Recurrent benign cementoblastoma: A case report and literature review

  • A 16-year-old male presented with pain in the right posterior mandible on chewing that had lasted for several months. The radiographic features of the lesion included a radiolucent-radiopaque mixed-density mass with a radiolucent rim attached to the root of the mandibular right first molar. The preliminary radiographic diagnosis was benign cementoblastoma, which was confirmed by histopathological examination following surgical excision. The lesion recurred 3 years after treatment; radiographically, it consisted of 3 round foci with mixed radiopacity, each with a radiolucent rim near the root of the mandibular right second premolar and the edentulous postoperative region. The lesion was diagnosed as recurrent benign cementoblastoma and a second surgery was scheduled. This report presented an unusual case of recurrent benign cementoblastoma following surgical excision and extraction of the involved tooth, along with a literature review on reported cases of recurrent benign cementoblastoma with a focus on its clinical features and the best treatment options.

Periosteal reaction as a crucial radiographic finding for desmoplastic fibroma of the jaw bone in children: A case report

  • This report presents the case of a 5-year-old boy with a hard swelling on the right side of the mandible body. An important point of this case is that the primary imaging finding was fine spicules in the inferior border of the mandible on panoramic radiography without significant changes in bone density. Cone-beam computed tomography views revealed a lytic lesion on the lingual side of the right mandibular body with the destruction of the lingual cortex and periosteal reaction from the midline to the first molar area. Careful attention to this radiographic finding in the primary stage in the absence of other significant imaging findings, particularly in children, could result in the early diagnosis of desmoplastic fibroma. Therefore, a better prognosis can be expected following early surgical treatment.

  • This paper will present the result of research which was done with 201 places on the actual condition of using dental diagnostic radiography unit and the protection of radiography. The purpose of this paper is to comprehend the actual condition of using dental x-ray unit and to protect when they do radiation work. Moreover this paper was completed to prepare basic materials that could be helpful to reduce the exposure from radiation. This paper obtains the following result. 1. On radiation photographing work in the dentist office, 50.3% of dental hygienists treat this job, and 19.2% of assistants, 10.8% of dentists, 5.6% of radiolotechnologists and 4.2% others performed this job. 2. The case that radiation worker is educated about diagnostic radiography safety supervision has been shown 14.4% and uneducated case has been shown 78.1%. 3. The result about the actual condition of using the oral diagnostic radiation per day was that a number of film which take photograph again (less than 1 exposure) was 40.3%. Normal photographing($1{\sim}10$ exposure) was 85.1% which is the highest percentage. Using the bitewing film and occlusal film was 7.0%, and 12.4% respectively. The percent that they use cephalo film and panoramic film was 16.4% 29.8% respectively. 4. Dental intra diagnostic radiography unit made in $1996{\sim}2000$ was 24.9% and the one made in $1991{\sim}1995$ was 19.9%, in $1986{\sim}1990$ was 19.9%, in 1985 was 9.5% according to the answer. On kVp, they use 60 kVp mostly(61.7%) and On mA, they use 10 mA with the highest percent(66.7%). On the dental extra diagnostic radiography units which are used for doing the extra oral radiography, the one made in $1996{\sim}2000$ was 13.4%, in $1991{\sim}1995$ was 9.5%, in $1985{\sim}1990$ was 2.0% according to the answer. They use $71{\sim}80\;kVp$ with 10.9% and $60{\sim}75\;kVP$ with 9.5%. They use less than 10 mA with 19.4% and $11{\sim}15\;mA$ with 2.5%. $16{\sim}20\;mA$ with 1.5%. But the case they exactly do not know how much mA they use or they do not have any mA was 76.6%. 5. General characteristics and the part of protection through the protective equipment by operator are completed with 89.1%. They have shown the similar difference in the relationship with age(p<0.001), experience(p<0.05) and in-patient(p<0.05). 6. When they take photographs of radiation with general quality, how far they keep the proper distance from the cone is as follows, Keeping safe distance is 12.9% according to the answer with the low percent. This result is similar with the difference related in experience(p<0.05) and work(p<0.05), the area of working(p<0.05) and in-patient(p<0.05). 7. The answer about the question-if they hold the tube head when they take photographs with general characteristics is as follows. The answer that they never hold the tube head and cone occupies 62.7% with the highest percent. It is shown the similar difference with age(p<0.05). 8. According to the study that they put on dosimeter with general characteristics is as follows. 64.7% has never put on the dosimester with the highest percent. 33.8% showed similar differences with experience(p<0.05), work(p<0.001), the area of work(p<0.005)and in-patient(p<0.001).

