동결견(凍結肩) 환자의 오공약침(蜈蚣藥鍼) 치료에 대한 임상적 고찰 (Effect of Scolopendrid Pharmacupuncture Therapy on Frozen Shoulder Patients)

  • Objectives : The purpose of this research was to study the effect of Scolopendrid Pharmacopuncture on frozen shoulder patients. Methods : All of the 46 frozen shoulder patients who visited the department of Acupuncture & Moxibustion in Wonkwang Sanbon Medical Center from 1st March 2007 to 30th August 2008 had taken either the Scolopendrid Pharmacopuncture therapy or acupuncture therapy 10 times. There are 23 patients in each group. We evaluated the improvement of their shoulder pain and disability using the Visual Analog Scale(VAS), Shoulder Pain And Diability Index(SPADI), Range of Motion(ROM), and the Apley Scratch test at their first and last visit. Then we analyzed and compared the improvement shown by the two groups. Results : 1. Measures on all tests for the acupuncture group showed statistically significant improvement. 2. Measures on all tests for Scolopendrid Pharmacopuncture showed statistically significant improvement. 3. The Scolopendrid Pharmacopuncture group's VAS and ROM(external, internal rotation) showed statistically significant difference compared with those of the acupuncture group. SPADI, ROM(flexion, extension, abduction, adduction) and Apley scratch test had no statistically significant difference between the two groups. Conclusions : The above results show that Scolopendrid Pharmacopuncture therapy can be used as an effective treatment for reducing frozen shoulder pain.

고유수용성신경근촉진법 재활운동이 수술 후 유방암 환자의 관절운동범위와 통증 및 기능에 미치는 영향 (The Effect of Proprioceptive Neuromuscular Facilitation Rehabilitation Exercise on Range of Motion, Pain, and Function of Breast Cancer Patients after Surgery)

  • Purpose: This study examined the effect of proprioceptive neuromuscular facilitation (PNF) rehabilitation exercise on postoperative range of motion (ROM), pain, and function of breast cancer patients. The purpose of this study was to provide fundamental data regarding the use of PNF rehabilitation exercise for patients with breast cancer. Methods: Fourteen patients who underwent surgery due to breast cancer were randomly divided into an experimental group (n=7) that performed PNF rehabilitation exercise and a control group (n=7) that performed general rehabilitation exercise. Both groups performed the respective exercises for 30 minutes, five times a week for 4 weeks. For the measurement of ROM, the range of shoulder abduction was measured using a clinometer smartphone application. The shoulder pain and disability index (SPADI) was used to measure the level of pain and functional activity. A paired t-test was conducted to compare within-group changes before and after the PNF rehabilitation exercise. Differences between the experimental group and control group were analyzed by an independent t-test. For all tests, the level of statistical significance was set at ${\alpha}=0.05$. Results: After the exercise, there was a significant within-group change in the ROM and SPADI in both the experimental group and control group (p<0.01). There was also a significant between-group difference in the ROM and SPADI after the intervention (p<0.05). Conclusion: Rehabilitation exercise is generally applied as a treatment for patients with breast cancer after surgery and is relatively effective. The application of PNF rehabilitation exercise may be useful in such patients, considering its effects on ROM improvement, pain reduction, and functional enhancement.

  • Objectives : The purpose of this study was to identify the effect of electro-acupuncture stimulation regarding restoration of range of motion on patients with frozen shoulder. Methods : Experimental group measured with shoulder flexion and abduction angle at pre and post electro-acupuncture stimulation. Results : There were statistically significant difference regarding change of flexion angle of electro-acupuncture stimulation(p<0.01) and the testing value of all levels had a significant difference with the value of final post-test(p<0.01). There were statistically significant difference regarding change of abduction angle of electro-acupuncture stimulation(p<0.01). Conclusions : Electro-acupuncture stimulation of distal part increased range of motion of frozen shoulder. Therefore, application of electro-acupuncture should be developed as a therapeutic method in treatment of frozen shoulder. Also, it can contribute as one of good therapeutic methods offered to patients through approach of oriental rehabilitation medicine.

