RI 폐기물 내 방사성요오드 분석을 위한 분리 및 회수 (Separation and Recovery for the Analysis of Radioiodine in RI Wastes)

  • RI 폐기물 내에 있는 낮은 방사능의 요오드의 함량을 결정하기 위해 산분해법과 BPGe 감마 선분광계를 이용하는 방법이 개발되었다. 분석에 앞서 모의시료인 제염지 내에 $^{131}I$이 일정량 첨가되었으며, 100 mL의 0.4 N $K_2Cr_2O_7$와 100 mL의 9 M $H_2SO_4$, 10 mL의 30% $H_3PO_3$, 1 mL의 $H_2O_2$를 넣고 산분해과정을 거치면서 증류된 용액을 응축하여 포집하였다. $CCl_4$의 용매추출에 의한 화학 분리과정을 거친 후 $AgNO_3$를 첨가해서 얻은 AgI 침전물을 여과하고 건조하여 측정하였다. 산분해 과정, 화학 분리과정, 여과 및 침전과정 등 세 단계로 나눠 회수율을 측정한 결과, 각각 94% 이상의 회수율을 나타냈으며, 본 연구의 측정조건에서 최소검출방사능은 0.6 Bq/g이었다.

방사성동위원소옥소(放射性同位元素沃素)($^{131}I$)에 의(依)한 갑상선질환(甲狀腺疾患)의 임상적연구(臨床的硏究) -제 4 보-(第 4 報) (Clinical Investigation and Treatment of Thyroid Diseases with Radioactive Iodine($^{131}I$) -Report 4-)

  • Over the past 6 years, from May 1960 to June 1966, 1,716 patients with various diseases of thyroid were examined and thyroid function tests with $^{131}I$ were done. Among them, 545 patients with hyperthyroidism were treated with $^{131}I$. A summary of the clinical data of the $^{131}I$-thyroid function tests and the therapeutic results of $^{131}I$ were presented and discussed. 1. The patients examined consisted of; 596 cases(34.7%) with toxic diffuse goiter, 412 cases(24.0%) with non-toxic nodular goiter, 278 cases(16.2%) with euthyroidism, 236 cases(13.8%) with non-toxic diffuse goiter, 89 cases(5.2%) with hypothyroidism, 53 cases(3.1%) with toxic nodular goiter, 32 cases(1.9%) with thyroiditis and 20 cases(1.2%) with dyshormonogenesis. 2. There were 218(12.7%) male patients and 1,498(87.3%) female patients, showing a ratio of 1:6.9. female predominantly. 3. The majority of patients(79.6%) were in the 3rd through 5th decades of their lives showing the peak in the 4th decades(35.9%). 4. The diagnostic values and normal ranges of $^{131}I$ uptake test, 48 hour serum activity, $T_3$ red blood cell uptake and $PB^{131}I$ conversion ratio were discussed. 5. An attention was given to dyshormonogenesis, a qualitative hypothyroidism, due to its characteristic findings of clinical and $^{131}I$ thyroid function tests, and its pathogenesis was briefly reviewed. 6. Among 545 patients with hyperthyroidism treated with $^{131}I$, 68.3% was cured after single. therapeutic dose and another 24.0% was cured after second dose. 7. The complications of $^{131}I$ therapy were discussed in some details and myxedema had developed. in 3.9% of our cases. No thyroid cancer was found after $^{131}I$ therapy.

GEANT4를 이용한 치료용 I-131 캡슐의 안정성 시뮬레이션 (Safety Simulation of Therapeutic I-131 Capsule Using GEANT4)

  • 국내 갑상선암 환자의 발병률이 증가함에 따라 방사성요오드 치료가 필요한 환자가 증가하고 있다. 방사성 요오드 치료에 이용되고 있는 캡슐용기들 중 1.1 GBq과 5.5 GBq의 국산제품에 설계도면을 바탕으로 차폐체를 구현한 후 366 keV 이상에 감마선들에 대하여 캡슐용기의 방사선 누설선량을 GEANT4 전사모사를 이용하여 평가하였다. 치료용 I-131 캡슐용기 누설선량을 측정하기 위해 각각의 용기에 대하여 용기 표면에서 10 cm 거리 및 100 cm 거리에서의 누설선량을 측정하였다. 용기표면에서 10 cm 거리와 100 cm 거리에서의 누설선량 측정은 방사형으로 발생되는 방사선을 위치별로 측정하기 위해 $10{\times}10{\times}10cm^3$ 부피의 정육면체 형태의 물 팬텀(phantom)을 상부, 상부측면, 측면, 하부측면, 하부 다섯 방향에 설치하여 누설선량을 계산하였다. 용기별로 5개 방향에서 용기표면으로 부터 10 cm, 100 cm 거리에서 전산모사를 수행한 결과 법적 허용기준인 10 cm 거리에서 2.0 mSv/h, 100 cm 거리에서 0.02 mSv/h 이하의 선량 규정과 비교하였을 때 법적기준치보다 현저히 낮은 누설선량이 방출되는 것을 확인하였다.

