코로나19의 증거생성 질병관리 (Evidence-Developing Disease Control of Coronavirus Disease 2019)

  • Coronavirus disease 2019 (COVID-19) has been going on for nearly a year. As of December 1, 2020, more than 60 million confirmed cases and 1.4 million deaths have occurred in 220 countries and territories. There is no effective treatment for COVID-19 so far, and vaccines are being developed. However, the vaccine of COVID-19 can be freed from COVID-19 only if there are positive answers to some questions: "Aren't there any major side effects from the vaccine?"; "Is the vaccine effective for enough time?"; "Does the vaccine sufficiently reduce the mortality rate?"; and "Does the vaccine sufficiently reduce the reproduction number?" The fight against COVID-19 will continue for a long time. Therefore, we need to respond by developing the evidence for control of COVID-19, the emerging infectious disease. Extensive quarantine measures in the early stages of COVID-19 need to be elaborated through developing the evidence. Korea Disease Control and Prevention Agency should establish a database for evidence developing COVID-19 control. In this database, based on more than 3 million tested individuals, epidemiological investigation results, health insurance claims data, and government data such as the Ministry of Public Administration and Security, etc. should be linked. It is necessary to disclose this database to academia, etc. to create the evidences of COVID-19 and manage COVID-19 through these. And through the developed evidences, guidelines for physical health and mental health in the era of Corona 19 should be developed.

Indonesian plastic surgeons' attitude during early period of the COVID-19 pandemic

  • Background: This study aims to report how the practice of plastic surgeons and their attitude was during the first measure period of coronavirus disease 2019 (COVID-19) pandemic. Methods: A survey study was held among members of the Indonesian Association of Plastic Reconstructive and Aesthetic Surgeons during week 5 after the first report of COVID-19. A 10 multiple-choice questions (MCQs) and 1 essay covered key questions on the area of surgery and operating room, clinics, internal meeting, and consultation. The only open-ended question relates to the last MCQ addresses a future "flipped" medical practice. Results: Response rate was 45.6% among 228 members, with 89.4% did no practice or limited their service to emergency and urgent cases only. Only 1.9% kept their official meeting as usual, while the majority modified it. The practice in the operating theatre and clinic were also altered to comply with the measures; with 21.2% from the total respondents only allowed patients with exposure to come for visit after taking 14 days of self-quarantine. Teleconsultation was practiced by 50% of the respondents, while 41.3% agreed and 10.6% disagreed upon the future "flipped" medical practice. Conclusion: In general plastic surgeons have made supportive actions during the pandemic. Surgery was performed with all precautions at the utmost as a reflection of high alert of viral infection. Teleconsultation has been embraced via existing social media. Agreement upon the future "flipped" medical practice is reasonable. All in all, the actions were considered as most relevant.

  • The aviation industry has continued to grow despite the influence of external environmental factors such as SARS and MERS. However, this growth has led to a sharp decrease in air traffic and a crisis of flight suspension due to the collapse of the global value chain and the decline of the world economy as the World Health Organization (WHO) declared a COVID-19 pandemic. Due to the decrease in the number of international flights, aviation workers such as pilots and cabin crew were exposed to high psychological stress and fatigue, such as large-scale layoffs, job instability, decrease in income, and increased risk of infection during. Recently, the international community has eased immigration restrictions through quarantine activities, and airlines are taking a step further to recover existing air demand. However, during the crisis period, a significant number of professional workers have been turned off or fired, and the physical and mental fatigue of those who can perform their duties is increasing. Therefore, this paper intends to examine the direction of policy improvement and the identification of problems in aviation safety and fatigue management after the pandemic.

