The Prognostic Factors Related to Traumatic Brain Stem Injury

  • Objective : This study was conducted to assess the clinical significance of traumatic brain stem injury (TBSI) reflected on Glasgow Coma Score (GCS) and Glasgow Outcome Score (GOS) by various clinical variables. Methods : A total of 136 TBSI patients were selected out of 2695 head-injured patients. All initial computerized tomography and/or magnetic resonance imaging studies were retrospectively analyzed according to demographic- and injury variables which result in GCS and GOS. Results : In univariate analysis, mode of injury showed a significant effect on combined injury (p<0.001), as were the cases with skull fracture on radiologic finding (p<0.000). The GCS showed a various correlation with radiologic finding (p<0.000), mode of injury (p<0.002), but less favorably with impact site (p<0.052), age (p<0.054) and skull fracture (p<0.057), in order of statistical significances. However, only GOS showed a definite correlation to radiologic finding (p<0.000). In multivariate analysis, the individual variables to enhance an unfavorable effect on GCS were radiologic finding [odds ratio (OR) 7.327, 95% confidence interval (CI)], mode of injury (OR; 4.499, 95% CI) and age (OR; 3.141, 95% CI). Those which influence an unfavorable effect on GOS were radiologic finding (OR; 25.420, 95% CI) and age (OR; 2.674, 95% CI). Conclusion : In evaluation of TBSI on outcome, the variables such as radiological finding, mode of injury, and age were revealed as three important ones to have an unfavorable effect on early stage outcome expressed as GCS. However, mode of injury was shown not to have an unfavorable effect on late stage outcome as GOS. Among all unfavorable variables, radiological finding was confirmed as the only powerful prognostic variable both on GCS and GOS.

  • Relapsing polychondritis is an uncommon disorder of unknown etiology. That involves multisystem characterized by recurrent inflammation, degenerative change of multiple cartilaginous structures. The most common clinical manifestation is an auricular chondritis. Other involving systems are joint, nose, eye, larynx, trachea, aorta, heart and kidney. A 26-year-old man who complained of hoarseness and sore throat was referred to the ENT department from internal medicine department. On initial video-laryngoscopic and radiologic examination, there was no abnormal finding. Treatment with high doses of corticosteroid and methotrexate was not effective. Symptoms were aggrevated with dyspnea. Three months later, on video-laryngoscopic examination, bilateral cord fixation with swelling was noted. The radiologic finding showed subglottic stenosis due to collapse of anterior wall of cricoid cartilage. The condition was managed with tracheotomy. We report a case of relapsing polychondritis involving larynx and trachea with bilateral vocal cord palsy which was managed with tracheotomy.

Imaging diagnosis of clonorchiasis

  • Among several diagnostic tools for clonorchiasis (Clonorchis sinensis infection), radiologic examinations are commonly used in clinical practices. During the 2 past decades, many reports regarding imaging findings of clonorchiasis were introduced. The basic imaging finding of clonorchiasis is diffuse dilatation of the peripheral intra-hepatic bile ducts, without dilation of the large intrahepatic or extrahepatic bile ducts. By this finding, however, active clonorchiasis cannot be differentiated from cured infection. Some recent radiologic studies suggested specific findings of active clonorchiasis. Besides direct demonstration of worms, increased periductal echogenicity on sonography and periductal enhancement on dynamic contrast-enhanced CT or MR imaging possibly represent active clonorchiasis. Those images of the liver clonorchiasis are known to be correlated with worm burdens (EPG counts) in their frequency and also severity. The images of cholangiocarcinoma associated with clonorchiasis show both the tumor with obstruction images and diffuse dilatation of the peripheral intrahepatic bile ducts. Radiological images can be a good practical alternative diagnostic method of clonorchiasis.

  • 우측 암면부, 특히 안구주위에 심한 통증을 호소하고 안검하수와 더불어 우측안구의 완전마비를 동반한 선천성 난장이 환자를 경험하였던 바 이는 신경방사선과학적소견, 검사실 검사소견, 전신적 corticosteroid치료에 대한 반응이 전형적인 Tolosa-Hunt Syndrome과는 다른 비전형적인 Tolosa-Hunt Syndrome이었기에 문헌적 고찰과 함께 보고하는 바이다.

  • Disorders related to the cervical spine can present a variety of signs and symptoms because of the many tissues and systems associated with this region. Therefore the challenge for the clinician is to organize the information from examination into a treatment plan that addresses the potential influences of the problem. the examination process should gather pertinent information from the patients, relating the pain pattern resulting loss of function. Our patients' X-rays showed absence of any abnormalities except loss of curvature. We therefore concluded that the main cause of posterior neck and shoulder pain was caused by muscle spasm, not cervical disc herniation not degenerative change.

