Long-term Oncologic Outcomes of Robotic Total Gastrectomy for Advanced Gastric Cancer

  • Purpose: Although laparoscopic distal gastrectomy has rapidly replaced open distal gastrectomy, laparoscopic total gastrectomy (LTG) is less frequently performed owing to technical difficulties. Robotic surgery could be an appropriate minimally invasive alternative to LTG because it alleviates the technical challenges posed by laparoscopic procedures. However, few studies have compared the oncological safety of robotic total gastrectomy (RTG) with that of LTG, especially for advanced gastric cancer (AGC). Herein, we aimed to assess the oncological outcomes of RTG for AGC and compare them with those of LTG. Materials and Methods: We retrospectively reviewed 147 and 204 patients who underwent RTG and LTG for AGC, respectively, between 2007 and 2020. Long-term outcomes were compared using inverse probability of treatment weighting (IPTW). Results: After IPTW, the 2 groups exhibited similar clinicopathological features. The 5-year overall survival was comparable between the 2 groups (88.5% [95% confidence interval {CI}, 79.4%-93.7%] after RTG and 87.3% [95% CI, 80.1%-92.0%]) after LTG; log-rank P=0.544). The hazard ratio (HR) for death after RTG compared with that after LTG was 0.73 (95% CI, 0.40-1.33; P=0.304). The 5-year relapse-free survival was also similar between the 2 groups (75.7% [95% CI, 65.2%-83.4%] after RTG and 76.4% [95% CI, 67.9%-83.0%] after LTG; log-rank P=0.850). The HR for recurrence after RTG compared with that after LTG was 0.93 (95% CI, 0.60-1.46; P=0.753). Conclusions: Our findings revealed that RTG and LTG for AGC had similar long-term outcomes. RTG is an oncologically safe alternative to LTG and has technical advantages.

Impact of concurrent inspiratory muscle and aerobic exercise training on pulmonary function and cardiopulmonary responses (흡기근육 훈련과 유산소운동의 동시적용이 심폐반응과 폐기능에 미치는 영향)

  • The effects of inspiratory muscle training in conjunction with aerobic exercise on inspiratory muscle strength, pulmonary function, and maximal oxygen uptake(VO2max) were examined. Twenty four healthy collegiate men were divided into three groups; respiratory muscle training group(RTG; n=8), running exercise group(REG; n=8), and both respiratory muscle training and running group(BTG; n=8). Their pulmonary function, maximal inspiratory pressures(PImax), and VO2max were assessed before and after intervention. RTG underwent inspiratory muscle training(IMT) with load set to 50 % of PImax, 30 times per session, twice a day, 4 days a week REG ran on a treadmill at 70-75 % of VO2max for 30 min a day, 4 days a week. BTG participated both IMT and the running exercise. Participant's anthropometric parameters and pulmonary function were not changed. VO2max increased by 6.1±3.3 %, 5.9±6.6 %, and 10.0±8.3 % in RTG, REG, and BTG, respectively(p< .05), and PImax also increased by 21.7±14.3 %, 19.7±12.0 %, and 27.0±12.1 % in RTG, REG, and BTG, respectively, but no group differences were found. Based on the study, although statistically insignificant, BTG showed the biggest increase of VO2max and PImax indicating a possible synergic effect of inspiratory muscle training and aerobic exercise on respiratory responses.

  • This paper presents a heuristic method for the Assembly Line Balancing(ALB) of the large-sized product. In the ALB problem of the large-sized product such as bus and truck, the assignments of the Related Task Groups(RTG), the same side tasks, and team tasks should be considered. In this paper, a new concept of the RTG and two kinds of assignment rules are proposed to resolve the above considerations. The first assignment rule allots the RTG with the constraint of the same side tasks to the station while the second allots the RTG to the station, relaxing the above constraint to increase the applicability of the method. An assignment rule for team tasks is also presented. The benefits of the method are to improve work methods, to give more job satisfaction to workers, and to allow greater flexibility in the design of assembly lines.