T Cell Immune Responses against SARS-CoV-2 in the With Corona Era

  • After more than two years of efforts to end the corona pandemic, a gradual recovery is starting in countries with high vaccination rates. Easing public health policies for a full-fledged post-corona era, such as lifting the mandatory use of outdoor mask and quarantine measures in entry have been considered in Korea. However, the continuous emergence of new variants of SARS-CoV-2 and limitations in vaccine efficacy still remain challenging. Fortunately, T cells and memory T cells, which are key components of adaptive immunity appear to contribute substantially in COVID-19 control. SARS-CoV-2 specific CD4+/CD8+ T cells are induced by natural infection or vaccination, and rapid induction and activation of T cells is mainly associated with viral clearance and attenuated clinical severity. In addition, T cell responses induced by recognition of a wide range of epitopes were minimally affected and conserved against the highly infectious subsets of omicron variants. Polyfunctional SARS-CoV-2 specific T cell memory including stem cell-like memory T cells were also developed in COVID-19 convalescent patients, suggesting long lasting protective T cell immunity. Thus, a robust T-cell immune response appears to serve as a reliable and long-term component of host protection in the context of reduced efficacy of humoral immunity and persistent mutations and/or immune escape.

Covid-19와 한국 수출 변화 관계 분석 (Changes in Exports of Korea in the COVID-19 Era)

  • The purpose of this study is to analyze how Korea's exports amount changed due to COVID-19 and what factors played a role in these changes. We analyze Korea's exports with 40 countries around the world. In analyzing the impact of COVID-19, we estimate the gravity model using international trade data for five years from 2015 to 2019 and and compare the results with those for 2020. Several factors such as mortality, quarantine intensity, industry characteristics are considered for the analysis. The following results were obtained. First, as a whole, exports of Korea decreased significantly to countries with strong containment measures. In addition, Korea's exports (increase further) or decrease less to countries with a large number of deaths and confirmed cases in importing countries. Second, these results were similar in the major industries, classified by HS two units. Exports to countries with strong containment decreased a lot while exports decreased less to the countries with severe COVID19 (based on the number of deaths or confirmed cases). Third, however, different results were obtained in the analysis of reagents and vaccines, which are detailed items directly related to COVID-19. Rather, the degree of containment does not matter for these items, and it seems that Korea exported more to countries to more severe Corona countries.

코로나19 팬데믹 위기 대응 역량의 국가별 비교분석 (Comparative Analysis of COVID-19 Pandemic Crisis Response Capacities by Countries)

  • 목적: 국가별 감염병 대응 역량을 분석하여 이를 바탕으로 우리나라의 감염병 관리 대응에 대한 개선점을 찾아보고자 한다. 방법: 본 연구에서는 첫 번째로 2022년 WHO가 공개한 전 세계 96개국 SPAR 점수로 코로나19 감염병 대응 역량을 국가별로 분석하였다. 둘째, Our World in Data와 글로벌보건안보지수(GHSI)를 활용하여 각국의 구체적인 코로나19 방역 성과를 분석하였다. 결과: 첫째, 2021년 1월 24일의 방역강도 지수는 동남아시아 지부 방역이 67.6으로 가장 높아 강한 방역대책을 가지고 있었고, 아프리카 지부는 44.5로 가장 낮았다. 2022년 12월 31일의 방역강도 지수는 유럽이 11.6으로 대폭 낮아졌다. 둘째, SPAR 지표가 인구 백만명당 총환자수에 미친 영향 요인은 국가 실험실(C4)로 p=.027이고, 인구 백만명당 총사망수에 미친 영향 요인은 감염 예방과 관리(C9) p=.005, 위험 의사소통 및 지역사회 참여(C10) p=.040이었다. 1인당 GDP의 영향 요인은 감염 예방과 관리(C9) p=.009이고, GHSI에 미친 영향 요인은 감염 예방과 관리(C9) p=.002이었다. 결론: 이상의 연구결과로 감염병 역량 정도를 각 국가가 자체평가한 결과인 SPAR가 코로나19의 누적 환자수를 낮추거나 방역강도를 결정하는 것과 연관성을 발견하기 어려웠지만 사망율과 GHSI, 국민소득 등과는 일정 부분 영향을 받은 것으로 판단이 된다. 향후 우리나라의 감염병 관리 대응에 대한 개선점은 향후 미지의 신종감염병이 발생했을 때를 대비하여 JEE 혹은 GHSI 등과 같은 보다 과학적이고 신뢰성 높은 데이터를 중심으로 대응역량을 분석하여 사회·경제적 비용 감소 효과를 절감할 수 있는 방역대책 수립이 필요하다. 이를 기초로 공중보건학적 국가 위기에 대응하여 전문가 그룹을 중심으로 한 콘트롤타워의 주도적인 의사결정과 효과적 보건 의사소통도 요구된다.

