Background: Radioiodine therapy has been widely used for thyroid disease patients, but hyposalivation and xerostomia may occur in 10~30% of patients. Sialocentesis is a procedure that removes inflammatory substances in the salivary duct and expands the duct for the secretion and delivery of saliva. In this study, thyroid disease patients treated with radioactive iodine were selected among the patients with xerostomia who visited the hospital, and the effect of sialocentesis was compared and analyzed. And then, comparison between the radioiodine therapy-experienced group and the non-radioiodine therapy-experienced group was conducted. Results: In this study, we studied xerostomia patients who underwent radioiodine therapy due to thyroid diseases and who underwent sialocentesis at the Korea University Anam Hospital. Sialocentesis is conducted by one surgeon. The study also compares the clinical symptoms before and after the surgery. After the procedure, the discomfort due to xerostomia was reduced, and the symptom was improved effectively. Conclusions: The results of this study showed that sialocentesis has a clinical effect in the treatment of xerostomia, which is a side effect of radioiodine therapy. In addition, the possibility of further clinical application of sialocentesis in the future is found.

Background and Objectives: Recently the tenn 'papillary microcarcinoma' has been proposed to designate carcinoma of 10 mm or less in diameter. In some cases, cervical lymph node metastasis preceding the occurrence of the primary tumor may be the first and sole manifestation of the disease. The objective of this study is to assess the clinical features of cervical metastasis in papillary microcarcinoma of thyroid glands. Materials and Methods: 9 cases with papillary microcarcinoma with neck metastasis were analyzed retrospectively. 5 cases are men and 4 are women. All patients complained of painless, movable neck mass. The symptom had been present from 1 month to 36 months. We reviewed clinical history, imaging studies, the results of fine needle aspiration, the surgical method, the pathologic results. Results: In 9 cases, no abnormalities of the thyroid gland were shown by imaging studies and thyroid scan. 3 cases were diagnosed by fine needle aspiration cytology. the others were not. Total thyroidectomy and neck dissection were performed in 9 cases and then pathology reports showed 2 case of multiple, 2 case of contralateral single and 5 cases of unilateral single thyroid microcarcinoma. They have no recurrence during follow-up period. Conclusions: Cervical metastasis from papillary microcarcinoma is variable clinical manifestation. The diagnosis of cervical metastasis from papillary microcarcinoma should be considered in patient with neck mass. We recommend total thyroidectomy with neck dissection and postoperative radioactive iodine ablation therapy in neck metastasis from papillary thyroid microcarcinoma.

Background and Objectives : The main purpose of this study is to clarify perceptual and acoustic analysis for the patients with hypothyroidism after thyroidectomy especially focused on the characteristics of speech articulation with special reference to the consonant production. Materials and Methods : The subjects of the research were 40 male and female adults (males : 5, females : 35). They were all received radioactive iodine treatment which after total thyroidectomy. Voice samples were collected during the three stages of after surgery, pre-radioisotope treatment (RIT), and post-RIT. The acoustic analysis was conducted by using Pratt (ver.5.2.21) after measuring voice onset time (VOT). The subjective evaluation of the voices used CAPE-V. Results : A significant decrease in overall severity was displayed in the CAPE-V following RIT. It may be conjectured that this is connected to the change in voice following RIT. The loudness of the sound displayed a significant decrease in the CAPE-V following RIT. It is conjectured that this is connected to the decrease in vocal intensity following RIT. No statistically significant results were revealed for the comparative analysis on the voice onset time (VOT) in all plosives during the three periods. Conclusion : Perceptually, the overall severity of the voice with hypothyroidism was changed significantly before and after RIT. Eventhough VOT were not significantly changed, it tended to decrease VOT in patients with hypothyroidism.

L-thyroxine (LT4) withdrawal prior to radioactive iodine (RAI) ablation therapy is a commonly used method for successful treatment of patients with papillary thyroid cancer (PTC). However, a prolonged period of hypothyroidism induced by LT4 withdrawal is sometimes associated with impaired quality of life and cardiopulmonary dysfunction in PTC patients. Furthermore, LT4 withdrawal may have a trophic effect on residual cancer by means of increased thyrotropin. We report on 2 cases of metastatic PTC patients with malignant pleural effusion (MPE) whose disease showed rapid worsening after LT4 withdrawal and RAI therapy. The first case is a 65-year-old woman who had PTC with multiple distant metastases and MPE. During LT4 withdrawal for RAI therapy, MPE showed rapid worsening, and the patient required repetitive therapeutic thoracentesis. The second case is a 49-year-old woman with PTC who underwent 3 additional operations for cancer recurrence in the neck lymph nodes and 6 times of RAI treatments. While preparing for the $7^{th}$ RAI treatment by withdrawing LT4, she developed MPE which became progressively aggravated after RAI therapy. Both patients experienced increased pleural effusion during the LT4 withdrawal period and a rise in the thyroglobulin level was observed after RAI therapy. MPE was not controlled with therapeutic thoracentesis and pleurodesis. Eventually, both patients died of rapid disease progression after RAI therapy. In summary, LT4 withdrawal may have an adverse effect on metastatic PTC patients, particularly those with MPE.
