Risk Factors for the Development and Progression of Atlantoaxial Subluxation in Surgically Treated Rheumatoid Arthritis Patients, Considering the Time Interval between Rheumatoid Arthritis Diagnosis and Surgery

  • Objective : Rheumatoid arthritis (RA) is a systemic disease that can affect the cervical spine, especially the atlantoaxial region. The present study evaluated the risk factors for atlantoaxial subluxation (AAS) development and progression in patients who have undergone surgical treatment. Methods : We retrospectively analyzed the data of 62 patients with RA and surgically treated AAS between 2002 and 2015. Additionally, we identified 62 patients as controls using propensity score matching of sex and age among 12667 RA patients from a rheumatology registry between 2007 and 2015. We extracted patient data, including sex, age at diagnosis, age at surgery, disease duration, radiographic hand joint changes, and history of methotrexate use, and laboratory data, including presence of rheumatoid factor and the C-reactive protein (CRP) level. Results : The mean patient age at diagnosis was 38.0 years. The mean time interval between RA diagnosis and AAS surgery was $13.6{\pm}7.0$ years. The risk factors for surgically treated AAS development were the serum CRP level (p=0.005) and radiographic hand joint erosion (p=0.009). The risk factors for AAS progression were a short time interval between RA diagnosis and radiographic hand joint erosion (p<0.001) and young age at RA diagnosis (p=0.04). Conclusion : The CRP level at RA diagnosis and a short time interval between RA diagnosis and radiographic hand joint erosion might be risk factors for surgically treated AAS development in RA patients. Additionally, a short time interval between RA diagnosis and radiographic hand joint erosion and young age at RA diagnosis might be risk factors for AAS progression.

  • 본 증례에서 폐결핵의 치료 중에 발생한 일시적인 방사선학적 악화의 초기 CT 소견은 폐 주변부에 발생한 폐소엽간 혹은 폐소엽 내 중격비후, 그리고 미세결절들이었다. 후기 CT 소견은 폐 주변부에 위치하고 내부에 저음영을 가지며 조영 증강이 잘되는 폐결절이었다. 폐결핵을 처음 치료하는 젊은 환자에서 임상적인 악화 없이 이러한 CT 소견이 보일 때 일시적인 방사선학적 악화를 의심해야 한다.

Osteosarcoma of the mandible: A case report with an early radiographic manifestation

  • Osteosarcoma is a classical malignant bone-forming neoplasm which usually presents with an aggressive clinical course. The current case is presented with the radiographic feature of widening of the periodontal ligament space of the involved teeth, which is considered to be the earliest radiographic manifestation of osteosarcoma involving the jaw bone. The main aim of this case report was to focus on the importance of early diagnosis of this tumor based on clinical and radiographic examinations, and confirmation by histopathology. Considering the rarity of the disease type and particularly taking into account the fast progression and aggressiveness of this neoplasm, it is clear that the presentation of a clinical case represents a major contribution to better understanding of osteosarcomas involving the jaw bone.

  • Purpose: To detect the progression of experimentally induced periapical lesions on periapical radiograph and cone beam computed tomograph (CBCT) by quantitative analysis. Materials and Methods: After the removal of coronal pulps from premolars of two Beagle dogs, the root canals of premolars were exposed to oral environment during one week and then sealed for 70 days. Digital periapical radiographs and CBCTs were taken at baseline and every 7 days for 77 days after pulp exposure. We examined occurrence and areas of periapical bone resorption. Three comparative groups of CBCT radiographs were prepared by average projection of thin slabs with different bucco-lingual thicknesses (0.1, 3.0, and 8.0 mm) using a 3D visualization software. Radiographic densities were compensated by image normalization. Digital images were processed with mathematical morphology operations. The radiographic density and morphological features of periapical lesions were compared among three groups of CBCT in different time points. Results: In the CBCT group with 0.1 mm thickness, radiographic density (p<0.05) and trabecular bone area (p<0.01) were significantly decreased at the fifth week. However, in the CBCT groups with 3 mm and 8 mm thickness and periapical radiographs, none of densitometric and morphological features showed any significant differences in different time points. Radiographic density of periapical lesion showed increasing tendency at the eleventh week after pulp exposure. Conclusion: Radiographic detection of periapical lesions was possible at the fifth week after pulp contamination by quantitative method and was affected by buccolingual bone thickness.

임상증세를 보이지 않는 개심장사상충증의 방사선학적 소견 (Radiographic diagnosis of canine heartworm disease without clinical signs)

  • Dirofilaria immitis, the causative agent of canine heartworm disease, inhabits mainly in the pulmonary arteries and the heart of dogs and cats, causing circulatory and respiratory disorders. Although diagnosis of the disease is based on the presence of microfilaria in the peripheral blood or of specific antigens released from the adult worms into the peripheral blood, the severity of the infection and the assessment of disease progression are based on the clinical signs and radiographic image analysis. We analysed 12 mixed-bred Jindo dogs naturally infected with Dirofilaria immitis without any clinical signs and compared the radiographic images of the heart and the lung with the number of adult worms at necropsy. The dorsoventral radiographs of 12 infected dogs revealed that the right caudal lobar pulmonary arteries (RCaLPA) were dialated in 66.7% of dogs, whereas the main pulmonary artery segment was enlarged in 50.3% of dogs. The cranial lobar pulmonary arteries (RCrLPA) were dilated or pruned in 33.3% of dogs. All dogs displayed the interstitial lung pattern, while the vertebral heart size (VHS) was of normal range. Although a range of 9 to 166 adult worms (av. 45.6) was found in the pulmonary arteries, in the heart or in the vena cava, no correlation with the radiographic findings and/or with the clinical signs was observed.

