Radiotherapy Technique of High Energy Electron (고에너지 전자선의 방사선 치료 기술)

  • High energy electron beams took effect for tumor radio-therapy, however, had a lot of problems in clinical application because of various conversion factors and complication of physical reactions. Therefore, we had experimentally studied the important properties of high energy electron beams from the linear accelerator, LMR-13, installed in Yonsei Cancer Center. The results of experimental studies on the problems in the 8, 10, 12 Mev electron beam therapy were reported as following. 1. On the measurements of the outputs and absorbed does, the ionization type dosimeters that had calibrated by $^{90}Sr$ standard source were suitable as under $3\%$ errors for high energy electrons to measure, but measuring doses in small field sizes and the regions of rapid fall off dose with ionization chambers were difficult. 2. The electron energy were measured precisely with energy spectrometer consisted of magnet analyzer and tele-control detector and the practical electron energy was calculated under $5\%$ errors by maximum range of high energy electron beam in the water. 3. The correcting factors of perturbated dose distributions owing to radiation field, energy and material of the treatment cone were checked and described systematically and variation of dose distributions due to inhomogeneous tissues and sloping skin surfaces were completely compensated. 4. The electron beams, using the scatters; i.e., gold, tin, copper, lead, aluminium foils, were adequately diffused and minimizing the bremsstrahlung X-ray induced by the electron energy, irradiation field size and material of scatterers, respectively. 5. Inproving of the dose distribution from the methods of pendulum, slit, grid and focusing irradiations, the therapeutic capacity with limited electron energy could be extended.

  • $PKC{\delta}$-catalytic V5 Heptapeptide (FEQFLDI, FP7) interacts with heat shock protein 27 (HSP27) and inhibits HSP27-mediated resistance to cell death against various stimuli including radiation therapy. Here, we prepared radio-iodinated heptapeptide and further investigated its uptake properties in HSP27 expression cells. Peptide sequence of FP7 and a negative control peptide (WSLLEKR, QP7) was modified by substituting their C-terminus residue to tyrosine (FP6Y and QP6Y) to label radio-iodine. Iodinated peptides were confirmed by LC mass analysis with cold iodine reaction mixture. Accumulation of [$^{125}I$]iodo-FP6Y and [$^{125}I$]iodo-QP6Y in NCI-H1299 cell line, with higher level of HSP27, and NCI-H460 cell line, with lower level of HSP27, was measured by NaI(Tl) scintillation counter. The modification of substituting C-terminus residue of FP7 to tyrosine (FP6Y) did not affect its interaction with HSP27. Accumulation of [$^{125}I$]iodo-FP6Y in NCI-H1299 cells was 3 fold higher than in NCI-H460 cells. The novel radio-iodinated FP6Y would be used as a tracer for targeting HSP27 protein.

  • The effect of radio- and chemotherapy for cancer are excellent, but their toxicities to normal tissue and organ of the body is relatively strong, which leads secondary side effect to patients during therapies. Particularly, due to the response for bone marrow suppression such as agranulocytosis limits the therapy periods and dose of drugs, new drug development that reproduces lymphocytes has been focused.   • The definite molecular mechanisms underlying the genesis of nasopharyngeal carcinomas (NPCs) remain to be completely elucidated. miRNAs are small non-coding RNAs which are implicated in cell proliferation, apoptosis, and even carcinogenesis through negatively regulating gene expression post-transcriptionally. EBV was the first human virus found to express miRNAs. EBV-encoded BART-miRNAs and dysregulated cellular miRNAs are involved in carcinogenesis of NPC by interfering in the expression of viral and host cell genes related to immune responses and perturbing signal pathways of proliferation, apoptosis, invasion, metastasis and even radio-chemo-therapy sensitivity. Additional studies on the roles of EBV-encoded miRNAs and cellular miRNAs will provide new insights concerning the complicated gene regulated network and shed light on novel strategies for the diagnosis, therapy and prognosis of NPC.

  • 현재 선진국에서는 고출력 양성자 선형 가속기를 기반으로 한 의료용 암치료기인 BNCT(Boron Neutron Capture Therapy)에 대해 활발히 연구 중이며 다원시스도 2016년부터 A-BNCT 사업을 진행 중이다. A-BNCT에 적용된 양성자 선형 가속기의 RF(Radio Frequency)전원을 공급하기 위해 352 MHz, 1.5 MW의 고출력을 가지는 클라이스트론을 사용하였다. 클라이스트론의 출력인 RF의 크기와 위상을 안정적으로 제어하기 위해 90 kV, 30 A, 120 Hz, 1.7 ms의 구형파 출력을 가지는 고전압 전원장치를 적용하였다. 또한 고전압 전원장치의 출력전압 변동률을 0.5% 이내로 유지시키기 위해 전압보상회로를 적용하여 회로 시뮬레이션과 실부하 실험을 통해 펄스전원장치의 성능을 검증하였다.