The methods for determining C-14 and tritium contents in the spent nuclear fuel sample were developed. The carbon-14($^{14}CO_2$) released during the dissolution of the spent fuel sample and $CaCO_3$ ($CO_2$ carrier) with 8 M $HNO_3$ at $90^{\circ}C$ was collected in trap containing 1.5 M NaOH. The volatile radioactive iodine evolved when the spent fuel was dissolved, was trapped on to Ag-silicagel (Ag-impregnated silicagel) adsorbent in column which is connected to two NaOH traps. The solutions which contain tritium as HTO after fuel dissolution were decontaminated by deionization with a mixture of cation and anion exchange resins and inorganic ionexchangers. The amount of C-14 in the trap solutions and the HTO concentration in the resulting deionization water were then determined by liquid scintillation counting.
Objectives: The development of thyroid tumor has a relationship with carcinogen, oncogene and tumor suppressor gene. With aminotriazole, radioactive iodine and nitrosomethylurea as carcinogens in rat, authors investigate the incidence in type of the thyroid tumors, p21 and p53 protein expression pattern by immunohistochemical stain and the relationship between the tumors and p21-p53 protein expressions. Materials and Methods: 80 experimental animals were divided into four groups; group 1(control, no carcinogen, n=20), group 2(oral administration of aminotriazole for 36 weeks, n=20), group 3(intraperitoneal injection of 131I for one time and oral administration of aminotriazole for 36 weeks, n=20), group 4(oral administration of nitrosomethylurea for 3 days and aminotriazole for 36 weeks, n=20). After 40 weeks they were sacrificed with pathologic examination and we performed immunohistochemical staining with pan-ras monoclonal antibody for p21 protein and CMI polyclonal antibody for p53 protein with paraffin-embedded specimens. Results: 1) No tumors were observed in group I, but 38.3% of nodular goiters, 11.7% of adenomas and 50.0% of carcinomas were observed in carcinogen treated groups(group 2, 3, 4). 2) The incidence of nodular goiter, adenoma and carcinoma were 70%, 20% and 10% in group 2, 40%, 15% and 45% in group 3 and 5%, 0% and 95% in group 4. 3) p21 protein was not expressed in normal thyroid tissues but was expressed in 26.1% of nodular goiters, 42.9% of adenomas and 6.7% of carcinomas. On the other hands, p53 protein was not expressed in normal thyroid tissues, nodular goiters, adenomas and in well differentiated thyroid carcinomas by immunohistochemical stain. Conclusion: The authors suggest that aminotrizole, 131I, nitrosomethylurea can be etiologic agents in the development of thyroid tumor and the p21 protein can be expressed in the early stage and in benign condition of thyroid tumor but p53 protein is not expressed in all conditions of development in rats.

When a patient with thyroid cancer is released from isolation after I-131 treatment and return to home using a vehicle, travel time should be controlled to reduce the amount of radiation to accompanying person. As the calculation of appropriate travel time is difficult, there is no patient-specific guideline until now. If we assume that there is no excretion and no physical decay during the relatively short travel time, calculation become quite simple; total radiation dose = dose rate ${\times}$ travel time. Results of this simple calculation and conventional calculation were compared using datum from 120 patients. Travel time calculated by simple method was 56% of conventional method in 0.3 m, 91% in 0.5 m and 96% in 1 m. Simple method was safe. It can be applied easily and also can be applied to the patients with hyperthyroidism treated by I-131.
Thyroglossal duct cyst (TGDC) carcinoma generally shows a favorable prognosis. If metastasis is present latently, it may not threaten the patient's life immediately. It has been shown, however, that larger than 1 cm papillary carcinoma (PC), level VI metastasis to the lymph node (LN), which is the nearest to the thyroid, independently predicts a worse prognosis. In the case presented herein, a 61-year-old female patient was diagnosed with an about 3 cm PC in the TGDC, particularly the columnar variant subtype, one of the aggressive variants. She had occult papillary thyroid microcarcinoma, but no LN metastasis. Even though she underwent the Sistrunk procedure and total thyroidectomy with central compartment neck dissection followed by high-dose radioactive iodine remnant ablation, however, the cancer cells spread to level IV neck LN, and finally to the lung. Therefore, when a patient is diagnosed with an aggressive histologic variant of PC in the TGDC, even without LN metastasis, the invasive surgical approach and close postoperative surveillance are necessary, with consideration of the risk of disease progression. Therefore, if it is possible to stratify the risk for patients, higher-risk patients can be offered a more invasive therapeutic approach.