Analysis of UCP1 Expression in Rainbow Trout Gonadal Cell Line RTG-2 Indicates its Marginal Response to Adipogenic Inducers Compared to Mammalian Cell Lines

  • Uncoupling protein 1 (UCP1) is a unique mitochondrial membranous protein expressed in brown adipose tissue (BAT) in mammals. While its expression in response to cold temperatures and adipogenic inducers is well-characterized in mammals and human infants, the molecular characterization and expression of UCP1 in fish remain unexplored. To address this gap, we analyzed UCP1 expression in response to adipogenic inducers in a fish cell line, rainbow trout gonadal cells (RTG-2), and compared it with UCP1 expression in three mammalian preadipocytes, 3T3-L1, T37i, and WT1 exposed to the Peroxisome proliferator-activated receptor gamma (PPARγ) agonists, rosiglitazone (Rosi). In mammalian preadipocytes, UCP1 protein was highly expressed by Rosi, with an induction of adipogenesis observed in a time-dependent manner. This suggests that UCP1 plays a significant role in adipogenesis in mammals. However, RTG-2 cells showed no response to adipogenic inducers and exhibited only marginal expressions of UCP1. These results imply that RTG-2 cells may lack crucial responsive mechanisms to adipogenic signals or that the adipogenic response is regulated by other mechanisms. Further studies are needed to confirm these phenomena in fish preadipocytes when an appropriate cell line is established in future research.

Purification and characterization of biochemical properties of hemolysin from Vibrio fluvialis (Vibrio fluvialis 유래의 hemolysin 정제와 생화학적 특성)

  • Hemolysin (VFH) of V. fluvialis, which is a pathogenic bacteria, causing watery diarrhea with vomiting, abdominal croup, was purified. V. fluvialis was cultivated in BHI medium and the culture supematant was precipitated by ammonium sulfate. The protein was purified by chromatographies on columns of DEAE-cellulose and Mono-Q. Molecular weight of the purified VFH was estimated as 79kDa by SDS-PAGE. The optimal temperature for a maximum hemolytic activity was at around 35$^{\circ}C$ and the activity was decreased at 4$0^{\circ}C$ Cytotoxicity of VFH was also investigated using RTG-2 cell line. LDH assay study showed that 50$\mu\textrm{g}$/m1 of VFH release 80% of total cellular LDH (lactate dehydrogenase) from RTG-2 cell and microscopic observation also showed the morphological change of cell.

A Study on Predictability of Snowfall Amount due to Fine Difference of Spatial Distribution of Remote Sensing based Sea Surface Temperature (원격 탐사 기반 해양 표면 온도의 미세 분포 차이에 따른 강설량 예측성 연구)

  • In order to understand the relation between the distribution of sea surface temperature and heavy snowfall over western coast of the Korean peninsula, several numerical assessments were carried out. Numerical model used in this study is WRF, and sea surface temperature data were FNL(National Center for Environment Prediction-Final operational global analysis), RTG(Real Time Global analysis), and OSTIA(Operational Sea Surface Temperature and Sea Ice Analysis). There were produced on the basis of remote sensing data, such as a variety of satellite and in situ observation. The analysis focused on the heavy snowfall over Honam districts for 2 days from 29 December 2010. In comparison with RTG and OSTIA SST data, sensible and latent heat fluexes estimated by numerical simulation with FNL data were higher than those with RTG and OSTIA SST data, due to higher sea surface temperature of FNL. General distribution of RTG and OSTIA SST showed similar, however, fine spatial differences appear in near western coast of the peninsula. Estimated snow fall amount with OSTIA SST was occurred far from the western coast because of higher SST over sea far from coast than that near coast. On the other hand, snowfall amount near coast is larger than that over distance sea in simulation with RTG SST. The difference of snowfall amount between numerical assessment with RTG and OSTIA is induced from the fine difference of SST spatial distributions over the Yellow sea. So, the prediction accuracy of snowfall amount is strongly associated with the SST distribution not only over near coast but also over far from the western coast of the Korean peninsula.

  • In this study, GFP reporter gene was transfected into a fibroblast cell line RTG-2 using a polycationic transfection reagent (Superfect) and showed a successful expression of GFP. The transfection efficiency by Superfect was compared to the commonly used transfection method, i.e. DNA-calcium phosphate coprecipitatlon. Transfection by Superfect was more effective than calcium phosphate coprecipltation method (frequency of cell expressing orr was 11.3% and 3.5%, respectively). The optimal expression of GFP and {\beta}-galactosidase was observed when $5-6\;{\mu}{\ell}$ of Superfect per ${\mu}g$ DNA was used for transfcction, 1:5-6 ratio between DNA(${\mu}g$) and Superfect ($\mu\ell$).