  • Bone Mineral Densitometry (BMD) values is determined numerically with T or Z-score while the image is not considered in the interpretation. Abnormal finding was observed in the left femoral bone density image of a 64-year-old female patient who underwent a left kidney transplantation. This was due to the ethiodized oil used for embolization in the treatment of lymphatic leakage. Radiologic technologists should not only monitor changes of BMD values, but keep in mind to assess the changes in the image to provide accurate information to the patient.

폐의 원발성 비호지킨림프종의 임상상 (Clinical Feature of Primary Pulmonary Non-Hodgkin's Lymphoma)

  • Background: Primary non-Hodgkin's lymphoma of the lung is a rare entity. It is represented commonly as marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue (MALT) type. Although there have been a few reviews of this lymphoma, clinical features, radiologic findings, management and prognosis have not been well defined. Methods: We reviewed the medical records of 24 patients with primary pulmonary lymphoma between January 1995 and September 2008; all diagnoses had been confirmed based on pathology. Results: The median follow-up time was 42.3 months (range, 0.1~131.2 months). Five (20.8%) patients were asymptomatic, 17 (70.8%) patients had pulmonary symptoms, and the remaining 2 (8.3%) patients presented with constitutional symptoms. There were 16 (66.7%) patients with MALT lymphoma, 4 (16.7%) patients with diffuse large B-cell lymphoma and 4 (16.7%) patients with lymphoma that had not received a WHO classification. Radiologic findings of primary pulmonary lymphoma were diverse and multiple nodule or consolidation was the most common finding regardless of pathologic lymphoma type. PET scan was carried out in 13 (54.2%) patients and all lesions showed notable FDG uptake. MALT lymphoma showed a trend of better prognosis (3-year survival, 78.8% vs. 70.0%; 5-year survival, 78.8% vs. 52.5%; p=0.310) than non-MALT lymphoma. Conclusion: Primary non-Hodgkin's lymphoma of the lung occurs with nonspecific clinical features and radiologic findings. MALT lymphoma is the most common pathologic type of primary pulmonary lymphoma. This entity of lymphoma appears to have a good prognosis and in this study, there was a trend of better outcome than non-MALT lymphoma.

  • There are several respiratory diseases that show chronic granulomatous inflammation for the histologic finding. Among them, sarcoidosis and tuberculosis are not easy to differentiate when the clinical and radiological features present similar patterns. The increasing incidence of nontuberculous mycobacteria pulmonary disease is making it more difficult for clinicians to arrive at a proper diagnosis. A 69 year old male patient visited our hospital with chronic cough as his chief compliant. His radiologic findings were multiple enlarged mediastinal lymphadenpathies with innumerable micronodules and multiple patch infiltrations. The spleen biopsy finding showed chronic granulomatous inflammation, and Mycobacterium avium was identified on the bronchoscopic culture. Because of these findings, we treated him with drugs for nontuberculous mycobacteria disease other than sarcoidosis. However, during the treatment, his symptoms and radiological features became aggravated. Thus, we reviewed the radiologic and pathologic findings and decided to treat him with steroid, which relieved his symptoms and improved the radiologic findings. We report here on a case of sarcoidosis that was initially misdiagnosed as nontuberculous mycobacteria pulmonary disease.

  • 골반문증은 장관골의 골단 및 골간단에 다발성의 경화성 병변이 발생하는 매우 드문 골 이형성 질환으로 아직 원인이 명확히 알려지지 않았다. 일반적으로 상염색체 우성으로 유전되며 대부분 무증상으로 단순방사선학적 검사에 의해 우연히 발견되는 경우가 대부분이다. 저자들이 경험한 증례는 전형적인 골반문증의 방사선학적 소견 및 임상적 특징을 보이고 있었고, 감별진단을 위하여, 조직검사를 시행하였다.

  • Carcinoma ex pleomorphic adenoma is rare, aggressive, poorly understood malignancy that occurs in the salivary glands. The patient was 70-year-old male presenting ovoid elevated submandibular mass that has been present for 30 years and rapid growing for 6 months. The initial cytologic finding by fine needle aspiration biopsy showed highly malignant cells and the radiologic finding revealed a $3.6{\times}3.7{\times}4.8cm$ sized mass with cervical lymphadenopathy. After wide excision of submandibular gland and modified radical neck dissection, the histologic examination revealed that tumor was composed partly of a benign pleomorphic adenoma and partly of an adenocarcinoma component with areas of calcification and frequent angiolymphatic invasion. We present a rare case of adenocarcinoma ex pleomorphic adenoma with tumor thrombus in the submandibular gland.