견비통에 대한 약침요법의 임상적 연구 (Clinical Study on Effect of Herbal acupuncture therapy on shoulder pain)

  • Objectives : This study was performed to examine effects of herbal acupuncture therapy on shoulder pain. Methods : Control group were treated by common acupuncture therapy and sample group were treated by common acupuncture therapy and herbal acupuncture therapy. We compared control group with sample group by VAS and ROM. Results : VAS of sample group were more decreased than control group. ROM of sample group were more improved than control group. Conclusion : The herbal acupuncture therapy may be good effects on the shoulder pain.

  • Objectives : The purpose of this study is to report the improvement after Sweet BV herbal acupuncture therapy of shoulder impingement syndrome with Subacromial bursitis. Methods : We treated a patient having shoulder pain due to shoulder impingement syndrome with Sweet BV herbal acupuncture therapy. We checked visual analogue scale (VAS) score and range of movement (ROM). Results : We treated shoulder pain. Thereafter ROM improved and VAS score dropped to the level 3 points from 10 points at onset. Conclusions : This report shows Sweet BV herbal acupuncture therapy has effectiveness on shoulder impingement syndrome with Subacromial bursitis.

The Effects of Group Exercise, Manual Therapy and Home Exercise on Pain, Range of Motion and Function in Patient with Adhesive Capsulitis

  • Purpose: The purpose of this study was to determine the effects of group exercise, manual therapy, and a home exercise program on VAS (visual analogue scale), ROM (range of motion) of shoulder, and functional outcome in adhesive capsulitis. Methods: The study was conducted with 45 patients. All subjects were randomly assigned to 3 groups: group exercise (n=15), manual therapy (n=15), home exercise (n=15). The subjects performed an intervention program 3 times a week for 4 weeks a total of 12 times. Outcome measurements were VAS, ROM, and Constant-Murley score outcome measures. Measurements were taken four times, at baseline, 4 weeks later, 8 weeks later, and 12 weeks later. Results: VAS, ROM of the shoulder, and functional outcomes showed improvement in all groups at the final follow-up (p<0.05). VAS and ROM showed greater improvement with group exercise and manual therapy than home exercise, however, there were no significant differences between the two groups (p>0.05). Functional outcomes showed the greatest improvement in group exercise at the final follow-up. This improvement was greater than with manual therapy or home exercise (p<0.05). Conclusion: Group exercise and manual therapy were more effective for improving pain, ROM, and function than home exercise in adhesive capsulitis. In particular, group exercise is more effective in functional recovery than other physiotherapy interventions.

신바로3 약침과 한방치료를 병행한 견관절 관절와순 파열 환자 치험 3례 (A Case Report of Patients with Labrum Tear in Shoulder Treated by Oriental Therapy, Including Sinbaro3 Pharmacopuncture)

  • Objectives : The purpose of this report is to show that oriental therapy, using Sinbaro3 pharmacopuncture in particular, is effective for symptom improvements and ROM in patients diagnosed with labrum tear in shoulder. Methods : 3 patients were admitted to Jaseng Oriental Medicine Hospital, were diagnosed with a tear of the labrum, and were treated with oriental medicine including Sinbaro3 pharmacopuncture. 3 case were measured and assessed by SPADI(Shoulder Pain and Disability Index), shoulder range of motion (ROM) and a shoulder physical examination. Results : After treatment, patients' SPADI decreased and they showed improvement of ROM(range of movement). In patients with labrum tear, oriental medicine treatment and Sinbaro3 pharmacopuncture was effective for relieving pain and facilitating better movement. Conclusion : In line with the above results, it can considered that oriental medical therapy including Sinbaro3 pharmacopuncture is effective for the relief of pain and for recovering function in the shoulder joint.

