Osborne-Cotterill Lesion a Forgotten Injury: Review Article and Case Report

  • Osborne-Cotterill lesion is an osteochondral fracture located in the posterolateral margin of the humeral capitellum, which may be associated with a defect of the radial head after an elbow dislocation. This lesion causes instability by affecting the lateral ulnar collateral ligament over its capitellar insertion, which is associated with a residual capsular laxity, thereby leading to poor coverage of the radial head, and hence resulting in frequent dislocations. We present a 54-year-old patient, a physician who underwent trauma of the left elbow after falling from a bike and suffered a posterior dislocation fracture of the elbow. The patient subsequently presented episodes of instability, and additional work-up studies diagnosed the occurrence of Osborne-Cotterill lesion. An open reduction and internal fixation of the bony lesion was performed, with reinsertion of the lateral ligamentous complex. Three months after surgery, the patient was asymptomatic, having a flexion of 130° and extension of 0°, and resumed his daily activities without any limitation. Currently, the patient remains asymptomatic 2 years after the procedure. Elbow instability includes a large spectrum of pathological conditions that affect the biomechanics of the joint. The Osborne-Cotterill lesion is one among these conditions. It is a pathology that is often forgotten and easily overlooked. Undoubtedly, this lesion requires surgical intervention.

  • 발파에 의한 암반의 손상이나 파쇄는 폭약의 폭굉 과정에서 발생하는 충격파와 가스팽창의 영향에 의해 야기된다. 발파에 의한 파괴 메커니즘을 완전히 이해하기 위해서는 두 메커니즘을 같이 연구해야한다. 본 연구에서는 개별 요소법에 기초한 수치해석 프로그램인 PFC2D를 이용하여 발파공 벽면에 작용하는 폭굉압과 가스압을 동시에 모델링 할 수 있고 이에 따른 암반 내 균열 발생을 확인할 수 있는 알고리즘을 개발하였다. 또한 시멘트-모르타르 블록에서의 모형 발파시험을 수치해석을 수행함으로써, 개발된 알고리즘을 검증하였다.

집성재(集成材)의 혼합모드 하중시 파괴(破壞) 기준(基準)에 관한 연구(硏究) (Studies on Criterion for Mixed Mode Fracture in Glulam)

  • This study was carried out to investigate the fracture criterion of glulam. The mixed mode fracture of glulam was investigated by means of single edge notched specimens with various crack inclination in the longitudinal-radial plane. While fracture of wood is not completely understood, the study on linear-elastic fracture mechanics is a rational and valuable tool for studying the strength behavior of glulam. The results are summarized as follows : 1. Glue line has no effect on fracture strength. 2. There is a definite interaction between fracture toughness $K_I$ and $K_{II}$ during the mixed mode fracture of glulam. Several criterions for mixed mode failure were compared. The criterion was expressed in the following form: $(\frac{K_I}{K_{IC}})^2+(\frac{K_{II}}{K_{IIC}})^2=1$ 3. As crack inclination increases, $K_{IC}$ value and $K_{IIC}$ value decreases. The equations relating crack angle to $K_{IC}$ and $K_{IIC}$, respectively, were obtained as follows; $K_{IC}$ = -77.42${\gamma}$+153.72 ($R^2$ = 0.78) $K_{IIC}$ = -9.17${\gamma}$+34.90 ($R^2$ = 0.48)

An experimental study on the hydraulic fracturing of radial horizontal wells

  • Combining the radial well drilling and hydraulic fracturing technique, the production capacity of the reservoirs with low-permeability can be improved effectively. Due to the existence of radial holes, the stress around the well is redistributed, and the initiation and propagation of hydraulic fractures are different with those in traditional hydraulic fracturing. Therefore, it is necessary to study the influences of radial horizontal wells on hydraulic fracturing. The laboratory experiment was conducted to simulate the hydraulic fracturing on the physical model with radial holes. The experimental results showed that, compared with the borehole without radial holes, the sample with radial hole in the direction of maximum horizontal stress was fractured with significantly lower pressure. As the angle between direction of the horizontal hole and the maximum horizontal stress increased, the breakdown pressure grew. While when the radial hole was drilled towards the direction of the minimum horizontal stress, the breakdown pressure increased to that needed in the borehole without radial holes. When the angle between the radial hole and the maximum horizontal stress increase, the pressure required to propagate the fractures grew apparently, and the fracture become complex. Meanwhile, the deeper the radial hole drilled, the less the pressure was needed for fracturing.

  • Critical erosion kinetic energy models for radial/median cracks and lateral cracks in a workpiece are established in this study. We used experimental results to demonstrate that the fracture erosion resistance and erosion machining number could be used to evaluate the brittle fracture resistance and machinability of a workpiece. Erosion kinetic energy models were developed to predict brittle fracture and ductile shear, and a critical erosion kinetic energy model was developed to predict the transition from brittle fracture to ductile shear. These models were verified experimentally.