Identification of Mesiodens Using Machine Learning Application in Panoramic Images (기계 학습 어플리케이션을 활용한 파노라마 영상에서의 정중 과잉치 식별)

  • The aim of this study was to evaluate the use of easily accessible machine learning application to identify mesiodens, and to compare the ability to identify mesiodens between trained model and human. A total of 1604 panoramic images (805 images with mesiodens, 799 images without mesiodens) of patients aged 5 - 7 years were used for this study. The model used for machine learning was Google's teachable machine. Data set 1 was used to train model and to verify the model. Data set 2 was used to compare the ability between the learning model and human group. As a result of data set 1, the average accuracy of the model was 0.82. After testing data set 2, the accuracy of the model was 0.78. From the resident group and the student group, the accuracy was 0.82, 0.69. This study developed a model for identifying mesiodens using panoramic radiographs of children in primary and early mixed dentition. The classification accuracy of the model was lower than that of the resident group. However, the classification accuracy (0.78) was higher than that of dental students (0.69), so it could be used to assist the diagnosis of mesiodens for non-expert students or general dentists.

The effects of pentoxifylline and tocopherol in jaw osteomyelitis

  • Objectives: Pentoxifylline (PTX) is a methylxanthine derivative that has been implicated in the pathogenesis of peripheral vessel disease and intermittent lameness. The purpose of this study was to investigate the effect of PTX and tocopherol in patients diagnosed with osteoradionecrosis (ORN), bisphosphonate-related osteonecrosis of the jaw (BRONJ), and chronic osteomyelitis using digital panoramic radiographs. Materials and Methods: This study was performed in 25 patients who were prescribed PTX and tocopherol for treatment of ORN, BRONJ, and chronic osteomyelitis between January 2014 and May 2018 in Seoul National University Dental Hospital. Radiographic densities of the dental panorama were compared prior to starting PTX and tocopherol, at 3 months, and at 6 months after prescription. Radiographic densities were measured using Adobe Photoshop CS6 (Adobe System Inc., USA). Blood sample tests showing the degree of inflammation at the initial visit were considered the baseline and compared with results after 3 to 6 months. Statistical analysis was performed using the Mann-Whitney test and repeated measurement ANOVA using IBM SPSS 23.0 (IBM Corp., USA). Results: Eight patients were diagnosed with ORN, nine patients with BRONJ, and the other 8 patients with chronic osteomyelitis. Ten of the 25 patients were men, average age was 66.32±14.39 years, and average duration of medication was 151.8±80.65 days (range, 56-315 days). Statistically significant increases were observed in the changes between 3 and 6 months after prescription (P<0.05). There was no significant difference between ORN, BRONJ, and chronic osteomyelitis. Only erythrocyte sedimentation rate (ESR) was statistically significantly lower than before treatment (P<0.05) among the white blood cell (WBC), ESR, and absolute neutrophil count (ANC). Conclusion: Long-term use of PTX and tocopherol can be an auxiliary method in the treatment of ORN, BRONJ, or chronic osteomyelitis in jaw.

Radiographic study of the odontogenic keratocyst (치성각화낭의 방사선학적 연구)

  • Purpose : To acquire the useful diagnostic information through the analysis of the clinical and radiological characteristics of mandibular odontogenic keratocyst. Materials and Methods : The researchers compared and analysed the clinical and radiological features of 112 cases of mandibular odontogenic keratocyst confirmed by histopathlogic examination. Results : Mandibular odontogenic keratocysts occurred more frequently in males than in females and the incidence is the highest in the 2nd and 3rd decades. These cysts occurred in the mandibular posterior area, angle-ramus area and anterior area $51.8\%$, $31.2\%$ and $17.0\%$ respectively. These cysts had undulating border ($69.6\%$) rather than smooth border ($30.4\%$). Most of these cysts had well-defined hyperostotic border ($94.6\%$). These cysts caused cortical thinning or expansion ($78.6\%$) rather than no cortical reaction ($21.4\%$). Loss of lamina dura was observed in $72.3\%$, displacement of tooth appeared in $35.7\%$ and root resolution appeared in $12.5\%$ of cases. In $71\%$ of cases, displacement of mandibular canal was observed. Internal patterns of lesional radiolucency were even ($61.6\%$) or uneven ($38.4\%$). Conclusion : These results would be helpful in diagnosing of mandibular odontogenic keratocyst. (Korean J Oral Maxillofac Radiol 2005; 35 : 51-4)

Radiographic features of desmoplastic ameloblastoma: Report of 3 cases (결합조직형성 법랑모세포종의 방사선학적 소견 : 증례보고 3례)

  • Desmoplastic ameloblastoma is a rare histologic variant of ameloblastoma. It shows important differences in anatomic distribution, histologic appearance, and radiographic findings compared with the general type of ameloblastoma. It is histologically characterized by an abundance of densely collagenous stroma and radiographically a mixed radiolucent-radiopaque lesion. We present three cases of desmoplastic ameloblastoma. All the patients complained on buccal swelling with or without pain and the sites of occurrence were the anterior and the premolar region. Plain radiographs showed mixed radiopaque-radiolucent lesion with ill-defined or diffuse sclerotic margin and no external root resorption. Additionally, CT scans revealetl buccal expansion and relatively well-defined margin of the lesions. The clinical and radiographic features of the presented cases were compared with those of the desmoplastic ameloblastoma in the previous literatures.

