivation of University Students in Radiology Using Research Facilities at KAERI)

  • 본 연구는 연구용원자로(하나로)와 국가방사선연구시설을 이용한 전문교육과정을 개발 운영함으로써 방사선학 전공 대학생들에게 전문화된 교육 기회를 제공하고 현장적응 능력을 증진시킬 목적으로 수행되었다. 이를 위해 하나로와 국가방사선연구시설을 이용한 전문 교육과정을 개발하고 운영함으로써 대학생들에게 하나로 활용 기회를 제공하였고, 대학생을 대상으로 하는 방사선학과 실험 실습을 운영함으로써 차세대 전문가를 양성하였다. 방사선학과 대학생들에게 현장실험실습 기회를 제공함으로써 향후 방사선학 연구 분야의 잠재 이용자를 육성하여 방사선학 인력의 저변 확대를 하고자 하였다. 본 실험실습 과정을 위해 서술형 교재와 강의용 교재, 설문지를 개발하고 활용하였다. 2006년부터 2017년까지 방사선학과 대학생을 대상으로 운영된 실험실습교육과정에 714명이 수료하였다. 방사선학과 대학생들을 대상으로 개발 운영되는 교육과정은 하나로와 국가방사선연구시설의 이용을 더욱 활성화하여 궁극적으로 방사선 산업인력의 저변을 확대하고 올바른 정책 결정 및 기술기반을 확립하는 주요 자원이 될 것이며 방사선 분야의 활성화에도 기여하게 될 것이다.

An Open Medical Platform to Share Source Code and Various Pre-Trained Weights for Models to Use in Deep Learning Research

  • Deep learning-based applications have great potential to enhance the quality of medical services. The power of deep learning depends on open databases and innovation. Radiologists can act as important mediators between deep learning and medicine by simultaneously playing pioneering and gatekeeping roles. The application of deep learning technology in medicine is sometimes restricted by ethical or legal issues, including patient privacy and confidentiality, data ownership, and limitations in patient agreement. In this paper, we present an open platform, MI2RLNet, for sharing source code and various pre-trained weights for models to use in downstream tasks, including education, application, and transfer learning, to encourage deep learning research in radiology. In addition, we describe how to use this open platform in the GitHub environment. Our source code and models may contribute to further deep learning research in radiology, which may facilitate applications in medicine and healthcare, especially in medical imaging, in the near future. Suggestion for a New Grading Scale for Radiation Induced Pneumonitis Based on Radiological Findings of Computerized Tomography: Correlation with Clinical and Radiotherapeutic Parameters in Lung Cancer Patients

  • Background: The objective of this research is the computed axial tomography (CT) imaging grading of radiation induced pneumonitis (RP) and its correlation with clinical and radiotherapeutic parameters. Materials and Methods: The chest CT films of 20 patients with non-small cell lung cancer who have undergone threedimensional conformal radiation therapy were reviewed. The proposed CT grading of RP is supported on solely radiological diagnosis criteria and distinguishes five grades. The manifestation of RP was also correlated with any positive pre-existing chronic obstructive pulmonary disease (COPD) history, smoking history, the FEV1 value, and the dosimetric variable V20. Results: The CT grading of RP was as follows: 3 patients (15%) presented with ground glass opacity (grade 1), 9 patients (45%) were classified as grade 2, 7 patients (35%) presented with focal consolidation, with or without elements of fibrosis (grade 3), and only one patient (5%) presented with opacity with accompanying atelectasis and loss of pulmonary volume (grade 4). Both univariate and multivariate analysis revealed as prognostic factors for the radiological grading of RP the reduction of FEV1 and the V20 (P=0.026 and P=0.003, respectively). There was also a significant (P<0.001) correlation of radiological grading of RP with FEV1 and V20 (spearman rho 0.92 and 0.93, respectively). Conclusions: The high correlation of the proposed radiological grading with the FEV1 and the V20 is giving a satisfactory clinical validity. Although the proposed grading scale seems relevant to clinical practice, further studies are needed for the confirmation of its validity and reliability.