공간 빅데이터를 활용한 행위자 기반 전염병 확산 예측 모형 구축에 관한 연구 -서울특별시 메르스 사태를 중심으로- (A Study on the Agent Based Infection Prediction Model Using Space Big Data -focusing on MERS-CoV incident in Seoul-)

  • 역학 모델은 질병 확산에 대한 시뮬레이션 및 관련 방역대책을 수립하는데 유용하며, 개체들의 접촉을 통해 전파되는 질병의 공간 확산에 대한 자세한 이해를 가능하게 한다. 이 연구에서는 공간에서 개체 간의 상호작요에 의한 결과로 메르스 전염병의 확산을 실시간적으로 시뮬레이션하기 위해 공간 빅데이터와 통합된 행위자 기반 공간 모델을 제안하고자 한다. 설계된 모델은 모집단, 시간, 공간이라는 세 요소를 고려하여 병원간의 직접접촉을 묘사하였다. 모집단의 역학관계는 2015년 서울특별시에서 발생한 메르스 사례를 기준으로 하였으며, 도로를 이동하는 사람과 메르스 환자가 발생한 병원과의 직접접촉으로 전염병이 전파하는 것으로 설계하였다. 모델을 이용하여 메르스 발생 상황을 예측하면서 시계열별로 실제 메르스 확산과 본 모형의 결과를 비교분석 하여 모형의 타당성을 검증하였으며, 다양한 시나리오를 적용해서 모의실험을 수행하였다. 메르스 발생 상황에서 방역 전략을 선정하기 위해 제시된 방법을 이용하여 방역조치를 다양하게 실험하는 것은 메르스 확산을 통제하는데 중요한 역할을 할 것으로 기대된다.

위기관리단계별 생물테러 관리체계에 관한 연구 (A Study on the Management System of Bioterrorism by the Phases of Crisis Management)

  • 9${\cdot}$11 테러 이후 우리는 뉴테러리즘이라는 용어를 많이 접하고 있다. 뉴테러리즘의 형태중 가장 전형적인 모습이 생물테러리즘이다. 그러나 생물테러리즘에 대하여는 자료의 특수성으로 인해 접근이 제한되고 있으며 이로 인해 일반시민들의 인지도 부족하고, 전문가들의 연구도 활성화되어 있지 아니한 분야이기도 하다. 생물테러는 국가적인 위기이므로 위기관리 관점에서 다루면 좀 더 구체적은 대비책을 찾을 수 있을 것이다. 따라서 위기관리 4단계를 중심으로 하는 단계별 대비 활동을 구체화하여 생물테러에 적용한다면 단계별 활동할 사항을 도출할 수 있고 우리의 대비실태를 점검할 수 있을 것이다. 생물테러는 발생하기는 어렵지만, 발생하면 국가적으로 많은 혼란을 가져올 것이다. 따라서 지구상 생물테러가 발생한 사례와 생물테러와 유사한 사례를 찾아 연구된다면 우리나라의 생물테러대비에 많은 시사점과 도움을 줄 수 있을 것이다 .이러한 관점에서 최근 한구고가 유사한 문화와 환경을 가진 중국에서 발생한 SARS 사례는, 생물테러 사태는 아니지만 예상하지 못한 시기와 장소에서 발생한 강력한 전염병에 대응한 사례로서 생물테러 대비에 많은 시사점을 준다고 할 것이다. 본 고에서는 연구목적을 달성하기 위하여 중국에서 SARS에 대응한 사례를 중심으로 생물테러라는 국가위기를 관리/대응하기 위한 활동을 위기관리 4 단계 즉 완화 및 예방단계, 대비단계, 대응단계, 복구단계로 구분하여 알아보았다.