An assessment of maxillary sinus and alveolar bone in cross-sectional linear tomogram of panorama (파노라마촬영장치의 협설선형단층상에 의한 상악동과 치조골 평가)

  • Purpose: To evaluate the precision of measurements taken of dental implants in bucco-lingually sectioned views of the maxilla by linear tomograms of the panorama and to assess the visibility of the inferior wall of the maxillary sinus. Materials and Methods : Eighty sites prepared with implants of gutta percha cone in the sockets of the upper premolars and molars of 10 dry skulls were radiographically examined using linear tomograms of panorama, and scanned coronally and axially by computed tomography. The differences in mm between the measurements in bucco-lingually sectioned images of maxillary alveolar bone and the true length and width of the implanted gutta percha cones were compared as mean values (mean) and standard deviations (SD) for each radiographic technique. Linear tomography of panorama was compared with computed tomography for visualization of the relationship between the inferior wall of maxillary sinus and the end of each implant. Results: The deviations between the actual implant length and the measured values taken from the linear tomograms (0.44±0.39 mm) was significantly less than the measured values from the multiplanar reconstructed images of the axially scanned computed tomogram (1.21 ± 0.90 mm). There was statistically significant difference (p < 0.05) between two techniques in the differences between the measurements and true implant length. The relationship of the inferior border of maxillary sinus with end of implant was worse identified with the linear tomogram of panorama (68%) than the multiplanar reconstructed image of axially scanned computed tomogram (99%). Conclusion: We could not find any differences in the accuracy of length measurement between the linear tomogram of panorama and computed tomogram, but computed tomogram allowed for a better visualization of the inferior wall of the maxillary sinus than the linear tomogram.

Relationship between tooth loss and carotid intima-media thickness in Korean adults

  • PURPOSE. The aim of this study was to examine the relationship between tooth loss and sub-clinical atherosclerosis in Korean adults. MATERIALS AND METHODS. The subjects were part of a cohort study conducted in Ansan city by the Korea University medical school as part of the Korean Genome project. 749 subjects over than 40 years old were evaluated. After taking panoramic radiography, the amount of tooth loss was calculated. The intima-media thickness (IMT) was assessed by using ultrasonography at the common carotid artery. Traditional cardiovascular risk factors for atherosclerosis were also evaluated. The relationship between tooth loss and the IMT was evaluated using ANOVA with Scheffe's multiple comparison method in univariate analysis. Multiple regression analysis was also performed to determine the significance between the IMT and tooth loss. RESULTS. With age, tooth loss increased, but there was no significant increase in other traditional cardiovascular risk factors. Univariate analysis revealed the IMT to be positively related with the amount of tooth loss. Regression analysis of the IMT in the anterior and posterior tooth loss revealed that only the posterior tooth loss was significantly related with the IMT at all sites of the common carotid artery (right far wall, P = .015; left far wall, P = .008; right near wall, P < .001; left near wall, P = .001). CONCLUSION. This study verified the positive relationship between the increased tooth loss at the posterior area and the accumulation of atheroma in arteries.

Maxillary sinus augmentation with biphasic calcium phosphate(Osteon$^{(R)}$); A clinical and radiographic study (Original Article 1 - 합성골 이식재인 Osteon$^{(R)}$을 이용한 상악동 거상술-임상적, 방사선 계측학적 연구)

  • Purpose: The aim of this study was to evaluate 1 year cumulative survival rate of implants placed on augmented sinus using Osteon$^{(R)}$, bone graft material and to assess height of the grafted material radiographically. Material and Methods: 10 maxillary sinuses were augmented in 10 patients and 25 implant fixtures were installed simultaneously or after 6 months healing period. The height of the sinus graft material was measured using panoramic images immediately after augmentation and up to 19 months subsequently. Changes in the height of the sinus graft material were calculated with respect to implant length and original sinus wall height. Results: The cumulative survival rate was 100% in all 25 implants. Additionally, normal healing process without any complication was observed in all patients. The mean crown/Implant ratio was 1.25. The mean marginal bone loss was 0.95mm and the mean resorption rate of Osteon$^{(R)}$ was 0.05mm/month. The fastest resorption site of Osteon$^{(R)}$ is the first molar area. The grafted material was well maintained in sinus and decreased slightly over 1 year. Conclusion: In conclusion, It can be suggested that Osteon$^{(R)}$ may have predictable result when it was used as a grafting material for sinus floor augmentation.
