암환자의 영양, 통증 및 피로 관련 논문분석 (Analysis of Researches about Nutrition, Pain and Fatigue of Cancer Patients)

  • Objectives: This study is aimed to analyze the trend of research on nutrition, pain and fatigue of cancer patients in Korea, suggest direction for future research of nutrition, pain and fatigue of cancer patients. Methods: 74 studies published from 1991 to 2001 were examined according to the year of publication, types of journal, research design, types of disease, care methods, major concepts, tool and research findings. Results: 1) The number of studies related nutrition, pain and fatigue of cancer patients had increased rapidly since the 1996's(78.4%). 2) 42 nutrition, pain and fatigue of cancer patients studies(56.1%) were done for a thesis for a degree and 32 were nondegree research studies(43.9%). 3) 70 studies(94.7%) were quantitative study, which included in 40 descriptive studies(54.1%), 22 experimental studies (29.75), 5 correlative studies(6.8%), 2 comparative studies(2.7%) and 1 case study(1.4%), and 4 studies(5.3%) were qualitative study, which included in 3 content analysis studies(3.9%) and 1 phenomenological study(1.4%). 4) Researches about cancer more than 2 were the most by 48 studies (66.1%), following leukemia researches were 8 studies(10.8%), breast cancer researches were 7 studies(9.5%), gastric cancer researches were 4 studies(5.4%), pediatric cancer researches were 3 studies(4.1%), uterine cancer researches were 2 studies(2.7%). 5) Researches about chemotherapy were the most by 39 studies(52.6%), following analgesic researches were 14 studies(18.9%), researches that do not present treatment method were 9 studies(12.2%), radiation researches were 7 studies(9.5%). 6) In 22 experimental studies, the effects of 13 types of nursing interventions were tested. Research findings were effective almost but muscle relaxation therapy to decrease nausea and vomiting was no effect. Conclusion: We need more researches about research of nutrition, pain and fatigue of cancer patients, especially need to prove the effect of intervention or program for nutrition, pain and fatigue of cancer patients by experimental research designs and need more qualitative researches to identify indepth the meaning of nutrition, pain and fatigue of cancer patients.

혈액 투석용 카테터에 의한 상대정맥증후군의 수술적 치료 -2예 보고- (Surgical Treatment of Superior Vena Cava Syndrome Caused by Hemodialysis Catheter - Report of 2 cases-)

  • 상대정맥증후군의 원인은 악성과 비악성 두 가지로 나눌 수 있다. 항암 화학요법이나 방사선 치료에 반응이 없는 악성 종양에 의한 상대정맥증후군의 경우, 수술보다는 방사선학적 중재적 시술로 치료하는 것이 일반적이다. 그러나 비악성 원인에 의한 경우 수술과 중재적 시술 중 어느 것이 우수한지 아직 정립되어 있지 않다. 저자들은 혈액 투석용 카테터에 의한 상대정맥증후군 환자 2명에 대해 수술적 치료를 시도하였다. 2명 모두 중재적 시술이 불가능하거나 실패한 경우였다. 첫 번째 환자는 expanded polytetrafluoroethylene를 이용하여 무명정맥-우심방 단락술을 시행하였고, 다른 한 환자는 혈전 제거술과 함께 자가 심막을 이용하여 상대정맥과 무명정맥의 첩포 확장술을 시행하였다. 두 환자 모두 증상의 현저한 호전이 있었으며, 수술 후 시행한 혈관 조영술에서 우회혈관의 현저한 저하가 관찰되었다.

소아암 환자에서 방사선 치료 후 영구치 치근발육장애 : 증례보고 (DEVELOPMENTAL DISTURBANCE OF PERMANENT TEETH AFTER RADIOTHERAPY FOR TREATMENT OF MALIGNANT TUMOR : REPORT OF CASES)

