방사선 교육 동향: 한국, 호주, 미국 방사선 교육 시스템 비교 (Changing Trends in Radiographic Education: A Comparison of Korean, Australian and the United States of America Radiographic Education Systems)

  • 지난 20년간 방사선 기술 분야는 비약적으로 발전하였고 확대되었다. 이를 기반으로 하는 방사선사의 역할 및 업무영역은 해를 거듭할수록 더욱 고도화, 전문화 되고 있다. 그리고 현재 대한민국의 방사선 교육을 서구화 하고자 하는 부단한 노력이 각 대학에서 활발히 진행 중이다. 이 논문은 대한민국의 방사선 교육과 면허 제도를 재조명하고, 호주와 미국의 방사선교육, 면허제도와 비교하였다. 대한민국 방사선 교육과정과 면허 제도를 호주와 미국의 동향과 비교 분석하여, 향후 전문 직업 교육과정으로서의 방사선 교육 발전 전략을 모색하고자 한다.

서울지역 대학병원의 방사선과 카세트 소독에 관한 연구 (A Survey of the Radiographic Cassettes Disinfection of University Hospitals in Seoul)

  • The purpose of this study is to prevent nosocomial infection in patients through contact of radiographic cassettes. Data were collected from radiographers working in 29 university hospitals in Seoul in February and March 2001. Radiographic cassettes were disinfected daily in 5 hospitals, weekly in 4 hospitals, monthly in 5 hospitals, bimonthly in 1 hospital and once every three months in another hospital. 12 other hospitals do not practice regular disinfections of radiographic cassettes. Gauze soaked in disinfectant solution is used in 7 hospitals while 11 hospitals used cotton and cloth soaked in disinfectant solution to clean the radiographic cassettes. 26 hospitals used 99% alcohol based disinfectant solutions while 3 hospitals used 75% alcohol based disinfectant. 26 hospitals use of intercourse cassettes outpatients and in patients. In 26 hospitals, all patients shared the same set of radiographic cassettes used in the hospitals, or in 26 hospitals, separate sets of radiographic cassettes are used for outpatients and inpatients. Separate sets of cassettes are used for ICU and inpatients in 6 others hospitals. 23 hospitals used the same sets of radiographic cassettes for all their patients. radiographic cassettes are cleaned in wash area in the study room of the radiographic department in 17 hospitals. 12 other hospitals do not have designated cleaning areas for the cassettes. All radiographers practiced hands washing with soap. All 29 hospitals surveyed have infection control committee. However, only 9 out of the 29 hospitals surveyed provided Infection disinfections control education to radiographers. Only 3 hospitals have radiographers sitting in the infection control committee. Infection management education is conducted in 63 hospitals annually, twice a year in 1 hospital and once every 3 months in 2 hospitals.

Case series of cleidocranial dysplasia: Radiographic follow-up study of delayed eruption of impacted permanent teeth

  • This report describes 3 cases of cleidocranial dysplasia (CCD) and presents relevant findings on long-term follow-up radiographic images of impacted permanent teeth with delayed eruption. Radiographic images of 3 CCD patients were reviewed retrospectively. These images were mainly composed of panoramic and skull radiographs, and the follow-up periods were 3, 13, and 13 years, respectively. The distinct features revealed by the images were described, and the eruption state of impacted permanent teeth was evaluated. The features common to the 3 cases were multiple supernumerary teeth, the presence of Wormian bone, underdevelopment of the maxilla and the maxillary sinus, and clavicular hypoplasia. The eruption of impacted permanent teeth was not observed without proper dental treatment in adult CCD cases, even after long time periods had elapsed. When proper orthodontic force was applied, tooth movement was observed in a manner not significantly different from the general population.

  • The system of Oral and Maxillofacial Radiology education, radiographic equipments and oral health care were evaluated in Korea. The objectives, the length, curriculum guidelines, instructional method and teaching means for undergraduate, and postgraduate education in 9 dental college were described. To investigate the radiographic ewuipments, surveys were received from 6 Dental College Hospitals(DCH), 23 Dental Hospitals(DH) and 373 Dental Clinics(DC). And the results of oral health care were obtained from the surveys of Kyunghee Dental College Hospital(KDCH), 14 Dental Hospital Hospitals and 52 Dental Clinics.