Brucella abortus 국내 분리주의 세포외막 단백질 분석 및 혈청학적 비교 (Comparative serological analysis of outer membrane proteins extracted from Brucella abortus Korean isolates and 1119-3 strains)

  • Brucellosis is one of the most important zoonosis in worldwide. As one of the control measures, attempts have been made to develop new diagnostic methods using filed isolates as a national policy in many countries. Currently, bovine brucellosis in Korea have been received attention in both public health and economical aspects due to sudden increase of outbreak. Based on the situation, we compared standard strain (B. abortus 1119-3) with field isolates to reveal the differences among them. Biological and biochemical charateristics, antibiotic resistance profiles, outer membrane proteins (OMPs) and lipopolysaccharide analysis of the strains were included in this study. For the diagnostic purpose, an attempt was made to find out a novel antigen from the Korean isolates by serological analysis. There were differences about 55 kDa, 36-38 kDa and 20 kDa in analysis of OMPs by SDS-PAGE and Western blot with positive sera ($\geq$ 1:400 in SAT titer). Also, a serological diagnostic method, ELISA was conducted using OMPs of the strains as novel antigen. Relationships between O.D. and SAT titer were analyzed using field sera showing different SAT titer. High correlation coefficient was observed between SAT titer and ELISA. Results from this study suggested that a new diagnsotic method should be developed using their own field isolates in each country.

다층모형을 이용한 국내 양돈농가의 돼지생식기호흡기증후군 위험요인 분석 (The Use of Multilevel Model to Evaluate the Risk Factors for Porcine Reproductive and Respiratory Syndrome in Swine Herds)

  • The goal of this study was to investigate risk factors associated with porcine reproductive and respiratory syndrome (PRRS) in pig farms in the Republic of Korea using logistic regression and a multilevel model. A cross-sectional study was applied to 305 pig farms with a questionnaire-based interview by veterinarians between March 2014 and February 2015. The questionnaire comprised eight categories: proximity to neighbors, disinfection, visitors, vehicles, insecticides, wild animals, gilts, and feeding. In total, 61 questions in eight categories related to pig farm biosecurity were investigated. Farms were classified as PRRS stable or unstable based on the results of an antibody test and PCR. For univariate analysis, keeping production records with computers (OR = 0.283, 95% CI = 0.056 - 1.425), accredited farm with no use of antibiotics (OR = 0.412, 95% CI = 0.134 - 1.269), reviewing health record of semen prior to purchasing (OR = 0.492, 95% CI = 0.152 - 1.589), complete isolation of runt pigs (OR = 0.264, 95% CI = 0.084 - 0.829), compulsory registering for visitors (OR = 0.424, 95% CI = 0.111 - 1.612), keeping records of insecticide history (OR = 0.406, 95% CI = 0.089 - 1.846), routine on-farm monitoring by veterinarians (OR = 0.314, 95% CI = 0.069 - 1.423), and use of on-farm checklist for biosecurity monitoring (OR = 0.313, 95% CI = 0.063 - 1.553) were found to decrease the probability of PRRS infection. Multivariate and multilevel analysis revealed only two factors, complete isolation of runt pigs (OR = 0.165, 95% CI = 0.045 - 0.602 and OR = 0.208, 95% CI = 0.055 - 0.782) and compulsory registering for visitors (OR = 0.106, 95% CI = 0.017 - 0.655 and OR = 0.119, 95% CI = 0.017 - 0.809) were found to decrease the probability of PRRS infection. The intracluster correlation coefficient of a province for multilevel model was 0.05. The results of this study might facilitate biosecurity measures for individual farms to reduce the probability of PRRS infection.