세라밴드 운동이 포함된 시각 및 청각 피드백이 둥근 어깨 자세에 미치는 영향 (The Effect of Visual and Auditory Feedback Combined with Theraband Exercise in Rounded Shoulder Posture)

  • Purpose: The purpose of this study was to investigate the effect of visual and auditory feedback combined with theraband exercise in rounded shoulder posture. Methods: There were 43 adults with rounded shoulder posture who had a distance of 2.5 cm or more from the posterolateral of the acromion to the table in the supine position that participated. The participants were randomly divided into four groups: those with visual feedback from the lateral view (visual feedback; VFB, n = 11) provided, those with auditory feedback of praise (auditory feedback; AFB, n = 10) provided, those with visual feedback and auditory feedback (visual auditory feedback; VAFB, n = 11) provided, and those without any feedback (control group; CON, n = 11). Theraband exercise with or without feedback was carried out three times per week for three weeks. To confirm the effect of theraband exercise with visual feedback and auditory feedback on pain, range of motion (ROM), posture, and psychological variables were measured before and after exercise in participants with rounded shoulder posture. Results: The VAFB group showed significant differences in pain, ROM, posture, and psychological variables when compared before and after treatment. However, the differences among the VAFB, VFB, AFB, and CON groups were significant in the ROM of abduction, the New York Posture Rating, and the scapular index. Conclusion: In conclusion, theraband exercise combined with visual feedback from the lateral view and auditory feedback by praise improved rounded shoulder posture. Moreover, auditory feedback was more significant statistically than visual feedback.

경추통 환자에 대한 신장운동과 도수치료가 즉각적인 통증과 ROM에 미치는 효과 (The Effect of Immediate Pain and Cervical ROM of Cervical Pain Patients on Stretching and Manipulation)

  • Purpose: Cervical pain is caused mainly by a static position, lasting work, bad habits and stress, and is accompanied by pain in the upper trapezius. Traditionally, heat and traction, exercise, mobilization, manipulation have been used to treat cervical pain. This study examined the effect of stretching and manipulation (high-velocity low-amplitude: HVLA) on the pain and ROM in women with chronic cervical and upper shoulder pain. Methods: Fifty-two women diagnosed with chronic cervical and upper shoulder pain were enrolled in this study. Among them, 26 patients (experiment group) were managed by passive stretching and manipulation, and another 26 patients (control group) were treated with physical therapy intervention (hat pack: HP, transcutaneous electrical nerve stimulation: TENS, ultra sound: US). Each group made use of a Visual Analogue Scale (VAS) and the highly reliable (flextion: 0.92, extention: 0.99) cervical range of motion (CROM) to compare the possible changes in pain and ROM in the two groups after treatment. Results: In the experimental group, the pain decreased and the ROM has increased in all directions. In the control group, the pain decreased but the ROM was not changed in all directions. Conclusion: According to the results, passive stretching and manipulation is effective for increasing the ROM and decreasing the level of pain. Moreover, physical therapy intervention (HP, TENS, US) is effective for immediately decreasing the pain but has little effect on the ROM.

회전근 개 간격의 가교 반흔 유착에 의한 외상성 견관절 강직증의 관절경적 치료 - 증례 보고 - (Arthroscopic Treatment of Post-traumatic Stiff Shoulder by Rotator Interval Bridging Scar Adhesion - Case Report -)

  • In adhesive capsulitis of the shoulder of no response to nonoperative treatment, an arthroscopic capsular release and manipulation improves range of motion and pain relief. We performed an arthroscopic examination in the stiff shoulder, of which she had no response to nonoperative treatment, after the conservative treatment of a clavicular shaft fracture by motorcycle-driver traffic accident. We found the intra-articular 'rotator interval bridging scar adhesion' between subscapularis tendon and antero-superior glenoid fossa under the rotator interval which was no adhesion and contracture itself. We performed the scar adhesion removal and synovectomy, maintaining the rotator interval. We recommended nonsteroidal anti-inflammatory drug for postoperative pain relief and continuous active and passive range of motion (ROM) exercise to gain motions. Preoperatively, active and passive range of motion were 70° for forward elevation, 60° for abduction and especially 0° for external rotation. After postoperative 2 months, active ROM were 150° for forward elevation, 130° for abduction and 80° for external rotation. After postoperative 6 months, passive and active ROM were full. UCLA score improved from preoperative 9 points to postoperative 29 points.
