일반촬영 검사 시 검사안내 유인물의 유용성에 관한 고찰 (A Study on the Usefulness of an Examination Information Printout during Ordinary X-Ray Examinations)

  • In a normal x-ray examination, the cooperation of a patient in positioning greatly influences the improvement of work performance, the quality of the images and the reduction in time spent on the examination. An attempt was made to discover the connection between an examination information printout and the effects of the examination by informing the patient before the examination on the examination positions in order to increase the level of cooperation by the patient and by surveying the degree of satisfaction of the patient during the examination proceedings. An examination information printout was prepared on five positions that patients consider to be difficult while receiving normal x-ray examinations and were shown to 257 male and female patients between the ages of 30 to 79 who had visited this hospital between January 1 to February 29, 2012 or to a person who had accompanied the patients before the examination to research the degree of satisfaction regarding the printout. In addition, the examination explanation printout was given to Radiologic Technologists with over 1 year experience in this hospital to use while taking the x-rays and the degree of satisfaction was surveyed and it was examined if the increase of the degree of satisfaction had an effect on the reduction in the examination time. The patients who utilized the examination information printout revealed results of approximately a 7.8% higher degree of satisfaction among the patients who had previous experience receiving x-ray examinations compared to those were having an x-ray for the first time. In the age groups between 30 to 70 years of age, the age group in the 30's revealed the lowest level of satisfaction at approximately 69% and the group in the 50's showed the highest degree of satisfaction of 87.94%. In the survey of the Radiologic Technologists, 85% responded that they were satisfied. In the question that stated "Will the examination information printouts reduce the time involved in unnecessary conversations with the patients?," the responses showed a 95% degree of satisfaction. The degree of satisfaction was high over the examination information printout by both the patients and the Radiologic Technologists. Although the effects of examination time reduction varied based on the ages of the patients and physical conditions that they were in and could not be given measurement values, it was able to be seen through the survey that reduction of the unnecessary communication between the patient and the Radiologic Technologists contributed to the increase in the degree of satisfaction. As a result, both the patients and the Radiologic Technologists had an increased degree of satisfaction over the examination information printout and it was possible for improvements to be made in the quality of medical service provided as well. Nevertheless, it is considered that a more developed manual in terms of quality and quantity must be produced using a more systematic approach and design.

전산화단층촬영의 소요시간 분석에 기반한 방사선사의 적정인력 산정에 관한 연구 (Estimation of Appropriate Number of Radiologic Technologist Based on Analysis of Time Required for Computed Tomography)

  • Although the number of computed tomography(CT) is increasing every year, it is insufficient to establish appropriate workload calculation standards of radiologic technologist to provide optimal medical services to patients, such as patient safety management and infection management. The purpose of this study is to present guidelines for calculating the appropriate workload of radiologic technologist by analyzing the work flow of CT procedures and the time required for CT examination in major hospitals. As for the study subjects and methods, the appropriate process for each step of CT examination was investigated to systematically present the process and time required for the actual examination, and the CT procedure time of 104,105 adult patients and 465 pediatric patients under the age of 6 were analyzed. For the time required, data according to the use of contrast medium, procedure type, and adult/child were collected and compared. The test time of CT examination using contrast medium took about 13 minutes when one radiologic technologist worked and about 9 minutes when two radiologic technologists worked. The time required for the procedures were statistically significant depending on the presence or absence of contrast medium, multi-phase procedure, and patient age (considering pediatric patients). As a result, in order to thoroughly perform patient safety and infection management, the appropriate workload increased by about 40% when there were two radiologic technologists. The limit workload was an average of 32 people per day with one radiologic technologist per 15 minutes, and 48 people per day with two radiologic technologist per 10 minutes. This is a marginal workload, and in the case of procedures that require more time to acquire radiographic images, the interval between reservations should be widened.