전위된 요골두 골절의 보존적 치료의 결과 (The Result of Conservative Treatment in Displaced Radial Head Fracture)

  • 목적: 전위된 요골두 골절 환자에 있어 보존적 치료 후의 결과를 확인하고자 하였다. 대상 및 방법: 1992년 4월부터 2002년 5월까지 본원에서 보존적 치료 받은 전위된 요골두 골절 환자 중 1년 이상 추시가 가능한 15명의 환자를 대상으로 하였다. 보존적 치료를 받은 15명 모두 남자였으며 평균 연령은 20.2($11{\sim}24$)세였으며 평균 추시기간은 13.4($12{\sim}15$)개월이었다. 그리고 내원 당시에 능동적 회내전 및 회외전이 70도 이상 가능한 환자를 대상으로 하였으며 모든 환자에서 일주일 정도의 장상지 석고 부목 고정술 후 조기 주관절 운동을 시행 하였다. 치료 결과는 변형된 Morrey 등의 기능적 평가와 방사선적 평가로 더 이상의 골편 전위 유무와 유합 소견으로 평가 하였다. 결과: 보존적 치료를 시행한 전위된 요골두 골절 환자 15명 중 변형된 Morrey 등의 기능적 평가에서 우수 8명, 양호 7명의 결과를 보였으며, 조기 주관절 운동을 시행하더라도 골편의 더 이상의 전위나 불유합 소견은 보이지 않았다. 결론: 전위된 요골두 골절 치료시 내원 당시에 회내전 및 회외전의 정도를 확인하고 능동적으로 70도 이상의 소견이 보이는 환자에 대해 부종이 감소된 이후에 조기 주관절 운동을 시킴으로써 관절 운동 범위의 제한을 막을 수 있고 일상 생활으로 복귀가 빨라 질 수 있을 것으로 사료된다.

전방 잠금형 금속판을 이용한 요골 원위부 골절수술 후 요골구 접선방향 촬영법의 임상적 유용성 (The Clinical usefulness of the tangential projection view of Radius groove after Distal Radius Fracture operation using the T-type Plat)

  • 서론 : 본 논문은 전방 잠금형 금속판을 이용한 요골 원위부 골절 수술 후 나사못(screw)의 요골구 관통 유무를 확인하기 위한 방법으로 방사선 요골구 접선방향촬영법의 새로운 검사법을 제시하고자 한다. 재료 및 방법 : 요골구 접선방향 촬영 시 요골 원위부 거상 각도는 2.5, 5, 7.5, 10도와 전완골과 방사선 검출기와의 각도를 60, 70, 80도로 각각 변화시켜 촬영한 방사선영상에서 나사못의 요골구 관통유무를 분석하였다. 결과 : 요골구 접선방향 영상에서는 요골구를 관통하지 않은 경우와 요골구 내관통, 요골구 외관통인 경우가 각각 55명(63.95%), 9명(10.47%), 22명(25.58%)으로 분석 되었고, 요골구를 관통한 나사못의 길이는 0.43~2.72mm의 범위로 평균 1.06mm이었다. 요골구가 잘 묘사되는 방사선 촬영법은 요골 거상각도 2.5~7.5도에서 90%이상, 전완골과 방사선 검출기의 각도 70도에서 요골구가 80%이상 잘 묘사되는 것으로 분석 되었다. 결론 : 요골구 접선방향 촬영법은 요골 원위부 거상 각도 5도, 전완골과 검출기의 각도는 70도를 추천 한다.

성인 주관절의 요골두와 구상돌기 골절을 동반한 탈구의 수술 적 치료 (성인 주관절에 발생한 위험3증주의 수술적 치료) (Operative Treatment of Terrible Triad in Elbow of Adults)

  • Purpose: The nonoperative outcome of elbow dislocations with associated radial head and coronoid fractures are often unsatisfactory because of chronic instability and stiffness from proloned immobilization, Therefore we managed these injuries with well programed surgical appproaches. Method: Ten patients with this injury were evaluated retrospectively from May 1998 to June 2004 after a minimum of 12 months. These injuries include elbow dislocation and associated fractures of both the radial head and the coronoid process. All ten patients were treated by one clinic operatively with similar scheduled surgical methods which started on the lateral side and terminated on the medial side of the elbow. Radial head and neck fractures were classified Mason types, as two and three types respectively with six and four cases and six cases were fixated. Coronoid process were fixated with screws anteroposterior directly or anchor suture in all cases, each type was classified one, two and three. where were three type one, four type two, and three type three were according to Regan and Morrey classification. Results: The outcome was three resulting in excellent, four good, two normaland and the remaining case was one poor according to the Mayo Elbow Performance score. At a terminal follow up, the range of motion of the elbow averaged flection contracture, $6^{\circ}(0{\sim}20^{\circ})$ and further flection, $129^{\circ}(115{\sim}140^{\circ})$. Two patients had complications requiring additional care. One, displaced coronoid process which was repaired with capsule and the other patient experienced, palsy of ulnar nerve and contracted elbow joint. Conclusions: Usage of early operation as the minimum injury of medial ligaments complex and the rigid fixation of fractures to prompt motion with our scheduled management for elbow dislocations with associated radial head and coracoid fractures provided excellent results.

Anterior Dislocation of the Radial Head Combined with Plastic Deformity of the Ulnar Shaft in an Adult: A Case Report

  • A 25-year-old woman presented to the emergency room with a painful and swollen right forearm. She had just sustained an injury from an accident during which her arm was tightly wound by a rope as she was lowering a net from a fishing boat. Before being released, her arm was rigidly trapped in the rope for approximately ten minutes. Radiographs revealed anterior dislocation of the radial head that was accompanied by plastic deformation of the proximal ulna, manifested as a reversal of the proximal dorsal angulation of the ulna (PUDA); suggested a Monteggia equivalent fracture. With the patient under general anesthesia, we reduced the radial head by posterior compression at $90^{\circ}$ of elbow flexion and at neutral rotation of the forearm. However, the reduction was easily lost and the elbow re-dislocated with even slight supination or extension of the arm. After the osteotomy of the ulnar deformity to restore the PUDA to normal, the reduction remained stable even with manipulation of the arm. We found that the patient could exercise a full range of motion without pain at the 3-month follow-up, and neither residual instability nor degenerative changes were observed at the final 3-year follow-up.