Attitude of the Korean dentists towards radiation safety and selection criteria

  • Purpose: X-ray exposure should be clinically justified and each exposure should be expected to give patients benefits. Since dental radiographic examination is one of the most frequent radiological procedures, radiation hazard becomes an important public health concern. The purpose of this study was to investigate the attitude of Korean dentists about radiation safety and use of criteria for selecting the frequency and type of radiographic examinations. Materials and Methods: The study included 267 Korean dentists. Five questions related to radiation safety were asked of each of them. These questions were about factors associated with radiation protection of patients and operators including the use of radiographic selection criteria for intraoral radiographic procedures. Results: The frequency of prescription of routine radiographic examination (an example is a panoramic radiograph for screening process for occult disease) was 34.1%, while that of selective radiography was 64.0%. Dentists' discussion of radiation risk and benefit with patients was infrequent. More than half of the operators held the image receptor by themselves during intraoral radiographic examinations. Lead apron/thyroid collars for patient protection were used by fewer than 22% of dental offices. Rectangular collimation was utilized by fewer than 15% of dental offices. Conclusion: The majority of Korean dentists in the study did not practice radiation protection procedures which would be required to minimize exposure to unnecessary radiation for patients and dental professionals. Mandatory continuing professional education in radiation safety and development of Korean radiographic selection criteria is recommended.

Augmented Reality Technology-based Dental Radiography Simulator for Preclinical Training and Education on Dental Anatomy

  • It is important that students are provided opportunities to practice their skills in acquiring radiographic images. However, these opportunities are currently limited because of the risk of radiation exposure. To overcome this limitation, a new augmented reality-based radiography simulator was developed that enables students to practice radiographic techniques as part of self-directed learning without time and space constraints. Subsequently, cross-sectional images of a manikin phantom head obtained via computed tomography were reconstructed into a three-dimensional object. An image marker that could be recognized by a mobile device and could allow users to practice dental radiography techniques was devised. The three-dimensional object was augmented to the mobile device; consequently, among 106 stored dental radiographs on the device, a radiograph corresponding to specific imaging conditions was opened when users performed radiographic procedures. This technology could improve dental students' understanding of dental anatomy and contribute to improving their competency in acquiring dental radiographs.

Dental radiology reporting status and recording frequency of reporting items in Korea

  • Purpose: This study investigated the current dental radiology reporting methods and the recording rate of 10 mandatory reporting items in Korea. Materials and Methods: An original online survey created using Google Forms was distributed to dental practitioners. The survey asked about the participants' age, experience, workplace, use of radiologic equipment, radiology reporting methods, and recording reporting items. Results: In total, 354 responses were analyzed. Radiologic reporting in dental charts was the most commonly used method for each modality. Four out of 10 mandatory items were recorded at a high rate, but the remaining 6 items had substantially lower recording rates, often below 50%. The participants who reported radiographic findings through other separate methods had higher item scores than those who wrote findings in dental charts(P<0.05). Conclusion: Radiologic societies and dental associations should encourage the use of separate reports for radiographic examinations. Education regarding radiology reports and the justification for reporting items should be reinforced in dental schools, training courses on radiology, and the continuing education curriculum.

3D 프린팅 구조물 기반 블라인드박스를 이용한 실습교육이 다차원 방사선영상해독력에 미치는 효과 (Effects of Practical Training Using 3D Printed Structure-Based Blind Boxes on Multi-Dimensional Radiographic Image Interpretation Ability)

  • 본 연구에서는 3D 프린팅 구조물 기반 블라인드박스를 이용한 실습교육이 다차원 방사선영상해독력에 미치는 교육적 효과를 알아보고자 하였다. 대상은 2020년부터 2022년까지 3년간 진행된 디지털의료영상학 실습에 참여한 방사선학과 2학년의 83명(남자: 49명, 여자: 34명)이었다. 학습방법은 3D 프린팅 기술을 이용하여 자체 설계한 블라인드박스 내 구조물을 출력하였다. 이를 엑스선 촬영한 한 후 소그룹별로 블라인드박스 내 구조물영상을 분석한다. 분석 결과로 3차원구조물을 지점토로 자체 제작하고 엑스선 촬영한 후 블라인드박스 내 구조물영상과 일치하는지 비교한다. 평가방법은 수업성실도(평소 수업태도, 예·복습 정도, 학습목표이해도), 방사선영상해독력(엑스선감쇠개념, 대조도개념, 윈도우닝개념, 3차원해독력), 수업만족도(흥미도, 외부추천도, 수업몰입도)를 5점 리커트 척도로 무기명 자기 기입방식으로 설문조사하였다. 그 결과 모든 평가항목이 남녀 간의 유의한 차이 없이 높은 긍정적인 효과가 있었다. 블라인드박스를 이용한 실습교육은 3D 프린팅 기술을 활용한 방사선교육공학의 의미있는 사례로써 학생들의 문제해결능력 향상과 전공과목의 만족도를 증대시키는 콘텐츠로 활용되기를 기대한다.

