Comparison Study of Positive Rates According to the Methods of EIA, RHA and PHA in Detecting of HBsAg, Anti-HBs Among -119 Emergency Medical Technicians and Rescuers in Korea (우리나라 일부지역 119구조구급대원의 HBsAg 및 Anti-HBs 검출에 있어서 EIA법과 RPHA법 및 PHA법에 따른 양성률비교)

  • This study of the positive rates of HBsAg, anti-HBs among 119 Emergency Medical Technicians and Rescuers who were working in Pohang, Kyoung-Buk, Korea was performed from March to June 1997 according to three methods of EIA, RPHA and PHA. The number of samples were 201. The results were as follows; The overall positive rate of HBs Ag by EIA and RPHA methods were 4.98%, 5.47%, the overall positive rate of anti-HBs by EIA and PHA methods were 58.71%, 63.68%. In detecting of HBs Ag, concurrence rate, sensitivity, specificity and predictability of PHA to EIA were 99.5%, 90.9%, 100% and 99.75% respectively. In detecting of anti-HBs, concurrence rate, sensitivity, specificity and predictability of PHA to EIA were 91.04%, 89%, 94.5% and 89.87% respectively. Based on this study, there were no significant diffrences in the positive rates of HBsAg and Anti-HBs in 119 Emergency Medical Technicians and Rescurers in Pohang, Korea as compared with the general population according to other studies. In terms of concurrence rate, sensitivity, specificity and the costs of RPHA with EIA for detecting HBsAg, RPHA is more cost effective than EIA for mass screening of HBsAg detection. Also, In terms of concurrence rate, sensitivity and specificity of PHA with EIA, PHA is more cost effective and less problems of procedure than the EIA for mass screening of Anti-HBs detection.

  • Primary screening test for serum HBsAg by RPHA from 4,805 persons who were clinically well through preemployment examination for the period of one calendar year of 1983 revealed 476 (9.9%) positive individual carriers. There were no significant differences in distribution of positives of serum HBsAg by age group, profession, or province area. Among positives of serum HBsAg, 356 (74.8%) showed normal findings and 120 (25.2%) showed abnormal findings in liver function test, respectively. Radioimmunoassay was done in 169 positives of HBsAg and RIA detected 10 negative persons who were positive by RPHA revealing 5.9% of false positive rate and 94.1% of sensitivity of RPHA. In RIA profile of HBV markers, pattern I (HBsAg+, Anti-HBe+) was 46.6%, pattern II (HBsAg+, HBeAg+) was 33.3%, pattern III (HBsAg+only) was 18.3%, pattern IV (HBsAg+, HBeAg+, Anti-HBs+) was 1.3%, pattern V (HBsAg+, HBeAg+, Anti-HBe+) was 0.6%, respectively. There were no positives of HBsAg among 10 persons who were negatives of HBsAg by RIA.

Actual Conditions and Improvement Strategies regarding the Nursing Workforce in Public Health Centers (보건소의 간호인력 실태와 개선 방안)

  • Purpose: Public health centers (PHCs) provide a variety of healthcare services according to the Regional Public Health Act (RPHA). We aim to analyze the changes in public healthcare services according to the legislation and the number of public health nurse (PHN) involved in the service, and propose an alternative to effectively distribute the public health nursing workforce for protecting the health rights of people. Methods: This is a review study that deduced the results from literature review. Results: During the revision of the PHC law to the RPHA, several services were added. Health related laws have forced the PHCs to provide services for these legislations. Consequently, the workload on the PHNs has been increasing. However, the PHNs have been consistently lacking. In 2017, there are 16.2 full-time nurses per center. Furthermore, About 50% of the PHCs are not meeting the minimum requirements of licensed or qualified health professional workforce outlined in 1997. In addition, 43% of the nurses in the PHCs are part-time nurses. Conclusion: We suggest that the minimum requirements of health professional workforce should be modified to reflect the increase in the workload of PHNs, and a legislation to enforce PHCs to fulfill these requirements is needed.