  • 악성종양을 치료하는데 있어서 수술, 화학요법, 방사선 치료 등을 포함한 항암치료는 악성도의 증가를 방지할 뿐만 아니라 항암제의 독성과 용량을 줄여주는 역할을 한다. 방사선 치료의 효과는 일반적으로 수회에 걸쳐 조사시 종양이 더 효과적으로 파괴된다. 환자는 주로 두경부에 국소적으로, 때로는 전신에 걸쳐 방사선을 조사받게 된다. 그러나 방사선 요법의 부작용으로 구강건조증, pH가 낮은 타액의 생성, 구강미생물의 변화 등으로 인한 방사선 우식증과 미각 이상, 점막염 등 다양한 구강내 합병증을 동반하게 된다. 그리고 성장하고 있는 소아의 경우에는 발육 중인 치열과 지지구조들에 큰 영향을 줄 수 있다. 치근 발육의 장애, 법랑질 형성부전, 왜소치, 무치증, 치아맹출 장애, 하악이나 상악의 저형성 등과 같은 다양한 현상이 나타날 수 있으며 이러한 현상은 비가역적이다. 특히 발생과정 중에 있는 치아는 그 발생단계에 따라 영향을 받는다. 본 증례는 치열 발육 단계에서 국소적 혹은 전신적으로 방사선 치료를 받은 어린이에서 나타난 영구치 치근 발육장애에 대해 보고하는 바이다. 이 환자들은 생후 $3{\sim}4$세 경에 악성 종양 치료를 위해 방사선 치료를 받았고 영구치 치근의 저형성, 조기 치수 폐쇄 소견을 보였다. 방사선 조사로 인한 구강환경 변화로 인해 우식발생률이 높으므로 철저한 구강 위생교육과 불소도포 등이 필요할 것이며, 짧은 치근을 가진 치아는 동요도의 증가 여부와 관리를 위해 정기적인 방사선 촬영 및 검진이 필요하다.

유방암 생존자의 장기간 복합 운동중재에 따른 기능적 체력과 골밀도의 변화 (The Change of Functional Fitness and Bone Mineral Density on a Long-Term Combined Exercise Intervention in Breast Cancer Survivors.)

  • 본 연구는 유방암 절제술을 받은 40-60대의 기혼 중년을 대상으로 복합운동을 1년간 실시하여 골다공증약과 운동을 병행한 군과 운동군으로 나누어 기능적 체력과 골밀도에 미치는 효과를 분석하고 이에 따른 효과적인 운동프로그램의 기초 자료를 제공하는데 있다. 대상자총 24명을 두군으로 나누어, 이중 18명은 골다공증 범위(T-score=$-2.2{\pm}0.8$)에 있으며 복합운동만을 실시하였다. 나머지 6명은 골다공증진단(T-score=$-4.6{\pm}0.9$)을 받고 골다공증약(Alendronate 70 mg/주 당)을 처방 받아 복용하면서 복합운동을 병행하여 실시하였다. 본 연구 결과, 기능적 체력 요소 중 좌전굴, 악력, 윗몸일으키기에서는 시기간에 유의한 향상을 보였다. 12개월 후, 골 밀도의 변화는 복합운동군(N=18)에서는 변화가 없었으며, 복합운동과 골다공증약 복용(N=6)을 병행하고 있는 그룹은 유의한 개선을 나타냈다. 따라서 하타요가, 탄성밴드 및 볼 운동이 포함된 장기간 점진적 강도의 복합운동프로그램이 유방암절제술 여성의 기능적 체력의 개선 및 골밀도 지연에 효과가 있었고, 특히 운동과 골다공증약을 병행한 그룹에서 골다공증개선에 긍정적인 영향을 미쳤으므로, 유방암 수술 후 회복기 운동프로그램으로 적극 활용할 수 있도록 제안한다. 앞으로 대상자 수를 늘려 골다공증약과 운동과의 병행관계를 심층적으로 연구하고, 유방암 수술 후 운동중재프로그램의 유형 및 강도를 달리하여 유방암과 골밀도 및 운동과의 관련성을 연구 해 볼 필요가 있겠다.

Retrospective Appraisal of Cancer Patients from Vientiane Capital City, Lao People's Democratic Republic (PDR), Seeking Treatment in Thailand

  • Background: Recent estimates suggest that in the Lao People's Democratic Republic (Lao PDR) the burden of cancer in terms of DALYs lost is amongst the highest in South East Asia. As such, increasingly cancer is becoming an important public health concern in the country. Lao PDR however has no population-based cancer registry and only one hospital-based registry. Cancer treatment within the country is extremely limited. Patients who can, may travel to neighboring countries for treatment, but little information about this is available in the country. The aim of this study was to estimate some of the otherwise largely unknown parameters of the cancer burden in Lao PDR. Materials and Methods: This is a retrospective, descriptive study based on the records of 847 Lao cancer cases treated with surgery, radiation and chemotherapy at Srinagarind Hospital, Khon Kaen University, in Thailand between 1988 and 2010. Results: The annual rate of registration of Lao cancer cases fluctuated, but showed an increasing trend. Most cancers were diagnosed by histology (65.2%), and a combination of endoscopy and radiology (15.6%). In most cases (70.2%) the stage of cancer at diagnosis could not be determined. In those whose stage could be identified, 54.0% were at the final stage (Stage IV). Among males, the commonest cancer sites were the liver (16.1%), blood (12.3%) and nasopharynx (10.6%). Those in female patients were the cervix (22.2%), breast (14.6%) and blood (8.1%). Conclusions: This study indicates that despite some fluctuations, the number of Lao cancer patients presenting at Srinagarind Hospital, Khon Kaen, gradually increased between 1988 and 2010. The unfavorable pattern of late-stage cancer diagnosis among male and female patients suggests a need for cancer control interventions and the establishment of cancer registration and treatment facilities within Lao PDR.