  • 현대사회의 IT 정보통신 인터넷 관련 산업은 눈부신 발전을 거듭하여 초고속 인터넷 보급률이 세계 1위를 차지하고 있으며, 이동통신의 종주국을 자랑이라도 하듯이 단말기의 관련기술 및 생산도 세계의 상위를 차지하고 있다. 이처럼 IT 분야는 하루가 다르게 빠르게 변화하고 있으나, 방사선학과의 학부로서 정보통신 관련교육은 이에 부응하지 못하는 실정이다. 학부로서의 인격 및 지식교육과 방사선사로서의 기술 및 직업교육의 교과 과정은 현재의 IT 환경과 실제 임상의 디지털 관련 장비의 인터페이스에 적응하기는 다소 힘들다. 특히 의료 디지털 환경 및 병원의 정보화로 방사선과에서의 방사선사의 역할이 증대되고 있는 상황에서의 정보 통신영역의 교육과정의 개선 및 새로운 과정의 제안은 디지털 사회의 변화에 적응할 뿐만 아니라 방사선사의 자질 및 위상을 높일 수 있는 계기로 생각된다. 따라서 본 논문에서는 디지털 방사선 환경에서의 방사선학과의 교육과정에 대한 각 대학 및 대학교별 현황과 문제점을 제시하며, 그 현황을 바탕으로 새로운 교육과정의 도입을 위한 개선 방향을 제안하고자 한다.

Determination of Scattered Radiation to the Thyroid Gland in Dental Cone Beam Computed Tomography

  • Background: Cone beam computed tomography (CBCT) is a specialized medical equipment and plays a significant role in the diagnosis of oral and maxillofacial diseases and abnormalities; however, it is attributed to risk of exposure of ionizing radiation. The aim of the study was to estimate and determine the amount of scattered radiation dose to the thyroid gland in dental CBCT during maxilla and mandible scan. Materials and Methods: The average scattered radiation dose for i-CAT 17-19 Platinum CBCT (Imaging Sciences International) was measured using a Multi-O-Meter (Unfors Instruments), placed at the patient's neck on the skin surface of the thyroid cartilage, with an exposure parameter of 120 kVp and 37.07 mAs. The surface entrance dose was noted using the Multi-O-Meter, which was placed at the time of the scan at the level of the thyroid gland on the anterior surface of the neck. Results and Discussion: The surface entrance dose to the thyroid from both jaws scans was 191.491±78.486 µGy for 0.25 mm voxel and 26.9 seconds, and 153.670±74.041 µGy from the mandible scan, whereas from the maxilla scan the surface entrance dose was 5.259±10.691 µGy. Conclusion: The surface entrance doses to the thyroid gland from imaging of both the jaws, and also from imaging of the maxilla and mandible alone were within the threshold limit. The surface entrance dose and effective dose in CBCT were dependent on the exposure parameters (kVp and mAs), scan length, and field of view. To further reduce the radiation dose, care should be taken in selecting an appropriate protocol as well as the provision of providing shielding to the thyroid gland.

Morphometric evaluation of great vein of Galen and its clinical implications

  • The Galenic venous system plays a vital role in the drainage of blood from deeper parts of the brain. This venous system is contributed by many major veins. These veins are located closer to the pineal gland making the surgical approach in this region difficult. Any accidental injury or occlusion of the vein of Galen could lead to devasting results. Thus, studying the dimensions of the vein of Galen is more important. Hence, we aimed to evaluate the morphometry and trajectory to the vein of Galen. About 100 computed tomographic venography records were evaluated and the length, diameter of vein of Galen, angle between straight sinus and vein of Galen and distance from internal occipital protuberance and roof of fourth ventricle to vein of Galen were studied. The mean length and diameter of vein of Galen were 9.8±2.7 and 4.08±1.04 respectively. The mean angle between straight sinus and vein of Galen was 64.2°. The mean distance between external occipital protuberance and roof of fourth ventricle to vein of Galen were 52±6.9 and 33.3±4.5 respectively. No significant morphometric differences were observed between the age groups as well as between the sexs. The results obtained from this study may be helpful for the neurosurgeons in better understanding of the anatomy of the Galenic venous system and to adopt a safe surgical approach to improve the efficacy of the surgeries of the pineal gland and also in the region of vein of Galen.