  • A summary of the clinical data of the $^{131}I$-thyroid function tests and the therapeutic results of $^{131}I$ among the 2,658 patients of various thyroid diseases treated over the past 10 years from May 1960 to Oct. 1969 at the Radioisotope Clinic and Laboratory, SNUH were presented and dscussed. 1. The patients examined consisted of: 929 cases (34.9%) of diffuse toxic goiter, 762 cases (28.7%) of diffuse nontoxic goiter, 699 cases (26.3%) of nodular nontoxic goiter, 58 cases (2.2%) of nodular toxic goiter and 210. cases (7.9%) of hypothyroidism. 2. There were 300 (11.4%) male and 2358 (88.6%) female, showing a ratio of 1:8. 3. The majority of patients (79.1%) were in the 3rd-5th decades of their lives. 4. The normal ranges, diagonstic values of $^{131}I$ uptake test, 48 hrs, serum activity, BMR and main subjective symptoms of various thyroid diseases were discussed. 5. In the 579 patients among 867 cases with hyperthyroidism treated with $^{131}I$, 47.8% were confirmed to be cured completely after single therapeutic doses. 6. The complications of $^{131}I$ therapy were discussed and myxedema had developed in 6.75% of our patients. 7. The results of $^{131}I$ thyroid function tests were analysed among the 160 cases of thyroid diseases which were confirmed the diagnosis with histopathological measures.

대장직장암 및 기타 소화기암에서의 단세포군항체를 이용한 방사면역신티그라피의 진단 (Immunoscintigraphy of Colorectal and Other Gastrointestinal Cancers with Radioactive Monoclonal Antibodies to CEA and CA 19-9)

  • The cocktails of two $^{131}I$ labeled Monoclonal antibody (MCAB) (Anti CA 19-9 F$(ab')_2$ + Anti CEA $F(ab')_2$ fragment), which react specially, with human gastrointestinal cancers, were administered to 10 patients with colorectal (7), stomach(2) and pancreas(1) cancer for scintigraphic detection. All patients were known or postoperatively recurrent cases, and serum tumor markers, CA 19-9 and CEA, were measured with immunoradiometric assay, just before immunoscintigraphy (ISG). The tumor marker's level in serum is not correlated with positive tumor uptake in ISG. The sensitivity and specificity of ISG in detection of 21 tumor sites, based on surgery, CT, ultrasonography and pathology, were 90.5% and 100% One case of colon cancer showed gall bladder metastasis, which was neglected on CT study. Tumor/non tumor uptake ratio of radiolabelled antibody were progressively increased from day 3 to day 7 during study. We summerized as follows 1) The use of cocktails of CEA and CA 19-9 MCAB $F(at')_2$ increased sensitivity and specificity in ISG. 2) Delayed imaging (later than 5 days) increases sensitivitv and specificity due to exclusion of nonspecific iodine accumulation in stomach and lung. 3) Second tracer technique is essential for anatomical landmark by use of a double isotope scan, but subtraction technique, a possible source of artifacts, is no longer necessory when delayed imaging is performed. 4) It may be possible to use two MCAB cocktails of CA 19-9 and CEA in Radioimmunodetection of stomach and pancreas cancer. In conclusion, ISG using MCAB cocktails, $F(ab')_2$ fragment of anti CA 19-9 and Anti CEA, provide additional opportunity for tumor localization and detection of colorectal and other G-I cancer, such as stomach and pancreas.

국내 PWR 사용후핵연료에서 세슘, 스트론튬과 요오드의 갭 및 입계 재고량 측정 (Measurement of the Gap and Grain Boundary Inventories of Cs, Sr in and I in Domestic Used PWR Fuels)

  • 처분장에서 지하수로 쉽게 유출될 수 있는 방사성 핵종들의 양을 예측하기 위하여 국내 PWR 사용후핵연료 팰렛들의 갭(gap) 및 입계에 있는 용해성 원소들의 재고량을 측정하였다. 연소도가 $45{\sim}66$ GWD/MTU를 갖는 연료봉에서 얻은 펠렛들에서 세슘의 갭 재고량이 $0.85{\sim}1.7%$M로 나타났으며, 이는 핵분열 생성기체 유출률의 $1/6{\sim}1/3$에 해당하였다. 그러나 핵분열 생성기체 유출 률이 1%이하인 연료봉에서 취한 40 GWD/MTU이하의 연소도를 갖는 펠렛들의 경우, 세슘의 갭 재고량들을 핵분열 생성기체 유출률과 연관시키기는 곤란하였다. 갭 및 입계내 스트론튬의 재고량은 동일 연료봉내 펠렛에서는 크게 다르지 않았으며, 요오드의 갭 재고량은 핵분열 생성기체 유출률보다 작거나 유사한 값을 갖는 것으로 평가되었다.

