종격동섬유종증의 방사선학적 소견 (Radiologic Findings of Mediastinal Fibromatosis)

  • The fibromatosis is a rare tumorous with local invasion, but is not metastasized distantly. This term should not be applied to nonspecific reactive fibrous proliferations that are part of an inflammatory process of are secondary to injury of hemorrhage and have no tendency toward growth or recurrence. It arises principally from the connnective tissue of muscle and overlying fascia or aponeurosis(musculaponeurotic fibromatosis), and chiefly affects the muscle of shoulder, pelvic girdle, and extremity. The term 'aggressive fibromatosis' is also employed to describe this disease, but it is impossible to predict the clinical course in the individual case. The fibromatosis arising in the mediastinum is very rare, and the report about it is nearly absent. The plain radiography shows merely mass with soft tissue density. The CT demonstrates a poorly defined homogenous or heterogenous mass, isodense with skeletal muscle on precontrast-images, and slightly hyperdense to muscle on postcontrast-scan. Accurate delineation between the tumor & surrounding tissue is vague or frequently impossible. The authors experienced one case of the mediastinal fibromatosis recently and report the case with review of concerned literature.

류마티스 질환에서 골스캔의 역할 (Role of Bone Scan in Rheumatic Diseases)

  • Rheumatic diseases can be categorized by pathology into several specific types of musculoskeletal problems, including synovitis (e.g. rheumatoid arthritis), enthesopathy (e.g. ankylosing spondylitis) and cartilage degeneration (e.g. osteoarthritis). Skeletal radiographs have contributed to the diagnosis of these articular diseases, and some disease entities need typical radiographic changes as a factor of the diagnostic criteria. However, they sometimes show normal radiographic findings in the early stage of disease, when there is demineralization of less than 30-50 %. Bone scans have also been used in arthritis, but not widely because the findings are nonspecific and it is thought that bone scans do not add significant information to routine radiography. Bone scans do however play a different role than simple radiography, and it is a complementary imaging method in the course of management of arthritis. The Image quality of bone scans can be improved by obtaining regional views and images under a pin-hole collimator, and through a variety of scintigraphic techniques including the three phase bone scan and bone SPECT. Therefore, bone scans could improve the diagnostic value, and answer multiple clinical questions, based on the pathophysiology of various forms of arthritis.

  • 디지털 방사선시스템에서의 의료영상 획득의 방법은 X선을 조사하고, 반도체 디텍터(Detector)를 이용하여 직접 및 간접으로 변환하여 기존 업체마다 여러 가지 알고리즘을 적용하여 적절한 이미지 프로세싱을 거쳐서 임상의 적정한 영상을 획득한다. 방사선과에서 적절한 의료 영상 형성을 위하여 적용하는 이미지 프로세싱 파라미터(Image Processing Parameters)는 Edge, Frequency, Contrast, Latitude, LUT, Noise 등의 영상 증강의 과정은 기술력 및 업체 알고리즘에 따라 다르게 적용되고 있다. 따라서 본 논문에서는 디지털 방사선 환경에서의 최종의 임상 영상을 위한 이미지 증강의 파라미터들의 적정 세팅 값의 기준을 제시하고자 한다. 그리고 각 병원들의 의료 영상을 바탕으로 이미지 프로세싱 파라미터들을 변화하여 각 파라미터들의 세부적인 기준 세팅값을 연구하며, 실제적인 파라미터 변화에 대한 적합한 의료 영상을 디지털방사선시스템의 영상 평가 방법을 도식화하여 결과를 제시하고, 향후 임상에서 적응 및 활용 가능한 객관적인 영상 파라미터에 대한 특성 평가의 응용을 정립하고자 한다. 또한 다양한 표본 병원의 디지털 방사선 환경에서 적정 파라미터 값들을 조사하여 임상에서 영상의 화질에 미치는 영향으로 특성 평가의 객관적인 기준의 변조전달함수(MTF)의 공간해상력을 제시하고 한다.

A Case of Mediastinal Cavernous Hemangioma

  • We report a case of cavernous hemangioma of the anterior mediastinum that was incidentally detected by chest radiography taken at a routine health check-up. A mass lesion was seen in the anterior mediastinum on computed tomography and magnetic resonance imaging. Direct surgical removal was performed for diagnosis and treatment through right thoracotomy. Histopathology confirmed the mass as a cavernous haemangioma. Post-operative course was uneventful.