  • 악성 조직의 온열 치료는 성공적인 암 치료 방법의 하나로서 방사선 치료 및 화학 요법에 비해 생체 적합성이 우수하고 비교적 온화한 조건에서 사용할 수 있어 최근 큰 주목을 받고 있다. 본 연구에서는 온열 치료를 목적으로 생체 적합성 고분자인 poly(2-methacryloyloxyethyl phosphorylcholine/fluorescein O-methacrylate) (P(MPC/FOM))를 코팅한 초상자성 산화철 나노 입자 (IONP)를 제조하고 관련 특성을 분석하였다. 15 nm 직경을 갖는 IONP는 먼저 공침법에 의해 제조된 후, 4-cyanopentanoic acid dithiobenzoate (CTP) 을 사용하여 IONP의 표면을 개질하였으며, 이 후 MPC 및 FOM 단량체의 reversible addition-fragmentation chain transfer (RAFT) 공중합을 통해 P(MPC/FOM)의 코로나 층을 형성시켰다. 투과 전자 현미경 (TEM)과 동적 광 산란 (DLS) 분석을 통해 IONP@P(MPC/FOM)의 형태 및 수력학적 크기를 확인할 수 있었으며, 열 중량 분석 (TGA)을 통해 P(MPC/FOM) 코로나 층의 형성을 확인하였다. 또한 교류 자기장을 이용해 IONP 분산액을 노출시킨 결과, 0.2 중량 %의 IONP @ P(MPC / FOM) 수분산액이 온열 치료에 사용될 수 있음을 확인하였다.

비인강암의 방사선치료성적 (Radiation Therapy of Nasopharyngeal Cancers)

  • 1986년 4월부터 1992년 3월까지 6년간 영남대학교 의과대학 부속병원 치료방사선과에 내원하여 근치적인 방사선치료를 시행한 후 6개월 이상 추적조사가 가능하였던 31명의 치료성적은 다음과 같다. 대상환자 31명중 남자가 22명으로 여자보다 2.4배 많았으며 30세미만 3명을 제외하면 30대에서 60대까지 고른분포를 보였다. 병리조직학적 분류상 편평세포암이 13명, 미분화세포암이 18명이었다. 임상적병기는 I기 1명, II기 2명, III기 6명, IV기 22명이었다. 31명중 방사선치료만 시행된 경우가 11명, 항암제등과 병합치료를 시행한 경우가 20명이었다. 편평세포암 환자 11명중 6명에서 방사선치료만을 시행하였고 5명에서 병합치료를 시행하였으나, 미분화세포암에서는 18명중 5명은 방사선치료만을 시행하였고 13명은 병합치료를 실시하였다. 치료에 대항 반응은 치료방법에 따른 차이가 없었으나, 방사선치료만 시행한 경우에 편평세포암 6명중 3명이 완전관해를 보인 반면에, 미분화세포암 5명 모두 완전관해를 보여 큰 차이를 보였으나 병합치료에서는 세포형태에 따른 차이가 소실되었다. 치료 후 1년 생존율은 치료방법에 따른 차이가 없었다. 완전관해를 보인 환자 22명중에서 방사선치료만 시행한 8명중 6명에서 재발을 보였으며 그중 3명은 원격전이를 동반하였으나, 병합치료를 시행한 14명중에서 3명만이 재발을 보였으며 그중2명은 원격전이를 보였다. 이상의 결과에서 치료방법에 따른 관해율은 비슷하였으나 치료 후 추적과정에서 발생하는 재발은 방사선치료만 시행한 경우보다 병합치료를 실시한 경우에 현저히 줄어듬을 보여주어 비인강암에 대한병합치료가 국소재발의 감소 및 원격전이의 감소를 통하여 생존율의 향상에도 기여하리라 사료된다.

