The Medial Sural Artery Perforator Flap versus Other Free Flaps in Head and Neck Reconstruction: A Systematic Review

  • The medial sural artery perforator (MSAP) flap is a versatile fasciocutaneous flap, and yet is less commonly utilized than other free flaps in microvascular reconstructions of the head and neck. The aim is to conduct a high-quality Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA)- and Assessment of Multiple Systematic Reviews 2 (AMSTAR 2)-compliant systematic review comparing the use of the MSAP flap to other microvascular free flaps in the head and neck. Medline, Embase, and Web of Science databases were searched to identify all original comparative studies comparing patients undergoing head and neck reconstruction with an MSAP flap to the radial forearm free flap (RFFF) or anterolateral thigh (ALT) flap from inception to February 2021. Outcome studied were the recipient-site and donor-site morbidities as well as speech and swallow function. A total of 473 articles were identified from title and abstract review. Four studies met the inclusion criteria. Compared with the RFFF and the ALT flaps, the MSAP flap had more recipient-site complications (6.0 vs 10.4%) but less donor-site complications (20.2 vs 7.8%). The MSAP flap demonstrated better overall donor-site appearance and function than the RFFF and ALT flaps (p = 0.0006) but no statistical difference in speech and swallowing function following reconstruction (p = 0.28). Although higher quality studies reviewing the use of the MSAP flap to other free flaps are needed, the MSAP flap provides a viable and effective reconstructive option and should be strongly considered for reconstruction of head and neck defects.

요골 두 및 경부 골절의 경피적 도수 정복술 (Percutaneous Mini-open Reduction for Mason II or III Radial Head and Neck Fracture)

  • 목적: Mason 분류 제 2형 및 일부의 3형 요골 두 및 경부 골절에 대하여 제한적 피부절개 및 경피적 도수 정복술 시행 후 기능 및 방사선학적 평가를 통해 임상적 결과를 분석하고자 하였다. 대상 및 방법: 2001년 5월부터 2007년 2월까지 Mason 분류 제 2형 또는 3형의 요골 두 및 경부 골절로 경피적 도수 정복술을 시행받은 13명의 환자 (요골 경부 골절: 9예, 요골 두 및 경부골절: 4예)를 대상으로 하였으며, 환자의 평균 연령은 29세 (11세~59세)였고 남자는 8명, 여자는 5명 이었다. 평균 추시 기간은 1년 6개월 (12개월~2년 9개월)이었다. 요골 두 및 경부의 도수 정복술은 방사선 투시기를 사용하여 골절부위를 확인 후, 골절부위 2~3 cm 원위부에서 1 cm 정도의 피부절개 후 경막 거상기를 이용하여 압박이나 감입 (impacted)형태의 요골 경부골절 및 두부의 부분 함몰된 관절면 골편의 정복을 시행하였다. 정복 후 불안정한 3예에 대해 1예에서 골절부의 경피적 핀고정술을 시행하였고, 2예에서 근위 요척골 간에 경피적 횡 핀고정을 추가 하였으며, 나머지 10예에서는 석고 부목 고정술만 시행하였다. 결과: 방사선학적 평가상 전 예에서 골유합이 관찰되었다. 평균 골유합 기간은 6.2주였다. 요골두의 각형성은 수술 전 평균 33.2도 (30도~42도)에서 수술 후 평균 7.8도 (4도~15도)로 감소하였다. 수술 직후에서 최종 추시시까지의 각변형은 0.7도 (-1도~2도) 였다. 수술 후 주관절의 운동 범위는 굴곡 평균 133.1도 (120도~140도), 신전 평균 7.3도 (0도~25도), 회내전은 80도(72도~90도), 회외전은 84.3도 (78도~90도)였다. Mayo Elbow Performance Index는 평균 96.2점 (80점~100점), American Shoulder and Elbow Surgeons elbow score는 평균 97점 (83점~100점), Disabilities of the Arm, Shoulder and Hand score는 평균 1.2점 (0점~5.8점) 으로 우수한 결과를 얻었다. 합병증으로 각각 한 예에서 일시적인 후골간 요골 신경 마비 증상과 경도의 주관절 외반 변형이 발생하였다. 결론: 경피적 도수 정복술은 관절내 독립적인 감입이 없는 Mason 분류 제 2형 및 일부의 3형 요골 두 골절 및 경부 골절의 치료에 있어서 간단하고 합병증 발생이 적으면서 만족할 만한 결과를 얻을 수 있는 유용한 방법 중의 한가지로 생각된다.