A Study on the Positive Rates of HBsAg and Anti-HBs from Old People's Home and Casual Ward in Daegu, Korea

  • Serum hepatitis is still recognized as a major public health problem in many countries. The most common etiologies are acute viral hepatitis A and B, and hepatitis B virus (HBV) was known as the pathogen of serum hepatitis. since the HBV causes chronic hepatitis. HBV infection is hyperendemic in Korea. It is known that 8% and 1% of the Korean population is chronic carriers of HBV. The total number of 487 serum specimen (old people's home 190, causal ward-Daegu hope village 297) were collected at the laboratory of Department of Clinical Pathology of Daegu medical center. The laboratory tested the specimen to detect for hepatitis B surface antigen (HBsAg) and anti-HBs using reverse passive hemag-glutination (RPHA) and passive hemagglutination (PHA) method between April, 2011 and June, 2011. In one-step test, HBsAg and anti-HBs of Humasis were used. The positive rate for HBsAg was 6.80% (33/487), and the rate in male was 6.93% (19/274) higher than that of female 6.57% (14/213). In the positive rate of HBsAg by age group, the highest positive rate group was in the group of 40-49 years in male (12.0%), and 50-59 years in female (17.65 %). The overall positive rate for anti-HBs was 42.70% (208/487), showing the higher positive rate of 43.80% in male (120/274) than that of 41.30% (88/213) in female. The highest positive rate of anti-HBs in age group was the 40-49 years group in male (52.00%) and the group of under 29 in female (66.67%).

Positive Rate of HBsAg in School Children in Incheon Area (인천 지역 초, 중, 고등학생의 B형 간염 바이러스 표면항원 양성률에 대한 조사 연구)

  • Purpose : The incidence of hepatitis B virus infection has gradually decreased since 1983 when hepatitis B vaccine was firstly produced. This study was performed to evaluate the efficacy of hepatitis B immunization. Methods : The elementary, middle and high school children in Incheon area were enrolled in this study in 1997 and 1998. Hepatitis B virus surface antigen(HBsAg) was measured using reversed passive hemagglutination(RPHA). Results : The results were as follows The positive rates of HBsAg in elementary, middle and high school children were 0.7%(337/46,861), 2.5%(381/15,026) and 3.1%(681/21,938) respectively in 1997 and 0.6%(257/41,946), 2.7%(379/13,652) and 2.4%(628/25,277) respectively in 1998. The positive rates of HBsAg in children under 19 years of age in 1985, 1990 and 1995 were 6.1, 5.2 and 3.5% respectively. Conclusion : The positive rates of HBsAg in elementary school children in 1997 and 1998 has decreased compared with those of the previous studies. The majority of elementary school children were given hepatitis B vaccination. These results suggest that hepatitis B vaccines used in Korea were effective for prevention of hepatitis B infection in school children.

  • We experienced two cases of Tsutsugamushi disease which occurred in October, 2000. The patients, who were over 70 years old and living in a rural area, visited Wonkwang University Oriental Chonju Medicine Hospital because of suffering fever, chill, skin rash, abdominal discomfort, and general weakness for a duration of seven days. The diagnosis was confmned as Tsutsugamushi disease by clinical findings such as eschar and high antibody titers on Reverse Passive Hemagglutination(RPHA). As for treatment, we used Sengmagalgentang-gamibang during the acute and Chojungikitang during convalescent phases of the illness. The patients were treated with Doxycycline of western medicine. The patients improved within 3 days of admission and were discharged within one week.

An OTHBVS Cell Line Expresses the Human HBV Middle S Protein

  • An OTHBVS cell line from HepG2 was established. This cell line stably expresses the human hepatitis B virus (HBV) middle S protein that includes the preS2 region which is important for HBV particle entry into the hepatocyte. To establish this cell line, the middle S open reading frame (ORF), with a promoter located in the 5' region and enhancer located in the 3' region, was cloned downstream from the metallothionine (MT) promoter of the OT1529 vector. In this vector, expression of the middle S protein was constructed to be regulated by its own promoter and enhancer. Expression of the large S protein which contains the preS1 region in addition to the middle S protein was designed to be regulated by the MT promoter. When extracts of OTHBVS cells were examined with an S protein detection kit (RPHA, Korea Green Cross Co.), an S protein was detected. Total mRNA of OTHBVS cell examined by northern blot analysis with an S ORF probe revealed small/middle S transcripts (2.1 kb). When the MT promoter was induced by Zn, large S transcripts (2.4 kb) were detected. The GP36 and GP33 middle S proteins were presumably detected, but large S proteins were not detected by immunostain analysis using anti-preS2 antibody.

