A Case Report of IgG4-Related Disease Clinically Mimicking Pleural Mesothelioma

  • An immunoglobulin G4 (IgG4)-related disease is a recently emerging entity, and a few cases of IgG4-related disease in lung and pleura have been reported. Herein, we report the case of a 74-year-old man with IgG4-related disease of lung and pleura, clinically suspicious of malignant mesothelioma. Chest computed tomography showed diffuse nodular pleural thickening, and microscopic finding disclosed diffuse thickening of visceral pleura with infiltrations of many lymphoplasma cells with increased number of IgG4-positive plasma cells and a few multinucleated giant cells. It is important for pathologists and clinicians to recognize this rare entity and its histologic finding, because it can be confused with malignant tumors on the radiologic examination although it can be treated with steroid therapy.

  • Pelioid hepatocellular carcinoma(HCC), a type of atypical HCC, is a rare histologic type of HCC. The radiologic findings of the pelioid HCC is differ from the typical type of HCC. To our knowledge, this case report is the second literature to show the enhancing features of a pelioid HCC on dynamic computed tomography (CT).Here we describe the dynamic CT findings in a case of surgically confirmed pelioid HCC.

Pulmonary Thromboembolectomy of Chronic Pulmonary Thromboembolism (만성 폐전색증의 전색 제거술 치험 -1례 보고-)

  • Pulmonary thromboembolism originated most commonly from the venous thrombus, especially deep vein thrombus in the leg, which migrated to and occluded the pulmonary vasculatures. The failure of clot lysis and repeated embolic episodes resulted in the hemodynamic compromise -that is- in the increasing in the pulmonary vascular resistance, which would cause the right ventricle failure[Car Pulmonale]. Under the cardiopulmonary bypass, 20 year old male patient was treated successfully by thromboembolectomy of pulmonary thromboembolism with pulmonary hypertension, which originated from the deep vein thrombus in the leg. The results of radiologic studies and clinical evaluations were excellent in that the postoperative lung perfusion scan showed the newly increased perfusion of post-embolectomy territories and in the arterial blood gas finding of 76 from 66[mmHg] in PaO2. The patient was uneventful and discharged on postop. $ 14 days with anticoagulant continued.

  • Giant cell reparative granuloma(GCRG) is a non-neoplastic rapidly expanding and locally destructive tumor that occurs almost exclusively within the mandible and maxilla. A 58-year-old man, complained of a mass on the left infra-auricular area starting 2 months ago. The radiologic finding suggests a mass that originate from mandible, pathology diagnosed the lesion as a giant cell reparative granuloma. The tumor was surgically excised without complications. We report the case with a review of literature.

  • Non-Hodgkin's Lymphoma(NHL) is a malignant tumor that is derived from the lymphatic system. The most common symptoms of NHL are painless lymph node enlargement. However, we should not diagnose NHL by only fragmentary clinical symptom and radiologic finding because of the various lymphoma characters. We have treated a patient with such preoperative findings of Branchial cleft cyst. However, the pathologic diagnosis of the surgical specimen was diffuse large b-cell lymphoma.

  • Bronchioloaveolar cell carcinoma is an uncommon primary lung cancer and may exhibit various pathologic, radiologic, and clinical presentations. We experienced a case of Bronchioloalveolar cell carcinoma with Air-bronchogram in chest CT scans. The features of Bronchioloalveolar cell carcinoma on CT scans have not been extensively described.

  • We investigated whether poly-L-histidine (PLH) significantly affect mucin release from cultured airway epithelial cells. Confluent primary hamster tracheal surface epithelial (HTSE) cells were metabolically radiolabeled with $^$^3H$$glucosamine for 24 hr and chased for 30 min in the presence of PLH to assess the effect on $^3H$-mucin release. PLH 9,850 and PLH 6,700 specifically inhibit mucin release from airway goblet cells without significant cytotoxicity. This finding suggests that poly-L-histidine might function as an airway mucoregulative agent.

  • We report 4 cases of malignant thymoma which were composed of 2 cases of invasive thymoma and 2 cases of thymic carcinoma. The cytologic findings of invasive thymoma were similar to those of benign thymoma. The distinctive cytologic features of thymic carcinoma were necrotic background, irregular clusters and individually scattered arrangement of anaplastic epithelial cells, and some scattered mature small lymphocytes. These findings may be found in the Hodgkin's lymphoma, seminoma, and metastatic squamous ceil carcinoma, undifferentiated carcinoma, and large ceil carcinoma of the lung. But, the feature of irregular clustering of anaplastic epithelial cell haying scanty cytoplasm was different from Hodgkin's lymphoma and seminoma. Clinical and radiologic findings as well as cytologic finding were helpful in differential diagnosis of thymic carcinoma from metastatic carcinoma.

  • Discal cyst is a very rare lesion that can result in refractory low back pain and leg radiating pain. Because they are so uncommon, their exact origin and pathophysiology are still unknown. A 31-year-old man visited our institute due to low back pain and severe left leg radiating pain. Magnetic resonance images [MRI] revealed spherically shaped extradural cystic lesion at L2-L3 level. Computed tomography [CT] discography demonstrated obvious communication between the intervertebral disc and the cyst. The patient underwent posterior decompression and excision of cyst. The symptoms were remarkably improved immediately after surgery.

Small Solitary Ground-Glass Nodule on CT as an Initial Manifestation of Coronavirus Disease 2019 (COVID-19) Pneumonia

  • The 2019 novel coronavirus (2019-nCoV) outbreak in Wuhan, Hubei Province, China in 2019 led to large numbers of people being infected and developing atypical pneumonia (coronavirus disease 2019, COVID-19). Typical imaging manifestations of patients infected with 2019-nCoV has been reported, but we encountered an atypical radiological manifestation on baseline computed tomography (CT) images in three patients from Wuhan, China infected with the 2019-nCoV. Surprisingly, the only similar CT finding was a solitary sub-centimeter ground-glass nodule adjacent to bronchovascular bundles, which could be easily overlooked. In addition, the follow-up images in these patients showed how COVID-19 pneumonia evolved from these small nodules. The radiologic manifestation of the three cases will expand contemporary understanding of COVID-19.