장장근건과 전완유리피판술을 이용한 성대.인두재건술 (Glottic and Pharyngeal Recostruction Using Radial Forearm Free Flap with Palmaris Longus Tendon)

  • Background and Objectives: As the laryngopharyngeal cancer is usually found at a advanced stage, it is difficult to get a wide surgical margin that preserves functional aspect and that is oncologically safe simultaneously. There were many operative technique to fulfill this principle, but none were satisfactory. Recently there were some reports about glottic and pharyngeal reconstruction using radial forearm free flap(RFFF) with palmaris longus tendon, which provided satisfactory oncologic and functional results. We attempted to perform this technique and to test usefulness at patients of laterally localized laryngopharyngeal tumor. Materials and Methods: Three patients were reconstructed glottis and pharynx using radial forearm free flap with palmaris longus tendon. Two hypopharyngeal cancer (T2N0M0) patients were performed wide vertical hemilaryngopharyngectomy and one supraglottic cancer(T2N0M0) patient was performed horizontovertical laryngopharyngectomy. Deglutitional function was evaluated with modified barium swallow and speech function was evaluated by speech pathologist. Results: Mean follow-up time was 29.3 months. There were no cancer recurrence. Their speech was satisfy-actory at social communication and oral feeding. They all have a complete oral nutrition from 26 days to 53 days. Decanulation time was from 71 days to 30 months. Conclusion: Glottic and pharyngeal reconstruction with radial forearm free flap could be accepted as a promising technique which offers a wide resection margin but satisfactory functional result in lateralized laryngohypopharyngeal cancer patients.

Robot-Assisted Free Flap in Head and Neck Reconstruction

  • Background Robots have allowed head and neck surgeons to extirpate oropharyngeal tumors safely without the need for lip-split incision or mandibulotomy. Using robots in oropharyngeal reconstruction is new but essential for oropharyngeal defects that result from robotic tumor excision. We report our experience with robotic free-flap reconstruction of head and neck defects to exemplify the necessity for robotic reconstruction. Methods We investigated head and neck cancer patients who underwent ablation surgery and free-flap reconstruction by robot. Between July 1, 2011 and March 31, 2012, 5 cases were performed and patient demographics, location of tumor, pathologic stage, reconstruction methods, flap size, recipient vessel, necessary pedicle length, and operation time were investigated. Results Among five free-flap reconstructions, four were radial forearm free flaps and one was an anterolateral thigh free-flap. Four flaps used the superior thyroid artery and one flap used a facial artery as the recipient vessel. The average pedicle length was 8.8 cm. Flap insetting and microanastomosis were achieved using a specially manufactured robotic instrument. The total operation time was 1,041.0 minutes (range, 814 to 1,132 minutes), and complications including flap necrosis, hematoma, and wound dehiscence did not occur. Conclusions This study demonstrates the clinically applicable use of robots in oropharyngeal reconstruction, especially using a free flap. A robot can assist the operator in insetting the flap at a deep portion of the oropharynx without the need to perform a traditional mandibulotomy. Robot-assisted reconstruction may substitute for existing surgical methods and is accepted as the most up-to-date method.

Surgical outcomes of suprafascial and subfascial radial forearm free flaps in head and neck reconstruction

  • Background: Conventional radial forearm free flaps (RFFFs) are known to be safe, but can result in donor site complications. Based on our experiences with suprafascial and subfascial RFFFs, we evaluated the safety of flap survival and surgical outcomes. Methods: This was a retrospective study of head and neck reconstructions using RFFFs from 2006 to 2021. Thirty-two patients underwent procedures using either subfascial (group A) or suprafascial (group B) dissection for flap elevation. Data were collected on patient characteristics, flap size, and donor and recipient complications, and the two groups were compared. Results: Thirteen of the 32 patients were in group A and 19 were in group B. Group A included 10 men and three women, with a mean age of 56.15 years, and group B included 16 men and three women, with a mean age of 59.11 years. The mean defect areas were 42.83 cm2 and 33.32 cm2, and the mean flap sizes were 50.96 cm2 and 44.54 cm2 in groups A and B, respectively. There were 13 donor site complications: eight (61.5%) in group A and five (26.3%) in group B. Flexor tendon exposure occurred in three patients in group A and in none in group B. All flaps survived completely. A recipient site complication occurred in two patients (15.4%) in group A and three patients (15.8%) in group B. Conclusions: Complications and flap survival were similar between the two groups. However, tendon exposure at the donor site was less prevalent in the suprafascial group, and the treatment period was shorter. Based on our data, suprafascial RFFF is a reliable and safe procedure for reconstruction of the head and neck.

  • 진행된 하인두암의 수술 후 시행한 유리 공장 이식술을 시행한 본원의 경험에서 수술 후 누공 형성, 수술 부위 협착 등의 다양한 합병증과 그 치료방법을 경험하였다. 수술 방법에 있어서 한 개의 동맥과 정맥을 연결하였고 이식한 혈관의 혈전에 의한 이식 실패는 없었다. 수술 후 연하 기능에 있어서 영향을 주는 가장 중요한 요인은 이식 부위 협착이 었으며 협착의 치료 결과에 따라 술 후 연하 기능의 회복에 차이가 있음을 확인하였다.