Reconstruction of Radiation Dose Received by Diagnostic Radiologic Technologists in Korea

  • Objectives: Diagnostic medical radiation workers in Korea have been officially monitored for their occupational radiation doses since 1996. The purpose of this study was to design models for reconstructing unknown individual radiation doses to which diagnostic radiation technologists were exposed before 1996. Methods: Radiation dose reconstruction models were developed by using cross-sectional survey data and the personal badge doses of 8167 radiologic technologists. The models included calendar year and age as predictors, and the participants were grouped into six categories according to their sex and facility type. The annual doses between 1971 and 1995 for those who were employed before 1996 were estimated using these models. Results: The calendar year and age were inversely related to the estimated radiation doses in the models of all six groups. The annual median estimated doses decreased from 9.45 mSv in 1971 to 1.26 mSv in 1995, and the associated dose variation also decreased with time. The estimated median badge doses from 1996 (1.22 mSv) to 2011 (0.30 mSv) were similar to the measured doses (1.68 mSv to 0.21 mSv) for the same years. Similar results were observed for all six groups. Conclusions: The reconstruction models developed in this study may be useful for estimating historical occupational radiation doses received by medical radiologic technologists in Korea.

우리나라 방사선사 과잉공급 해결방안 (Restructuring Enrollment Quotas of College for Radiological Technologists)

  • 본 논문에서는 보건의료인의 한 직종인 방사선사의 수요와 공급관련 문헌을 조사하여 방사선사를 양성하는 대학과 전문대학의 방사선(학)과의 입학정원 조정방안을 제시하였다. 오영호 등이 연구한 보건의료인력 중 장기 수급추계연구에서 방사선사는 2015년에 1,134명 ~ 2,052명의 공급과잉 현상이 전망되고, 2030년에는 12,632명 ~ 14,196명의 공급이 과잉될 것으로 전망되었다. 방사선사 공급과잉문제를 해결하기 위해서는 방사선사 교육기관의 정원 외 신입생을 5%이하로 모집하여 방사선사 응시자를 308명까지 감축하고, 3년제 방사선사 양성 교육기관을 4년제 교육기관으로 학제를 일원화하여 매년 방사선사 응시자 387명을 감축하고, 가칭 한국방사선사평가원을 설립하여 대학과 전문대학의 교육여건을 평가하고 최소한의 수준에 미달하는 대학의 방사선사 응시자격을 제한하여야 한다. 이를 위해서는 방사선사 관련단체뿐만 아니라 관련된 보건의료인 단체와의 협력이 필요하다.

방사선사의 직무요구와 소진간의 관계에서 감성지능의 역할 (Moderating Effect of Emotional Intelligence on the Relationships Between Job Demand and Burn-out of Radiologic Technologists)

  • 본 연구의 목적은 병원에서 근무하고 있는 방사선사의 직무요구와 소진간의 관계에서 감성지능의 조절효과를 파악하는데 있다. 본 연구의 결과를 요약하면 첫째, 둘째, 방사선사의 직무요구(역할갈등, 인지요구, 업무량)가 소진(감정고갈, 개인성취감저하, 비인격화)에 유의한 영향을 미치는 변수로 나타났다. 인지요구와 업무량이 감정고갈에 유의한 영향을 미쳤으며, 역할갈등과 업무량은 개인성취감 저하에 유의한 영향을 미쳤다. 그리고 역할갈등과 인지 요구는 비인격화에 유의한 영향을 미치는 것으로 나타났다. 셋째, 직무요구(역할갈등, 인지요구, 업무량)와 소진(감정고갈, 개인성취감저하, 비인격화)과의 관계에서 감성지능의 조절효과를 검증하였는데, 검증결과 인지요구와 개인성취감저하 관계에서 감성지능의 조절효과가 있었다. 그리고 역할갈등과 비인격화 관계에서 감성지능의 조절효과가 있었고, 인지요구와 비인격화 관계에서 감성지능의 조절효과가 있는 것으로 나타났다.

일부 방사선과 재학생의 방사선사 이미지 결정에 대한 조사 (Investigation about Decisions of Radiological Technologist Image of Some Students Majoring inradiology)

  • The objectives of this study were to determine radiologic technologist image of some of the majored in department of radiology and collected data of 196 students were analyzed. The results obtained from this study are as follows 1. Department of radiology selective aspect appears the many have a positive opinion but now department satisfaction dropped 2. Department of radiology educational aspect appears the important things of clinical training programs and the national exam special lectures are high 3. Radiologic technologist's futural aspect appears high the many hope to get the university hospital for job plan after graduation and have a positive opinion 4. Radiologic technologist's professional aspect appears high the many think high skilled workers as the essential prerequisites of the radiologic technologist.