Efficacy of Permanent Iodine-125 Seed Implants and Gemcitabine Chemotherapy in Patients with Platinum-Resistant Recurrent Ovarian Carcinoma

  • Background: The aim of this study was to explore the efficacy and adverse reactions of CT-guided radioactive 125I-seed implantation treatment combined with chemotherapy for platinum-resistant recurrent ovarian carcinoma. Materials and Methods: From September 2010 to December 2012, 23 patients with platinum-resistant recurrent ovarian carcinoma were enrolled. All the patients refused, could not bear, or were not suitable for surgery. They all had no more than 3 lesions, which were detected and could also be measured by CT. All were clarified as single-lesion or multiple-lesion groups. A total of 41 lesions underwent implantation of from 8 to 106 125I seeds (median=43). Multi-plane implanting was adopted and 125I-seeds of (0.4-0.7)mCi were placed at intervals of (0.5-1.0) cm. After implantation treatment, all patients underwent 4 cycles of chemotherapy with gemcitabine $800mg/m^2$ (days 1, 8 and 15). Results: The outcome was evaluated with CT 3 weeks and every 3 months after implantation treatment. After 6 months, the volume of 32 out of 41 lesions (78.0%) was reduced at least 30%, within which 9 lesions completely disappeared(22.0%). Complete response was observed in 7 cases (30.4%), with a partial response in 4 cases (17.4%), 4 cases stable(17.4%)and 8 cases showing progression (34.8%). The total clinical remission rate was 47.8% (11/23). The clinical remission rate was 77.8% (7/9) in the single-lesion group and 28.6% (4/14) in the multiple-lesion group with a significant difference between the two(P=0.036). The common side effects observed were mild gastrointestinal reactions. Conclusions: 125I-seed implantation combined with chemotherapy applies an effective way in the treatment of platinum-resistant recurrent ovarian epithelial carcinoma with the advantages of high local control rates, good short-term effects, little trauma and less side effects.

한국 소아 및 청소년 Graves병의 자연 경과 및 예후 인자 (The natural history and prognostic factors of Graves' disease in Korean children and adolescents)

  • 목 적: Graves병은 소아 및 청소년의 갑상선 기능항진증의 가장 흔한 원인으로 한국 소아 및 청소년 Graves병의 자연 경과 및 예후 인자를 알아보기 위하여 이 연구를 시행하였다. 방 법: 1991년 11월부터 2006년 7월까지 15년간 서울아산병원 소아청소년과에서 Graves병으로 진단 받고 2년 이상 추적관찰이 가능하였던 환자 113명(여자 88명, 남자 25명)을 대상으로 하였다. 성별, 진단 시 연령, 치료 기간, 혈액 검사, 임상 증상, 가족력을 후향적으로 분석하였다. 결 과 : 모든 환자들은 항갑상선제 투여로 치료를 시작하였고 7명(6.2%)에서 PTU, 106명(93.8%)에서 MZ을 투여하였다. 전체 환자 113명 중 75명(66.4%)은 첫 번째 관해가 되었으며, 이 중 23명(전체의 20.4%)은 약물 치료를 중단한지 평균 $25.5{\pm}33.7$개월 후 재발하였다. 재발한 23명 중 13명(전체의 11.5%)은 두 번째 관해가 되었으나 이 중 2명은 다시 재발하였다. 1명은 4년간의 약물 치료에도 정상 갑상선 기능이 되지 않아 방사선 요오드 요법을 시행하였다. 완전 관해군의 진단 시 연령이 질병 지속 상태군에 비해 의미 있게 높았다(12.7세 vs. 11.7세, $P$=0.034). 즉, 연령이 높은 경우 관해율이 높을 것으로 나타났다. 결 론: 진단 시 연령을 소아 및 청소년 Graves병의 예후 인자로 고려할 수 있으며 이는 Graves병의 치료 방법 및 기간을 결정하는데 도움이 될 것으로 생각된다.