  • In this study, we investigated the skin safety, antioxidant, moisturizing, and anti-inflammatory effects of a composite extract of Prunus mume flower, Lonicera japonica flower, Chrysanthemum indicum flower, and Phyllostachys bambusoides (Royal TheraTea GuardTM, RTG), which has been used as an internal medicine in the royal medical treatment for preventing skin damage caused by external environmental factors. In a cytotoxicity test on HaCaT cells, RTG complex extract showed no change in cell viability at concentrations of 0.125, 0.25, 0.5, and 1%, and a skin irritation index of 0.00 in a human patch test, confirming that it is non-irritating to the skin. The antioxidant effects was confirmed by the presence of 497.83 ㎍ GAE/g of polyphenols and increased DPPH radical scavenging activity, with a significant increase in catalase activity in the stratum corneum, showing potential as an antioxidant that protects cells from oxidative stress. The anti-inflammatory effects was observed through reduced erythema in skin stimulated by tape stripping and treated with fine dust in keratinocytes. The moisturizing effects was shown by increased expression of hyaluronan synthase (HAS)2, 3 and keratin1 in keratinocytes treated with fine dust compared to the control group, as well as increased skin moisture content and decreased skin roughness. These results suggest RTG can be used as a cosmetic ingredient to prevent skin damage caused by external environmental factors.

Change of Infection Properties of Subcultured Marine Birnavirus in Several Fish Cell Lines (어류 주화세포에서의 계대배양에 의한 해양버나바이러스의 감염특성의 변화)

  • Marine birnavirus (MABV) has wide host range in marine organisms. To clarify various infection properties of MABV in different host species, in vitro study was performed by subculture for 10 passages in several fish cell lines. In CHSE-214, RTG-2 and RSBK-2 cells, the virus produced high yield of virus. Typical CPE with high protein expression was observed in these cells. On the contrary, the virus grown in EPC, FHM and BF-2 cells exhibited no CPE appearance although virus protein was detected. In EPC and FHM cells, the virus titer increased in later passages. The plaque size was distinctly bigger in CHSE-214, RTG-2 and RSBK-2 cells than in other cell lines. The nucleotide sequence of VP2/NS junction region on genome segment A exhibited one specific nucleotide change at 195. The different infection properties in several cell types performed in the present work might reflect in vivo MABV infection in various host species occurring in natural conditions.

A Study on MR Cholangiography using Breathing Hold Target Techniqu by Prospective Acquisition Correction and Respiration Trigger Gating (Non Breathe Hold Technique를 이용한 MR 담도계조영술에 대한 고찰 : Prospective Acquisition Correction(PACE)기법과 Respiration Trigger Gating(RTG) 기법의 비교)

  • Recently, MR Cholangiography used mainly bu controlling of patient's breathing. There is breathing hold techniques to get images within shopt time and gating technique adjusted to respiration cycle for high resolution image. In this study, the aim of this experiment is to know on clinical usefulness compared with PACE and RTG thchniques. This study's period is from 2006 in November to 2007 in January. A total of 21 patients investigated at MAGNETOM Sonata 1.5T (SIEMENS Erlangen) with use of 12ch body coil. MR acquisition protocol used 3D turbo spin echo coronal sequence. Scan parameters applied to potimal setting in use as gating techniques, respectively. Analysis of consuming timing evaluated with rapidness. As analysis of quantity, the common bile duct, gall bladder measured in signal intensities, then these data were calculated by signal to noise ratio and contrast to noise ratio. Qualitative analysis, experienced 2radiologists and 3 RTs were evaluated into 3groups about artifact, accuracy of lesions, sharpness of the common bile duct or gall bladder. As a result of analysis, when compared to PACE, consuming time of the RTG took less than PACE, On both CNRs and SNRs, PACE technique was slightly high values than RTG(p<0.05). Qualitative analysis' results, discrimination of lesions in the common bile duct, gall bladder get a significance level in both RTG and PACE techniques but presence's artifact of breathing and pulsation highly demonstrate in PACE techniques. In conclusion, both PACE and RTG methods at MRCP provided prominently clinical information for the common bile duct, gall bladder. If machines have not limitation with performance, induction of breathing holding also will help getting diagnistic quality.