Clinical Application of Endoscopic Inguinal Lymph Node Resection after Lipolysis and Liposuction for Vulvar Cancer

  • Aim: To examine lymph nodes obtained after lipolysis and liposuction of subcutaneous fat of the inguinal region of female vulvar cancer patients to explore the feasibility of clinical application. Methods: The field of operation was on the basis of the range of the conventional resection of inguinal lymph nodes. We injected lipolysis liquid fanwise, started liposuction after 15-20 minutes; then the subcutaneous fatty tissue was sucked out clearly by suction tube. We selected the first puncture holes located on 2-3 cm part below anterior superior spine, the others respectively being located 3cm and 6cm below the first for puncturing into the skin, imbedding a trocar to intorduce $CO_2$ gas and the specular body, and excise the lymph nodes by ultrasonic scalpel. The surgical field chamber was set with negative pressure drainage and was pressured with a soft saline bag after surgery. Results: A lacuna emerged from subcutaneous of the inguinal region after lipolysis and liposuction, with a wide fascia easily exposed at the bottom where lymph nodes could be readily excised. The number of lymph nodes of ten patients excised within the inguinal region on each side was 4-18. The excised average number of lymph nodes was 11 when we had mature technology. Conclusion: Most of adipose tissue was removed after lipolysis and liposuction of subcutaneous tissue of inguinal region, so that the included lymph nodes were exposed and easy to excise by endoscope. This surgery avoided the large incision of regular surgery of inguinal region, the results indicating that this approach is feasible and safe for used as an alternative technology.

Gamma Knife Radiosurgery for Brainstem Metastasis

  • Objective : Brainstem metastases are rarely operable and generally unresponsive to conventional radiation therapy or chemotherapy. Recently, Gamma Knife Radiosurgery (GKRS) was used as feasible treatment option for brainstem metastasis. The present study evaluated our experience of brainstem metastasis which was treated with GKRS. Methods : Between November 1992 and June 2010, 32 patients (23 men and 9 women, mean age 56.1 years, range 39-73) were treated with GKRS for brainstem metastases. There were metastatic lesions in pons in 23, the midbrain in 6, and the medulla oblongata in 3 patients, respectively. The primary tumor site was lung in 21, breast in 3, kidney in 2 and other locations in 6 patients. The mean tumor volume was $1,517mm^3$ (range, 9-6,000), and the mean marginal dose was 15.9 Gy (range, 6-23). Magnetic Resonance Imaging (MRI) was obtained every 2-3 months following GKRS. Follow-up MRI was possible in 24 patients at a mean follow-up duration of 12.0 months (range, 1-45). Kaplan-Meier survival analysis was used to evaluate the prognostic factors. Results : Follow-up MRI showed tumor disappearance in 6, tumor shrinkage in 14, no change in tumor size in 1, and tumor growth in 3 patients, which translated into a local tumor control rate of 87.5% (21 of 24 tumors). The mean progression free survival was 12.2 months (range, 2-45) after GKRS. Nine patients were alive at the completion of the study, and the overall mean survival time after GKRS was 7.7 months (range, 1-22). One patient with metastatic melanoma experienced intratumoral hemorrhage during the follow-up period. Survival was found to be associated with score of more than 70 on Karnofsky performance status and low recursive partitioning analysis class (class 1 or 2), in terms of favorable prognostic factors. Conclusion : GKRS was found to be safe and effective for management of brainstem metastasis. The integral clinical status of patient seems to be important in determining the overall survival time.

Phase II Study on Dose Escalating Schedule of Paclitaxel Concurrent with Radiotherapy in Treating Patients with Locally Advanced Non-small Cell Lung Cancer

  • Objective: To evaluate clinical efficacy of a dose escalating schedule of paclitaxel concurrent with radiotherapy in treating patients with locally advanced non-small cell lung (NSCLC). Methods: Patients with locally advanced NSCLC were treated with conventional fractionated radiotherapy or three dimensional conformal radiotherapy (3 DCRT), concurrently with a dose escalating schedule of paclitaxel. All patients were divided into three groups, A with paclitaxel $30mg/m^2$, B with paclitaxel $60mg/m^2$ and C with paclitaxel $90mg/m^2$. Paclitaxel was repeated every week for a total of 4 or 6 weeks. Results: Among 109 patients, response rates were 68.8%, 71.1% and 71.8% (p>0.05) for group A (n=32), B (n=38), and C (n=39) respectively. Accordingly, disease control rates were 81.3%, 81.6% and 82.1% (p>0.05). Progression-free survival time was $8.0{\pm}5.0$ months, $11.6{\pm}6.1$ months, and $14.8{\pm}7.9$ months (p<0.05), respectively. Overall survival time was $15.4{\pm}7.6$ months, $18.2{\pm}8.0$ months, and $22.0{\pm}7.6$ months (p<0.05), one-year survival rates were 62.5%, 73.1% and 90.0% (p>0.05) and two-year survival rates were 31.3%, 38.5% and 50.0% (p<0.05). Main side-effects were bone marrow suppression, radiation related esophagitis and gastrointestinal reaction. Conclusion: In treating patients with NSCLC, concurrent chemoradiotherapy with paclitaxel improves early response compared with conventional fractionated radiotherapy or 3 DCRT. The survival rate was improved with the addition of paclitaxel, but there was an increase in adverse reactions when the dose of paclitaxel was increased.