흰쥐의 내분비선 및 혈장성분에 미치는 뇌하수체척출의 영향과 이에 대한 성 Hormone의 효과 (The Effect of Hypophysectomy and Subsequent Administration of Sex Hormone on Several Endocrine Glands and Plasma Components in Rats)

  • The present experiments were carried out to elucidate the effects of hypophysectomy and subsequent administration of sex hormone on thyroid, adrenal gland, gonads and blood plasma components in the rat. The jresults obtained were summarized as follows: The weight of the thyroid gland of both male and female hypophysectiomzed rats decreased markedly from 7 days up to 56 days after the hypophysectomy as compared to the control group. The administration of sex hormone (6 mg of testosterone propionate to male and 6 mg of hexestrol to female) to the hypophysectomized rat gave on effect on the change in the weight of the thyroid gland. The hopophysectomy decreased the uptake of radioactive iodine in the thyroid gland in both male and female rats with time. Subsequent administration of the sex hormone caused no effect. With regard to the histological changes of the thyroid gland, the hypophysectomy caused significant changes in the gland showing a remarkable degeneration. The function of the gland seemed to disa, pp.ar almost completely on 56th day after the hypophysectomy. Upon the administration of sex hormone after the hypophysectomy, however. the epithelia of the follicle which has changed to flat from has partly returned to its functional cubicfrom and nuclei recovered as nearly as normal. These recovery were more remarkable in the female than in the male. The hypophysectomy kept causing a significant decrease in the weight of the adrenal gland in male and female rats during the period of observation (up to 56 days) as in the case of thyroid gland. The administration of sex hormone has on effect in this respect either. The hypophysectomy also caused a marked morphological change in the gland: zona fasciculata and zona reticularis were dicreased in size quichly after the hypophysectomy. The administraton of the sex hormone to the hypophysectomized rat resulted in clear distinction among the three layers of the adrenal cortex which otherwise very diffused. In the male, this phenomenum was more remarkable than in the female and the pattern of the cell arrangements and the thickening of each layer became similar to those of normal rats. The gonads of both sexes have also kept decreasing in the weight and degenerated in morpohology after the hypophysectomy. However, the degenerate follicle became enlarged after the administration of hexestrol in the female. Furthermore, the vacuoles found in interstitial cells of hypophysectomized rat disa, pp.ared after the administration of testosterone in the male and the formation of spermatocytes seemed to be recovered. Hypophysectomy also caused a gradual increase in the contents of total protein, non-protein nitrogen, total lipid, cholesterol and calcium in the blood plasma with time. The concentrations of sodium, potassium and chloride in the blood did not change after the hypophysectomy. Sex hormone caused practically no change in above tendency.

한국남자 성인을 대상으로 한 방사성옥소($^{131}I$)의 갑상선 및 각 장기별 잔류율과 소변 일일배설률 측정 (Measurement of Uptake Rates of Internal Organs Including Thyroid Gland and Daily Urinary Excretion Rates for Adult Korean Males)

  • 본 연구는 한국인 특성에 대해 보다 신뢰할 수 있는 방사성핵종의 체내 흡수선량 평가를 위한 일환으로 $^{131}I$을 선정하여 체내 각 장기별 잔류율 및 소변 일일배설률을 측정하였다. 실험방법은 성인남성 28명을 대상으로 $^{131}I$을 경구 투여한 후, 시간대별(2, 4, 6, 24시간) 갑상선, 간, 위, 소장, 신장, 소변의 방사능을 측정하고 이를 이용하여 각 장기별 잔류율 및 소변 일일배설률을 산출하였다. 그 결과, $^{131}I$ 투여 24시간 후 갑상선이 평균 19.70%의 잔류율과 71.12%의 소변 일일배설률을 나타냈으며, 갑상선을 제외한 각 장기는 투여 2시간 후 최고 잔류율 및 최고 소변 일일배설률을 보이나, 이후 감소하는 경향을 보였다. 또한 잔류율이 높은 장기 순서는 갑상선을 제외하고 위, 왼쪽 신장, 간, 소장, 오른쪽 신장으로 나타났다. 본 연구를 통해 산출된 방사성옥소의 섭취 24시간 후 갑상선 잔류율 변화는 기존 30%로 보고된 ICRP-54/67 및 25% 잔류율로 보고된 ICRP-78의 자료와 차이를 나타냈다. 한국인의 특성에 맞는 체내 흡수선량 평가의 올바른 접근과 그에 따른 기초 자료의 확보는 향후 원자력 발전소의 작업 종사자 내부피폭 및 임상에서 발생 가능한 체내 피폭의 정량적 평가에 도움을 줄 수 있을 것으로 판단된다.