A Research on the Positive Rate of HBsAg Among the Healthy Korean (한국(韓國)의 건강인(健康人)에서 간염(肝炎) B항원(抗原)(HBsAg) 양성율(陽性率)에 대한 조사연구(調査硏究))

  • In the 2,218 healthy cases-male 1,574 female 644-who were conducted serum examination from May 1st 1983 to November 1st 1983 by the medical selection of Dae Han Kyouk life insurance Co., the results which investigated the HBsAg by using the Reverse Passive Hemagglutination(RPHA) method are as follows; 1. The number of 167 from 2,218 cases proved to be positive, showing 7.52%, the number of 143 from 1,574 male positive, showing 9.08%, and the number 24 from 644 female positive, showing 3.73%. The positive rate of the male was indicated by 5.36% higher than that of the female as the former marked 2.4 times higher distribution than the latter. 2) The positive rate by the type of occupations appears as follows; The highest positive rate group is that of discharger from military service in 1983, the number of 70 from 736 persons marking 9.51% and the next is the merchant 8.79%(8/91), employee of a company markes 8.53%(44/516), high school boy 7.69%(19/247), house wife 5.71%(10/210), carrier woman 3.45%(10/290) and the group of the high school girl records the minimum rate of all, 3.13%(4/128). 3. The positive rate by the age group appears as follows; The highest rate group is that of 31 to 35 years, marking 9.80% positive rate, and the next is $26{\sim}30$(9.3%), $21{\sim}25$(8.01%), $41{\sim}45$(6.96%), $36{\sim}40$(6.9%), those below 20(5.99%), 46 to 50 years shows the minimum rate of all(2.94%) Among the male case, 26 to 30 years shows the highest positive rate by 10.59% and female, 31 to 35 years shows the highest rate by 7.89%. 4. The positive rate by the blood type represents as follows; Blood type A shows 7.30%, type B 7.17%, type O 6.11% and type AB indicates 5.88% in order.

Domain Decomposition Strategy for Pin-wise Full-Core Monte Carlo Depletion Calculation with the Reactor Monte Carlo Code

  • Because of prohibitive data storage requirements in large-scale simulations, the memory problem is an obstacle for Monte Carlo (MC) codes in accomplishing pin-wise three-dimensional (3D) full-core calculations, particularly for whole-core depletion analyses. Various kinds of data are evaluated and quantificational total memory requirements are analyzed based on the Reactor Monte Carlo (RMC) code, showing that tally data, material data, and isotope densities in depletion are three major parts of memory storage. The domain decomposition method is investigated as a means of saving memory, by dividing spatial geometry into domains that are simulated separately by parallel processors. For the validity of particle tracking during transport simulations, particles need to be communicated between domains. In consideration of efficiency, an asynchronous particle communication algorithm is designed and implemented. Furthermore, we couple the domain decomposition method with MC burnup process, under a strategy of utilizing consistent domain partition in both transport and depletion modules. A numerical test of 3D full-core burnup calculations is carried out, indicating that the RMC code, with the domain decomposition method, is capable of pin-wise full-core burnup calculations with millions of depletion regions.

An Epidemiological Study on the Selected Risk Factors of Hepatitis B Virus Infection (B형 간염 전파관련요인에 관한 역학적 연구)

  • An attempt to confirm the associations of some selected risk factors of HBV infection and measure their risks, a cross-sectional study with 1,209 urban office workers was carried out. For the study, a simple questionnaire which contained several questions on personal experience and behaviors on several known selected risk factors of HBV infection was applied to each subject, and the Hepatitis B virus surface antigen and its antibody were checked by RPHA and PHA method, respectively. Risk factors chosen for this study were experience of blood transfusion and personal contact variables, such as frequencies of eating-out, drinking after office hours, going to tea room, sharing cigarettes, etc. The results obtained were as follows: 1. The proportion of HBsAg positive was 10.6%, and total HVB infected including the Anti-HBs positive cases without vaccination was 44.2%. Both were higher in male than in female. 2. Frequent personal contact through glasses and dishes in eating-outs and drinkings turned out not to be a significant risk factor of Hepatitis B surface antigenecity. 3. Frequent visits to tea room was a significant risk factor of HBV infection which combined HBsAg positive cases and Anti-HBs cases who had not received HBV vaccination. The odds ratio was 1.56 4. Blood transfusion was not a significant risk factor of both HBsAg positive and total HBV infection. In summary, indirect oral contacts through eating-outs and drinkings was not significant risk factor in Korea at least between adults. Blood transfusion is no more major source of HBV infection in Korea probably because the adquate screening test of HBsAg for the blood donors is being made.