Osteocutaneous flaps for head and neck reconstruction: A focused evaluation of donor site morbidity and patient reported outcome measures in different reconstruction options

  • With significant improvements in success rates for free flap reconstruction of the head and neck, attention has turned to donor site morbidity associated with osteocutaneous free flaps. In this review, we address the morbidity associated with harvest of the four most commonly used osteocutaneous flaps; the free fibula flap, the scapula flap, the iliac crest flap and the radial forearm flap. A comprehensive literature search was performed to identify articles relevant to donor site morbidity for these flaps. We assessed morbidity in terms of incidence of delayed healing, chronic pain, aesthetic outcomes, site specific complications and patient satisfaction/quality of life. Weighted means were calculated when sufficient studies were available for review. The radial forearm and free fibula flaps are associated with high rates of delayed healing of approximately 20% compared to the scapular (<10%) and iliac flaps (5%). The radial forearm flap has higher rates of chronic pain (16.7%) and dissatisfaction with scar appearance (33%). For the majority of these patients harvest of one of these four osteocutaneous does not limit daily function at long-term follow-up. The scapular osteocutaneous flap is associated with the lowest relative morbidity and should be strongly considered when the recipient defect allows. The radial forearm is associated with higher morbidity in terms of scarring, fractures, chronic pain and wrist function and should not be considered as first choice when other flap options are available.

중안면부 복합조직결손부의 재건을 위한 다양한 유리피판술의 선택 (Reconstruction of Midfacial Defect Using Various Free Flap)

  • Various vascularized free flaps have been used for midfacial reconstruction after ablative head and neck cancer surgery. The most common donor sites for free flap include latissimus dorsi, rectus abdominis, and radial forearm. Between 1994 and 2004, 14 patients underwent free flap operation after head and neck cancer ablation, and were reviewed retrospectively. Among 14 free flaps, 8 were latissimus dorsi myocutaneous flaps, 3 rectus abdominis myocutaneous flaps and 3 radial forearm flaps, respectively. The overall survival rate of the flap was 100%. Complications were wound dehiscence(5 cases) and ptosis(1 case). We designed multiple dimensionally folded free flap for midfacial reconstruction. For 3-dimensional flap needs, we used latissimus dorsi myocutaneous flap. 2-Dimensional flap was latissimus dorsi or rectus abdominis myocutaneous flap and 1-dimensional flap was radial forearm flap. In this study we produced an algorithm for midfacial reconstruction. Large volume with many skin paddle defects were best reconstructed with latissimus dorsi myocutaneous flap or rectus abdominis myocutaneous flap. Radial forearm flap was used for reconstruction of small volume and little skin paddle defects.

요전박유리피판을 이용한 하인두협착 재건 (A Case of Reconstruction of Hypopharyngeal Stricture with Radial Forearm Free Flap)

  • Reconstruction of the pharyngoesophagus is one of the most difficult challenges in head and neck surgery. The goals of pharyngoesophageal reconstruction include restoration of a person's ability to swallow and to speak with minimal morbidity, but no current reconstruction modality is clearly best. Following its first introduction as fasciocutaneous flap by Yang in 1981, the forearm flap based on radial artery has become recognized as a very reliable and relatively easy one to use. The forearm flap has thin, pliable and predominantly hairless skin and scant subcutaneous layer In addition, its vascular pedicle is long and of large caliber, which greatly increases the chance of successful revascularization. The forearm flap shows the potentiality for better functional rehabilitation in swallowing and speech as well as the possibility of three dimensional reconstruction. We experienced a case of radial forearm free flap for the reconstruction in a patient with the hypopharyngeal stricture. The early return of oral feeding was possible and successfully enough to return to the normal daily activity.

성상신경절 차단후 총경동맥, 액와동맥, 상완동맥, 요골동맥의 혈류속도변화 (Flow Velocity Changes of Carotid, Axillary, Brachial and Radial Artery after Stellate Ganglion Block)

  • Stellate ganglion block (SGB) is applicated frequently to increase the blood flow and to reduce the pain in head, neck and upper extremity. The effects of SGB are able to be estimated by clinical signs and symptoms of Horner's syndrome, skin warmth, anhydrosis, etc. The effects are also estimated by sympathetic function and the blood flow. Blood flow velocities and pulsatility indices of common carotid,d axillary, brachial and radial artery were measured by Doppler flowmeter after SGB with 1% lidocaine at C6 level. Blood velocities of all arteries were increased and pulsatility indices of all arteries were decreased. This results suggest that SGB increase the blood flow of head and upper extremity and Doppler flowmeter is a good indicator of the effects of SGB.