Purpose: We conducted a comparative study to evaluate the diagnostic values of T1-201, Tc-99m MIBI and I-131 scans in the follow-up assessment after ablative I-131 therapy in differentiated thyroid cancer. Materials and Methods: The study population consisted of 20 patients who underwent surgical removal of thyroid cancer and ablative radioactive iodine therapy, and followed by one or mote times of I-131 retreatments (33 cases). In all patients, T1-201, Tc-99m MIBI, diagnostic and therapeutic I-131 scans were performed and the results were analyzed retrospectively. Also serum thyroglobulin levels were measured in all patients. The final diagnosis of recurrent or metastatic thyroid cancer was determined by clinical, biochemical, radiologic and/or biopsy findings. Results: Positive rates (PR) of Tc-99m MIBI, T1-201, diagnostic and therapeutic I-131 scans in detecting malignant thyroid tissue lesions were 70% (19/27), 54% (15/28), 35% (17/48) and 63% (30/48), respectively. The PR in the group of 20 cases (28 lesions) who underwent concomitant T1-201 and I-131 scans were in the order of therapeutic 131 scan 71%, T1-201 scan 54% and diagnostic I-131 scan 36%. There was no statistically significant difference between T1-201 and diagnostic I-131 scans (p>0.05). In the group of 20 cases (27 lesions) who underwent concomitant Tc-99m MIBI and I-131 scans, the PR were in the order of Tc-99m MIBI scan 70%, I-131 therapeutic scan 52% and I-131 diagnostic scan 33%. The PR of Tc-99m MIBI was significantly higher than that of diagnostic I-131 scan (p<0.05). Conclusion: Tc-99m MIBI scan is superior to diagnostic I-131 scan in detecting recurrent or metastatic thyroid cancer following ablation therapy in patients with differentiated thyroid cancer. T1-201 scan did not showed significantly higher positive rate than diagnostic I-131 scan. Instead of diagnostic I-131 scan before the I-131 retreatment, Tc-99m MIBI scan without discontinuing thyroid hormone replacement would be a prudent and effective approach in the management of these patients.
Objectives: It is certain that Radioactive iodine thyroid uptake(RAIU) rate should be measured with the standard counts considering the thyroid gland depth in enlarged thyroid patients for the variation from geometric factors. The purpose of this paper is to consider the effects of geometric factors according to detector to source distance and the effective thyroid depth on RAIU rate with experiment test. Materials and Methods: I-131 370 kBq ($10{\mu}Ci$) point source was measured by Captus-3000 thyroid uptake system (Capintec, NJ, USA) with a change Detector-Source Distance from 20 cm to 30 cm at an interval of 1 cm. And we changed the Neck phantom surface-Source Depth in the phantom with 1 cm, 2 cm, 5 cm using the neck phantom in order to reproduce the effective thyroid depth. Results: Every experimental group follows power curve as inverse square curve ($$R2{\geq_-}0.915$$). The average count rates in the case not using a phantom and the every case applied the effective thyroid depth using a phantom was not identical each other. There was significant fluctuations upon the effective thyroid depths applied the effective thyroid depth above 1 cm in $364.4 keV{\pm}10%$ energy ROI (p<0.01). There was not significant difference between the count rates of 1 cm and 2 cm in $364.4keV{\pm}20%$ and $637.1keV{\pm}6.2%$ (p=0.354, p=0.397). In assumed RAIU rate from regression equation, $364.4keV{\pm}20%$ was lower difference than $364.4keV{\pm}10%$ as 6.42% and 5.09% per 1 cm. Every change of count rate upon depth appears decreased line on Linear Regression, but the case of $284.3keV{\pm}10%$ increased only. And also, The graphs of coefficient of variation upon depth increased as straight line on every experimental group. Conclusion: The result appears that application of $364.4keV{\pm}20%$ energy ROI is more suitable for reducing error from the effective thyroid depth. And also, we can estimate the error of 20 cm should be highly reduced than 30 cm for Inverse Square Law. Therefore, If there is not information of the thyroid depth, it is considered that the error from thyroid depth can reduce through set up energy ROIs for $364.4keV{\pm}20%$, and increase Detector-Source Distances.