경부 경막외강내 카테테르의 X-선상 위치 및 조영제의 확산 (The Radiological Location of the Catheters in Cervical Epidural Space and the Spread of Radiopaque Dye)

  • Continuous epidural block can be useful in the management of acute and chronic pain. For the most effective analgesia, it is important to localize the tip of epidural catheter and the spread of radiopaque dye. Epidural catheterization was performed in 12 patients on the sitting position. Catheters were advanced by 10 cm cephalad in the cervical epidural space by median approach and radiopaque dye 3 ml was injected through the catheters. The position of cervical epidural catheters and the spread of dye was confirmed by radiography. The course of epidural catheter were: coiled 3/12 (25%), loop 2/12 (16.7%), straight 2/12 (16.7%). In 8 cases, the tip of epidural catheters were located within one vertebral segment from the level of insertion site. Radiopaque dye spreaded average 3.68 vertebral segment to cranially and 1.67 vertebral segment to caudally from the insertion site.

Long-term Fistula Formation Due to Retained Bullet in Lumbar Spine after Gunshot Injury

  • We here report a case of long-term fistula formation due to bullet retention for 30 years in the lumbar spine after a gunshot injury, and describe its treatment. A 62-year-old male visited our hospital due to pus-like discharge from his left flank. The discharge had been present for 30 years, since his recovery from an abdominal gunshot injury. A spine radiography showed radiopaque material in the body of the third lumbar vertebra. Foreign body was removed using an anterolateral retroperitoneal approach. The postoperative course was uneventful. The patient was discharged 7 days after the operation and was followed-up for 8 months, during which time, the fistula did not reoccur. A bullet retained long term in the vertebral body may cause obstinate osteomyelitis and fistula formation. A fistula caused by a foreign body in the spine can be effectively treated by surgical removal.

Giant Cauda Equina Schwannoma with Dystrophic Calcifications : Case Report and Review of the Literature

  • Giant spinal schwannoma of the cauda equine involving many nerve roots is rare, and ossification is usually not observed in the schwannoma. A 21-year-old man presented with a 12-month history of urinary dysfunction and numbness below the buttocks. Plain radiography showed scalloping of the posterior surface of the vertebral bodies from L4 to the sacrum, and magnetic resonance imaging and computed tomography revealed a giant cauda equina tumor with dystrophic calcification. The tumor was completely removed, with intraoperative neurophysiologic monitoring. Histopathologic examination showed that the tumor was a schwannoma. The patient's postoperative course was uneventful, with urinary function and numbness gradually improving. Although a giant schwannoma accompanied by dystrophic calcification is extremely rare, such a tumor can be removed safely and completely by meticulous dissection and careful neuromonitoring of the cauda equina spinal nerves involved in the tumor.

Successful Treatment of Lead Poisoning in a Whooper Swan (Cygnus cygnus) and a Cinereous Vulture (Aegypius monachus) in Korea

  • We described the successful treatment of cases of lead poisoning in a Whooper swan (Cygnus cygnus) and a Cinereous vulture (Aegypius monachus) in Korea. The swan and vulture rescued were diagnosed with lead poisoning based on clinical signs, the presence of suspected lead-like particles in the ventriculus detected by radiography and elevated blood lead levels. Endoscopic retrieval for the swan and gastric lavage for the vulture were used to remove the lead respectively. After the removal of lead particles, a course of chelation treatment was administrated. This would be the first report of successfully treating wild birds with lead poisoning in Korea.

십이지장 무공증을 동반한 복부 내장 전위증 - 1예 보고- (Situs Inversus Abdominis Associated with Duodenal Atresia - A Case Report-)

  • Situs inversus abdominis is a rare congenital condition commonly associated with serious cardiac and splenic malformations. The importance of recognizing the presence of situs inversus abdominis preoperatively is emphasized by the fact that the surgical incision is placed on the incorrect side of the abdomen. A 6 day-old girl was referred to our hospital because of bile stained vomiting. A plain radiography of abdomen and chest showed the heart to be normal position and a reversed "double-bubble" picture with no other gas shadow in the rest of the abdomen. Abdominal computed tomography scan revealed situs inversus with the stomach and polysplenia on the right side and the liver on the left side. A laparotomy confirmed the diagnosis of situs inversus with duodenal atresia. The obstruction was bypassed by constructing a side-to-side duodenoduodenostomy. The postoperative course was uneventful.

A Case of Pulmonary Mycobacterium kansasii Disease Complicated with Tension Pneumothorax

  • Pneumothorax is an extremely rare complication of non-tuberculous mycobacterial infection. A 52-year-old man presenting with difficulty breathing and chest pain was admitted to our hospital. A right-sided pneumothorax was observed on chest radiography and chest computed tomography showed multiple cavitating and non-cavitating nodules with consolidation in the upper to middle lung zones bilaterally. Serial sputum cultures were positive for Mycobacterium kansasii, and he was diagnosed with pulmonary M. kansasii disease complicated by tension pneumothorax. After initiation of treatment including decortications and pleurodesis, the patient made a full recovery. We herein describe this patient's course in detail and review the current relevant literature.