비공학도를 위한 X-ray 영상촬영 시스템 해상력 평가 방법 (An Evaluation Method of X-ray Imaging System Resolution for Non-Engineers)

  • Digital Radiography(DR) 시스템은 임상현장에서 아날로그 시스템을 대체하고 널리 이용되고 있다. DR을 이용하여 얻어진 X선 영상의 해상력을 결정짓는 요소에는 이용되는 검출기의 고유 해상력, 피사체의 대조도 및 특성, X선 선질, X선원의 산란, DR 검출기의 성능, X선 변환효율 및 초점의 크기, 피사체의 움직임 등이 있다. DR 검출기를 구성하는 요소에는 X선 포획 요소, 커플링 요소, 정보수집 요소가 있는데 이들은 시스템의 성능에 영향을 미치며, 그 성능은 해상력으로 평가된다. 의료영상 시스템의 해상력은 촬영대상물의 조직 간의 해부학적 영상을 구분하는 능력을 나타낸다. 해상력 평가를 위해 Modulation Transfer Function(MTF)이 보편적으로 이용되고, MTF는 입력 공간주파수 성분에 대한 출력 공간주파수 성분의 비를 나타내는데, 수학적으로 MTF는 Point Spread Function(PSF) 입력에 대한 시스템의 주파수 응답이며 Edge Phantom을 이용한 결과 영상에서 추출된 Line Spread Function(LSF)을 Fourier Transform하면 얻을 수 있다. 일반적으로 임상현장에서 의료영상시스템의 이용 및 관리의 책임은 방사선사가 맡고 있지만, MTF를 측정하기 위해서는 공학적, 수학적 기초 및 C, Fortran, Matlab등의 프로그램 작성 능력이 필요하기 때문에 비 공학도는 정확한 측정이 불가능하다. 의료영상 시스템의 성능 관리 및 최상의 상태를 유지하기 위해 시스템의 성능평가가 이뤄져야 하는데, 이를 위해 본 연구에서는 비공학도가 해상력 성능평가를 할수 있도록 ImageJ 및 Excel을 이용하여 해상력 평가를 할 수 있도록 방법을 제시하고, 제안된 방법을 이용해 계산된 결과와 프로그래밍을 이용해 계산된 결과의 비교를 통해 본 논문에서 제시하는 방법의 유용성을 확인하였다.

방사선사의 업무소진과 업무자율성이 이직의사에 미치는 연구 (A study on the impact of Professional burnout and job autonomy on turnover intention in radiologic technologists)

  • 본 연구의 목적은 병원에서 근무하고 있는 방사선사의 업무소진과 업무자율성이 이직의사에 미치는 영향에 대해 분석하였다. 본 연구의 결과를 요약하면, 방사선사의 업무소진과 업무자율성 및 이직의사의 평균값과 표준편차는 전체적인 업무소진은 3.15점을 보여 방사선사의 업무소진은 보통수준 이상인 것으로 나타났으며, 업무자율성에서는 2.88점으로 업무에 대한 자율성이 보통수준에 미치지 못하는 것으로, 이직의사에서는 3.30점으로 이직하려는 경향이 높은 쪽으로 나타났으며, 이직의사에 영향을 미치는 요인 중에서 정서적 소진, 일에 대한 흥미상실, 일에 대한 열의상실이 유의한 영향을 미치는 것으로 나타났다. 방사선사가 방사선검사와 관련하여 자신의 전문지식과 판단을 근거로 방사선 업무를 수행하고, 업무상 의사결정을 충분히 가질 수 있으면서, 적극적으로 참여할 수 있는 환경이 조성되어진다면, 방사선사의 업무소진은 최소화 할 수 있으며, 업무의 자율성을 강화하기 위한 프로그램을 개발하여 적용하게 될 때, 이직의사를 줄이면서 방사선 인력관리 및 방사선사의 질적인 면을 향상시켜, 병원 조직 내에서 방사선사의 위상을 높일 수 있을 것으로 사료된다.