Clinical outcome of high-dose-rate interstitial brachytherapy in patients with oral cavity cancer

  • Purpose: To evaluate the clinical outcome of high-dose-rate (HDR) interstitial brachytherapy (IBT) in patients with oral cavity cancer. Materials and Methods: Sixteen patients with oral cavity cancer treated with HDR remote-control afterloading brachytherapy using $^{192}Ir$ between 2001 and 2013 were analyzed retrospectively. Brachytherapy was administered in 11 patients as the primary treatment and in five patients as salvage treatment for recurrence after the initial surgery. In 12 patients, external beam radiotherapy (50-55 Gy/25 fractions) was combined with IBT of 21 Gy/7 fractions. In addition, IBT was administered as the sole treatment in three patients with a total dose of 50 Gy/10 fractions and as postoperative adjuvant treatment in one patient with a total of 35 Gy/7 fractions. Results: The 5-year overall survival of the entire group was 70%. The actuarial local control rate after 3 years was 84%. All five recurrent cases after initial surgery were successfully salvaged using IBT ${\pm}$ external beam radiotherapy. Two patients developed local recurrence at 3 and 5 months, respectively, after IBT. The acute complications were acceptable (${\leq}grade$ 2). Three patients developed major late complications, such as radio-osteonecrosis, in which one patient was treated by conservative therapy and two required surgical intervention. Conclusion: HDR IBT for oral cavity cancer was effective and acceptable in diverse clinical settings, such as in the cases of primary or salvage treatment.

  • Colorectal cancer (CRC) is one of the major healthcare problems worldwide and its processes of genesis include a sequence of molecular pathways from adenoma to carcinoma. The discovery of microRNAs, a subset of regulatory non-coding RNAs, has added new insights into CRC diagnosis and management. Together with several causes of colorectal neoplasia, aberrant expression of oncomiRs (oncogenic and tumor suppressor miRNAs) in cancer cells was found to be indirectly result in up- or down-regulation of targeted mRNAs specific to tumor promoter or inhibitor genes. The study of miRNAs as CRC biomarkers utilizes expression profiling methods from traditional tissue samples along with newly introduced non-invasive samples of faeces and body fluids. In addition, miRNAs could be employed to predict chemo- and radio-therapy responses and be manipulated in order to alleviate CRC characteristics. The scope of this article is to provide a comprehensive review of scientific literature describing aberrantly expressed miRNAs, and consequently dysregulation of targeted mRNAs along with the potential role of miRNAs in CRC diagnosis and prognosis, as well as to summarize the recent findings on miRNA-based manipulation methods with the aim of advancing in anti-CRC therapies.

This study was designed to develop a digilog game play therapy contents that was combined analog and digital. We infer that this contents is effective game play therapy not only for ordinary people but also for dementia old man, or intellectual disorder man. Motion Recognition for Kinect Sensor Data Using Machine Learning Algorithm with PNF Patterns of Upper Extremities

  • Purpose: The purpose of this study was to investigate the availability of software for rehabilitation with the Kinect sensor by presenting an efficient algorithm based on machine learning when classifying the motion data of the PNF pattern if the subjects were wearing a patient gown. Methods: The motion data of the PNF pattern for upper extremities were collected by Kinect sensor. The data were obtained from 8 normal university students without the limitation of upper extremities. The subjects, wearing a T-shirt, performed the PNF patterns, D1 and D2 flexion, extensions, 30 times; the same protocol was repeated while wearing a patient gown to compare the classification performance of algorithms. For comparison of performance, we chose four algorithms, Naive Bayes Classifier, C4.5, Multilayer Perceptron, and Hidden Markov Model. The motion data for wearing a T-shirt were used for the training set, and 10 fold cross-validation test was performed. The motion data for wearing a gown were used for the test set. Results: The results showed that all of the algorithms performed well with 10 fold cross-validation test. However, when classifying the data with a hospital gown, Hidden Markov model (HMM) was the best algorithm for classifying the motion of PNF. Conclusion: We showed that HMM is the most efficient algorithm that could handle the sequence data related to time. Thus, we suggested that the algorithm which considered the sequence of motion, such as HMM, would be selected when developing software for rehabilitation which required determining the correctness of the motion.