Periodontal regenerative therapy in endo-periodontal lesions: a retrospective study over 5 years

  • Purpose: The aim of this study was to evaluate clinical and radiographic changes and the survival rate after periodontal surgery using deproteinized bovine bone mineral (DBBM) with 10% collagen or DBBM with a collagen membrane in endo-periodontal lesions. Methods: A total of 52 cases (41 patients) with at least 5 years of follow-up were included in this study. After scaling and root planing with or without endodontic treatment, periodontal regenerative procedures with DBBM with 10% collagen alone or DBBM with a collagen membrane were performed, yielding the DBBM + 10% collagen and DBBM + collagen membrane groups, respectively. Changes in clinical parameters including the plaque index, bleeding on probing, probing pocket depth, gingival recession, relative clinical attachment level, mobility, and radiographic bone gains were evaluated immediately before periodontal surgical procedures and at a 12-month follow-up. Results: At the 12-month follow-up after regenerative procedures, improvements in clinical parameters and radiographic bone gains were observed in both treatment groups. The DBBM + 10% collagen group showed greater probing pocket depth reduction ($4.52{\pm}1.06mm$) than the DBBM + collagen membrane group ($4.04{\pm}0.82mm$). However, there were no significant differences between the groups. Additionally, the radiographic bone gain in the DBBM + 10% collagen group ($5.15{\pm}1.54mm$) was comparable to that of the DBBM + collagen membrane group ($5.35{\pm}1.84mm$). The 5-year survival rate of the teeth with endo-periodontal lesions after periodontal regenerative procedures was 92.31%. Conclusions: This study showed that regenerative procedures using DBBM with 10% collagen alone improved the clinical attachment level and radiographic bone level in endo-periodontal lesions. Successful maintenance of the results after regenerative procedures in endo-periodontal lesions can be obtained by repeated oral hygiene education within strict supportive periodontal treatment.

Middle superior and anterior superior alveolar nerve injury following trauma to the maxillary sinus: a prospective clinico-radiographic evaluation

  • Objectives: Anterior maxillary sinus wall fractures are common in all types of maxillofacial trauma. They can result in various complications, including injury to the surrounding nerves. Owing to its anatomy, trauma to the maxillary antrum can result in injury to the middle superior alveolar nerve (MSAN) and the anterior superior alveolar nerve (ASAN). The purpose of this study is to evaluate neurosensory deficits (NSD) present in maxillary gingiva, incisors, and premolars after injury to the anterior wall of the maxillary antrum. Materials and Methods: This prospective study was conducted among 39 patients sustaining unilateral fractures of the anterior maxillary sinus wall. Clinical neurosensory tests including two-point discrimination and fine touch discrimination were performed to classify the extent of nerve injuries as mild, moderate, severe, or anesthetic. Additional temperature discrimination and pulpal sensibility tests (electric pulp testing and cold testing) were carried out. A comparison of radiographic fracture patterns and severity of nerve injury was done. Testing was carried out immediately after trauma and at 2-month follow-up. Results: More than half of the patients assessed in the study group presented with NSD of the teeth and gingiva after trauma. The incidence of deficits varied with the type of test used to measure them. Most frequently, patients presented with both loss of two point as well as fine touch discrimination thresholds. Severe nerve injuries were associated with loss of temperature discrimination clinically and displaced fractures radiographically. There was no significant relationship between the recovery of pulpal and gingival sensation. The patterns of injury and recovery in ASAN and MSAN were similar. Conclusion: NSD after trauma to the maxillary antrum is relatively common. Clinical loss of temperature discrimination and radiographic signs of fracture lines passing through the canalis sinuosus are predictors of persistent and severe oral NSD.