Primary intraosseous squamous cell carcinoma mimicking periapical disease: a case report

  • Primary intraosseous squamous cell carcinoma (PIOSCC) is a rare carcinoma, which arises within the jaws without connection to the oral mucosa and presumably develops from a remnant of odontogenic epithelium. We present a case of solid type PIOSCC in a 52-year-old male patient complaining of dull pain on his left lower molar. In this case, early stage PIOSCC mimicking a periapical lesion might lead to a one-year delay in treatment due to the misdiagnosis of osteomyelitis after extraction of the third molar. The clinical, radiological, and histologic features are described. In this case, there was initial radiographic evidence for PIOSCC mimicking a periapical lesion. Incautious radiographic interpretation and treatment procedures had delayed the correct diagnosis and resulted in extensive bony destruction during the patient's disease progression.

Parry-Romberg Syndrome 환자에서 악정형 및 교정 치료 (Orthopedic and Orthodontic Treatments of a Patient with Parry-Romberg Syndrome)

  • Parry-Romberg syndrome(PRS) is a degenerative disease characterized by progressive hemifacial atrophy. A 10-year-old girl who had been treated for linear scleroderma at the dermatologic department visited the orthodontic department. The frontal facial photograph showed mild facial asymmetry. On the left side, mild atrophy of soft tissue, enophthalmos, cheek depression, and dry skin with dark pigmentation were observed. The radiograph showed the hypoplasia of both the maxilla and mandible on the left side. This case report describes the treatment of a patient with PRS for 7 years. To minimize the effect of progressive atrophy on the facial growth, a hybrid appliance was used. The facial photos and radiographic records were periodically taken to analyze the progression of PRS. Although it is impossible to prevent the progression of facial asymmetry, it appears to be possible to limit the atrophic effect. After the stabilization of PRS, the orthodontic treatment by the fixed appliance was performed. Additionally, autologous fat graft was performed three times at 6 month intervals. After the treatment, the patient had a confident smile and facial asymmetry was improved.

Teeth discoloration during orthodontic treatment

  • Objective: Teeth discoloration is a rare orthodontic complication. The aim of this study was to report the clinical progression of discoloration during orthodontic treatment. Methods: Discolored teeth, detected during orthodontic treatment between January 2003 and December 2012 by a single dentist using similar techniques and appliances, were analyzed. Results: The total number of teeth that showed discoloration was 28. Progression of discoloration was evaluated in only 24 teeth that were observed without any treatment. During the observation period, the discoloration "improved" in 8 of the 24 teeth (33.3%) and was "maintained" in 16 (66.6%). The electric pulp test performed at the time of initial detection of discoloration showed 14.3% positivity, which improved to 21.4% at the final follow-up. None of the initial and final follow-up radiographic findings showed any abnormalities. Conclusions: When teeth discoloration is detected during orthodontic treatment, observation as an initial management is recommended over immediate treatments.

Assessment of radiographic left atrial dimension and C-reactive protein in dogs with myxomatous mitral valve disease

  • Radiographic left atrial dimension (RLAD) is a valuable metric for assessing left atrial enlargement in dogs. While there have been studies on the use of RLAD and the increase in C-reactive protein (CRP) levels based on heart disease stages, there has been no prior research on the correlation between RLAD and CRP. In this study, the objective was to investigate the relationship between the rise in RLAD as myxomatous mitral valve disease (MMVD) stages advance and the increase in CRP levels with MMVD stage progression. In this study, a total of 30 small-breed dogs were included as subjects. These dogs were diagnosed with MMVD at the American College of Veterinary Internal Medicine (ACVIM) stage B1 or B2, or stage C, based on a comprehensive assessment including physical examination, thoracic radiography, and echocardiography. Measurements of VHS and RLAD were compared to assess any significant differences. There were significant differences in RLAD between dogs with MMVD ACVIM stage B1 and those with stage C. The monocytes and CRP levels showed significant differences between ACVIM stage B1, B2 and ACVIM C. Additionally, a significant correlation was observed between the RLAD and VHS measurements. This underscores the notable association between MMVD stage advancement and elevated monocyte and CRP levels. The RLAD scores exhibited a significant difference among dogs with ACVIM stages B1, B2, and C, and significant variations were also observed in monocyte and CRP levels. These results suggest that monocyte and CRP levels may be a valuable diagnostic indicator for heart disease in dogs during the diagnostic evaluation.

An Assessment of Vertebral Left Atrial Size in Relation to the Progress of Myxomatous Mitral Valve Disease in Dogs

  • Left atrial enlargement (LAE) is an important diagnostic factor in dogs with myxomatous mitral valve disease (MMVD). It is associated with the onset of congestive heart failure (CHF). Recently, a new radiographic left atrial measurement called vertebral left atrial size (VLAS) was introduced. This can be considered as a left atrial enlargement above 2.3. It appears to be related to the severity of MMVD. However, serial changes in VLAS in relation to disease progression and improvement in patients have yet to be studied. This study aims to assess the value of VLAS as a left atrial size monitoring indicator by examining correlations with VHS, LA/Ao ratio and LVIDDN, and comparing serial changes in dogs. A total of 126 dogs were studied with their owners' consent. The dogs were classified into four MMVD groups (Control, B1, B2, C-D) following the ACVIM Guideline by performing a physical examination, radiography and echocardiography. Besides, 24 and 17 dogs were reevaluated to compare values in relation to the progression and improvement of MMVD. VLAS showed significant increase according to the progress of the MMVD stage. This was the same in the Maltese breed group. A strong positive correlation was found between LVIDDN, VHS, LA/Ao ratio, and VLAS. The results of this study found VLAS to be significantly different according to left atrium size, and there was a correlation between disease progression and VLAS levels in each dog. Therefore, VLAS may be used to detect changes in left atrium size as an additional monitoring index of MMVD.
